Лечение раны, пораженной остеомиелитическим процессом, расположенной на боковой поверхности ногтевой фаланги пальца кисти, осуществляется следующим образом: выполняется радикальная хирургическая обработка мягкотканого остеомиелитического дефекта по боковой поверхности ногтевой фаланги (1) пальца (2) и раны (3) пораженной остеомиелитическим процессом (фиг. 1), производится резекция участка деструкции ногтевой фаланги. Костной ложкой удаляют секвестры и гнойные грануляции, границу резекции ногтевой фаланги обрабатывают шаровидной фрезой до достижения здоровой кровоточащей ткани. Полость дефекта обрабатывают раствором антисептика и временно закрывают марлевой салфеткой. По боковой поверхности ногтевой фаланги пальца (2) выкраивают дистальный несвободный кожный лоскут (4) (фиг. 2) основанием на апикальной части пальца на дистальной сосудистой ножке. Кожный лоскут (4) включает общую ладонную пальцевую артерию (5), собственную ладонную пальцевую артерию 6 (фиг. 3). С дистальной части кожного лоскута (4) срезают эпидермис, из раны удаляют салфетку. Дистальную часть кожного лоскута прошивают рассасывающим шовным материалом. Лоскут (4) фиксируют к резецированной ногтевой фаланге 1 (фиг. 4) на поверхности кости с противоположной стороны, с максимальным отклонением по оси не более ста пятидесяти градусов. Рану после радикальной хирургической обработки остеомиелитического очага восстанавливают за счет мобилизации местных тканей. Рану в донорской зоне ушивают по виду «шеврона», а на месте иссеченного мягкотканого остеомиелитического дефекта в линию.Treatment of a wound affected by the osteomyelitis process, located on the lateral surface of the nail phalanx of the finger, is carried out as follows: radical surgical treatment of the soft tissue osteomyelitic defect is performed along the lateral surface of the nail phalanx (1) of the finger (2) and the wound (3) affected by the osteomyelitis process (Fig. 1), resection of the site of destruction of the nail phalanx is performed. Sequesters and purulent granulations are removed with a bone spoon, the border of the resection of the nail phalanx is treated with a spherical cutter until a healthy bleeding tissue is reached. The defect cavity is treated with an antiseptic solution and temporarily covered with a gauze napkin. On the lateral surface of the nail phalanx of the finger (2), a distal non-free skin flap (4) (Fig. 2) is cut out with its base on the apical part of the finger on the distal vascular pedicle. The skin flap (4) includes the common palmar digital artery (5), own palmar digital artery 6 (Fig. 3). The epidermis is cut from the distal part of the skin flap (4), the napkin is removed from the wound. The distal part of the skin flap is sutured with absorbable suture material. The flap (4) is fixed to the resected nail phalanx 1 (Fig. 4) on the bone surface from the opposite side, with a maximum deviation along the axis of not more than one hundred and fifty degrees. The wound after radical surgical treatment of the osteomyelitic focus is restored by mobilizing local tissues. The wound in the donor area is sutured in the form of a "chevron", and in the place of the excised soft tissue osteomyelitic defect in a line.
Сущность предлагаемого способа заключается в формировании дистального несвободного кожного лоскута по боковой поверхности ногтевой фаланги пальца кисти пораженного остеомиелитическим процессом, устранении гнойного очага ногтевой фаланги пальца кисти, сохранение формы и функциональной способности пальца кисти. Это достигается тем, что способ включает радикальную хирургическую обработку остеомиелитического очага по боковой поверхности ногтевой фаланги пальца кисти, удаление гнойных грануляций и секвестров, обработку раны раствором антисептика с последующим тампонированием марлевой салфеткой, по боковой поверхности пораженного остеомиелитическим дефектом ногтевой фаланги пальца выкраивают несвободный дистальный кожный лоскут основанием на апикальной части пальца на дистальной сосудистой ножке, при этом на кисти используют лоскут с общей ладонной пальцевой артерией и собственной ладонной пальцевой артерией, срезают с дистальной части несвободного кожного лоскута эпидермис и удаляют салфетку, лоскут фиксируют к ногтевой фаланге пальца с отклонением по оси не более ста пятидесяти градусов, рану восстанавливают с мобилизацией местных тканей и ушивают по виду «шеврона», а место иссеченного дефекта ушивают в линию.The essence of the proposed method lies in the formation of a distal non-free skin flap along the lateral surface of the nail phalanx of the finger affected by the osteomyelitis process, the elimination of the purulent focus of the nail phalanx of the finger, the preservation of the shape and functional ability of the finger. Свищевая форма. An.morbi: травма была получена 14 дней назад, в последующем отметила появление болей, отека гиперемии мягких тканей 3 пальца правой кисти. На рентгенограмме деструкция костной ткани ногтевой фаланги 1/3. St.Localis: остеомиелитический очаг по боковой поверхности ногтевой фаланги третьего пальца кисти справа с дефицитом мягких тканей размером 1,0×0,5 см, дно раны - кость ногтевой фаланги. На рентгенограмме имеется деструкция кости размером 0,5×0,3 см. Проводилась антибактериальная терапия с учетом микрофлоры. Выполнена операция предлагаемым способом.
