Description
Foreliggende oppfinnelse angår fremstilling av den nye forbindelse 2-amino-2<1>,6'-propion-xylidid til bruk som legemiddel mot hjerte-arytmi hos pattedyr.
Helt siden man begynte å legge vekt på hjertebehand-ling har man lagt stor vekt på behandling av ventrikulære eks-trasystoler og andre hjerte-arytmier. Intet kjent legemiddel er helt tilfredsstillende for langvarig behandling og regulering av slike arytmier. Stoffer som chinidin, procainamid, propran-olol og difenylhydantoin (fenytoin (Brit. Pharm)), har vært be-nyttet, men oppviser uønskede bivirkninger. Visse fenoxyderi-vater av aminopropan har også vært studert, men deres virkning på sentralnervesystemet er av lignende art som hos fenytoin, Proceedings of the British Pharmacological Society, bind 39,
s. 183P, (1970). I tillegg til ovennevnte stoffer oppviser andre farmasøytiske preparater antiarytmiske egenskaper. For eksempel er den lokalanestetiske forbindelse lidocain med kjem-isk navn 2-dietylamino-2<1>,6'-acetoxylidid et antiarytmisk legemiddel som er egnet for intravenøs eller intramuskulær bruk, Parkinson, P.I., og medarbeidere, Brit. Med. J., bind 2, s. 29-30, (1970) og The Merck Index, 8. utgave, (Merck & Company, Inc., Rahway, New Jersey, 1968), s. 618, men er ikke virksomt oralt
på grunn av de lave stoffnivåer i blodet. Eisinger og Hellier, Lancet, 1969, II, 1303 og Boyes et al, Clin. Pharmacol. Therap. 12, nr. 1, s. 105-116 (1971). Når lidocain gis oralt finner man et utpreget tap av legemidlet som sannsynligvis skyldes leverfunksjonene som mesteparten av stoffet må passere umiddelbart etter absorpsjonen fra fordøyelseskanalen. Varigheten av
disse konsentrasjoner i blodet med lidocain er også temmelig lav, hvilket utelukker langvarig beskyttelse.
Det er også kjent at visse 2-aminotetraliner har antiarytmiske egenskaper. D.M. Graeff et al, Journal of Medicinal Chemistry, bind 14, s. 60-62 (1971) . Videre, er det fra Archiv der Pharmacie, bd. 301 (1968), side 780-785, kjent en med forbindelsen fremstilt ifølge oppfinnelsen beslektet forbindelse, nemlig 2-dimetylamino-2<1>,6<1->propionxylidid og med liknende terapeutisk virkning.
Ifølge foreliggende oppfinnelse fremstilles den åntiarytmisk virksomme, forbindelsen 2-amino-2',6'-propionxylidid med formelen:
samt forbindelsens farmasøytisk akseptable salter, og fremgangsmåten til fremstilling av denne forbindelse er kjennetegnet ved at man omsetter 2-klor-2<1>,6<1->propionxylidid, 2-jod-2<1>,6<1->propionxylidid eller 2-brom-2',6<1->propionxylidid med ammoniakk, hvoretter, om ønsket, det erholdte produkt omdannes til et terapeutisk akseptabelt salt derav ved omsetning med en farma-søytisk akseptabel syre.
Den fremstilte forbindelse og dens salter har, som nevnt,, anti-arytmisk virkning og selv ved oral administrasjon, oppnås et høyt nivå av legemidlet i blodet. Varigheten av dette høye nivå er også relativ lang.
Uttrykket "terapeutisk akseptabelt salt" betegner på området et syreaddisjonssalt som er fysiologisk uskadelig når det gis i en slik dose og med slike intervaller (f .eks. administrasjonsintervaller) at forbindelsen er effektiv for den angitte terapeutiske bruk av moderforbindelsen. Typiske terapeutisk anvendelige addisjonssalter av 2-amino-2<1>,6'-propionxylidid omfatter salter av mineralsyrer som saltsyre, fosfor-syre eller svovelsyre og av organiske syrer som ravsyre og vin-syre, samt sulfonsyrer som metansulfonsyre•
Ved klinisk praksis vil de aktive forbindelser nor-malt gis oralt eller ved injeksjon i form av farmasøytiske preparater som inneholder forbindelsen i form av en fri base eller som ett av de vanlige terapeutisk brukbare salter, f.eks. hydro-kloridet, i forbindelse med farmasøytisk brukbare bærestoffer som kan være faste, halvfaste eller flytende, eller som fordøy-elig kapsel.' Vanligvis vil den aktive forbindelsen utgjøre fra 0,1 til 10 vektprosent av preparatet, f.eks. i vandig oppløs-ning i form av det oppløselige syresaltet, selv om forbindelsen når den foreligger i form av faste preparater som tabletter eller kapsler, kan ha en konsentrasjon på opptil 100 vektprosent av tabletten eller kapselen.
Farmasøytiske preparater i form av doseringsenheter på 100 til 250 mg hver for oral bruk kan lages ved å blande enten basen eller syresaltet med et fast pulverformet bærestoff. Eksempler er lactose, saccharose, sorbitol; mannitol og stivel-ser som potetstivelse, maisstivelse eller amylopektin, cellulosederivater eller gelatin. Bærestoffet kan også være smøre-midler som magnesium- eller kalsiumstearat, en "Carbovoks" eller andre polyetylenglykol-vokser som presses til tabletter eller kjerner som deretter kan belegges med enten en konsentrert sukkeroppløsning som også kan inneholde gummiarabikum, gelatin, talkum og/eller titandioksyd eller som eventuelt kan belegges med en lakk oppløst i et lettflyktig organisk oppløsningsmiddel eller en blanding av slike oppløsningsmidler. Man kan tilsette fargestoffer til disse belegg. Langsomt-oppløsende tabletter kan fremstilles ved å bruke flere lag av den aktive forbindelsen atskilt av langsomt-oppløsende belegg. En annen måte for fremstilling av langsomt-oppløsende tabletter er å oppdele dosen av aktivt stoff i granulater med belegg som har forskjellige tykkelser og presse granulatene til tabletter sammen med bære- . stoffet. Den aktive forbindelsen kan også innarbeides i lang-somt-oppløsende tabletter av fett eller vokser eller kan for-deles jevnt i en tablett eller en uoppløselig forbindelse som f'.eks. et fysiologisk inert plast-stof f, f or eksempel som be-skkevet i U.S. patent 3.317.394.
Myke gelatinkapsler (perleformede lukkede kapsler) og andre lukkede, kapsler består for eksempel av en blanding av gelatin og glyserol og kan inneholde blandinger av den aktive forbindelse og en vegetabilsk olje. Hårde gelatinkapsler inneholder granulater av den aktive forbindelsen med faste pulver-formede bærestoffer som lactose, saccharose, sorbitol, mannitol eller stivelse som potetstivelse, kornstivelse eller amylopektin eller cellulosederivater eller gelatin, eventuelt magnesium-stearat eller stearinsyre. Doseringsenhetene for alle disse kapsler kan ligge på mellom 100 og 250 mg for enten basen eller syreaddisjonssaltet.
For parenteral bruk ved injeksjon består preparatene fortrinnsvis av en vandig oppløsning av et vannoppløselig, farm-asøytisk anvendelig syresalt av den aktive forbindelsen, eventuelt tilsatt et stabiliseringsmiddel og/eller en bufferforbindel-se. Oppløsningene kan gjøres isotoniske ved tilsetning av nat-riumklorid. Den foretrukne doseringsenhet for disse oppløsnin-ger ligger også på 100 til 250 mg 2-amino-2<1>,6<1->propionxylidid
eller dets terapeutiske salt.
Evnen hos 2-amino-2<1>,6<1->propionxylidid til å under-trykke hjerte-arytmi har vært demonstrert både på mus og hunder.
Eksperimentene på mus ble utført i henhold til en modifikasjon av fremgangsmåten beskrevet av J.W. Lawson, "Anti-arrhytmic activity of some isoquinoline derivatives determined by a rapid screening procedure in the mouse" (Antiarytmisk virkning for enkelte isochinolinderivater bestemt ved en hurtig screening-prosess på mus), J. Pharm. Exp. Therap., bind 160,
s. 22-31, (1968) . Metoden er basert på den observasjon at når en ubedøvet og ubehandlet mus utsettes for kloroformdamp vil
åndedrettet snart stanse og på dette tidspunkt viser både elek-trokardiografisk og visuell undersøkelse at hjertekamrene flimrer. Hvis musen behandles på egnet måte med kjente antiarytmika før kloroformbehandlingen vil åndedrettsstansen ikke følges av
hjertekammerflimring.
Grupper på 10 hunnmus av sveitsisk albinotype (HAM/ICR) som hver veier 18 til 25 g, ble forbehandlet med en dose (ca. LD , - den dosen som dreper en av tusen mus) av
0, 1
2-amino-2<1>,6<1->propionxylidid i 10, 20, 40, 80 og/eller 160 minutter, respektivt, før de ble anbragt i et 2 liters begerglass inneholdende bomull og 50 ml kloroform. Umiddelbart etter at dyrene hadde opphørt å puste ble de tatt opp av begerglasset, itoraks ble åpnet og hjertet undersøkt for å finne om hjertekam-.ret fibrillerte eller ikke. Hjerterytmen ble bekreftet av
elektrokardiografiske målinger. Hvis man ikke fant flimring, ble hjertet berørt med en tang. Man anser.at hjertet flimrer hvis det finnes fine skjelvebevegelser på hjertemuskelens over-flate som holder seg i minst 5 sekunder etter åpningen av tor-aks eller den mekaniske stimulus. Man betrakter hjertet som fritt for hjertekammerskjelving hos dyr hvor en organisert hjertekammeraktivitet var tydelig etter ovenstående inngrep..
Den følgende tabell viser virkningene ved administrasjon av lidocain, chinidin, procainamid og 2-amino-2',6<1->propionxylidid.
Forbindelsen 2-amino-2<1>,6<1->propionxylidid ble også prøvet på hunder etter en modifikasjon av metoden som er beskrevet av A.S. Harris i "Delayed Development of Ventricular Ectopic Rhythms Following Experimental Coronary Occlusions"
(Forsinket fremkalling av ektopiske rytmer i hjertekamret etter eksperimentelle hjerteoklusjoner), Circ. bind 1, s. 1318-1328
(1950). Ved denne fremgangsmåten ble hundene bedøvet, det ble åpnet inn til hjertet og den fremre nedstigende gren av venstre hjerteartérie ble avbundet i to trinn. Brystet ble lukket og hunden ble tillatt å komme seg av bedøvelsen. I løpet av de første 2 til 3 dager etter dette kirurgiske inngrep viste elek-trokardiogrammene hjertekammer-arytmi og disse ujevne rytmer viste seg å undertrykkes eller oppheves av de kjente antiarytmika. (1956). For å undersøke virkningen av 2-amino-2<1>,6'-propionxylidid på disse arytmier, ble ubedøvede hunder anbragt i strop-per og behandlet intravenøst eller oralt med legemidlet. De elektrokardiografiske, kardiovaskulære eller andre virkninger av legemidlet ble avlest og skrevet opp. Eksperimentene viste at intravenøse doser fremkalte en tydelig undertrykkelse av eller nedsettelse av hjerte-arytmien, uten at man fant uheldige side-virkninger. Andre hunder ble gitt orale doser som også fremkalte tydelig nedsettelse av hjertekammer-arytmien.
Tabell II oppsummerer resultatene av den orale behandling.
Reaksjonen hos ubedøvede hunder overfor intravenøs infusjon med lidocain og 2-amino-2',6'-propionxylidid på første dag etter hjertearterie-avbindingen er angitt nedenfor i tabell III. Doseringsskjemaet var: 1. time: 15 mg/kg/time, 2. time: 30 mg/kg/time, 3. time: 60mg/kg/time. Infusjonen ble stoppet ved første tegn på toksisitet av annen karakter enn oppkast.
Avbindingen ble foretatt nær den fjerneste kanten av venstre hjertekammerappendiks. En dobbelt ligatur ble ført under den løsnede arterie og skåret over slik åt man fikk to ligaturer i samme stilling på arterien. Den første ble strammet omkring, arterien med mellomlegg av en nål nr. 20 som ble trukket opp etterpå. Etter 30 minutter ble den andre ligaturen strammet for å lukke arterien.
Forbindelsen 2-amino-2<1>,6<1->propionxylidid viser uventet antiarytmiske virkninger. Stoffet har meget svak lokalanestetisk virkning sammenlignet med lidocain, et kjent bedøvel-sesmiddel og antiarytmisk legemiddel. For å vise dette, ble
20 ml oppløsninger av 2-amino-2<1>,6'-propionxylidid og lidocain sammenlignet-in vitro ved måling av virkningen på hoftenerven fra frosk i henhold til metoden beskrevet av Camougis og Takman, "Nerve and Nerve-Muscle Preparations (As Applied to Local Anes-thétics)" (Nerve- og nerve-muskel-preparater (anvendt på lokalanestetika)), kapittel 1, i Methods of Pharmacology, A. Schwartz, ed., Appleton-Century-Crofts, New York, New York (.1970). Man
fant at lidocain-oppløsningen sperret 78% av nervens normale virkningspotensial etter 5 minutter, mens forbindelsen fremstilt ifølge oppfinnelsen ikke blokkerte virkningspotensialen i det hele tatt etter at nerven var utsatt for oppløsningen i 96 minutter.
For å demonstrere hva som oppnås ved den ifølge oppfinnelsen fremstilte forbindelse i forhold til nevnte kjente forbindelse, nemlig 2-dimetylamino-2',6'-propionxylidid, ble det foretatt sammenligningsforsøk med disse forbindelser (racemat-ene) som følger: Ved kontinuerlig infusjon i hund ble det oppnådd et suksessivt øket innhold i blodplasmaet (venøst blod) med forbindelsen i foreliggende søknad. Derimot ble- det oppnådd en platåverdi med den tidligere kjente forbindelse, hvilket antyder en kortere farmakologisk halveringstid og dermed mindre gode muligheter for anvendelse som oralt administrerbart antiarytmi-middel.
Ved kontinuerlig infusjon av 0,5 mg/kg/min. av forbindelsen fremstilt ifølge oppfinnelsen ble det således oppnådd et blodnivå på' 40,1 ug/ml etter 160 min. Nivået øket lineært, under hele perioden.
Ved kontinuerlig infusjon av 2-dimetylamino-2-', 6 '-propionxylidid (0,25 mg/kg/min) ble det oppnådd en platåverdi på 7 Mg/ml etter 30 min. Når man øket infusjonshastigheten til 0,5
mg/kg/min., ble det oppnådd en ny platåverdi på 10 ;ig/ml etter
10 min. Ved en ytterligere øking av infusjonshastigheten til
1 mg/kg/min. steg blodnivået til 20 pg/ ml (platåverdi) etter 30.
min. Dette antyder en hurtig metabolisme og/eller eliminering av stoffet med påfølgende vanskelighet å- opprettholde farmakologisk virksomme blodnivåer.
Resultatene fra sammenligningsforsøk av CNS-toksisitet og beskyttelseseffekt mot kloroforminduserte arytmier (kfr. side 5 i søknaden) hos mus, fremgår av nedenstående tabell:
De ovenfor angitte tallverdier innebærer et rela-tivt terapeutisk indeks på 2 for ataksi, 2 for konvulsjon og 3 for tap av korrigeringsrefleks, for den nye forbindelse fremstilt ifølge foreliggende oppfinnelse sammenlignet med nevnte tidligere kjente forbindelse.
Til tross for den generelt aksepterte tidligere lære at antiarytmisk virkning og lokalanestetisk virkning er nøye for-bundet og at primære aminer er mye svakere lokalanestetika enn de tilsvarende sekundære aminer, viser det meget svake lokalanestet-ikum, 2-amino-2<1>,6'-propionxylidid sterke antiarytmiske egenskaper som har lang varighet. 2-amino-2',6'-propionxylidid syntetiseres som nevnt ved å omsette det tilsvarende 2-klor-, 2-brom- eller 2-jod-2',6'-propionxylidid med ammoniakk i alkoholisk-vandig oppløsning eller alkoholisk oppløsning. Ammoniakken tilsettes som gass enten kontinuerlig eller med avbrudd ved lav temperatur. En blanding av ammoniumhydroksyd, alkohol og gassformig ammoniakk kan også brukes. Reaksjonsdeltagerne kan holdes ved romtemperatur eller høyere temperatur i lukket kar. Blandingen omrøres i
4-9 dager hvis man bruker romtemperatur.
Oppfinnelsen illustreres ytterligere av følgende eksempel:
Eksempel:
Syntese av 2- amino- 2', 6'- propionxylidid. Forbindelsen 2-amino-2',6'-propionxylidid ble synt-etisert ved å mette med gassforraig ammoniakk og ved romtemperatur en suspensjon av 50 g (0,195 mol) 2-brom-2',6<1->propionxylidid i en blanding av 500 ml 95% alkohol og 400 ml konsentrert vandig ammoniakk. Metningen ble gjennomført under mekanisk rør-ing. Etter 25 timer ble blandingen mettet på nytt med ammoniakk-gass. Røringen ved romtemperatur ble fortsatt i løpet av ialt 116 timer og man tok en prøve på dette tidspunkt. Gasskromatografisk analyse viste at ca. 95% av bromforbindelsen var omdan-net til det ønskede produkt. Oppløsningene ble avdampet i vakuum og residuet tatt opp i 80 ml 3 molar saltsyre. Etter tilsetning av 220 ml vann ble uoppløselig stoff frafiltrert, vasket med 100 ml vann og tørket. Det uoppløselige stoffet veide 9,5 g og besto hovedsakelig av uomsatt bromforbindelse. Filtratet ble omsatt med 50 ml 7 molar NaOH, ekstrahert tre ganger med metylenklorid (50 ml + 2 x 25 ml posjoner), tørket over kaliumkarbonat og inn-dampet. Utbyttet var 26,8 g som utgjorde 71,4% av teoretisk ut-bytte. Denne rest var en fargeløs stivnende olje som ble oppløst i 200 ml kloroform. Hydrogenklorid ble gjennomboblet til en prøve av oppløsningen viste sur reaksjon på fuktig pH indikator- • papir. Man fikk en felling som ble frafiltrert. Denne ble vasket med kloroform og tørket. Smeltepunktet ble bestemt til 246-247,5°C. Gasskromatografisk analyse av stoffet viste bare en komponent. De beregnede verdier for 2-amino-2',6<1->propionxylidid-hydroklorid (C^H^<C>ll^O med molvekt lik 228,5) er:
C, 57,8; H, 7,49; N, 12,2; Cl, 15,5; og analyseverdiene var:
C, 57,7; H, 7,69; N,12,2; Cl, 15,5. IR-analyse (KBr skive, hydroklorid). viste: 0 2000-1850 (et bredt bånd med skulder, NH3+), 1670 (amid I), 1540 (amid II), 1386, 1375 (symmetriske metyl-grupper), 760-1 (tre nabohydrogener på fenyl).
Forbindelsen 2-amino-2<1>,6'-propionxylidid og dens terapeutiske salter har vist seg å ha antiarytmiske virkninger når den gis til hunder i orale doser på fra 50 til 100 mg/kg, og en kumulativ dose på 7 5 mg/kg til mus i en maksimal oral dose på ca. 380 mg/kg og bør være effektiv på mennesker i dose på ca. 6 g pr. dag oralt.

- Analogifremgangsmåte til fremstilling av anti-arytmisk virksomt 2-amino-2<1>,6<1->propionxylidid med formelen:samt forbindelsens farmasøytisk akseptable salter, karak- terisert ved at man omsetter 2-klor-2',6'-propionxylidid, 2-jod-2<1>,6<1->propionxylidid eller 2-brom-2<1>,6'-propionxylidid med ammoniakk, hvoretter, om ønsket, det erholdte produkt omdannes til et terapeutisk akseptabelt salt derav ved omsetning med en farmasøytisk akseptabel syre.
