Vaatverwijdend middel.
De uitvinding betreft een farmaceutisch preparaat met daarin als een actieve component het vaatverwijdende middel H'-cyaan-N-U-pyridyl-Ii’-1,2,2-trimethylpropylguanidine (hieronder aangeduid als P 1134) en als verder actief bestanddeel een diureticum, hetzij van het thiazide-, het 5 thiazideachtige type of een tubelus-diureticum, een werkwijze voor het bereiden van deze preparaten, doseringseenheden daarvan, en een werkwijze voor het behandelen van patiënten met dit preparaat, in het bijzonder in de vorm van doseringseenheden.
De onderhavige preparaten zijn waardevol bij het behandelen van IQ zoogdieren met inbegrip van de mens, die lijden aan te hoge bloeddruk of congestief hartbezwaar.
Amerikaans octrooischriffc 4.057*636 beschrijft een aantal., hoge bloeddruk bestrijdende middelen met de formule 1 van het formuleblad of de tautomere vormen daarvan, waarin de R -gesubstitueerde cyaan-gua— 15 nidylgroep op de 2-, 3- of 4-plaats van de pyridinering voorkomt, en waarin R een rechte of vertakte, al of niet verzadigde alifatische koolwaterstofgroep met 1-8 koolstofatomen, een cycloalkyl- of cyclo- alkenylgroep met 3-7 koolstofatomen, een aryl- of aralkylgroep, en 2 ...
R waterstof, halogeen, hydroxyl, alkyl of alkoxy voorstellen, die bij 20 diverse dieren na enterale of parenterale toediening goede bloeddruk verlagende middelen zijn gebleken.
Van de verbindingen volgens Amerikaans octrooischrift 4.057*636 is P 1134 een materiaal met opmerkelijk verlengde activiteit gebleken bij toepassing in de kliniek en het behandelen van te hoge bloeddruk, 25 waardoor P 1134 voor dit doel bijzonder bruikbaar is. Met name het mono-hydraat van P 1134 is bruikbaar gebleken voor farmaceutische preparaten en doseringseenheden daarvan voor de klinische praktijk.
Zijn activiteitsprofiel vertoont karakteristieken van een perifere vaatverwijder, d.w.z. dat de verlaging van de verhoogde bloeddruk 30 wordt bereikt door een direct effect op de gladde vaat spiertjes.

Patienten die -worden "behandeld met vaatverwijdende middelen ondervinden als gebruikelijk neveneffect een vloeistofretentie; een neveneffect, dat ook is waargenomen hij een aantal patiënten die werden behandeld met P 113^· Een dergelijke vloeistofretentie verlaagt in vele 5 gevallen het antihypertensieve effect van de vaatverwijdende therapie.
Volgens de uitvinding werd nu gevonden, dat de onderhavige comr· binatie bijzonder gunstig is bij het behandelen van te hoge bloeddruk en congestief hartbezwaar omdat de mogelijke neveneffecten in de vorm van een vloeistofretentie worden tegengegaan en een waardevol additief 10 of zelfs synergistisch effect wordt bereikt wat betreft de verlaging van de bloeddruk.
Klinische proeven hebben nu nl. aangetoond dat door een toediening van een preparaat volgens de uitvinding de gewenste verlaging van de bloeddruk wordt bereikt wanneer P 113^ in hoeveelheden liefst tussen 15 10 en 200 mg/dag wordt gebruikt, hetgeen overeenkomt bij volwassen mensen met ca. 0,15 - 3 mg/kg lichaamsgewicht per dag, zonder dat vloeistofretentie optreedt.
De onderhavige combinatie is voorts gunstig door de duidelijk minder uitgesproken tachycardie dan bij de tot dusver bekende combina— 20 ties van een vaatverwij der en een diureticum, b.v. combinaties van de vaatverwijders hydralazine, guancydine of minoxidil met diuretica (Antihypertensive· Agents, aditor Franz Gross, Springer Verlag, 1977)·
Het is uit de literatuur bekend, dat de verlaging van een verhoogde arteriele bloeddruk bij een behandeling met een vaatverwijder 25 wordt vergezeld, door een via reflexen opgewekte compensatoire verhoging van de hartactiviteit, die in vele gevallen moet worden tegengegaan door een gelijktijdige toediening van een /β-blokkerend middel.
Verrassenderwijze werd nu gevonden, dat bij patiënten die doses aan P 113^ kregen waarmee de bloeddruk aanmerkelijk werd .verlaagd, 30 slechts een minimale verhoging van de hartactiviteit vertoonden, zodat een gelijktijdige toediening van /S -blokkerende middelen gewoonlijk niet nodig was.
De toepassing van de onderhavige combinatie maakt het dus mogelijk de toediening· van een derde actief bestanddeel, nl, een /3-hLokke-35 rend middel, in de meeste gevallen van een milde tot matige hypertensie * --------------*3 8201493 - 3 - * Ki

te vermijden.
Indien bij een ernstige hypertensie toch een β-blokkerend middel moet worden gebruikt, lean de dosis daarvan kleiner zijn dan bij combinaties met andere vaatverwijders, en ook dit zal resulteren in 5 minder neveneffecten.
P 113^· is een base die zouten kan vormen met zuren liefst met farmaceutisch geschikte zuren, als zoutzuur en waterstofbromide, fosfor-zuur, zwavelzuur, salpeterzuur, p-tolueensulfonzuur, methaansulfonzuur, mierezuur,. azijnzuur, propionzuur, citroenzuur, wijnsteenzuur, maleïne-10 zuur en embonzuur, welke opsomming beslist niet als beperkend moet worden beschouwd.
Bij de onderhavige combinatie kan P 113¾ zowel als zodanig-of' in de vorm van een boven weergegeven zout worden gebruikt.
Van de diuretica van het thiazidetype die kunnen worden gebruikt 15 kunnen worden genoemd hydroflumethiazide, bendroflumethiazide, chloor— thiazide, cyclothiazide, hydrochloorthiazide, trichloormethiazide, cyclopenthiazide en polythiazide,. alsmede de thiazideachtige verbindingen als clopamide, chinethazon, mefruside en chloorthalidon. Van de tube·* lusdiuretica kunnen worden genoemd furosemide, bumetanide, piretamide, • 20 ethacrynzuur en indacrinon. Deze opsomming is slechts illustratief en niet beperkend voor de uitvinding.
Het is een gunstige eigenschap van de gecombineerde therapie met P 113¾ en een diureticum dat een bloeddrukverlagend effect, zelfs in ernstige gevallen reeds met lage doses P 113¾ wordt bereikt.
25 De uitvinding betreft derhalve een farmaceutisch preparaat dat bruikbaar is voor een algemene behandeling van patiënten die aan te hoge bloeddruk of congestieve hartbezwaren lijden.
Met het oog hierop bevat het onderhavige preparaat als eerste actieve bestanddeel een verbinding P 113¾ of een zout daarvan met een 30 geschikt zuur en als tweede actieve verbinding een diureticum tezamen met vaste, halfvaste of vloeibare farmaceutische dragers en/of hulpstoffen.
Het onderhavige preparaat moet tenminste 0,1$ en liefst meer dan 1% van het totaal aan therapeutisch actieve bestanddelen bevatten 35 en kan op een gebruikelijke wijze in diverse farmaceutische vormen wor— 8201493 * — k — den gegoten» b.v. in die -van tabletten, pillen, dragees, capsules,, verlengd werkende tabletten, suspensies, suppositoria» injecties, met daarin steeds bet vaatverwijdende middel en bet diureticum volgens de uitvinding gemengd met dragers en/of hulpstoffen.
5 Farmaceutisch geschikte niet giftige organische of anorganische, vaste,, halfvaste of vloeibare dragers en/of hulpstoffen die geschikt zijn. voor orale of enterale toediening, kunnen worden gebruikt voor het vervaardigen van de onderhavige preparaten. Gelatine, suikers en suiker-alcoholen, .zetmeelstoffen, zetmeelderivaten, cellulose en cellulosederi-.
10 vaten,, calcium of magnesiumstearaat, talk, natuurlijke of gemodificeerde plantaardige en dierlijke vetten en oliën, minerale oliën, gommen» polyalkyleenglycolen» polyvinylderivaten, buffers, organische zuren, carbonaten of andere gebruikelijke en bekende dragersto’ffen en/of hulpstoffen voor geneesmiddelen zijn alle geschikt.
15 De uitvinding betreft voorts een keuze van passende doseringen van de verbindingen welke doseringen zo kunnen worden toegediend, dat de gewenste activiteit wordt bereikt zonder gelijktijdig neveneffecten te veroorzaken.
Volgens de uitvinding wordt het diureticum gebruikt in hoeveel- 20 heden die .gewoonlijk worden toegepast bij het .behandelen van te hoge bloeddruk b.v. 0,5 - 2000 mg/dag afhankelijk van het gebruikte diureticum, terwijl P 113^ bij voorkeur wordt toegepast in dagelijkse hoeveelheden tussen 10 en 200 mg.
Bij een humane therapie kunnen de componenten passend aan vol- 25 wassenen worden toegediend in doseringseenheden van niet minder dan 5 mg en ten hoogste 50 mg, liefst 10 - 25 mg P 113^,. hetzij als zodanig of in de vorm van een zout, berekend op de vrije vorm en van 0,1 - 1000 mg aan diureticum, waarbij de hoeveelheid van het specifieke diureticum afliangfc.
30 Met de uitdrukking "doseringseenheid1' wordt een enkelvoudige do sis bedoeld,, die aan een patiënt kan worden toegediend en gemakkelijk kan worden gehanteerd en verpakt, fysisch stabiel is en hetzij het actieve materiaal als zodanig of gemengd met vaste, halfvaste of vloeibare dragers en/of hulpstoffen bevat.
35 In de vorm van doseringseenheden kan het preparaat een- of meer- 8201493 •5- ' * * malen, daags met passende intervallen .worden toegediend,, steeds afhankelijk evenwel van de toestand van de patient en overeenkomstig het voorschrift van een arts..
Een dagelijkse dosering van het onderhavige preparaat omvat 5 meestal 0,15 - 3 mg/kg lichaamsgewicht P 113^ en 0,01 - 30 mg/kg lichaamsgewicht aan diureticum afhankelijk van het specifieke gebruikte diureticum: zo zal een volwassen menselijke patient gewoonlijk per dag 10 - 200. mg P 113^ en 0,5 - 2000 mg diureticum krijgen.
Bij een continue therapie kunnen tabletten of capsules als pas-10 sende farmaceutische vorm worden gebruikt. Het langdurig aanhoudende effect van P 113^ kan verder worden ondersteund door toepassing van preparaten met vertraagde afgifte.
De uitvinding omvat tevens de behandeling van zoogdieren met inbegrip van mensen die lijden aan te hoge bloeddruk en verwante situa— 15 ties met een preparaat volgens de uitvinding in therapeutisch effectieve doses. Er wordt hierbij evenwel op gewezen dat bij een dergelijke behandeling het diureticum en de vaatverwijder P 113^ of een zout daarvan ofwel tezamen^dan wel gescheiden, gelijktijdig of met intervallen kunnen worden toegediend en wel in passende farmaceutische vormen, 20 liefst in de bovenbeschreven doses.
De uitvinding wordt aan de hand. van de volgende voorbeelden nader toegelicht, welke niet als een beperking moeten worden opgevat.
Voorbeeld I
Snel uiteenvallend tablet 25 P H3i monohydraat 100 g bendroflumethiazide 25 g lactose 1000 g maïszetmeel 639 g

methylcellulose (waterige h%fs oplossing) UOO car 30 magnesiumstearaat 20 g
Het P 1 monohydraat, bendroflumethiazide, lactose, en het maïszetmeel werden in een menger gemengd, het verkregen poeder met de methylcellulose-oplossing tot granulair bevochtigd. Vervolgens werd gedroogd in een fluïde-beddroger en wel bij 50°C, waarna het gedroogde 35 granulaat door een zeef van 0,7 mm maaswijdte werd. gewerkt en dit granu— 82 0 1 4 9 3 3 - 6 - - >* laat met magnesiumstearaat in een tuimelmenger werd gemengd.
Van dit materiaal werden 10.000 tabletten met een gemiddeld gewicht. van 0,218- g geslagen onder toepassing van ronde stempels met een diameter van 8 mm.· Elk uiteenvallend tablet bevatte 10 mg P 113^ mono— * 5 hydraat en 2,5 mg bendroflumethiazide.
Voorbeeld II
Langzaam uiteenvallend tablet P 113^· monohydraat 200 g bendroflumethiazide 25 g 10 lactose. 1^35 g stearol (geatomiseerd) V' ITO g magnesiumstearaat 20 g
Het P 113^ monohydraat, bendroflumethiazide, lactose en verkleinde stearol werden in een snel lopende menger gemengd, het verkregen poe-15 der· in een rolinrichting tot slugs verwerkt en deze slugs tot granules gemalen, die door een zeef van 0,7 mm konden passeren. Dit granulaat werd met magnesiumstearaat gemengd en daarvan 10.000 tabletten met een gemiddeld gewicht van I85 mg geslagen onder toepassing van ronde stempels met een diameter van 8 mm.
20 Elk langzaam uiteenvallend tablet bevatte 20 mg P 113^ monohy— draat en 2,5 mg bendroflumethiazide.

Langzaam uiteenvallende laag: P 113^ monohydraat 150 g lactose 1050 g glycerylmonostearaat, verkleind 200 g 35 magnesiumstearaat 150 g 8201493 - τ -
Het b en droflumethi azide, de watervrije lactose, sta-Bx 1500, colloidaalsiliciumdioxyde en magnesiumstearaat werden gezeefd door mazen van 0,7 ma en 15 minuten in een tuimelmenger gemengde P 113^ monohydraat, lactose en. glycerylmonostearaat werden 15 5 minuten in een tuimelmenger gemengd en in een rolinrichting tot slugs verwerkt, vervolgens tot granules van ten hoogste 0,T mm gemalen en dit sluggranulaat met magnesiumstearaat gemengd in een tuimelmenger.
Vervolgens werden 10.000 tabletten geslagen met een rond stempel met een diameter van 8 mm waarbij het beoogde gewicht 155 mg bij 10 de langzaam uiteenvallende laag en 50 mg bij de snel uit een vallende laag bedroeg.
Elke tweelaagstablet bevatte 2,5 mg bendroflumethiazide in de snel uiteenvallende laag en 15 mg P 113^ monohydraat in de langzaam uiteenvallende laag*

Snel uiteenvallende tablet P 113^· monohydraat 100 g bumetanide 2,5 g lactose 1000 g 20 maïszetmeel 639 g methylcellulose als k%*s waterige oplos-

sing 1*00 cm magnesiumstearaat 20 g P 113^ monohydraat, bumetanide, lactose en maïszetmeel werden 25 in een snellopende menger gemengd. Het poeder werd bevochtigd met de methylcellulose-oplossing tot granulair. Vervolgens werd met een vloeiend bed-droger bij 50°C gedroogd en het gedroogde granulaat door een zeef van 0,7 mm gewerkt,, waarna het granulaat met magnesiumstearaat in een tuimelmenger werd gemengd. Van dit materiaal werden 10.000 ta— 30 bletten geslagen met een beoogd gemiddeld gewicht van 0,2l6 g onder toepassing van stempels met een diameter van 8 mm.
Elk uiteenvallend tablet bevatte 10 mg P 113^ monohydraat en 0,25 mg bumetanide.
Voorbeeld V
35 Langzaam uiteenvallende tablet 8201493 * w . ' -8 - P 113¾ monohydraat 200 g furosead.de ^00 g lactose 1^-35 g stearol (geatomiseerd) 170’g 5 magnesiumstearaat 20 g P 113¾ monohydraat, furosead.de, lactose en geatomiseerde stearol werden in een snellopende aienger gemengd, dit poeder tot slugs verwerkt en deze slugs tot granules gemalen die een zeef van 0,7 nm konden passeren. Het verkregen granulaat werd met magnesiumstearaat gemengd en 10 van dit geheel. 10.000 tabletten met een beoogd gewicht van 222 mg geslagen onder toepassing van vlakke ronde stempels met een diameter van 8 mm.
Elk langzaam uiteenvallend tablet bevatte 20 mg P 113¾ monohydraat en ^ mg furosemide.

Claims (11)
1. Farmaceutisch preparaat met daarin een mengsel van N"-cyaan-H-^-pyri dyl-H'-1,2,2~trimethylpropylguanidine en een diureticum, waarbij de twee actieve componenten als zodanig of in de vorm van niet giftige zouten daarvan worden toegepast.
2. Preparaat volgens conclusie 1, met het kenmerk, dat'het diureti cum een thiazide of thiazideachtig diureticum is.
3. Preparaat volgens conclusie 1, waarbij het diureticum een tubelus-diureticüm is. Preparaat volgens conclusie 1, met het kenmerk, dat het diureti— 10 cum bendroflumethiazide is. 5» Preparaat volgens conclusie 1, met het kenmerk, dat het diureti— cum hydroflumethiazide is.
6. Preparaat volgens conclusie 1, met het kenmerk, dat het diureti— cum. bumetamide is. 15 7· Preparaat volgens conclusie 1, met het kenmerk, dat het diureti cum furosemide is.
8. Preparaat volgens conclusies 1-7, in de vorm van doserings— eenheden.
9· Farmaceutisch preparaat volgens conclusie 8 voor het behandelen 20 van patiënten met te hoge bloeddruk of congestief hartbezwaar, met het kenmerk, dat dit een actief bestanddeel van niet minder· dan 5 mg en ten hoogste 50 mg Iï"-cyaan-ïf-^-pyridyl-Ii * -1,2,2-trimethylpropylguanidine, als zodanig of in de vorm van een niet giftig- zout daarvan en als verder actief bestanddeel 0,1 - 1000 mg van een diureticum bevat, waarbij 25 de hoeveelheid van het diureticum afhangt van het specifieke gebruikte diureticum, beide tezamen met farmaceutisch geschikte niet giftige dragers en/of hulpstoffen.
10. Farmaceutisch preparaat volgens conclusie 9 in de vorm van een tablet, pil, capsule of vouwpoeder voor orale behandeling van patiënten 30 die lijden aan te hoge bloeddruk of congestief hartbezwaar.
11. Werkwijze voor het behandelen van zoogdieren met inbegrip van de mens, die lijden aan te hoge bloeddruk of congestief hartbezwaar, * 8201493 V : - ίο - met het kenmerk, dat men aan de patiënt een effectieve hoeveelheid van ee preparaat volgens· conclusie 1 toedient.
12. Werkwijze volgens conclusie 11, met het kenmerk, dat men het preparaat in een doseringseenheid toepast volgens conclusies 8 - 10.

Werkwijze voor het bereiden van een preparaat volgens conclusie 1, met het kenmerk, dat men N ,r-cy aan-W-l-pyr idyl-Nf-1,2,2-trimethy 1-propylguanidine als zodanig of als niet giftig zout mengt met een diure-ticum als gespecificeerd in conclusie 1 in de hoeveelheden als gespecificeerd 'in conclusie 9, alsmede met farmaceutisch geschikte niet gifti-10 ge dragers en/of hulpstoffen. lU. Werkwijze voor het behandelen van te hoge bloeddruk of congestie? hartbezwaar bij zoogdieren met inbegrip van de mens, met het kenmerk,, dat men gelijktijdig aan een lijdende patiënt een therapeutisch geschikte hoeveelheid W’-cyaan-W- py r idy 1-Nf-1,2,2-trimethylpropy 1 gua-15 nidine en een therapeutisch geschikte hoeveelheid van een diureticum volgens conclusie 1 toedient.
