Известен способ хирургического лечения ожоговой раны, заключающийся в выполнении раннего удаления струпа с одномоментной кожной аутопластикой [Вихреев Б.С., Бурмистров В.М. К недостаткам способа относятся: оперативное лечение только ожогов IIIБ ст., самостоятельная эпителизация «пограничных ожогов» IIIА-IIIБ ст. с высоким риском развития рубцовой ткани в послеоперационном периоде; применение после некрэктомии при пластике сетчатых (перфорированных) трансплантатов, что снижает функциональные и косметические результаты, в связи с высоким риском развития патологической рубцовой ткани в области перфорационных отверстий, высокий риск лизиса трансплантата из-за высокой вероятности оставления в ране участков некроза. Определение жизнеспособности дна раны - субъективный признак. Способ осуществляется следующим образом. После очищения раны от некротических тканей и формирования зрелых готовых для операции грануляций пациента берут в операционную, обычно на 27-30 сутки. Операцию начинают после стандартной подготовки и обработки операционного поля. Сначала электродерматомом производят забор кожных аутотрансплантатов в свободных от ожогах местах, площадью, равной предполагаемой площади раны. На донорские раны накладывают повязки с антисептиками. Аутотрансплантаты заворачивают в повязки с фурацилином.The method is as follows. Во время операции перед свободной аутодермопластикой выполняется иссечение всех слоев грануляционной ткани с помощью ложки Фолькмана и скальпеля, а также иссекаются края ожоговой раны, отступя на 2-3 мм от краев, направляя разрез косо по направлению к ране, увеличивая толщину среза по направлению к дну раны под углом 45°. Выполняя срез, выравниваются края раны и дно раневой поверхности. Проверка радикальности удаления грануляционной ткани осуществляется с помощью лазерной доплеровской флоуметрии. Производят гемостаз, придавливая рану повязками с раствором адреналина на 3-5 минут, крупные сосуды коагулируют. Параллельная граница позволяет произвести аутопластику неперфорированным кожным трансплантатом по правилам пластической хирургии: эпидермис к эпидермису, дерма к дерме. Накладывают повязки с антисептиком. Рану плотно забинтовывают в 5-7 туров медицинским бинтом. После операции конечности создают возвышенное положение. Первую перевязку производят на 5-й день. Выполнение ранней некрэктомии с одномоментной САДП глубоких ожоговых ран не представлялось возможным в связи с отсрочкой поступления и развившимся перифокальным воспалением ожоговых дефектов. Больному проводились этапная некрэктомия, перевязки с растворами антисептиков, мазями на водорастворимой основе, покрытиями с антисептиками. Поверхностные ожоги эпителизировались. После полного очищения раны от некротических тканей были сформированы грануляции на всей поверхности ожоговых дефектов.Example. На 27 сутки больной взят в операционную. Выполнена операция по заявляемому способу. Первая перевязка после операции на 5-й день. Отмечается полная адаптация аутотрансплантатов. Признаков лизиса лоскута пересаженного нет. Больной выписан на амбулаторное лечение через 10 дней после приживления трансплантата. В отдаленном периоде (через 6 месяца) восстановление функции полное, не наблюдается образования патологической рубцовой ткани, отмечается ровная зона перехода трансплантата к здоровой коже, с совпадением краев. Отмечается идеальный косметический результат вследствие пластики неперфорированным трансплантатом, не требующий коррекции консервативными способами.On the 27th day the patient was taken to the operating room. Данный способ позволяет сократить сроки лечения больных с глубокими и пограничными ожогами с хорошим косметическим результатом. Способ доступен врачам-хирургам, не требует дорогостоящего оборудования и проведения большого количества лабораторных и инструментальных исследований. Выполнение заявляемого способа дает возможность не только эффективно предотвращать развитие осложнений во время лечения ожогов, привести к быстрому восстановлению ожогового дефекта и нормализации пересаженного лоскута с восстановлением его функции, но и избежать развития отдаленных осложнений ожоговой травмы в виде патологических рубцов, трофических язв, тем самым позволяет избежать инвалидизации пациентов, повторных операций, отсутствие функциональных и косметических нарушений.The implementation of the proposed method makes it possible not only to effectively prevent the development of complications during treatment of burns, to quickly restore a burn defect and normalize a transplanted flap with the restoration of its function, but also to avoid the development of long-term complications of a burn injury in the form of pathological scars, trophic ulcers, thereby avoid disability of patients, repeated operations, lack of functional and cosmetic disorders.
