Analgetische samenstellingen en toepassingsmethoden.
De uitvinding heeft betrekking op een methode voor het creëren van analgesie door de simultane of opvolgende toediening van butorfanol en zomepirac alsmede samenstellingen die butorfanol en zomepirac omvatten,. welke verhoogde (synergistische) analgetische sterkten ver-5 schaffen.
Het verzachten van pijn is sinds lange tijd en blijft één van de belangrijkste doeleinden van de geneeskunde. Helaas geven bijna alle sterke analgetische middelen ten minste in enige mate andere reacties dan de gewenste analgesie, in het bijzonder wanneer ze in grote doses worden 10 ingenomen voor matige tot hevige pijn. Dergelijke neveneffecten omvatten duizeligheid, vertroebeling of vervaging van het zicht, misselijkheid, braken, verhoogde of verlaagde adem- of polssnelheden en dergelijke. Derhalve moet bij het gebruik van analgetische middelen niet alleen rekening worden gehouden met de analgetische sterkte van het desbetref-15 fende middel, maar ook zijn potentiële neveneffecten. Het is bekend om mengsels van analgetica voor te schrijven, ook wanneer hun werkzaamheden slechts additief zijn, omdat neveneffecten kunnen worden verminderd door analgetica te combineren die volgens verschillende mechanismen werken.
Het is ook bekend dat in sommige gevallen mengsels van twee of meer 20 analgetische middelen elkanders activiteiten versterken. Dit is bijzonder gewenst omdat de totale hoeveelheid geneesmiddel kan worden verlaagd en de neveneffecten kunnen worden verminderd. Er bestaat derhalve een voortdurend onderzoek naar combinaties van analgetische middelen die een verhoogde (synergistische) analgetische werkzaamheid geven, in een poging 25 om bepaalde combinaties te vinden die een maximale analgesie creëren met geen of weinig neveneffecten.
A) Het Amerikaanse octrooischrift 3.819.635 openbaart butorfanol en verwante verbindingen, alsmede werkwijzen voor hun bereiding. De analgetische en narcose-antagonistische werkzaamheden van butorfanol in ver- 30 schillende diermodellen worden geopenbaard.
B) A.B.Dobkin et al. in Canad.Anaesth.Soc.J., 21, 600-610 (1974), beschrijven dubbel blind proeven waarbij de analgetische werkzaamheden van butorfanol en morfine bij postoperatieve patiënten met matige tot hevige pijn worden vergeleken. De resultaten laten zien dat butorfanol 35 in de orde van tienmaal zo sterk is als morfinesulfaat.

Pharmacodyn., 235, 116-123 (1978), A.W.Pircio et al. worden de resultaten beschreven van proeven in muizen aan de analgetische werkzaamheden van combinaties van butorfanol en aceetaminofeen (APAP). Combinaties van butorfanol:APAP van 1:125 en 1:10 gaven anal-5 getische werkzaamheden die groter waren dan verwacht werd op grond van een eenvoudige additieve werking, dat wil zeggen er was een synergistisch effect. Het synergistisch effect van de 1:125 combinatie was statistisch significant (p < 0,001) terwijl het synergistisch effect van de 1:10 combinatie statistisch niet significant was (p< 0,1 >0,05).

E) Drugs of the Future, _2, 698-701 (1977) en daarin geciteerde publicaties beschrijven methoden voor de bereiding van zomepirac en zijn 15 werkzaamheid als anti-inflammatoor middel alsmede als analgetisch middel.
In proeven van zijn analgetische werkzaamheid in patiënten met tandpijn, bleken 50 mg en 100 mg doses van zomepirac significant beter te zijn- dan 650 mg asperine.
F) Er zijn vele bekende anti-inflammatore middelen die tevens anal-20 getische werkzaamheid bezitten. -3-,- «f * H) Het Amerikaanse octrooischrift 4.243.673 openbaart ruwweg synergistische analgetische samenstellingen welke een mengsel omvatten van suprofen en "niet-narcotisch analgeticum", dat daarin gedefinieerd wordt als een analgeticum dat eerder dan het centrale zenuwstelsel primair 5 het perifere zenuwstelsel aangrijpt. Vermeld wordt dat de voor gebruik geschikte "niet-narcotische analgetica" de salycilaten, paraaminofenolen en pyrazolonen omvatten, maar de enige samenstellingen waarvan voorbeelden zijn gegeven en waarvoor rechten worden gevraagd zijn mengsels van suprofen en APAP, 10 Fig. 1 is een isobolografische analyse welke de synergistische analgetische werkzaamheden van verschillende mengsels van butorfanol en zomepirac toont.

De uitvinding heeft in een opzicht betrekking op een methode voor het creëren van analgesie in dieren, waaronder de mens, omvattende de hetzij gelijktijdige hetzij opvolgende toediening aan een dier dat aan pijn lijdt van butorfanol en zomepirac.
20 De uitvinding heeft in een ander opzicht betrekking op samenstel lingen die butorfanol en zomepirac omvatten, welke verhoogde (synergistische) analgetische sterkten geven.
Verrassenderwijze is gevonden dat de samengaande toediening van de bekende analgetische middelen butorfanol en zomepirac in verhouding van 25 ongeveer 1:1 tot ongeveer 1:100 onverwachts een verbeterde analgetische werkzaamheid geeft. De geprefereerde verhoudingen lopen van ongeveer 1:2 tot ongeveer 1:50, en de meest geprefereerde verhoudingen lopen van ongeveer 1:5 tot ongeveer 1:25. Het zal de deskundigen duidelijk zijn dat de verhoogde werkzaaheid zal worden verkregen ongeacht of het 30 butorfanol en het zomepirac gelijktijdig als een mengsel of opvolgend als de twee individuele componenten worden toegediend.
Hoewel voor het gemak hier gewoonlijk eenvoudigweg zal worden gesproken van "butorfanol" is het specifiek de bedoeling dat de uitvinding het gebruik omvat van niet-toxische farmaceutische aanvaardbare 35 zure additiezouten van butorfanol. Dergelijke zouten zijn aan de deskundigen wel bekend en omvatten bijvoorbeeld anorganischeen organische '-8T2TT5 4 4--------------------------------------......................
-4- zuuradditiezouten zoals het hydrochloride, hydrobromide, nitraat, sulfaat, methaansulfonaat, p-tolueensulfonaat, succinaat, maleaat, tartraat en dergelijke. Om redenen van oplosbaarheid heeft een zuur additiezout van butorfanol gewoonlijk de voorkeur boven de vrije basevorm. 5 Butorfanoltartraat is een bijzonder geprefereerd zuuradditiezout.
Evenzo, hoewel gewoonlijk hier eenvoudigweg van "zomepirac" zal worden gesproken, is het specifiek de bedoeling dat de uitvinding het gebruik van niet-toxischefarmaceutisch-aanvaardbare zouten van zomepirac omvat. Deze verbinding bevat een carbonzuurgroep en vormt derhalve zouten 10 met organische of anorganische basen. Geprefereerde zouten zijn de natrium en kaliumzouten.
Butorfanol is de United States Adopted Name voor (-)-17-cyclobutyl-ethyl-3,14-dihydroxymorfinan met formule 1 van het formuleblad.
Zomepirac is de United States Adopted Name voor 5-(4-chlorobenzyl)-15 1,4-dimethyl-lH-pyrrol-2-azijnzuur met formule 2 van het formuleblad.
In de praktijk van de uitvinding kunnen het butorfanol en zomepirac oraal, of parenteraal worden toegediend. Vanweg het gemak wordt de orale route geprefereerd. Bij het bereiden van de samenstellingen voor oraal gebruik, kunnen elk van de gebruikelijke farmaceutische media, 20 hulpmiddelen en excipientia worden toegepast. Voor vloeibare orale pre— peraten kan men gebruik maken van water, glycolen, oliën, alcoholen en dergelijke, tezamen met kleurmiddelen, conserveringsmiddelen, smaakmid-delen en dergelijke, teneinde oplossingen, elixers, siropen, suspensies enz. te maken. Voor vaste orale preparaten kan men gebruik maken van 25 conventionele granuleringsmiddelen, smeermiddelen, bindmiddelen, desintegratiemiddelen, zetmelen, suikers en dergelijke, teneinde vaste doseringsvormen zoals capsules, tabletten of poeders te bereiden. In het geval van parenterale preparaten zal de gebruikelijke vorm een oplossing zijn van butorfanol en zomepirac in steriel water, samen met 30 eventuele gewenste conserveringsmiddelen of oploshulpmiddelen.
De toe te dienen dosering van butorfanol en zomepirac zal natuurlijk variëren met factoren zoals de toedieningsroute, leeftijd, gewicht en algemene gezondheidstoestand van de patient, ernst van de symptomen, aard (indien daarvan sprake is) van een gelijktijdige behandeling met 35 andere geneesmiddelen, de frequentie van de behandeling en het gewenste 8201544 « e -5- effect. Gewoonlijk zal de dosering van het butorfanol ongeveer 1 mg tot ongeveer 20 mg bedragen, bij voorkeur ongeveer 2 mg tot ongeveer-10 mg en liefst ongeveer 4 mg. De dosering van zomepirac zal gewoonlijk van ongeveer 10 mg tot 500 mg bedragen, bij voorkeur van ongeveer 25 mg 5 tot ongeveer 250 mg en liefst van ongeveer 50 mg tot ongeveer 100 mg. Dergelijke doseringen zullen twee. tot ,v±}fiuaal per dag worden toegediend, en gewoonlijk drie tot viermaal per dag. Wanneer de samenstelling is geformuleerd in een vorm met vertragende afgifte, kunnen minder dagelijkse doses vereist zijn, afhankelijk van de desbetreffende formulering.
10 Het is bijzonder voordelig en geschikt om samenstellingen van butorfanol en zomepirac te formuleren in de vorm van een doseringseen-heid, dat wil zeggen fysisch discrete eenheden die elk de berekende hoeveelheid butorfanol en zomepirac voor het geven van het gewenste resultaat bevatten,tezamenmetelke eventueel gewenste farmaceutische drager.
15 Dergelijke doseringseenheden kunnen de vorm van capsules, tabletten, pillen, ampullen,volle theelepels, volle lepels en dergelijke hebben.
Proevenvoor het vaststellen van de analgetische werkzaamheid. ________
De in dit onderzoek toegepaste geneesmiddelen waren butorfanoltar-traat, zomepiracnatrium en codeinefosfaat. Mengsels werden bereid voor 20 isobolografisch onderzoek door porties van standaardoplossingen van de bovengenoemde geneesmiddelen te combineren. Alle oplossingen waren vers bereid met gedestilleerd water.
Het onderzoek van de analgetische werkzaamheden van butorfanol-zomepirac en codeine-zomepirac combinaties, alsmede van de individuele 25 geneesmiddelen, werd uitgevoerd in de .muis (fenylquinon geïnduceerde krimpingenproef) na orale toediening. De analgetische interactie werd onderzocht door bepaling van de orale ED^-waarden (effectieve dosis^) en het gebruik, van de isobolografische methode van Loewef(S.Loewe,
Arzneim. Forsch., _9, 429-456 (1959)] zoals gemodificeerd door Gessner 30 en Cabana [P.K.Gessner en B.E.Caban, J.Pharmacol.Exp.Ther., 174, 247-259 (1969)].
De methode met vaste verhouding werd gebruikt om de interactie van deze analgetische geneesmiddelen te onderzoeken. In deze procedure werden ED^q-waarden van binaire mengsels van butorfanol-zomepirac en 35 codeine-zomepirac bepaalt de variërende doses van de twee geneesmiddelen 82 0 1 6 44 ..........................
-6- 4 b in een mengsel met een constante verhouding toe te dienen. Onder toepassing van de isobolografische analyse van Loewe kan derhalve worden vastgesteld of de waargenomen ED^q waarden significant verschillen van de waarden die verwacht werden op basis van de effecten van de individu- 5. ele geneesmiddelen, en of deze interactie een synergistisch, antagonistische of additieve werking vertegenwoordigden.
De numerieke schatting van de ED^q-waarden en hun betrouwbaarheids-grenzen werd verkregen door de probit methode van Finney [D.J.Finney,
Probit Analysis, pp. 50-80, 230-268, University Press, Cambridge, 10 England, 3rd Ed., (1971)]. De evaluatie van de gegevens van de mengsels werd uitgevoerd met technieken, beschreven door Finney voor het eenvoudige (additieve) model voor soortgelijke werkzaamheid.
Antagonisme van buikkrimpingen in de muis werd gebruikt om de analgetische werkzaamheid te evalueren. De methode was in essentie zo-15 als beschreven door Slegmund et al. [E.Siegmund et al., Proc.Exp.Biol.
Med., 95, 729-731 (1957)] Swiss-Webster mannetjes-muizen werden gebruikt met. een gewicht van 18-20 g na een nacht zonder eten te zijn gelaten. Alle geneesmiddelen en binaire mengsels werden oraal 25 minuten voor de intraperitonealêy injectie van fenylquinon (2 mg/kg) toegediend.
20 De proefmaterialen werden door een enkelvoudige incubatie in de maag gebracht in een volume gedestilleerd water dat in een constante verhouding stond tot het lichaamsgewicht (10 ml/kg). Vanaf vijf minuten na de injectie met fenylquinon werden de dieren geobserveerd voor buikkrimpingen en het aantal krimpingen over een periode van 10 minuten werd 25 geteld. Een dier werd geacht analgesie te vertonen wanneer het aantal .door.het dier vertoonde krimpingen 50% of minder van het gemiddelde aantal krimpingen van de pekel controledieren bedroeg. De ED^gWaarden waren de doses de analgesie gaven in 50% van de behandelde muizen en werden berekend uit dosis-responsiekrommen, gebaseerd op ten minste vier 30 dosesniveaus. Ten minste 30 muizen werden bij elke dosesniveau gebruikt.
De isobologram die de interactie van butorfanol-zomepirac en codeine-zomepirac, gecombineerd in verschillende verhoudingen, tonen, zijn respectievelijk in de figuren 1 en 2 weergegeven. In het geval van 35 butorfanol-zomepirac kan men zien dat de meeste ED^q-waarden van de verschillende mengsels duidelijk beneden de ED^^-additielijn liggen.

De ED^Q-waarden voor de codeïne-zomepirac-combinaties liggen echter niet significant beneden ED^-additielijn. Deze gegevens laten zien dat de verschillende butorfanol-zomepirac combinaties synergistisch waren doordat een kleinere dosis van het mengsel dan verwacht een 50% anal-5 getische responsie gaf, terwijl· de codelne-zomepirac combinaties in essentie additieve analgerische responsie gaven.
Een vergelijking van de waargenomen en verwachte ED^-waarden wordt getoond in tabel A, tezamen met de resultaten van de statistische behandeling van de gegevens. De "waargenomen" ED^^ van de combinaties 10 is de experimenteel bepaalde waarde terwijl de "verwachte" ED^ van de combinaties de waarde is, gebaseerd op een additief effect van de twee ingrediënten, dat wil zeggen het snijpunt is van de ED^q additielijn met de lijn die correspondeert met de desbetreffende verhouding van het butorfanol/zomepirac of codelne/zomepirac mengsel. Een statistisch 15 significant synergisme werd waargenomen bij butorfanol/zomepirac combinaties van respectievelijk 1:100, 1:50, 1:10 en 1:5, met synergisme van een grensgevalsignificatie (55%). getoond door het 1:1 mengsel.
De codelne/zomepirac combinaties van respectievelijk 1:1 en 1:5 gaven geen significant.synergisme, maar resulteerde in een vrijwel algebraïsche 20 optelling van hun individuele analgetische werkzaamheden.
Voor vergelijkingsdoeleinden werden proeven zoals boven beschreven, ook uitgevoerd aan mengsel van butorfanol met respectievelijk suprofen, isoprofen, naproxen., sulindac, piroxican en voltaren. Alleen de butor-fanol/suprofen mengsels vertoonden een duidelijk significant synergisme 25 bij alle onderzochte verhoudingen. De mengsels van butorfanol met respectievelijk ibuprofen, naproxen, sulindac, piroxicam en voltaren vertoonden bij sommige verhoudingen een significant synergisme, maar in een veel lagere mate dan de mengsels van butorfanol met zomepirac of suprofen. De resultaten van deze proeven zijn getoond in tabel B.

Claims (13)
1. Analgetische samenstelling, omvattende (a) butorfanol of een niet toxisch farmaceutisch aanvaardbaar zuuradditiezout daarvan en (b) zomepirac of een niet-toxisch farmaceutisch aanvaardbaar zout daarvan, waarin de gewichtsverhouding van (a) tot (b) lil tot 1:100 5 bedraagt.
2. Samenstelling volgens conclusie 1, met het kenmerk, dat de gewichtsverhouding van (a) tot (b) 1:2 tot 1:50 bedraagt.
3. Samenstelling volgens conclusie 1, met het kenmerk, dat de gewichtsverhouding van (a) tot (b) 1:5 tot 1:25 bedraagt.
4. Samenstelling volgens een of meer van de conclusies 1-3, met het kenmerk, dat hij bovèndien een farmaceutisch aanvaardbare drager bevat.
5. Samenstelling volgens een of meer van de conclusies 1-3, met het. kenmerk, dat het butorfanol aanwezig is als het.tartraatzout en het zomepirac aanwezig is als het natriumzout.
6. Samenstelling volgens conclusie 5, met het kenmerk, dat hij bovendien een farmaceutisch aanvaardbare drager bevat..
7. analgetische samenstelling in de vorm van een doseringseenheid, welke 1 tot 20 mg butorfanol of een niet-toxisch farmaceutisch aanvaardbaar zuuradditiezout daarvan en 10 mg tot 500 mg zomepirac of een 20 niet-toxisch farmaceutisch aanvaardbaar zout daarvan omvat, plus een farmaceutisch aanvaardbare drager.
8. Samenstelling volgens conclusie 7, met het kenmerk, dat het butorfanol of zout daarvan aanwezig is in een hoeveelheid van 2 mg tot 10 mg en het zomepirac of zout daarvan aanwezig is in een hoeveelheid 25 van 25 mg tot 250 mg.
9. Samenstelling volgens conclusie 7, met het kenmerk, dat het butorfanol of zout daarvan aanwezig is in een hoeveelheid van 4 mg en het zomepirac of zout daarvan aanwezig is in een hoeveelheid van 50 mg tot 100 mg.
10. Samenstelling volgens een of meer van de conclusies 7-9, met het kenmerk, dat het butorfanol aanwezig is als het tartraatzout en het zomepirac aanwezig is als het natriumzout. 820 1 5 44 “ ^ -14-
11. Werkwijze voor het creëren van analgesie in een dier/ omvattende gelijktijdige of opvolgende toediening aan het dier van een analgetisch werkzame hoeveelheid van (a) butorfanol of een niet-toxisch farmaceutisch aanvaardbaar zuuradditiezout daarvan en (b) zomepirac of een niet 5 toxisch farmaceutisch aanvaardbaar zout daarvan/ waarin de gewichtsverhouding van (a) tot (b) 1:1 tot 1:100 bedraagt.
12. Werkwijze voor het creëren van analgesie in een dier omvattende toediening aan het dier van een analgetisch werkzame hoeveelheid van een samenstelling volgens conclusie 4.
13. Werkwijze voor het creëren van analgesie in een dier, omvattende toediening aan het dier van een analgetisch werkzame hoeveelheid van een samenstelling volgens conclusie 6. —8"2 0“l-5“4'4............................
