Abstract
Description
The invention relates to the field of medicine, veterinary medicine and the pharmaceutical industry and can be used to treat burn wounds, including deep wounds and using autodertoplasty.
Known use for the treatment of deep burn wounds preparations of various chemical nature. According to the Register of Medicinal Products of Russia (Moscow, RLS-2000, 1999), Betadine ointment (p. 776) is used for these purposes, the disadvantage of which is irritation of the skin and mucous membranes at the sites of application due to excreted iodine: Levovinisol (p. 847) is an aerosol preparation of complex composition, which also causes a burning sensation in places of application; Lorinden ointment (p. 876) can cause contact eczema, allergic reactions, atrophy of the skin and subcutaneous tissue; Oksinart ointment is contraindicated in case of viral infection; Argosulfan ointment (p. 874) may cause leukopenia and allergies; Dimexidum solution can cause vomiting, bronchospasm, itching, is contraindicated in violation of the liver or kidney function, angina pectoris, atherosclerosis, stroke, pregnancy. In addition, all these drugs are ineffective when used for the treatment of burn wounds by the method of autodermathoplasty.
The closest to the claimed is the drug Miramistin (p. 656), representing a 0.01% aqueous solution of myristamidopropyl dimethylbenzylammonium chloride. The drug is used in the form of dressings and tampons in the treatment of superficial and deep burn wounds. Its disadvantages include a slight burning sensation that passes on its own after 1015 s, and also the fact that dressings and tampons dry quickly and stick to the wound surface, which requires constant additional treatment of the dressings from the outside. The drug is ineffective in the preparation and conduct of autodertoplasty, as it quickly drains from the surface of the graft and does not protect it from suppuration; in addition, the concentration used is insufficient to ensure a reliable antimicrobial effect.
The aim of the present invention is to expand the range of funds for the treatment of burn wounds and its effective use in the practice of autodermathoplasty.
Поставленная цель достигается тем, что в известный состав, включающий мирамистин и воду, дополнительно вводят гелеобразователь при следующем соотношении компонентов, мас.%:This goal is achieved by the fact that in a known composition, including Miramistin and water, additionally injected gelling in the following ratio, wt.%:
1,0 -50,0Gelling agent 1,0 -50,0
Вода До 100Water Up to 100
3%.At the same time, as a gelling agent, for example, modified cellulose derivatives or methylsilicic acid hydrogel (enterosgel) are used in various concentrations. In the case of tylose, a concentration of from 1 to 3% is preferred.
Increasing the concentration of miramistin provides an effective antimicrobial barrier. In addition, with the joint use of Miramistin and a gelling agent, the synergistic reparative properties of the composition increase, while increasing its viscosity ensures a long-lasting effect.
1.To illustrate the invention, the compositions were prepared according to the table. one.
Burn wound formulations
Legend: W - liquid; LH - liquid gel; MG - soft gel; TG is a solid gel.
As can be seen from the table. 1, compounds 4 and 7 are solid gels, the use of which is difficult, and therefore they were not used in the work.
2.For the treatment of burn wounds (PR) the compositions according to the table. 1 and the prototype was applied to the wound surface using dressings or tampons, which were changed daily. At the same time, the rate of wound cleansing from purulent discharge (GO), the period of activation of reparative processes and the appearance of islands of epithelialization, the total duration of treatment were noted. The results are presented in table. 2
In addition, composition 5 was used in the preparation and transplantation of skin autotransplants in 6 patients and received a complete engraftment of the transplanted skin graft 3-5 days earlier than without the use of the claimed composition.
Results of treatment of deep burn wounds in humans
In 3 patients with whom the transplant was performed without using the claimed composition, it began to be applied after the appearance of signs of partial melting of autografts, which led to the rapid elimination of HO and the survival of the remaining areas of the transplanted skin.
In no case was there any adverse effect of the composition or complaints of patients to discomfort during their application. On the contrary, the gel-like composition when applied to the wound surface caused a decrease in pain symptoms and a burning sensation.
As can be seen from the table. 2, the effectiveness of composition 1 is practically the same as that of the prototype, which is associated with the same concentration of miramistin and insufficient concentration of the gelling agent.
В то же время эффективность состава 2 значительно эффективнее прототипа, что при равной концентрации мирамистина подтверждает синергизм его с гелеобразователем, который без мирамистина такого эффекта не дает (состав 8).At the same time, the effectiveness of composition 2 is much more efficient than the prototype, which, with an equal concentration of miramistin, confirms its synergy with a gel former, which does not produce such an effect without miramistin (composition 8).
The inventive composition can be produced by the pharmaceutical industry, since all components are industrial registered products:
