Abstract
Description
Изобретение относится к медицине, в частности хирургии кисти, и может быть использовано для восстановления функции захвата кисти после ее травмы. The invention relates to medicine, in particular hand surgery, and can be used to restore the function of capture of the hand after its injury.
Особую группу больных с последствиями травм кисти составляют пациенты с травматическими дефектами различных сегментов кисти. Они возникают в результате тяжелых механических травм с первичным разрушением тканей, а также после глубоких ожогов, отморожении, ишемии. Основной причиной инвалидизации и утраты профессиональной пригодности в 70-80% являются ампутационные дефекты пальцев и кисти (Водянов Н.М., 1979; Лернер П.Н. с соавт., 1981). A special group of patients with the consequences of hand injuries are patients with traumatic defects of various segments of the hand. They arise as a result of severe mechanical injuries with primary tissue destruction, as well as after deep burns, frostbite, ischemia. The main cause of disability and loss of professional fitness in 70-80% are amputation defects of the fingers and hand (Vodyanov N.M., 1979; Lerner P.N. et al., 1981).
Техника полицизации предусматривает пересадку любого пальца или его культи, или сегмента трубчатой кости на постоянно питающей сосудистой ножке на место дефекта первого луча. Эффективность всех способов полицизации определяется следующими условиями:
1) достаточной подвижностью сохранившихся суставов первого луча и сухожильно-мышечного аппарата;
2) сохранностью чувствительности рабочих поверхностей межпястного промежутка;
3) достаточной длиной первого луча для осуществления противопоставления и функции захвата предметов различного диаметра.The police technique involves the transplantation of any finger or its stump, or segment of the tubular bone on a constantly supplying vascular pedicle to the site of the defect of the first ray. Использование способа Morrison нарушает функцию опоры нижней конечности. Кожно-костная реконструкция по В.В Азолову предусматривает многоэтапное восстановление. Данный способ можно применять только в специализированных отделениях, оснащенных микрохирургическим инструментарием и подготовленными кадрами, владеющими микрохирургической техникой. Данный способ предусматривает транспозицию здорового второго пальца на первую пястную кость. Показание к операции - отсутствие одновременно первого, второго пальцев кисти с сохранением фрагмента основной фаланги культи второго пальца. Указанный результат достигается тем, что для полицизации первого луча используется Фрагмент второй пястной кости, второго пястно-фалангового сустава и фрагмент основной фаланги культи второго пальца на сосудисто-нервной ножке. Предлагаемый способ осуществляется следующим образом: под общим обезболиванием или проводниковой анестезией и обескровливанием оперируемой конечности производят разрезы по тыльной и ладонной поверхностям в I-II и II-III межпястных промежутках до основания пястных костей. Выделяют сосудисто-нервный пучок, идущий на второй палец. Выполняют остеотомию второй пястной кости в области метаэпифиза с сохранением пястно-фалангового сустава и сухожилия поверхностного сгибателя пальца. Сухожилие разгибателя пальца пересекают в области проксимальной части трансплантата. Фрагмент кости с мягкими тканями и питающими сосудами и нервами мобилизуют. Двумя косыми разрезами обнажают и освежают от рубцов дистальную часть первой пястной кости. Фрагмент культи 2 пальца с мягкими тканями на питающей ножке перемещают на культю первого луча и фиксируют двумя спицами в положении оппозиции к четвертому и пятому пальцам. Накладывают сухожильный шов на разгибатель первого пальца. Раны кисти ушивают местными тканями. Фиксирующие спицы удаляют через 3 недели после срастания первой пястной кости с аутотрансплантатом. Способ применен в клинической практике у 9 больных. У двух больных одновременно с полицизацией первого луча выполнена свободная пересадка вторых пальцев в позицию четвертого пальца при адактилии кисти с применением микрохирургической техники. Функциональный результат удовлетворяет всех пациентов: восстанавливается захват кисти, социально наступает реабилитация больных. The method is used in clinical practice in 9 patients. Клиническое наблюдение. Больной С., 42 года (История болезни 5346/2000), поступил в отделение малоинвазивной и пластической хирургии больницы скорой медицинской помощи г.Уфы на плановое оперативное лечение с жалобами на отсутствие первого и второго пальцев правой кисти. Из анамнеза: больной 1,5 года тому назад получил бытовую травму циркулярной пилой. Клинически: имеются ампутационные культи первого пальца на уровне пястно-фалангового сустава, второго пальца на уровне дистального метаэпифиза основной фаланги. Больному выполнена операция: под проводниковой анестезией и обескровливанием оперируемой конечности произведены разрезы по тыльной и ладонной поверхностям в I-II и II-III межпястных промежутках до основания пястных костей. Выделен сосудисто-нервный пучок, идущий на второй палец. Выполнена остеотомия второй пястной кости в области метаэпифиза с сохранением пястно-фалангового сустава и сухожилия поверхностного сгибателя пальца. Сухожилие разгибателя пальца пересечено в области проксимальной части трансплантата. Фрагмент кости с мягкими тканями и питающими сосудами и нервами мобилизован. Двумя косыми разрезами обнажены и освежены от рубцов дистальная часть первой пястной кости. Фрагмент культи 2 пальца с мягкими тканями на питающей ножке перемещен на культю первого луча и фиксирован двумя спицами в положении оппозиции к четвертому и пятому пальцам. Наложен сухожильный шов на разгибатель первого пальца. Раны кисти ушиты местными тканями. Фиксирующие спицы удалены через 3 недели после срастания первой пястной кости с аутотрансплантатом. A neurovascular bundle reaching the second finger was isolated. An osteotomy of the second metacarpal bone was performed in the metaepiphysis with preservation of the metacarpophalangeal joint and tendon of the superficial flexor of the finger. The extensor extensor tendon is crossed in the area of the proximal part of the graft. A fragment of bone with soft tissues and nourishing vessels and nerves is mobilized. By two oblique incisions, the distal part of the first metacarpal bone is exposed and refreshed from scars. Fixing knitting needles removed 3 weeks after the fusion of the first metacarpal bone with an autograft.
Послеоперационный период протекал без осложнений. Аутотрансплантат полностью прижил, раны зажили первичным натяжением. Осмотрен через 8 месяцев после операции. Основные виды захвата кисти в полном объеме, работает по специальности. The postoperative period was uneventful. The autograft completely healed, the wounds healed by primary intention. Преимущества предложенного нами способа - в одномоментности операции, применении местных тканей, дополнительном углублении I-II межпястного промежутка, сохранности чувствительности рабочих поверхностей и достаточной длины первого луча для осуществления функции захвата кисти.
