Одной из наиболее сложных проблем в хирургии кистевого сустава являются оперативные вмешательства при травматических разрывах дистального радиоульнарного сочленения. Особенность заключается в необходимости восстановления связочного аппарата сочленения и в сохранении функции движения в радиоульнарном сочленении. Существует множество известных способов лечения этой категории пациентов с разрывом дистального радиоульнарного сочленения. Так для предупреждения рецидива вывиха головки локтевой кости, ее фиксируют проволокой к эпифизу лучевой кости [1]. Однако, в этом случае, несмотря на стабильность, теряется подвижность супинационно-пронационных движений в суставе, что приводит к резкому ограничению функции в лучезапястном суставе.One of the most difficult problems in wrist surgery is surgical interventions for traumatic ruptures of the distal radioulnar joint. However, in this case, despite the stability, the mobility of the supination-pronation movements in the joint is lost, which leads to a sharp limitation of the function in the wrist joint.
Наиболее близким, по своему техническому решению, заявляемому способу, является «Способ восстановления дистального радиоульнарного сочленения предплечья» [2]. Сущность этого способа включает доступ к суставу, формирование каналов и аутотендопластику. Производят два продольных разреза по лучевой и локтевой стороне предплечья, на уровне радиоульнарного сочленения обнажают шейку локтевой кости. Через дистальный эпиметафиз лучевой кости, во фронтальной плоскости, просверливают два поперечных канала: проксимальный и дистальный. Конец свободного армированного аутотрансплантата проводят через дистальный канал с лучевой стороны предплечья - на локтевую, и, обойдя петлей вокруг шейки локтевой кости, трансплантат проводят через проксимальный канал в обратном направлении. Фиксируют головку локтевой кости в суставной выемке лучевой кости, подтягивая концы трансплантата, завязывают их узлом, крепят к трансплантату, фиксируют кости предплечья в нейтральном положении двумя спицами на 4 недели. Данный способ травматичен, сформированная связка не всегда стабильно удерживает головку локтевой кости, некровоснабжаемый аутотрансплантат подвержен рубцовому перерождению и потере прочности, в результате чего не исключается возможность ладонно-тыльного перемещения головки локтевой кости, и рецидива ее вывиха. При значительных физических нагрузках на сустав возрастает вероятность получения патологического перелома лучевой, и/или локтевой, кости, или разрыва аутотрансплантата.The closest, in terms of its technical solution, to the claimed method is the "Method for restoring the distal radial articulation of the forearm" [2]. This method is traumatic, the formed ligament does not always hold the head of the ulna stably, the non-blood-supplied autograft is subject to cicatricial degeneration and loss of strength, as a result of which the possibility of palmar-dorsal displacement of the head of the ulna and recurrence of its dislocation is not excluded. With significant physical exertion on the joint, the likelihood of obtaining a pathological fracture of the radial and / or ulnar bone, or rupture of the autograft increases.

Из отдельного доступа, на тыле 5 пястно-фалангового сустава, выделяют собственный разгибатель 5 пальца на дистальном уровне, отсекают его вместе с паратеноном, и выводят на уровень нижней трети предплечья. Выполняют продольный разрез по лучевой стороне предплечья в нижней трети, длиной 2 см, выделяют лучевой разгибатель запястья. Просверливают канал в лучевой и локтевой кости, на уровне дистального радиоульнарного сочленения, в сформированный канал проводят сухожилие собственного разгибателя 5 пальца, выводят его с лучевой стороны, и сшивают в расщеп с лучевым разгибателем запястья, подбирая умеренное натяжение, исключающее нестабильность головки локтевой кости. Пациентка С, 53 лет. Поступила в отделение Травматологии №2 ГАУЗ «РКБ МЗ РТ» 07.10.2019 г. Диагноз: закрытый разрыв дистального радиоульнарного сочленения левого предплечья, с вывихом головки локтевой кости к ладони, 4 недельной давности. Обстоятельства травмы: за 4 недели до обращения - упала на руку возле дома. Лечилась в гипсе 3 недели по поводу ушиба. При осмотре выявлен вывих головки локтевой кости. В области дистального лучелоктевого сочленения плотный отек, боли при пальпации и движениях. Активные ротации предплечья невозможны из-за болей. Пассивные ограничены, болезненны. Функция схвата так же нарушена из-за болей. Чувствительность пальцев сохранена. Сосудистых расстройств не выявлено. Пациентке. Выполнена рентгеновская компьютерная томография (РКТ) нижней трети предплечья с захватом кисти (фиг. 1), стандартное предоперационное обследование. Сопутствующих заболевай не выявлено. Active rotation of the forearm is impossible due to pain. Passives are limited, painful. The gripping function is also impaired due to pain. Finger sensitivity is preserved. No vascular disorders have been identified. To the patient. Performed X-ray computed tomography (RCT) of the lower third of the forearm with the capture of the hand (Fig. 1), standard preoperative examination. No associated diseases were identified. There are no contraindications to surgery.
14.10.2019 г. проведена операция по вышеописанной методике. Под проводниковой анестезией выполнили продольный С-образный разрез, длиной 5 см, по тыльно-локтевой поверхности кисти и предплечья в проекции дистального радиоульнарного сочленения и головки локтевой кости, рассекли 5 канала. Выявили разрыв тыльной части капсулы дистального радиоульнарного сочленения. Вскрыли дистальное радиоульнарное сочленение, удалили грануляции. Головку локтевой кости вправили в локтевую вырезку лучевой кости (фиг. 2). Из отдельного доступа на тыле 5 пястно-фалангового сустава выделили собственный разгибатель 5 пальца на дистальном уровне, отсекли его вместе с паратеноном (фиг. 3), и вывели на уровень нижней трети предплечья. Выполнили продольный разрез, длиной 2 см, по лучевой стороне предплечья в нижней трети, выделили лучевой разгибатель запястья. Просверлили канал в лучевой и локтевой кости, на уровне дистального радиоульнарного сочленения. В сформированный канал провели сухожилие собственного разгибателя 5 пальца, вывели его с лучевой стороны, и сшили в расщеп с лучевым разгибателем запястья (фиг. 5), подобрав умеренное натяжение, исключающее нестабильность головки локтевой кости. Кости предплечья зафиксировали в нейтральном положении спицей. Восстановили капсулу дистального радиоульнарного сочленения (фиг. 4). Гемостаз, кожные швы до дренажей, спиртовая повязка, гипс.On October 14, 2019, the operation was performed using the above-described method. Ротационные движения левого предплечья не ограничены(фиг. 7-а, фиг. 7-б, Фиг. 7-в). Симптомов вывиха головки локтевой кости нет. Результатом операции пациентка удовлетворена.
