При глубоких ожогах важных в функциональном и косметическом отношениях участков тела выполняют пластику целыми аутотрансплантатами (Ожоги: Руководство для врачей / Под ред. Б.С. Вихриева и В.М. Бурмистрова. - Л.: Медицина, 1981. - С. 158). Учитывая, что склонность полнослойных трансплантатов к вторичной ретракции по сравнению с расщепленными меньше (Азолов В.В., Дмитриев Г.И. Хирургическое лечение последствий ожогов. - Н. Новгород: ОАО «Дзержинская типография», 1995. - С. 42, С. 57), использование полнослойных кожных трансплантатов на функционально и эстетически значимых участках тела предпочтительнее. Использование кожных трансплантатов с живота является традиционным. Учитывая, что взятие полнослойного кожного трансплантата больших размеров требуется, как правило, при закрытии ран лица, локализация донорского участка на животе является оптимальной, т.к. целесообразно планировать донорские места на одной с закрываемыми ранами поверхности тела (Ожоги: Руководство для врачей / Под ред. Осуществление способа забора кожного трансплантата производят следующим образом. В условиях операционной после стандартной подготовки и обработки операционного поля на животе выполняют предоперационную разметку кожных покровов, обозначая границы забора с учетом необходимой длины и ширины кожного трансплантата. Скальпелем по линиям предоперационной разметки производят рассечение кожи и подкожно-жировой клетчатки. Выполняют забор полнослойного кожного трансплантата с подкожно-жировой клетчаткой до поверхностной фасции, а также гемостаз. Затем отдельными узловыми швами частично стягивают края донорской раны от периферии к центру, уменьшая тем самым площадь раневого дефекта. Затем производят аппликацию дермальных аутофибробластов или аллофибробластов в концентрации 1 млн. клеток в 1 мл на дно донорской раны из расчета расхода не менее 20 тыс.клеток на 1 кв. см. обрабатываемой поверхности с экспозицией 30 минут. По данному способу в клинических условиях прооперированы 4 больных.According to this method, 4 patients were operated on under clinical conditions.
Пример 1 (см. приложение 1). Пациент А., 9 лет, поступил в ожоговое отделение ГБУЗ «НИИ - ККБ №1 им. проф. С.В. Очаповского» с диагнозом: термический ожог (пламенем) головы, шеи, туловища, ягодиц, конечностей 41% I-II-III степени, термоингаляционная травма, ожоговая болезнь (рис. 1). После выполнения этапных некрэктомий на лице на 21-е сутки после травмы больной был взят в операционную (рис. 2). После стандартной подготовки и обработки операционного поля (рис. 3) на животе выполнили предоперационную разметку кожных покровов, обозначая границы забора с учетом необходимой длины (рис. 4) и ширины (рис. 5) кожного трансплантата. Скальпелем по линиям предоперационной разметки произвели рассечение кожи и подкожно-жировой клетчатки (рис. 6). Выполнили забор полнослойного кожного трансплантата с подкожно-жировой клетчаткой до поверхностной фасции, а также гемостаз (рис. 7). Затем отдельными узловыми швами частично стянули края донорской раны от периферии к центру, уменьшая тем самым площадь раневого дефекта (рис. 8 и 9). Затем произвели аппликацию дермальных аллофибробластов в концентрации 1 млн. клеток в 1 мл на дно донорской раны из расчета расхода не менее 20 тыс.клеток на 1 кв. см. обрабатываемой поверхности с экспозицией 30 минут. В это время произвели подготовку полнослойного кожного трансплантата (рис. 10 и 11), хирургическую обработку раны (рис. 12), требующей пластического закрытия полнослойным трансплантатом, взятие на задней поверхности туловища и перфорацию с коэффициентом перфорации 1:2 расщепленных кожных аутотрансплантатов, толщиной 0,25 мм, для закрытия раны донорского участка. Выполнили кожную пластику полнослойным кожным трансплантатом (рис. 13 и 14). Через 30 минут выполнили пластическое закрытие донорской раны расщепленными перфорированными кожными трансплантатами (рис. 15), кожные трансплантаты закрыли сетчатым атравматическим раневым покрытием (рис. 16), затем наложили вакуумную повязку на донорский участок живота (рис. 17 и 18) на 5 дней с постоянным давлением 110-120 мм.рт.ст. аппаратом для вакуумной терапии ран VivanoTec (рис. 19). Через 5 дней после операции вакуумную повязку сняли (рис. 20). Под сетчатым атравматическим раневым покрытием нагноения не наблюдали (рис. 21). На 7-е сутки после операции раневое покрытие было удалено (рис. 22), наблюдали приживление расщепленных кожных трансплантатов на донорском участке живота (рис. 23). Донорские участки после взятия расщепленных кожных трансплантатов на задней поверхности туловища эпителизировались под сухими повязками (рис. 24), смена повязок не требовалась. Пациент был выписан на 63-е сутки после травмы после полного восстановления, поврежденного ожогом кожного покрова и заживления донорских ран. Наблюдали удовлетворительное состояние восстановленного кожного покрова на донорском участке живота (рис. 25) и полное приживление полнослойного кожного трансплантата на лице без склонности к ретракции (рис. 26).Example 1 (see Appendix 1). Under reticulated atraumatic wound cover, suppuration was not observed (Fig. 21). On the 7th day after the operation, the wound cover was removed (Fig. 22), and the engraftment of split skin grafts in the donor area of the abdomen was observed (Fig. 23). Donor sites after taking split skin grafts on the rear surface of the body were epithelized under dry dressings (Fig. 24), no dressing changes were required. The patient was discharged on the 63rd day after the injury after complete recovery, damaged by a burn of the skin and the healing of donor wounds. A satisfactory condition of the restored skin on the donor abdomen was observed (Fig. 25) and complete engraftment of a full-layer skin graft on the face without a tendency to retraction (Fig. 26).
Пример 2. Пациент М., 26 лет, поступил в ожоговое отделение ГБУЗ. «НИИ - ККБ №1 им. проф. С.В. Очаповского» с диагнозом: термический ожог (пламенем) туловища, верхних конечностей 18% I-II-III степени, ожоговая болезнь. После выполнения этапных некрэктомий на кистях на 17-е сутки после травмы больной был взят в операционную. После стандартной подготовки и обработки операционного поля на животе выполнили предоперационную разметку кожных покровов, обозначая границы забора с учетом необходимой длины и ширины кожного трансплантата. Скальпелем по линиям предоперационной разметки произвели рассечение кожи и подкожно-жировой клетчатки. Выполнили забор полнослойного кожного трансплантата с подкожно-жировой клетчаткой до поверхностной фасции, а также гемостаз. Затем отдельными узловыми швами частично стянули края донорской раны от периферии к центру, уменьшая тем самым площадь раневого дефекта. Затем произвели аппликацию дермальных аутофибробластов в концентрации 1 млн. клеток в 1 мл на дно донорской раны из расчета расхода не менее 20 тыс.клеток на 1 кв. см. обрабатываемой поверхности с экспозицией 30 минут. В это время произвели подготовку полнослойного кожного трансплантата, хирургическую обработку раны правой кисти, требующей пластического закрытия полнослойным трансплантатом, взятие на правом бедре и перфорацию с коэффициентом перфорации 1:2 расщепленных кожных аутотрансплантатов, толщиной 0,25 мм, для закрытия раны донорского участка живота. Выполнили кожную пластику ран правой кисти полнослойным кожным трансплантатом. Через 30 минут выполнили пластическое закрытие донорской раны живота расщепленными перфорированными кожными трансплантатами, кожные трансплантаты закрыли сетчатым атравматическим раневым покрытием, затем наложили вакуумную повязку на донорский участок живота на 5 дней с постоянным давлением 110-120 мм.рт.ст. аппаратом для вакуумной терапии ран VivanoTec. Через 5 дней после операции вакуумную повязку сняли. Под сетчатым атравматическим раневым покрытием нагноения не наблюдали. На 7-е сутки после операции раневое покрытие было удалено, наблюдали приживление расщепленных кожных трансплантатов на донорском участке живота. Донорские участки правого бедра после взятия расщепленных кожных трансплантатов эпителизировались под сухими повязками, смена повязок не требовалась. Пациент был выписан на 32-е сутки после травмы после полного восстановления, поврежденного ожогом кожного покрова и заживления донорских ран.Example 2.
