Description
Invenţia se referă la medicină, în special la stomatologie şi poate fi utilizată pentru tratamentul afecţiunilor parodontale. The invention relates to medicine, in particular to dentistry, and can be used for the treatment of periodontal diseases.
Se cunoaşte metoda de tratament al afecţiunilor parodontale care constă în aceea că asupra regiunii afectate se acţionează cu unde de 0,85...0,95 µm, puterea de 0,5...4,0 W, frecvenţa de 0,1...3 kHz (câte 2 min, 3...5 proceduri) în combinare cu terapia medicamentoasă, inclusiv antibiotice [1]. Dezavantajul acestei metode constă în lipsa direcţionării patogenice a terapiei antibacteriale în ce priveşte combinarea remediilor din diferite grupe, ţinând cont de sensibilitatea unor importante tipuri de bacterii parodontopatogene. Totodată, are loc distrugerea în masă necontrolată şi nediferenţiată a microflorei cavităţii bucale şi în primul rând a microflorei saprofite. În afară de aceasta, terapia cu antibiotice duce la apariţia rezistenţei faţă de agenţi patogeni, ceea ce contribuie la generalizarea procesului inflamator (M.M. Pojariţcaia, L.B. Morozova, G.M. Melniciuc, C.C. Melniciuc, 1994). The disadvantage of this method is the lack of pathogenetic targeting of antibacterial therapy in terms of combining remedies from different groups, taking into account the sensitivity of important types of periodontal pathogenic bacteria. At the same time, there is an uncontrolled and undifferentiated mass destruction of the oral cavity microflora and primarily of the saprophytic microflora. În calitate de cea mai apropiată soluţie se prezintă metoda de tratament al afecţiunilor parodontale care constă în aceea că se administrează terapia medicamentoasă, după care asupra ţesuturilor parodonţiului se acţionează cu unde electromagnetice de frecvenţă înaltă (EFÎ). Totodată terapia medicamentoasă include administrarea preparatelor cu calciu (Biofit, Calciu) şi a vitaminelor (Complevit) după schemele standarde. Terapia EFÎ se realizează cu utilizarea aparatului Амфит-0,2/10-01 cu oscilaţii electromagnetice în diapazonul 53,57...78,33 GHz cu puterea specifică totală a zgomotului după diapazonul lungimilor de undă în limitele 1,0...4,0 mW/cm2 şi neuniformitatea distribuirii spectrului de zgomot după frecvenţă ce nu depăşeşte ±3 dB, care funcţionează după principiul legăturii inverse la frecvenţe de rezonanţă. Conform invenţiei, metoda revendicată constă în aceea că se înlătură factorii traumatizanţi locali, apoi zilnic, pe parcursul a 8…12 zile, se acţionează consecutiv cu radiaţie electromagnetică în diapazonul de 53,57…60,12 GHz, câte 2..3 min asupra punctelor de ieşire ale ramurilor II şi III ale nervului trigemen şi câte 10…15 min asupra regiunii parodonţiului afectat, concomitent, în regiunea joncţiunii muco-gingivale, în primele 3 zile, apoi peste fiecare 2 zile se administrează Traumel C - 2,2 ml şi Mucosa compositum - 2,2 ml, 5 injecţii, iar intramuscular odată în 3 zile se administrează Cerebrum compozitum H - 2,2 ml, 3 injecţii, totodată per os se administrează câte 10 picături de Verticogel, de 2…3 ori, zilnic, iar local se fac aplicaţii cu un amestec care conţine 10…15 g de Enterosgel şi 2,2 ml de Traumel C, câte 15…20 min, timp de 14…16 zile. Administrarea preparatului Traumel C în regiunea joncţiunii muco-gingivale contribuie la încetarea hemoragiilor gingivale datorită sporirii tonusului vaselor şi micşorării permeabilităţii lor, ceea ce de asemenea contribuie la micşorarea edemaţierii ţesuturilor moi. Prin metoda revendicată au fost supuşi tratamentului 30 de bolnavi (13 bărbaţi şi 17 femei cu vârstele cuprinse între 25 şi 55 ani) cu parodontită cronică generalizată de grad uşor şi mediu. În grupa de control au fost selectaţi bolnavi de aceeaşi vârstă cu diagnostic analogic, care au fost trataţi după metoda obişnuită de tratare a parodontitei. 30 patients (13 men and 17 women aged 25 to 55) with mild to moderate generalized chronic periodontitis were treated using the claimed method. Patients of the same age with an analogous diagnosis were selected in the control group and were treated according to the usual method of treating periodontitis.
Rezultatele tratamentului au fost evaluate după datele clinice în dinamică: examinarea clinică, determinarea adâncimii pungilor gingivale, indicele igienic Fedorov-Volodkin (II), indicele papilar-marginal-alveolar (PMA), testul Shiller-Pisarev, indicele hemoragic (IH), determinarea rezistenţei funcţionale a capilarelor după V.I. Kulajenko. Tratarea bolnavilor prin metoda propusă a permis în scurt timp (2 şedinţe în grupa de bază faţă de 5...6 şedinţe în grupa de control) de a obţine un efect terapeutic stabil. Ameliorarea clinică a fost confirmată de dinamica pozitivă a modificării indicilor clinici. Pe parcursul observaţiei (6 luni) la bolnavii din grupa de bază nu s-a menţionat acutizarea bolii. În grupa de control la 38% din persoanele supuse studiului s-au menţionat recidive ale bolii deja la 1,5...2 luni după tratamentul efectuat. Analiza indicilor clinici la 5 zile după tratamentul efectuat conform invenţiei propuse a atestat ameliorarea stării ţesuturilor parodontale, ce s-a manifestat prin diminuarea proceselor inflamatorii în gingii. La 6 luni după tratament s-a observat o micşorare însemnată a adâncimii pungilor gingivale, a dispărut reacţia inflamatorie în gingie. Bolnavul C, 45 ani, s-a adresat cu acuze la apariţia unor hemoragii gingivale periodice, halenă, mobilitatea dinţilor, îndeosebi a celor frontali, dezgolirea coletului dinţilor şi o sensibilitate sporită în sectorul frontal. Din anamneză, simptomele enumerate au apărut cu aproximativ 9 ani în urmă. La medic nu s-a adresat până acum. În ultimii 3 ani mobilitatea dinţilor a devenit mai pronunţată, la fel şi hemoragiile gingivale. La examenul obiectiv s-au depistat pungi parodontale cu adâncimea de 4 mm, din care se elimina un exudat purulent seros. Mobilitatea dinţilor era de gradul II. S-a observat dezgolirea coletelor şi parţial a rădăcinilor dinţilor. Înălţimea apofizelor alveolare s-a lăsat neuniform în jos până la 1/3... 1/2 din lungimea rădăcinilor. Pacienta R., 38 ani, s-a adresat cu diagnosticul: parodontită cronică generalizată de grad uşor. La prima vizită au fost eliminaţi factorii traumatizanţi locali. Au fost îndepărtate depunerile gingivale şi subgingivale, a fost familiarizat pacientul cu regulile ce ţin de igiena individuală şi de alegerea mijloacelor de îngrijire a cavităţii bucale, s-a efectuat selectiv şlefuirea dinţilor. Apoi a fost efectuată o şedinţă de terapie EFÎ pe punctele de ieşire ale ramurilor II, III şi în proiecţia nervului trigemen, după aceasta în regiunea parodonţiului timp de 15 min. S-a administrat o injecţie de Traumel C 2,2 ml şi Mucosa compositum 2,2 ml în regiunea joncţiunii muco-gingivale, iar intramuscular Cerebrum compositum H. 2,2 ml. Pacientei i s-a administrat per os Verticogel, câte 10 picături de 3 ori pe zi. Local, pe regiunea afecţiunii parodontale, pacienta zilnic, de 2 ori pe zi, timp de 15 min a făcut aplicaţii cu Enterosgel cu adăugarea a 2,2 ml de Traumel C. Apoi în fiecare zi, pe parcursul următoarelor 10 zile, pacientei i s-a aplicat terapie EHF după metoda descrisă. I s-au administrat încă 4 injecţii de Traumel C 2,2 ml şi Mucosa compositum 2,2 ml, în regiunea joncţiunii muco-gingivale, în fiecare zi în primele 2 zile, apoi peste două zile. După care i s-au administrat intramuscular 2 injecţii de Cerebrum compositum H. 2,2 ml în a 4-a şi a 7-a zi de tratament, iar per os câte 10 picături de Verticogel de 3 ori pe zi. Local, pe regiunea afectată a parodonţiului, pacienta a aplicat 10 g de Enterosgel cu adăugarea a 2,2 ml de Traumel C, zilnic, de 2 ori pe zi, timp de 15...20 min. Ameliorarea stării a fost observată din a 5-a zi, care a fost confirmată de modificarea tabloului clinic din cavitatea bucală. La sfârşitul tratamentului s-a micşorat semnificativ edemul, a dispărut cianoza papilelor, s-a micşorat adâncimea pungilor parodontale. La fel au dispărut acuzele pacientei cu privire la hemoragia gingivală, iritaţia, durerea acută pulsativă din gingii. Clinic s-a observat lipsa edemului papilelor dinţilor şi a cianozei marginii gingivale, precum şi ameliorarea indicilor de laborator, în particular, majorarea indicelui activităţii antioxidante generale a salivei, majorarea nivelului lizozimei şi al imunoglobulinei A secretorii. Analizând rezultatele cercetării putem trage concluzia că metoda propusă este comodă în aplicare, uşor suportabilă de către pacienţi, nu are acţiuni secundare şi contraindicaţii. After which she was administered 2 intramuscular injections of Cerebrum compositum H. 2.2 ml on the 4th and 7th days of treatment, and per os 10 drops of Verticogel 3 times a day. Locally, on the affected region of the periodontium, the patient applied 10 g of Enterosgel with the addition of 2.2 ml of Traumeel C, daily, 2 times a day, for 15...20 min. Improvement of the condition was observed from the 5th day, which was confirmed by the change in the clinical picture in the oral cavity. At the end of the treatment, the edema significantly decreased, the cyanosis of the papillae disappeared, the depth of the periodontal pockets decreased. The patient's complaints about gingival bleeding, irritation, acute pulsating pain in the gums also disappeared. Clinically, the absence of edema of the dental papillae and cyanosis of the gingival margin was observed, as well as the improvement of laboratory indices, in particular, the increase in the index of general antioxidant activity of saliva, the increase in the level of lysozyme and secretory immunoglobulin A. Analyzing the research results, we can conclude that the proposed method is convenient to apply, easily tolerated by patients, and has no side effects or contraindications.
