SISTEMA DE CIERRE ASISTIDO POR VACÍO CON PROVISIÓN DE CALENTAMIENTO Y ENFRIAMIENTO
CAMPO TÉCNICO
La presente invención se refiere al sanado de heridas. Más específicamente, la presente invención se refiere al cierre de las heridas asistido por vacío, en donde se utiliza calentamiento o enfriamiento localizados para acelerar o retardar la función metabólica del sistema inflamatorio con el fin de facilitar el sanado de la herida .
ANTECEDENTES DE LA INVENCIÓN
El cierre de la herida involucra la migración hacia adentro del tejido epitelial y subcutáneo adyacente a la herida. Esta migración es ordinariamente ayudada a través del proceso inflamatorio, mediante el cual el flujo sanguíneo es incrementado y son activados diversos tipos de células funcionales. A través del proceso inflamatorio, el flujo sanguíneo a través de los vasos dañados o rotos es detenido por la oclusión al nivel capilar, después de lo cual pueden comenzar las operaciones de limpieza y de reconstrucción. Hasta fechas recientes, tales heridas difíciles eran enfrentadas únicamente a través del uso de suturas o grapas. Aunque todavía ampliamente practicadas y frecuentemente efectivas, tales técnicas de cierre mecánico sufren de una desventaja principal, ya que éstas producen tensión sobre el tejido de la piel adyacente a la herida. Las heridas no reparables mediante sutura o engrapado requieren en general hospitalización prolongada, con su alto costo inherente, y procedimientos quirúrgicos mayores, tales como injertos de los tejidos circunvecinos. Los ejemplos de heridas no fácilmente tratables con grapas o sutura incluyen heridas abiertas, profundas, y grandes; las úlceras por decúbito; las úlceras resultantes de la osteomielitis crónica; y las quemaduras de espesor parcial que subsecuentemente se desarrollan en quemaduras de espesor completo. Como resultado de estos y otros inconvenientes de los dispositivos de cierre mecánico, han sido desarrollados métodos y aparatos para drenar las heridas mediante la aplicación de presiones negativas continuas. Cuando se aplican sobre un área suficiente de la herida, tales presiones negativas se he encontrado que promueven la migración hacia la herida de tejidos epiteliales y subcutáneos. En la práctica, la aplicación de presión negativa a una herida, comúnmente denominada como la terapia de cierre ayudado por vacío (VAC) , involucra típicamente la contracción parecida a la mecánica, de la herida con eliminación simultánea del fluido en exceso. De esta manera, la terapia VAC aumenta el proceso inflamatorio natural del cuerpo mientras que alivia muchos de los efectos colaterales intrínsecos conocidos, tal como la producción de edema provocado por ausencia incrementada del flujo sanguíneo, la estructura vascular es necesaria para retorno venoso adecuado. Mientras que la terapia VAC ha sido altamente exitosa en la promoción del cierre de la herida, el sanado de muchas heridas que previamente se pensaba que eran intratables en gran medida, permanece sin una dificultad. Debido a que el proceso inflamatorio es muy único para el paciente individual, incluso la adición de terapia VAC no da como resultado una respuesta lo suficientemente rápida para el cierre de algunas heridas, especialmente cuando se aplican durante la oclusión y las etapas iniciales de limpieza y reconstrucción. Es por lo tanto un objetivo principal de la presente invención el proporcionar un método y aparato mediante el cual se mejoren las modalidades de terapia VAC conocidas a través de la aceleración controlada de la respuesta inflamatoria. Además, y nuevamente al menos parcialmente atribuible a la variación entre pacientes, es posible que una respuesta inflamatoria iniciada adecuadamente pueda ser tomada demasiado rápido, dando como resultado edema y dolor. Por lo tanto, otro objetivo principal de la presente invención es proporcionar un método y un aparato mediante el cual se mejoren las modalidades de terapia VAC conocidas a través del retardo controlado de la respuesta inflamatoria.
DESCRIPCIÓN DE LA INVENCIÓN
De acuerdo con los objetivos anteriores, la presente invención - un método y un aparato para la aceleración controlada y/o el retardo de la respuesta inflamatoria del cuerpo - comprende en general una almohadilla de espuma para la inserción sustancialmente dentro de un sitio de herida, calentando y enfriando la almohadilla para la aplicación sobre el sitio de la herida, y una venda o esparadrapo para herida para sellar el alojamiento o encerramiento de la almohadilla de espuma, y el calentamiento y el enfriamiento de la almohadilla en el sitio de la herida. De acuerdo a la invención, la almohadilla de espuma es colocada en comunicación fluida con una fuente de vacío para la promoción del drenado del fluido mientras el fluido caliente o frío se hace circular a través de la almohadilla de enfriamiento y calentamiento para la aceleración o retardo controlados, respectivamente, de la porción de función metabólica de la respuesta inflamatoria del cuerpo.
De acuerdo a la modalidad preferida de la presente invención, se agrega una provisión de calentamiento y enfriamiento a la terapia VAC previamente conocida para controlar la función metabólica local como parte de la respuesta inflamatoria. Mediante la provisión de calentamiento localizado en combinación con la terapia VAC de otro modo ordinaria, la respuesta inflamatoria completa puede ser sinérgicamente acelerada para producir oclusión capilar rápida e inicio temprano de las etapas de limpieza y reconstrucción. De igual modo, en el caso en que el médico que atiende, determine que la respuesta inflamatoria ha sido sobreactivada, puede ser proporcionado enfriamiento localizado en combinación con terapia VAC para retardar la respuesta inflamatoria del cuerpo sin sacrificio del control del edema y otros aspectos de la terapia VAC de otro modo proporcionada. En la modalidad preferida de la presente invención, la almohadilla de enfriamiento y calentamiento comprende una capa acuosa flexible y respirable, que comprende en general dos hojas de material soldable por RF. Las dos hojas de la almohadilla son soldadas por RF conjuntamente en un patrón en forma de waffle, en donde una pluralidad de aberturas es formada entre una pluralidad de canales. Las aberturas permiten la transpiración de la humedad desde la piel del paciente mientras que los canales permiten la circulación, por medio de un tubo de suministro y un tubo de drenado, de agua caliente o fría, como es requerido, a través de la almohadilla para el calentamiento o enfriamiento de la misma. Mientras que la almohadilla de calentamiento y enfriamiento puede ser colocada dentro o fuera del esparadrapo para herida durante el aspecto de sanado de la presente invención, es crítico que la almohadilla de calentamiento y enfriamiento sea colocada dentro del esparadrapo de la herida durante el aspecto de enfriamiento de la presente invención. De esta manera, la formación del condensado sobre el interior del esparadrapo, que puede provocar que el adhesivo del esparadrapo se suelte y dé como resultado al final pérdida de vacío en el sitio de la herida, puede ser minimizada. En particular, la colocación de la almohadilla de calentamiento y enfriamiento dentro del esparadrapo para herida limita el contenido de humedad circunvecino a aquel existente y generado dentro de los confines del sitio de la herida, el cual es minimizado por el aspecto de succión de la terapia VAC. Debido a que el aspecto de enfriamiento de la presente invención debe ser implementado de esta manera y el médico debe indicar la necesidad para el enfriamiento a cualquier tiempo después del inicio de la terapia VAC, el método preferido de la presente invención comprende la colocación de la almohadilla de calentamiento y enfriamiento por debajo del esparadrapo para herida, adyacente a la almohadilla de espuma y al sitio de la herida, no obstante de si el calentamiento o el enfriamiento es inicialmente indicado. Después de la colocación de la almohadilla, el esparadrapo para herida es firmemente adherido alrededor del tubo de suministro y del tubo de drenado para prevenir la fuga de vacío. Finalmente, muchas otras características, objetivos y ventajas de la presente invención serán aparentes para aquellos de experiencia ordinaria en las técnicas relevantes, especialmente a la luz de las discusiones anteriores, los siguientes dibujos y la descripción detallada ejemplar, y las reivindicaciones anexas a la presente.
BREVE DESCRIPCIÓN DE LOS DIBUJOS

MEJOR MODALIDAD PARA LLEVAR A CABO LA INVENCIÓN
Aunque aquellos de experiencia ordinaria en la técnica reconocerán fácilmente muchas modalidades alternativas, especialmente a la luz de las ilustraciones proporcionadas en la presente, esta descripción detallada es ejemplar de la modalidad preferida de la presente invención - un sistema de cierre asistido por vacío, con provisión de calentamiento y enfriamiento, el alcance del cual está limitado únicamente por las reivindicaciones anexas a la presente . Con referencia ahora a las figuras, se muestra la presente invención 10 que comprende en general una almohadilla de espuma 11 para la inserción sustancialmente dentro de un sitio de herida 12, una almohadilla 13 de calentamiento y enfriamiento para la aplicación sobre el sitio de la herida 12 y un esparadrapo 14 para herida para sellar el alojamiento o encierro de la almohadilla de espuma 11 y la almohadilla de calentamiento y enfriamiento 13 en el sitio 12 de la herida. De acuerdo a la invención, la almohadilla 11 de espuma es colocada en comunicación fluida con una fuente de vacío para la promoción del drenado del fluido mientras que se hace circular fluido caliente o frío a través de la almohadilla 13 de calentamiento y enfriamiento para la aceleración o retardo controlados, respectivamente, de la porción de la función metabolica de la respuesta inflamatoria del cuerpo. De acuerdo a la modalidad preferida de la presente invención, la almohadilla 11 de espuma, el esparadrapo 14 de herida y la fuente de vacío son implementados como es conocido en la técnica anterior, cada uno de los cuales es detallado en la solicitud de patente de los Estados Unidos No. de Serie 08/517,901 presentada el 22 de Agosto de 1995. Por esta referencia, la descripción completa de la solicitud de patente de los Estados Unidos No. de Serie 08/517,901 ("la solicitud '901"), incluyendo las reivindicaciones y los dibujos, es incorporada en la presente como si se describiera en su totalidad. Además, tal sistema VAC es fácilmente comercialmente disponible de Kinetic Concepts Inc., de San Antonio, Texas, E.U.A. , y sus compañías subsidiarias.
Como se detalla en la solicitud '901, la almohadilla de espuma 11 comprende preferentemente una espuma de poliuretano o poliéter de celdas abiertas, altamente reticulada, para la buena permeabilidad de los fluidos de las heridas mientras está bajo succión. Como también se detalla en la solicitud '901, la almohadilla de espuma 11 es preferentemente colocada en comunicación fluida, por medio de una manguera 15 de material plástico o similar, con una fuente de vacío, la cual comprende preferentemente una lata colocada de manera segura a vacío a través de comunicación fluida, vía un filtro de membrana hidrofóbico interpuesto, con una bomba de vacío. Finalmente, la solicitud '901 también detalla el esparadrapo 14 para herida, el cual comprende preferentemente un material elastomérico al menos periféricamente cubierto con un adhesivo acrílico, sensible a la presión, para aplicación de sellado sobre el sitio 12 de la herida. De acuerdo al método preferido de la presente invención, aquellos componentes como los que se describen en la solicitud '901 son en general empleados como son conocidos en la técnica, con la excepción de que la provisión de calentamiento y enfriamiento de la presente invención es agregada para controlar la función metabólica local como parte de la respuesta inflamatoria. Mediante la provisión de calentamiento localizado en combinación con terapia VAC de otro modo ordinaria, la respuesta inflamatoria completa puede ser sinérgicamente acelerada para producir la oclusión capilar rápida y el inicio más temprano de las etapas de limpieza y reconstrucción. De igual modo, en el caso en que el médico que atiende determine que la respuesta inflamatoria ha sido sobreactivada, puede ser proporcionado enfriamiento localizado en combinación con la terapia VAC para retardar la respuesta inflamatoria del cuerpo sin sacrificio del control de edema y otros aspectos de la terapia VAC de otro modo proporcionada. En la modalidad preferida de la presente invención, la almohadilla 13 de calentamiento y enfriamiento comprende una capa acuosa 16 flexible y respirable, que comprende en general dos hojas 17, 18 de material soldable por RF. Las dos hojas 17, 18 de la almohadilla son soldadas por RF conjuntamente en un patrón en forma de waffle, en donde una pluralidad de aberturas 19 es formada entre una pluralidad de canales 20. Las aberturas 19 permiten la transpiración de la humedad a partir de la piel 21 del paciente, mientras que los canales 20 permiten la circulación, vía un tubo de suministro 22 y un tubo de drenado 23, de agua caliente o fría, como se requiera, a través de la almohadilla 13 para el calentamiento o enfriamiento de la misma. Mientras que la almohadilla 13 de calentamiento y enfriamiento puede ser colocada dentro o fuera del esparadrapo 14 para herida, durante el aspecto de calentamiento de la presente invención, es crítico que la almohadilla 13 de calentamiento y enfriamiento sea colocada dentro del esparadrapo 14 para herida durante el aspecto de enfriamiento de la presente invención. De esta manera, la formación del condensado sobre el interior y cerca de los bordes del esparadrapo 14, que pueden provocar que el adhesivo del esparadrapo se suelte y al final dé como resultado la pérdida del vacío en el sitio 12 de la herida, pueden ser minimizada. En particular, la colocación de la almohadilla 13 de calentamiento y enfriamiento dentro del esparadrapo 14 para herida limita el contenido de la humedad circunvecina a aquel nivel de humedad que existe y es generado dentro de los confines del sitio 12 de la herida, que es minimizado por el aspecto de succión de la terapia VAC. Debido a que el aspecto de enfriamiento de la presente invención debe ser implementado de esta manera y el médico puede indicar la necesidad para enfriar a cualquier tiempo después del inicio de la terapia VAC, el método preferido de la presente invención comprende la colocación de la almohadilla 13 de calentamiento y enfriamiento por debajo del esparadrapo 14 para herida, adyacente a la almohadilla de espuma 11 y al sitio 12 de la herida, no obstante de si es inicialmente indicado el calentamiento o el enfriamiento. Después de la colocación de la almohadilla 13, el esparadrapo 14 para herida es firmemente adherido alrededor del tubo de suministro 22 y el tubo de drenado 23 para prevenir la fuga de vacío. Mientras que la descripción anterior es ejemplar de la modalidad preferida de la presente invención, aquellos de experiencia ordinaria en las técnicas relevantes reconocerán las diversas variaciones, alteraciones, modificaciones, sustituciones y similares que son fácilmente posibles, especialmente a la luz de esta descripción, de los dibujos anexos y de las reivindicaciones anexas a la presente. Por ejemplo, aquellos de experiencia ordinaria en la técnica reconocerán que la almohadilla 13 de calentamiento y enfriamiento puede ser construida en una amplia variedad de formas, tamaños y estructuras internas. Tal modalidad alternativa puede comprender la integración de la almohadilla 13 de calentamiento y enfriamiento en una versión de capas múltiples del esparadrapo 14 para herida. En cualquier caso, debido a que el alcance de la presente invención es mucho más amplio que cualquier modalidad particular, la descripción detallada anterior no debe ser considerada como una limitación de la presente invención, la cual está limitada únicamente por las reivindicaciones anexas a la presente .
POSIBILIDAD DE APLICACIÓN INDUSTRIAL
La presente invención es aplicable a las técnicas de sanado de heridas .
Se hace constar que con relación a esta fecha, el mejor método conocido por la solicitante para llevar a la práctica la citada invención, es el que resulta claro de la presente descripción de la invención.
Claims (10)
1. Un método para promover el sanado de heridas en mamíferos, caracterizado el método porque comprende los pasos de : la aplicación de vacío a una región de un sitio de herida sobre un mamífero; y efectuar un cambio en la respuesta inflamatoria del mamífero en dicha región, mientras que es aplicado el vacío a ésta.
2. El método para promover el sanado de heridas de conformidad con la reivindicación 1, caracterizado porque la realización de un paso de cambio comprende el control de la función metabólica local del mamífero en dicha región.
3. El método para promover el sanado de heridas de conformidad con la reivindicación 2, caracterizado porque la realización de un paso de cambio comprende la aceleración de la función metabólica local del mamífero en dicha región, para promover la oclusión capilar rápida y el inicio acelerado de las etapas de limpieza y construcción de la respuesta inflamatoria del mamífero.
4. El método para promover el sanado de heridas de conformidad con la reivindicación 3, caracterizado porque la realización de un paso de cambio comprende el calentamiento de dicha región.
5. El método para promover el sanado de heridas de conformidad con la reivindicación 2, caracterizado porque la realización de un paso de cambio comprende el retardo de la función metabólica local del mamífero ya que dicha región puede prevenir la sobreactivación de la respuesta inflamatoria del mamífero.
6. El método para promover el sanado de heridas de conformidad con la reivindicación 5, caracterizado porque la realización de un paso de cambio comprende el enfriamiento de dicha región.
7. El método para promover el sanado de heridas de conformidad con cualquiera de las reivindicaciones precedentes, caracterizado porque la aplicación de un paso de vacío comprende los pasos de: el empaquetamiento de un sitio de herida por una almohadilla de espuma, estando dicha almohadilla de espuma en comunicación fluida con una fuente de vacío; el sellado de la región, incluyendo la almohadilla de espuma, con un esparadrapo para herida; y la comunicación del vacío desde la fuente de vacío a través de la almohadilla de espuma hacia dicha región.
8. El método para promover el sanado de heridas de conformidad con la reivindicación 7, caracterizado porque la realización de un paso de cambio comprende los pasos de : la circulación de un fluido alrededor de dicha región; y el control de la temperatura del fluido para obtener el cambio.
9. El método para promover el sanado de heridas de conformidad con la reivindicación 8, caracterizado porque el fluido se hace circular alrededor de dicha región en una envoltura flexible, la envoltura flexible tiene una entrada y una salida para la comunicación fluida a través de éste, del fluido.
10. El método para promover el sanado de heridas de conformidad con la reivindicación 9, caracterizado el método porque comprende además el paso de sellar la envoltura flexible con el esparadrapo para herida, entre la almohadilla de espuma y el esparadrapo para herida.
