Abstract
Description
The invention relates to the field of medicine, veterinary medicine and the pharmaceutical industry and can be used to treat burn wounds, including deep ones and using autodermatoplasty.
Known use for the treatment of deep burn wounds of drugs of various chemical nature. According to the Register of Medicinal Products of Russia (Moscow, "RLS-2000", 1999) for these purposes use: Betadine ointment (p.776), the disadvantage of which is irritation of the skin and mucous membranes at the sites of application due to the release of iodine; Levovinisol (p.847) - an aerosol preparation of a complex composition, which also causes a burning sensation in places of applications; Lorinden ointment (p. 876) can cause contact eczema, allergic reactions, atrophy of the skin and subcutaneous tissue; Oksinart ointment is contraindicated in viral infections; Argosulfan ointment - (p.874) can cause leukopenia and allergies; Limexide solution can cause vomiting, bronchospasm, itching, is contraindicated in case of impaired liver or kidney function, angina pectoris, atherosclerosis, stroke, pregnancy. In addition, all these drugs are ineffective when used for the treatment of burn wounds using autodermatoplasty.
Closest to the claimed is the drug Miramistin (p.656), representing a 0.01% aqueous solution of myristamidopropyl dimethylbenzylammonium chloride. The drug is used in the form of dressings and tampons in the treatment of superficial and deep burn wounds. Its disadvantages include a slight burning sensation that passes on its own after 10-15 seconds, as well as the fact that dressings and tampons quickly dry and stick to the wound surface, which requires constant additional processing of the dressing from the outside. The drug is ineffective in the preparation and conduct of autodermatoplasty, because quickly drains from the surface of the graft and does not protect it from suppuration; in addition, the concentration used is insufficient to provide a reliable antimicrobial effect.
The aim of the present invention is to expand the range of products for the treatment of burn wounds and its effective use in the practice of autodermatoplasty.
3%.This goal is achieved by the fact that in a known composition, including miramistin and water, an additional gelling agent is introduced in the following ratio of components, wt.%:
Miramistin - 0.01-1.0
Gelling agent - 1.0-50.0
Water - Up to 100
In this case, as a gelling agent, for example, modified cellulose derivatives or methylsilicic acid hydrogel (Enterosgel) in various concentrations are used. In the case of tylose, a concentration of from 1% to 3% is preferred.
Increased miramistin concentration provides an effective antimicrobial barrier. In addition, with the combined use of miramistin and a gelling agent, the synergistically reparative properties of the composition increase, and an increase in its viscosity provides a long-lasting effect.
1. To illustrate the invention, the compositions of table 1 were prepared.
As can be seen from the table. 1,
2. For the treatment of burn wounds (OR), the compositions of Table 1 and the prototype were applied to the wound surface using dressings or tampons, which were changed daily. At the same time, the rate of wound cleansing from purulent discharge (GO), the period of activation of reparative processes and the appearance of epithelialisation islands, and the total duration of treatment were noted. The results are presented in table.2.
In addition,
In 3 patients who underwent transplantation without using the claimed composition, they started to use it after the signs of partial melting of autografts appeared, which led to the rapid elimination of GO and preservation of the viability of the remaining sections of the transplanted skin.
In no case was there any side effect of the composition or complaints of patients about unpleasant sensations when applied. On the contrary, the gel-like composition when applied to the wound surface caused a decrease in pain symptoms and a burning sensation.
As can be seen from table 2, the effectiveness of
At the same time, the effectiveness of
