Titel : Aminozuusoplossingen voor kankerpatiënten.
De uitvinding heeft betrekking op aminozuuroplossingen voor kankerpatiënten.
Vanneer het bijvoorbeeld onmogelijk is om een patiënt na een operatie oraal of intestinaal te voeden wordt de patiënt gewoonlijk 5 met parenterale of niet-intestinale voeding gevoed om de lichamelijke gezondheid te handhaven en te verbeteren. Hiervoor zijn aminozuurop-lossingen met verschillende bestanddelen bekend die algemeen worden toegepast.
Dergelijke bekende aminozuuroplossingen, die slechts geschikt 10 zijn voor parenterale voeding, zijn in wezen sterk gelijkend wat betreft de aminozuursamenstelling, en bootsen moedermelk, kippeëie-ren, albumine uit menselijk bloedserum, enzovoort, na die de voor het menselijk lichaam essentiële amonizuurvoedingsstoffen bevatten.
De oplossingen bevatten derhalve alle acht essentiële aminozuren ais 15 onmisbare bestanddelen.
Tijdens het onderzoek van aminozuuroplossingen die in het bijzonder nuttig zijn voor kankerpatiënten, heeft men nu verrassender-wijze gevonden dat de bekende aminozuuroplossingen, wanneer deze geen methionine bevatten, het aantal kankercellen verminderen of on-20 derdrukken terwijl de voedende werkzaamheid van de in de oplossingen aanwezige aminozuren blijft gehandhaafd.
De uitvinding beoogt derhalve aminozuuroplossingen te verschaffen die de vorming van kajfereellen voorkomen of verminderen en tevens de voeding verschaffen.
25 De uitvinding beoogt eveneens aminozuuroplossingen te verschaf fen die tevens kunnen worden toegepast met bekende kader werende middelen om de werking van de middelen aanzienlijk te verbeteren.
De uitvinding zal nader worden toegelicht aan de hand van de volgende beschrijving.

Men verkrijgt de werkzaamheid van de aminozuuroplossingen van d ï onderhavige uitvinding vergeleken met de bekende aminozuuroplossingei en de parenterale therapie met toepassing van dergelijke oplossingen, op volkomen onverwachte wijze en met name de vermindering van het 5 aantal kankercellen of de kankerwerende werking en het bijzondere neven-effect van een langere levensduur waarbij het in stand houden en de verbetering van de fysieke gezondheid met de door de oplossing gevormde voeding in wezen wordt gewaarborgd, 33e oplossingen volgens de onderhavige uitvinding vergroten de werkzaamheid van het middel -jO aanzienlijk wanneer zij worden toegepast in combinatie vmet een bekend kankerwerend middel zoals mitomycine-C, 5-fluor/ar?°^neocarzino·· statine of bischloorethylnitrosoureum, De aminozuuroplossingen volgens de uitvinding verminderen of voorkomen derhalve kankercellen, terwijl zij geen ongewenst neveneffect bezitten. De uitvinding ver-15 schaft derhalve tevens een bruikbare werkwijze om kanker te behandelen. De bijzondere werkzaamheid van de aminozuuroplossingen volgens de uitvinding ..is·, ten opzichte van de bekende aminozuuroplossingen volkomen onverwachts, omdat bij toedienen van de oplossing aan de kankerpatiënt deze niet alleen dient om zijn fysieke sterkte met de 20 voeding in stand te houden, doch tegelijkertijd ook de voedingsstoffen aan de kankercellen in het levende lichaam worden geleverd om de cellen te laten groeien. De parenterale voeding vertraagt alleen de vermindering van het lichaamsgewicht van de patiënt, doch heft de symptomen niet op of doet het lichaamsgewicht in geen enkele mate 25 toenemen. De oplossing volgens de uitvinding, die geen methionine bevat, werkt daarentegen zodanig dat de groei van de kankercellen in het levende lichaam op selectieve wijze wordt verhinderd, hetgeen leidt tot een toename van het lichaamsgewicht en onderdrukking van de symptomen.
50 De aminozuuroplossingen volgens de uitvinding kunnen voor wat be treft de samenstelling gelijk zijn aan de hierboven genoemde bekende aminozuuroplossingen, behalve dat de aanwezige oplossingen geen me-.· thionine bevatten, De samenstelling van de onderhavige oplossingen is desgewenst vast te stellen afhankelijk van de toedieningsvijze, symp-, 35 tcasen van de patiënt, toedieningsduur, enzovoort. Be aminozuren worden gewoonlijk in vrije vorm toegepast doch dit vormt geen beperking; zij zijn eveneens bruikbaar in de vorm van farmacologisch aanvaardbare zouten met zuren' aöals zoutzuur, azijnzuur en dergelijke.
Be aminozuuroplossingen volgens de uitvinding kunnen naast de hierboven genoemde aminozuren, verschillende toevoegsels bevatten ^ die gewoonlijk worden toegepa3t voor dergelijke oplossingen, zoals koolhydraten, vitaminen, vetten, elektrolyten, antiseptische middelen, stabiliseringsmiddelen, enzovoort. Voorbeelden van dergelijke toevoegsels zijn zetmeel, saccharose, glucose, maltose, fructose, sorbitol, xylitol, ascorbinezuur, ,nicotinezuur, pantothsaizuur, thiamine, riboflavine, pyridoxine, menadion, biotine, folzuur, vitamine inositol, natriumbisulfiet, fosforzuur, en dergelijke.
Be aminozuuroplossingen volgens de uitvinding zijn evenals de bekende aminozuurpreparaten gewoonlijk steriele waterige oplossinger en worden als infusie-oplossingen op parenterale of niet-intestina-^ le wijze gegeven, bijvoorbeeld in de vorm van intraveneuze inspuitingen en druppels. Be oplossingen kunnen zowel oraal als enteraal wórden toegediend. Be dosis kan op geschikte wijze worden bepaald, afhankelijk van de symptomen van de patiënt, enzovoort. Tan de patiënt. De oplossingen worden gewoonlijk in een dagelijkse dosis van ongeveer 100 tot ongeveer 1500 ml, bij voorkeur ongeveer 500 tot ongeveer 1500 ml, gegeven.
Het kankerwerende middel dat tezamen met de aninozuuropiossing 5 van de uitvinding wordt toegepast, wordt gegeven in een dosis die in het algemeen wordt aangepast hoewel de dosis op geschikte wijze kan worden bepaald afhankelijk van de aard van het middel, de ioort kanker, toedieningswijze, enzovoort. Verschillende kankerwerende middelen kunnen alleen of in mengsels worden toegepast. Wanneer bijvonr-10 beeld S-fluGsr/^2·-alleen aan een patiënt met maagkanker wordt toegediend, tezamen met de onderhavige oplossing, wordt het middel druppelsgewijze intraveneus toegediend in een dosis van 15 mg/kg/dag gedurende 5 achtereenvolgende dagen en daarna met een dosis van 7,5 mg/kg/dag om de andere dag, of wordt intra-arteriaal toegediend in 15 [ een dosis van 5 tot 10 mg/kg/dag gedurende 10 tot 20 dagen. Mitomyci-ne-C wordt, wanneer het tezamen met de onderhavige oplossing voor eet patiënt met maagkanker wordt toegepast, intraveneus één of twee keer per week gegeven in'een dosis van 8 tot 10 mg/kg iedere keer.
De oplossingen volgens de uitvinding zijn bruikbaar voor patiën-20 ten met bijvoorbeeld maagkanker, oarcinoom van de karteldarm, kanker van de endeldarm carcinoom van de esofagus, kanker van de pancreas, colangioOm, leverkanker en verschillende andere vormen van kanker.
De uitvinding zal nader toegelicht worden aan de hand van de voorbeelden, waarin de waarden de hoeveelheden aangegeven van de ami-25 nozuren die in de vorm van zouten zijn toegepast en berekend zijn als vrije aminozuren.

Hierna worden proefnemingen en klinische proeven beschreven die zijn uitgevoerd onder toepassing van aminozuurinfusieoplossingen 20 volgens de onderhavige tri. tvinding.
EXPEPIHSNTEBI 700P3EBLD
De proefdieren zijn Donryu-ratten en Wlstar ratten met een gewicht van 150 tot 160 g. Ter vorming van kanker "bezittende dieren wordt een catheter volgens de volgende werkwijze in elk dier aange-25 bracht om zonder "beperkingen een continue infusie uit te voeren, gevolgd door èen acclimatisatie periode van drie dagen.
Onder narcose met ïïembutal snijdt men de huid van de hals van , de rat in om de jugulumader tot de vertakking met ,de sleutelbeena- •der"bloot te leggen. Men brengt een katheter in de jugulumader tot 50 deze het voorste uiteinde de bovenste holle ader bereikt en beves- ,/· tigi dan de katheter. Het andere uiteinde van de katheter wordt onder de huid geleid en in een ruggedeelte uit het lichaam getrokken. Men verkrijgt na afloop van de acclimatisatieperiode kankerbe- •j £ zittende dieren door 2,7x10' cellen/ml Yoshida sarcoom en 9 2 10 η cellen/ml AE 130 in Donryu ratten over te brengen, en 1 x 10 50 cellen/ml Walker sarcoom en Ehodamine sarcoom in een hoeveelheid van 0,2 ml als kankercellensuspensie in Wistar ratten. Be kankercellen brengt men in onder de huid in de linkerlies van de dieren. Zoals in Tabel A op blz. 10 is weergegeven bezitten de ratten kankercellen van dezelfde soort en die ratten die geen kankercellen bezitten 7906620 — 10- £ * zijn in twaalf groepen verdeeld: namelijk de groepen A-1 en Δ-4 waaraan alleen de aminozuurinfusieoplossing volgens de uitvinding (bereid in Yoorbeeld I, hierna aangeduid als A0-30), de groepen A-2 en A-5 waaraan zowel A0-30 als Mitomycine-C (hierna aangeduid als "MMC") en de groepen A-3 en A-é waaraan A0-30, MMC en een dieet, en als controlegroepen de graven P-1 en P-4 waaraan alleen PA, de groepen P-2 en P-5 waaraan zowel PA als MMC en de groepen P-3 en P-6 waaraan PA, MMC tezamen met een dieet is gegeven. Elk van de groepen bestaat uit 6 ratten. A0-30 en PA zijn continue met de snelheid van 1,5 ml/uur q toegediend. MMC is intraperitoneaal toegediend met een dosis van 0,1 mg/kg/dag, waarmee 24 uren na het inbrengen van de kankercellen is begonnen. Het dieet is het eiwitrrije dieet dat hierboven is vermeld.
In Tabel A duidt ”+n aan dat in de ratten kankercellen zijn aangebracht, of MMC of een dieet is toegediend. Het teken M-H geeft dat tj MMC of een dieet niet is ingebracht of toegediend.

Het Toshida sarcoom en AH 130 worden 12 dagen na het inbrengen uit de ratten verwijderd, en het Walker sarcoom en Hhodamine sarcoom op de 15e dag, en worden de kankercellen gewogen. De tabellen B tot 2 geven de resultaten weer. Het gemiddelde gewicht van de kankercellen 5 C in de kankercellen bezittende groep P-4 die alleen PA heeft ontvan- * gen heeft men berekend. Het gemiddelde gewicht van de kankercellen D in elk van de kankercellen bezittende groepen P-5» en A-4 tot A-6 heeft men eveneens berekend. De tabellen B tot en met E geven de verkregen Τ/C verhouding. De verkregen resultaten zijn als doeltref-10 fend beoordeeld wanneer de D/C verhouding kleiner is dan 0,50, matig doeltreffend Wanneer de verhouding 0,50 tot 0,70, en ondoelmatig wanneer de verhouding groter is dan 0,70.
Het gemiddelde gewicht van de kankercellen dat in de tabellen 3 tot en met E is weergegeven is het gemiddelde van de maximale en 15 minimale gewichten van de kankercellen in de 6 ratten van de betreffende groep. De waarde met is het maximale gewicht en de waarde met is het minimale gewicht.

Tnrmn-—- — 12 - I leidt tot een vermindering van het gewicht van de kankercellen van tenminste ongeveer eenderde van het gewicht van de kankercellen van de groep die alleen het bekende PA ontvangt,- en aldus toont dat A0-30 een kankerrreamende werking bezit. Yerder is de T/C verhouding 5 van de groep A-4, die gelijk is aan 0,é4» vergelijkbaar of beter dan die welke met de toediening van MMC (groq? P-5) en het gebruik van zowel MMC en het dieet (P-6) wordt bereikt. Groep A-5 met A0-30 en MMC, en groep A-6 met A0-30, MMC en dieet vertonen doeltreffende resultaten van 0,42 voor de T/C verhouding. Pit toont aan dat het ge-10 zamelijke gebruik van A0-30 en MMC de door de enkele toepassing hiervan bereikte werking verbetert.

Tabel C toont eveneens dat het gebruik van A0-30 (groep A-4) 30 leidt tot een vermindering van het gewicht zonder kankercellen, welke vermindering overeenkomt met ongeveer 1/3 van het gewicht van de kankercellen onder toepassing PA (groep P-4) en toont derhalve dat A0-30 zelfs wanneer het alleen wordt toegepast doelmatig is om de vorming van kankercellen te verhinderen. Pe tabel toont verder dat 35 AO-30 wanneer het tezamen met MMC of met MMC en een dieet wordt toe 7906620 - 13 - gepast, doelmatiger resultaten verschaft.

Be bovenstaande tabellen tonen dat de aminozuurinfusie-op-lossing volgens de uitvinding, anders dan de bekende aminozuuinfusie-oplossingen die methionine bevatten een uitstekende remmende werking bezitten ten opzichte van verschillende kankercellen.
2. Proef ter vergroting van de levensduur.
10 Men verkrijgt kankercellen-bezittende dieren door op intraperi- toneale wijze 1,7 x 107 cellen/ml Yoshida sarcoom of 1,1 x 10 cellen/ ml AH 150 aan Bonryu ratten, en 1,0 x 10** cellen/ml Walker sarcoom aan Wistar ratten toe te dienen. In deze proef dient men een infusie-oplossing van Ao-30 (of PA voor de controlegroepen), glucose, elektro-1 5 lyten en vitaminen toe volgens de zogenoemde totale parenterale voe- dingswerkwijze (hierna aangeduid als '’TPfT-werkwijze). Be samenstelling van de infusie oplossing is:

Zoals in onderstaande Tabel F is weergegeven zijn de ratten met dezelfde kankercellen in vier groepen van elk 10 ratten verdeeld: 5 groep 1-1* waaraan A0-30 en groep A-2’ waaraan zowel A0-30 als MMC, en als controlegroepen, de groep P-1* waaraan PA en groep P-21 waaraan zowel PA als MMC is toegediend. MMC is op intraperitoneale wijze met een dosis van 0,1 mg/kg/dag tcegediend.
In Tabel F duidt het teken ”+" de groep met de inenting of 10 waaraan MMC is toegediend aan, en het teken de groep zonder inenting of MMC.

Men neemt de proefdieren waar om de levensduur gedurende 50 dagen na de inenting te bepalen. De invloed op de vergroting van de overlevingsduur wordt bepaald op basis van de τ/C verhouding, dat wil zeggen de verhouding van de gemiddelde overlevingsduur C van groep P-11 die PA als een aminozuurinfusie-oplossing ontvangt ten opzichte van de gemiddelde „overlevingsduur T van elk van de andere kankercellen bezittende 7906620 -16- groepen. Het resultaat is doeltreffend wanneer de Τ/C verhouding groter is dan 1,25, doch ondoelmatig wanneer de verhouding kleiner is dan 1,25, Tabel G geeft de resultaten weer die zijn verkregen net de kankercellen.

Tahel G toont dat de infusie-oplossing volgens de uitvinding ^5 die voor elk van de kankercellen is .toegepast de levensduur die wordt verschaft door de PA infusie-oplossing met een factor 1,3 tot 1,5 vergroot. Be onderhavige oplossing vergroot bovendien de toename van de levensduur die wordt verkrgen met MC wanneer deze oplossing tezamen MC wordt toegepast (groep A-2’)· 30 . Hoewel dit niet in Tabel G is weergegeven verkrijgen de groepen P-2* en A-2r die met AH 130 zijn ingeënt 30 dagen na het inenten overlevingskansen van 40 % respektievelijk 60 Be groep P-2' met hst Walker sarcoom bezit een overlevingskans van 20 % terwijl de groep A-2' met dezelfde kankercellen een overlevingskans van 50 25 .bezit.

De hierboven beschreven resultaten tonen dat de pminczvxLrinïu-sieoplossing volgens de uitvinding, die vrij is van methionine, wanneer deze alleen gedurende enige tijd continn i3 toegediend, zeer doelmatig werkt ten opzichte van het Yoshida sarcoom, Walker sarcoom, AH 130 en Thodamine sarcoom om de groei van de kankercellen te verminderen en'dat de infusie-oplossing grotere werkzaamheid blijft vertonen wanneer deze tezamen met een kankerwerend middel zoals MC wordt toegepast. De infusieoplossing volgens de uitvinding verschaft daarnaast, wanneer deze continueals een enkele stifkstofbron wordt toege-) diend volgens de THiT-werkvijze een opmerkelijke toename in de levensduur van de proefdieren die met het Yoshida sarcoom, AH tJO en Walker sarcoom zijn ingeënt terwijl de werking van MC wanneer dit hiermee wordt toegepast vergroot.
Slinisch voorbeeld I
5 De patiënt was een man van 54 jaar die 2 jaren na een gehele verwijdering van de maag leed aan kankerhuikvliesontsteking. De patiënt onderging nogmaals cellotomie, die, omdat het een grote buikoperatie betrof, niettemin is gestaakt zonder enige herstelprocedure.
De patiënt klaagde over ernstig braken door carcinomateuse verstopr 0 ping van de ingewanden en was ernstig verzwakt. lien diende de patiënt de in Toonbeeld I volgens de uitvinding bereide aminczuurinfusie-cp-_ lossing A0-50 dagelijks toe volgens de THT-werkwijze met een dosis van 500 ml/dag gedurende 4 weken. Daanaast kreeg de patiënt MC, 5-fluóruracil (hierna aangeduid als 53TT”) en neoearzinostatine (hierna 5 aangeduid als'^TCS"), afzonderlijk op de volgende wijze. MC diende men intraveneus eenmaal per dag, gedurende de eerste vier dagen en gedurende 4 dagen na een pauze van 2 weken in een dosis van 10 mg/cm^ kankercellenoppervlak toe. 57ÏÏ diende men eenmaal per dag gedurende de éérste 5- dagen en gedsrende 5 dagen ha een onderbreking van·12 we- T? 10 ken'intraveneus - toe” in een ^hoeveelheid van 2GQ0 eenheden eenmaal per dag gedurende de periode van 4 weken. 4 weken na het begin van de infusie was de patiënt in staat voedsel te verteren, werd weer gezond en werd uit het ziekenhuis ontslagen.
Klinisch voorbeeld IT
55 De patiënt was een man van 39 jaar die weer in het ziekenhuis was opgenomen vanwege het verspreiden van kanker naar de lever, 7 zie biz. 13 — 18 — gepaard gaande met geelzucht, 1 jaar na de gehele verwijdering van de maag. Hij leed aan hepatische ader on t steking in de buikwand, buikwaterzucht en oedeem in de onderste helft van het lichaam en was in een kritieke toestand. De patiënt werd, evenals in klinisch voorbeeld 5 1, iftgespoten met de aminozuurinfusie-oplossing A0-30, bereid volgens voorbeeld 1, volgens de TPU-werkwijze en kreeg tevens MMC,55TJ en HOS. De oplossing en de kankerwerende middelen werden allen, in dezelfde hoeveelheden en op dezelfde wijze als in klinisch voorbeeld 1, toegediend, behalve dat de .aminozuuroplossing en ÏTCS dageli jkds gedurende *10 5 weken werden toegediend. 5 weken na het begin van de infusie verdwe nen het hepatisme en de aderontsteking, terwijl de andere symptomen waren verminderd, doch de patiënt overleed door inwendige bloedingen. Ha biopsie bleek dat de beschadigingen in de weefsels op uitgebreide schaal waren vernietigd.
15 ff in een hoeveelheid van 10 mg/kg lichaamsgewicht. HCS diende men intraveneus toe.

1. Aminozuuroplossing voor een kankerpatiënt met essentiële aminozuren met het kenmerk, dat de oplossing geen methionine "bevat dat één van de essentiële aminozuren is.
2.
