Предлагаемое изобретение относится к медицине, а именно к пластической хирургии, комбустиологии, травматологии, нейрохирургии и может быть использовано у больных с глубокими ожогами головы и их последствиями, а также с посттравматическими ранами с остеонекрозом костей свода черепа.The proposed invention relates to medicine, namely to plastic surgery, combustiology, traumatology, neurosurgery and can be used in patients with deep burns of the head and their consequences, as well as with post-traumatic wounds with osteonecrosis of the bones of the cranial vault.
Основной целью лечения пациентов с наличием раневых дефектов является пластическое восстановление целостности тканей, в первую очередь - кожного покрова. Повреждения костей свода черепа наиболее часто наступают от действия тока высокого напряжения. Самостоятельное отторжение омертвевших участков костей протекает очень долго, иногда даже через год они прочно фиксированы (Ожоги: Руководство для врачей. Под ред. Б.С. Вихриева и B.М. Бурмистрова. Аналогом предполагаемого изобретения нами принят способ лечения контактных ожогов с остеонекрозом черепа (Гинзбург Р.Л. Ожоги. М.: Медицина, 1971. - С. 114). Способ предусматривает оперативное лечение остеонекроза путем выполнения множественных краниотомии. Для этого фрезой делают множественные отверстия в некротизированной кости до твердой мозговой оболочки. Омертвевшая кость через 1,5-2 месяца отторгается, к этому времени твердая мозговая оболочка оказывается покрытой грануляциями и рана готова к трансплантации кожи, которую выполняют вторым этапом.An analogue of the alleged invention, we adopted a method for the treatment of contact burns with osteonecrosis of the skull (Ginzburg RL Burns. Л.: Медицина, 1981. - С. 187-188). Резекция костей производится путем нанесения отверстий в кости, которые соединяют распилами, произведенными пилой Джигли, края костного дефекта выравнивают кусачками. На этом заканчивается первый этап. Сущностью изобретения, включающего обработку операционного поля, хирургическую обработку ран, остеонекрэктомию, свободную кожную аутопластику, наложение вакуумной повязки, является следующее: остеонекрэктомию выполняют радикально до твердой мозговой оболочки до активного кровотечения из жизнеспособной костной ткани, образующей стенки раны, кожными трансплантатами одномоментно укрывают края, стенки и дно образовавшейся после хирургической обработки и остеонекрэктомии раны, располагая их внахлест к здоровой коже, а фиксацию кожных трансплантатов осуществляют при помощи вакуумной повязки с постоянным давлением 70 мм.рт.ст.The essence of the invention, including treatment of the operating field, surgical treatment of wounds, osteonecrectomy, free skin autoplasty, application of a vacuum bandage, is the following: osteonecrectomy is performed radically to the dura mater until active bleeding from the viable bone tissue forming the wound walls, skin grafts simultaneously cover the edges, the walls and bottom of the wound formed after surgical treatment and osteonecrectomy, overlapping the healthy skin, and the fixation of skin grafts is carried out using a vacuum bandage with a constant pressure of 70 mm Hg.
Технический результат: данный способ позволяет за одну операцию восстановить целостность кожных покровов на ранах головы с остеонекрозом костей свода черепа, уменьшить процессы рубцевания в области выполнения свободной кожной пластики за счет предотвращения образования грануляционной ткани. Вакуумная повязка обеспечивает плотное соприкосновение кожных аутотрансплантатов с дном раны, что в свою очередь сводит к минимуму вероятность миграции аутотрансплантатов, а также появления гематом под аутотрансплантатами, которые могут замедлить их приживление. Благодаря вакуумной повязке пациент может принять любое положение в кровати, с возможностью лежать на ране, не боясь смещения повязок.EFFECT: this method allows in one operation to restore the integrity of the skin integuments on head wounds with osteonecrosis of the bones of the cranial vault, to reduce scarring processes in the area of free skin grafting by preventing the formation of granulation tissue. The vacuum bandage ensures a tight contact of the skin autografts with the bottom of the wound, which in turn minimizes the likelihood of migration of autografts, as well as the appearance of hematomas under the autografts, which can slow down their engraftment. Thanks to the vacuum dressing, the patient can take any position in bed, with the ability to lie on the wound without fear of displacement of the dressings.
Осуществление способа лечения локальных ран головы с остеонекрозом костей свода черепа выполняют следующим образом. В условиях операционной после стандартной обработки операционного поля производят хирургическую обработку ран: скальпелем выполняют окаймляющий разрез на границе здоровой кожи, экономно иссекают некротически измененные мягкие ткани или грануляции по краям раны, выполняют гемостаз электрокоагулятором и давящими повязками. Затем в области остеонекроза осцилляционной пилой, долотом и кусачками выполняют остеонекрэктомию радикально до твердой мозговой оболочки до активного кровотечения из жизнеспособной костной ткани, образующей стенки раны. Электродерматомом ДЭ-60 производится забор расщепленных кожных аутотрансплантатов необходимых по площади размеров, толщиной 0,45-0,5 мм. Кожными трансплантатами одномоментно укрывают края, стенки и дно образовавшейся после хирургической обработки и остеонекрэктомии раны, располагая их внахлест к здоровой коже. Затем на раны головы накладывают сетчатые атравматические повязки, губку вакуумной системы и пленку. Подсоединяют вакуумный аппарат и устанавливают параметры давления (70 мм. рт. ст.). Через 5-6 суток после операции производят первую перевязку. Вакуумную повязку удаляют, выполняют санацию зоны выполненной кожной пластики. Полную адаптацию кожных трансплантатов наблюдают через 10-12 дней после операции.The implementation of the method for the treatment of local head wounds with osteonecrosis of the bones of the cranial vault is performed as follows. Then, in the area of osteonecrosis with an oscillating saw, chisel and nippers, osteonecrectomy is performed radically to the dura mater until active bleeding from the viable bone tissue forming the wound walls. The DE-60 electrodermatome is used to collect split skin autografts of the size required for the area, with a thickness of 0.45-0.5 mm. Skin grafts simultaneously cover the edges, walls and bottom of the wound formed after surgical treatment and osteonecrectomy, placing them overlapping the healthy skin. Then, mesh atraumatic dressings, a vacuum system sponge and a film are applied to the head wounds. Connect a vacuum apparatus and set the pressure parameters (70 mm Hg). The first bandaging is performed 5-6 days after the operation. The vacuum bandage is removed, the area of the performed skin plastic is sanitized. Full adaptation of skin grafts is observed 10-12 days after surgery.
По данному способу в клинических условиях оперировано двое больных с локальными послеожоговыми ранами головы с остеонекрозом костей свода черепа.According to this method, in a clinical setting, two patients with local post-burn wounds of the head with osteonecrosis of the bones of the cranial vault were operated on.
Клинический пример. Пациент М., 46 лет, поступил в ожоговое отделение ГБУ3-НИИ ККБ №1 им. проф. С.В. Очаповского с диагнозом: электроожог головы 0,75% (рис. 1). Переведен через 4 недели после травмы из ЦРБ, где проводились этапные некрэктомии на перевязках. В условиях операционной после стандартной обработки операционного поля была произведена хирургическая обработка ран: скальпелем выполнили окаймляющий разрез на границе здоровой кожи, экономно иссекли грануляции по краям раны, выполнили гемостаз электрокоагулятором и давящими повязками. Затем в области остеонекроза осцилляционной пилой нанесли распилы на наружной кортикальной пластине до губчатого слоя, долотом провели остеонекрэктомию до внутренней кортикальной пластины (рис. 2), в связи с ее остеонекрозом кусачками удалили внутреннюю кортикальную пластину радикально до твердой мозговой оболочки до активного кровотечения из жизнеспособной костной ткани, образующей стенки раны (рис. 3). Электродерматомом ДЭ-60 произвели забор расщепленных кожных аутотрансплантатов необходимых по площади размеров, толщиной 0,45 мм. Кожными трансплантатами одномоментно укрыли края, стенки и дно образовавшейся после хирургической обработки и остеонекрэктомии раны, располагая их внахлест к здоровой коже (рис. 4). Затем на раны головы наложили сетчатые атравматические повязки, губку вакуумной системы и пленку (рис. 5). Подсоединили вакуумный аппарат и установили параметры давления 70 мм. рт. ст.(рис. 6). Через 5 суток после операции произвели первую перевязку. Вакуумную повязку удалили (рис. 7), выполнили санацию зоны выполненной кожной пластики (рис. 8). Полную адаптацию кожных трансплантатов наблюдали через 11 дней после операции (рис. 9).Clinical example. Patient M., 46 years old, was admitted to the burn department of GBU3-NII KKB No. prof. S.V. Then, in the area of osteonecrosis with an oscillating saw, cuts were made on the outer cortical plate to the spongy layer, osteonecrectomy was performed with a chisel to the inner cortical plate (Fig. 2), in connection with its osteonecrosis, the inner cortical plate was radically removed with nippers to the dura mater until active bone bleeding from viable tissue forming the walls of the wound (Fig. 3). Electrodermatome DE-60 was used to collect split skin autografts of the required size, 0.45 mm thick. Skin grafts simultaneously covered the edges, walls and bottom of the wound formed after surgical debridement and osteonecrectomy, placing them overlapping the healthy skin (Fig. 4). Then, mesh atraumatic dressings, a vacuum system sponge, and a film were applied to the head wounds (Fig. 5). We connected a vacuum apparatus and set the pressure parameters to 70 mm. rt. Art. (Fig. 6). The first bandaging was performed 5 days after the operation. The vacuum bandage was removed (Fig. 7), the zone of the performed skin grafting was sanitized (Fig. 8). Complete adaptation of skin grafts was observed 11 days after surgery (Fig. 9).
