Invenţia se referă la medicină, în special la tratamentul combustiilor de gradul I-IIIA.Metoda, conform invenţiei, constă în aceea că în prima şi a doua zi de tratament se administrează intramuscular sol. Ketoprofen - 100 mg, o dată în zi, iar local plăgile se prelucrează cu sol. Betadină de 1%, totodată, din prima zi de tratament pe plăgi se aplică pansamente cu ulei din seminţe de dovleac, timp de 7 zile.The invention relates to medicine, in particular to the treatment of combustion of degree I-IIIA. Invenţia se referă la medicină, în special la tratamentul combustiilor de gradul I-IIIA. The invention relates to medicine, in particular to the treatment of I-IIIA degree burns.
Traumatismul combustional este o problemă de bază medicală şi socială. Anual se înregistrează un număr impunător de cazuri de traumatism combustional. Din pacienţii internaţi cu plăgi combustionale, la 60...80% dintre aceştia sunt înregistrate combustii termice superficiale de gradele I, II - IIA, care nu necesită tratament chirurgical. Epitelizarea plăgilor decurge de sine stătător, timp de 30…34 zile, în funcţie de grad. Pe lângă aceasta 1/3 din combustiile de gradul III se epitelizează formând o cicatrice hipertrofică vulgară, care ulterior necesită un tratament conservativ, uneori chirurgical, de durată. Problema pe care o rezolvă invenţia propusă constă în extinderea arsenalului de metode utilizate pentru tratamentul combustiilor şi reducerea duratei tratamentului plăgilor combustionale. Avantajele metodei revendicate se datorează faptului că din prima zi de tratament pe plăgi se aplică pansamente cu ulei din seminţe de dovleac care permite epitelizarea mai precoce a plăgilor cu 3…4 zile faţă de cea mai apropiată soluţie. Metoda propusă se efectuează în felul următor. The proposed method is performed in the following way.
Iniţial, se efectuează prelucrarea primară a plăgilor, cu rezecarea veziculelor cu lichid şi înlăturarea ţesuturilor necrotizate. Ulterior în prima şi a doua zi de tratament se administrează intramuscular sol. Ketoprofen - 100 mg, o dată în zi, iar local plăgile se prelucrează cu sol. Betadină de 1%, totodată, din prima zi de tratament pe plăgi se aplică pansamente cu ulei din seminţe de dovleac, timp de 7 zile. Pacientul D., 45 ani, internat cu plăgi de gradul II-IIIA rezultate în urma combustiilor termice a regiunii dorsale a antebraţului, braţului şi trunchiului. În salonul de pansamente a fost efectuată revizia primară a plăgilor şi prelucrarea lor cu soluţii antiseptice. Obiectiv se evidenţiază multiplu epiteliu necrotizat cu o hiperemie pronunţată a regiunii date şi edemul ţesuturilor. În regiunea antebraţului se evidenţiază o plagă de gradul III de dimensiuni 10 x 20 cm, în regiunea braţului - o plagă de 5 x 15 cm şi în regiunea posterioară dreaptă a cutiei toracice - o plagă de dimensiuni 3 x 25 cm. Epitelizarea plăgilor lipseşte. Patient D., 45 years old, hospitalized with grade II-IIIA wounds resulting from thermal burns of the dorsal region of the forearm, arm and trunk. In the dressing room, primary revision of the wounds and their processing with antiseptic solutions was performed. Objectively, multiple necrotic epithelium with pronounced hyperemia of the given region and tissue edema are evident. In the forearm region, a grade III wound measuring 10 x 20 cm is evident, in the arm region - a wound measuring 5 x 15 cm and in the right posterior region of the rib cage - a wound measuring 3 x 25 cm. Epithelization of the wounds is absent.

La a 7-a zi, după înlăturarea pansamentului se evidenţiază epitelizarea definitivă a plăgilor. Aplicarea pansamentului nu a mai fost necesară. În urma epitelizării plăgilor s-au format cicatrici ce nu determină incomodităţi sau lezări ale funcţiilor regiunilor anterior afectate. Comparativ cu termenii descrişi de tratare a plăgilor cu Regesan (10…11 zile), epitelizarea plăgilor tratate prin aplicarea metodei propuse a avut loc cu 3…4 zile mai precoce. Compared to the described terms of wound treatment with Regesan (10…11 days), epithelialization of wounds treated by applying the proposed method occurred 3…4 days earlier.
Utilizarea acestui tratament la alţi 5 pacienţi cu combustii termice superficiale internaţi în secţia de combustiologie a IMDSP SCRTO a provocat o epitelizare precoce a plăgilor cu formarea unei cicatrice favorabile din punct de vedere cosmetologic, fără apariţia reacţiilor adverse la tratamentul aplicat. The use of this treatment in 5 other patients with superficial thermal burns hospitalized in the burn unit of IMDSP SCRTO caused early epithelialization of the wounds with the formation of a cosmetically favorable scar, without the occurrence of adverse reactions to the treatment applied.
