Abstract
Description
Изобретение относится к области медицины, в частности к травматологии, в лечении последствий повреждений дистального отдела лучевой кости с разрывом дистального лучелоктевого сочленения. Известны несколько способов лечения застарелых повреждений дистального лучелоктевого сочленения: 1) способ Darrach - резекция головки локтевой кости по косой линии от основания шиловидного отростка к дистальному краю суставной поверхности головки локтевой кости; 2) способ Moora - Darrach - гемирезекция головки локтевой кости, когда удаляется ее суставная поверхность, но сохраняются шиловидный отросток и вся внутренняя часть (Ашкенази А.И. Хирургия кистевого сустава. /Москва: Медицина, 1990). Известны также способы Эдельштейна и Ю.Ю.Джанелидзе (Н.И.Реут, Ф.А.Кадыров. Лечение изолированных повреждений дистального лучелоктевого сочленения. /Ортопедия травматология и протезирование, 1971, №4).The invention relates to medicine, in particular to traumatology, in the treatment of the effects of damage to the distal radius of the radial bone with a rupture of the distal radiolactic joint. Известный способ Эдельштейна заключается в том, что отщеп сухожилия локтевого сгибателя проводится на тыл головки локтевой кости в обхват ее циркулярно и обратно на ладонную поверхность. Таким образом способ применим только при тыльных вывихах головки локтевой кости. Заявленный технический результат достигается тем, что создается прочная фиксация головки локтевой кости во вправленном положении в сигмовидной вырезке (своеобразный тенодез) с надежной внутрикостной и параоссальной фиксацией аутосухожильного трансплантата. Восстанавливается именно та часть треугольного фиброзно-хрящевого комплекса и связочного аппарата, которая функционально недостаточна в данном конкретном случае, поскольку известно, что при ладонных вывихах головки локтевой кости страдает тыльная порция лучелоктевой связки, а при тыльных вывихах - ладонная порция. Операция показана при возможности восстановления удовлетворительных соотношений в дистальном лучелоктевом суставе и необходимости стабилизации дистального конца локтевой кости.The claimed technical result is achieved by creating a solid fixation of the head of the ulna in the right position in the sigmoid notch (a kind of tenodesis) with reliable intraosseous and paraossal fixation of an autotendinous graft. Сущность заявляемого способа заключается в том, что из разреза по тыльной поверхности кисти в области головки локтевой кости выделяются сухожилие локтевого разгибателя кисти, дистальное лучелоктевое сочленение. При необходимости производится открытое вправление вывиха головки локтевой кости. От сухожилия локтевого разгибателя кисти (фиг.1, а) производится забор «отщепа» надистальной ножке (фиг.1, б). В головке локтевой кости тонким сверлом выполняется Г-образный канал продольно по оси локтевой кости и поперечно с выходом в подголовчатой зоне в межкостный промежуток (фиг.2, в).The essence of the proposed method lies in the fact that a tendon of the elbow extensor of the brush and a distal radiolbow joint are distinguished from an incision along the dorsum of the hand in the region of the head of the ulnar bone. После выхода в сторону межкостного промежутка свободный конец огибает локтевую кость с ладонной поверхности и возвращается на тыльную при ладонном вывихе и через тыльную поверхность на ладонную - при тыльном вывихе. Далее небольшой распатор проводится параоссально на уровне поперечной части канала в головки локтевой кости по тыльной поверхности лучевой кости при ладонном вывихе (фиг.3) и по ладонной - при тыльном вывихе (фиг.4). Свободный конец трансплантата прошивается и зажимом проводится параоссально поперек лучевой кости на противоположную сторону, где с натяжением и при вправленном положении локтевой кости фиксируется «на валике» (фиг.3-г). Дистальное лучелоктевое сочленение фиксируется спицами (фиг.3, 4-д). Пациент А., 43 лет, поступил в приемное отделение РосНИИТО с диагнозом: Закрытый перелом дистального метаэпифиза правой лучевой кости со смещением. Была произведена закрытая ручная репозиция, иммобилизация гипсовой повязкой. На контрольных Рг-граммах достигнуто удовлетворительное положение отломков. В дальнейшем перелом консолидировал, но проявилась несостоятельность дистального лучелоктевого сочленения с тыльным подвывихом головки локтевой кости. Обратился за помощью через 4 месяца после травмы с жалобами на ограничение объема ротационных движений и болезненность в крайних положениях пронации и супинации, снижении силы захвата пальцев кисти.Patient A., 43 years old, was admitted to the admission department of RosNIIITO with a diagnosis of Closed fracture of the distal metaepiphysis of the right radius with displacement. Выполнена операция по описанной методике с фиксацией дистального лучелоктевого сочленения спицами и гипсовой повязкой до 4 недель. После короткого курса восстановительного лечения достигнут практически полный объем ротационных движений без болевого синдрома, восстановилась сила захвата.
