Предлагаемое изобретение относится к медицине, в частности к пластической хирургии, и может быть использовано при лечении глубоких ожоговых ран. The present invention relates to medicine, in particular to plastic surgery, and can be used in the treatment of deep burn wounds.
Известен способ лечения ожогов, путем ежедневной санации раневой поверхности и наложения влажно-высыхающих повязок с антисептиками до их самостоятельного заживления или формирования гранулирующих ран с последующей аутодермопластикой (см. Как следствие, при локализации ожогов в области крупных суставов часто возникают контрактуры с нарушением двигательных функций, приводящих к снижению или стойкой утрате трудоспособности пострадавших даже после ограниченных ожогов. Однако известный способ не предупреждает развитие краевых рубцовых деформаций на границе пересаженных аутодермотрансплантатов и самостоятельно заживших участков ожоговых ран. Задача предлагаемого изобретения - профилактика развития послеожоговых рубцовых деформаций по границе пересаженных свободных кожных лоскутов и самостоятельно заживающих участков. Поставленная задача решается за счет того, что в способе лечения ожоговых ран, включающем свободную кожную аутопластику, дополнительно осуществляют хирургическое иссечение и аутодермопластику самостоятельно заживающих участков вокруг гранулирующей поверхности. Способ лечения ожоговых ран осуществляют следующим образом. Иссекают кайму самостоятельной краевой эпителизации вокруг гранулирующей раны шириной 0,5-1 см на глубину 0,3-0,5 мм. Осуществляют гемостаз салфетками с раствором адреналина 1: 1000. На грануляции и образовавшуюся после иссечения раневую поверхность вокруг гранулирующей раны распластывают предварительно взятые электродерматомом перфорированные кожные трансплантаты толщиной 0,3 - 0,5 мм и производят тугое бинтование. В дальнейшем ведут рану, как обычную асептическую, делая первую перевязку на 3-4 день после операции. A method of treating burn wounds is as follows. Больная К. , 6 лет, И/б N 179899; 1.02.98 г. поступила в Нижегородский НИИ травматологии и ортопедии по поводу ожога пламенем I-II-III-АБ ст. правого плеча, грудной клетки на площади 40% поверхности тела. 19.02.98 г. выполнена свободная кожная пластика на гранулирующие раны правого плеча с предварительным хирургическим иссечением участков краевой самостоятельной эпителизации. Операция выполнялась под общим обезболиванием, под жгутом. Толщина иссеченного слоя составила 0,3 мм. Первая перевязка 23.02.98 г. Имело место полное восстановление кожного покрова. Больная выписана на амбулаторное лечение 26.03.98 г. Контрольное обследование было проведено через 12 месяцев после выписки из стационара. Состояние оперативно восстановленного кожного покрова приближалось к нормальной коже при отсутствии деформаций и контрактур.Clinical example

Предлагаемый способ лечения ожоговых ран позволяет провести полноценное восстановление утраченного кожного покрова, предупредить развитие краевых послеожоговых рубцовых деформаций и контрактур, особенно в области крупных суставов конечностей.
