Abstract
예시적인 실시예에 따른 종양 혈관 파괴용 약학 조성물은 알칼리성 5-플루오로우라실 및 니트릭 옥사이드 생성억제제를 포함할 수 있다. 예를 들어, 알칼리성 5-플루오로우라실 및 니트릭 옥사이드 생성억제제를 포함하면, 종양 혈관내피 세포의 사멸효과와 종양 혈관의 폐색을 함께 제공하여 우수한 항암 효과를 제공할 수 있다.
Description
본 발명은 종양 혈관 파괴용 약학 조성물에 관한 것으로, 의료 분야 및 제약 분야 등에서 사용될 수 있다.
5-플루오로우라실(5-Fluorouracil, 5-FU)은 여러 암의 치료에 광범위하게 사용되는 항암화학요법제이다. 5-플루오로우라실은 인체에 존재하는 우라실(uracil)의 유사체로 주로 티미딜산 합성효소(thymidylate synthase)의 억제에 의한 DNA 합성 및 수선 저해, RNA 기능의 억제로 항암효과를 나타낸다.
5-플루오로우라실은 주로 정맥투여로 병원에서 직접 투여해야하고, 혈전증 등의 부작용도 있어 최근에는 경구용 5-FU 전구약물(prodrug) 형태로 개발되고 있다.
5-플루오로우라실은 일반적인 항암제와 같이 세포내 핵산의 합성을 억제하거나 핵산에 직접 결합하여 그 기능을 손상시킴으로써 항암 효과를 나타낸다. 그러나, 암세포에만 선택적으로 작용하는 것이 아니라, 정상세포, 특히 세포분열이 활발한 조직세포에도 손상을 입히기 때문에 골수독성(myelosuppression)이나 위장관내에서 인산화에 의한 위장관 독성이 빈번하게 발생한다. 또한, 심장독성은 빈도는 낮으나 생명을 위협할 수 있는 심각한 부작용이 1.5 내지 18%까지 보고되고 있다.
따라서, 기존 5-플루오로우라실의 항암 효과를 유지하면서 부작용을 감소시킨 항암제에 대한 연구가 필요하다. 예를 들어, 한국 공개특허 제10-2013-0074325호는 5-플루오로우라실을 포함하는 암 치료용 약학적 조성물을 개시하고 있지만, 부작용 감소와 우수한 항암효과를 동시에 제공하지 못한다.
본 발명의 목적은 우수한 항암 효과 및 부작용을 감소시킨 종양 혈관 파괴용 약학 조성물을 제공하는 것이다.
예시적인 실시예에 따른 종양 혈관 파괴용 약학 조성물은 알칼리성 5-플루오로우라실 및 니트릭 옥사이드 생성억제제를 포함할 수 있다.
일 실시예에 있어서, 상기 니트릭 옥사이드 생성억제제는 L-NMMA(NG-Monomethyl-L-arginine), L-NAME(NG-Nitro-L-arginine methyl ester), L-NA(Nitro Arginine) 및 7NI(Nitroindazole)에서 선택되는 하나 이상을 포함할 수 있다.
일 실시예에 있어서, 상기 니트릭 옥사이드 생성억제제는 L-NMMA(NG- Monomethyl-L-arginine)을 포함할 수 있다.
일 실시예에 있어서, 상기 알칼리성은 pH 8 내지 9일 수 있다.
일 실시예에 있어서, 상기 알칼리성 5-플루오로우라실(5-FU)은 알칼리 용매에 5-플루오로우라실이 용해된 형태일 수 있다.
일 실시예에 있어서, 상기 알칼리성 5-플루오로우라실의 농도는 전체 조성물에서 0.1 내지 600 mM일 수 있다.
일 실시예에 있어서, 상기 알칼리성 5-플루오로우라실은 종양 혈관내피세포에서 트롬보스폰딘-1 단백질 발현을 증가시켜 혈관내피세포를 사멸시킬 수 있다.
일 실시예에 있어서, 상기 니트릭 옥사이드 생성억제제의 농도는 전체 조성물에서 0.1 내지 300 mM일 수 있다.
일 실시예에 있어서, 상기 니트릭 옥사이드 생성억제제는 종양 혈관을 수축시키고, 5-플루오로우라실에 의해 손상된 종양 혈관 내부에 혈전증 발생을 촉진하여 혈관을 폐쇄시킬 수 있다.

예를 들어, 알칼리성 5-플루오로우라실을 종양의 저부에 주사하면 혈관내피세포의 사멸(apoptosis)을 효과적으로 유도하여 종양을 파괴시킬 수 있다. 이는 종양자체가 아닌 종양에 혈액을 공급하는 혈관의 파괴에 의한 결과이기 때문에 다른 암종에도 적용할 수 있다.
본 발명에 따른 종양 혈관 파괴용 약학 조성물은 알칼리성 5-플루오로우라실 및 니트릭 옥사이드 생성억제제를 포함하여 종양 혈관내피세포 사멸효과와 종양 혈관 폐색을 함께 제공하여 우수한 항암 효과를 제공할 수 있다.
본 발명에 따른 종양 혈관 파괴용 약학 조성물은 항암제 부작용도 감소시킬 수 있다.

예시적인 실시예에 따른 종양 혈관 파괴용 약학 조성물(이하, 조성물이라 함)은 알칼리성 5-플루오로우라실(5-FU) 및 니트릭 옥사이드 생성억제제를 포함한다. 따라서, 암세포 근처 혈관내피 세포의 사멸효과와 혈관의 폐색을 향상시킬 수 있어 우수한 항암 효과를 제공할 수 있다.
이하, 도면 및 실시예들을 참조하여 본 발명의 예시적인 실시예들에 따른 조성물에 대해 상세히 설명한다. 다만, 도면 및 실시예들은 예시적인 것에 불과하며, 본 발명이 이에 제한되는 것은 아니다.
예시적인 실시예에 따른 조성물은 알칼리성 5-플루오로우라실(5-FU)을 포함할 수 있다.
일 실시예에 있어서, 상기 알칼리성 5-플루오로우라실은 종양 혈관내피세포에서 트롬보스폰딘-1(thrombospondin-1, TSP-1) 단백질 발현을 증가시켜 혈관내피세포를 사멸시킬 수 있다.
5-플루오로우라실이 알칼리성 용매에 혼합되면 시너지를 발휘할 수 있다. 예를 들어, 중성 용매 또는 산성 용매보다 알칼리성 용매에 혼합될 때 우수한 항암효과를 제공할 수 있다. 예를 들어, 알칼리성 5-플루오로우라실이 암 조직내 혈관내피세포를 타겟하여 세포사멸을 유도할 수 있다. 예를 들어, 종양 조직의 저부에 알칼리성 5-플루오로우라실을 주입하면 종양에 혈액을 공급하는 혈관내피세포의 사멸을 유도하여 혈관이 파괴되어 종양의 괴사시킬 수 있다. 이 경우, 5-플루오로우라실의 pH가 혈관내피세포의 사멸 유도에 중요한 요소이며, pH가 약알칼리 예를 들어, 8.4 내지 9.0일때 중성 또는 산성의 5-플루오로우라실 보다 혈관내피세포의 사멸을 의미있게 증가시킬 수 있다.
다시 말해, 알칼리성 5-플루오로우라실은 암세포 근처의 혈관을 파괴하고 신생혈관합성을 억제하여, 암세포로의 영양분 또는 산소의 공급을 차단 및 억제함으로써 항암 효과를 나타낼 수 있다. 이는 종양자체가 아닌 종양에 혈액을 공급하는 혈관의 파괴에 의한 결과이기 때문에 다른 암종에도 적용할 수 있다. 또한, 항암제 부작용도 감소시킬 수 있다.
일 실시예에 있어서, 상기 알칼리성 5-플루오로우라실은 5-플루오로우라실이 알칼리성 용매에 용해된 형태일 수 있다. 상기 알칼리성 용매는 예를 들어, 알칼리수, 생리식염수 등일 수 있다.
일 실시예에 있어서, 상기 알칼리성은 pH가 8.0 내지 9.0일 수 있다. 예를 들면 8.0 내지 9.0, 8.2 내지 9.0, 8.4 내지 9.0, 또는 8.4 내지 8.8 일 수 있다. 다만 이에 제한되는 것은 아니다.
일부 실시예에 있어서, 알칼리성 5-플루오로우라실 농도는 당업자에 의해 제한없이 선택될 수 있다. 예를 들면 전체 조성물에서 0.1 mM 내지 600 mM, 0.1 mM 내지 500 mM, 0.5 mM 내지 390 mM, 1 mM 내지 350 mM, 10 mM 내지 300 mM, 50 mM 내지 250 mM 또는 100 mM 내지 200 mM일 수 있다. 다만 이에 제한되는 것은 아니다.
예시적인 실시예에 따른 조성물은 알칼리성 5-플루오로우라실 및 니트릭 옥사이드 생성억제제를 포함할 수 있다.
상기 니트릭 옥사이드 생성억제제는 예를 들어, L-NMMA(NG-Monomethyl-L-arginine), L-NAME(NG-Nitro-L-arginine methyl ester), L-NA(Nitro Arginine), 7NI(Nitroindazole)등 일 수 있다.
니트릭 옥사이드(Nitrile Oxide)는 혈관내피세포에서 NO synthase에 의해 합성 및 분비되어 혈관을 확장시키고, lipopolysaccharide (LPS), angiotensin II, caspase-3 overexpression 및 TNF-alpha와 같은 외부자극에 의해 혈관내피세포가 세포사멸로 진행되는 것을 억제하여 세포를 보호한다.
일부 실시예에 있어서, 상기 니트릭 옥사이드 생성억제제는 종양 혈관을 수축시키고 5-FU에 의해 손상된 종양 혈관 내부에 혈전증 발생을 촉진시킬 수 있다. 예를 들어, 알칼리성 5-플루오로우라실에 의해 종양 혈관내피세포가 손상되거나 파괴되어 종양 혈관내부에 혈전이 생길 수 있다. 니트릭 옥사이드 생성억제제는 이러한 종양 혈관의 페쇄를 촉진하여 종괴 괴사를 더 효과적으로 유도할 수 있다.
일부 실시예에 있어서, 상기 니트릭 옥사이드 생성억제제는 바람직하게 L-NMMA(NG-Monomethyl-L-arginine)일 수 있다.
L-NMMA는 nonspecific NO synthase 억제제로서 혈관으로 투여되면 효과적으로 혈압을 상승시킨다. 생리학적 조건에서 L-NMMA에 의한 NO 합성효소 단독의 억제는 생체 내에서 혈소판 활성화를 초래하지 않으나 종양 혈관의 내피세포에 손상이 발생하면 혈소판 활성화 및 혈전증을 유발할 수 있다. 다시 말해, 손상된 혈관 내피세포가 혈전이 생기도록 유도할 수 있다. 따라서, L-NMMA를 5-플루오로우라실과 함께 사용하면, 종양 혈관내피세포 사멸효과와 종양 혈관 폐색을 함께 제공하여 우수한 항암 효과를 제공할 수 있다.
예를 들어, 상기 니트릭 옥사이드 생성억제제 농도는 전체 조성물에서 0.1 mM 내지 300 mM, 0.1 mM 내지 250 mM, 1 mM 내지 200 mM, 10 mM 내지 200 mM일 수 있다. 다만 이에 제한되는 것은 아니다.
다른 예를 들면, L-NMMA는 약물을 처리하는 대상의 무게를 기준으로 0.5 mg/kg 내지 20 mg/kg, 1 mg/kg 내지 10 mg/kg, 2 mg/kg 내지 8 mg/kg, 또는 4 mg/kg 내지 6 mg/kg 이 되도록 포함될 수 있다. 다만, 이에 제한되는 것은 아니다.
일 실시예에 있어서, 상기 조성물은 베바시주맙, 카프르산 또는 이의 생리학적 허용 가능한 염, 및 폴록사머에서 선택된 하나 이상을 더 포함할 수 있다.
베바시주맙(Bevacizumab)은 혈관생성을 촉진하는 혈관내피성장인자(vascular endothelial growth factor, VEGF)를 억제하기 위한 재조합 인간화 단클론 항체이다.
통상적으로 베바시주맙은 단독 또는 다른 약물과 병용하여 항암제로 사용될 수 있다. 기존에는 주로 정맥 주사로 투여되었으며, 진행성 위암의 경우 fluoropyrimidine-cisplatin 치료와 병행했을 때 무진행 생존기간(progression free survival)과 전체반응률(overall response rate)에 호전을 보인다고 알려져있다.
그러나, 일 실시예에 있어서, 상기 조성물에 베바시주맙을 추가할 수 있고, 서방형으로 제공할 수 있다. 이에 암세포의 신생혈관생성을 억제하므로 종양내에 분포한 혈관을 효과적으로 소멸시킬 수 있다.
카프르산 또는 이의 생리학적 허용 가능한 염, 및 폴록사머는 독성이 낮아 생체적합적이고, 생체내 안정성이 더 우수하다. 예를 들어, 카프르산 또는 이의 생리학적 허용 가능한 염, 및 폴록사머가 혼합됨으로써, 점도를 증가시키고 폴록사머의 졸-겔 전이온도를 높일 수 있다. 예를 들면 상기 졸-겔 전이온도는 상온과 체온 사이, 예를 들면 20 내지 40℃, 25 내지 37℃일 수 있다. 상기 범위에서 상온에서 졸 상태로 보관, 수송 및 사용이 편리하며, 체내에서 작용할 때는 겔 상태로 조직(tissue)에서의 확산이 용이하다.
일부 실시예에 있어서, 카프르산 및 폴록사머의 중량비는 0.25 내지 3.4: 30, 1 내지 3.4: 30, 2 내지 3: 30일 수 있고, 구체적으로 상기 카프르산 및 폴록사머의 중량비는 2.75 내지 3.3: 30, 2.8 내지 3.2: 30일 수 있다. 상기 범위에서 전달체의 졸-겔 전이가 25 내지 36℃ 사이에서 일어나기 쉽다.
상기 카프르산(capric acid)는 하기 화학식 1로 표시되는 화합물로 포화지방산의 한 종류이다.

상기 폴록사머의 분자량은 예를 들면 중량 평균 분자량이 1,000 내지 100,000, 10,000 내지 100,000, 10,000 내지 20000, 1,0000 내지 15,000일 수 있다. 상기 분자량은 전달 대상이 되는 물질에 따라 당업자가 적절히 선택할 수 있다.
폴록사머는 일반적으로 폴록사머의 근사 분자량 및 폴리옥시에틸렌의 함량 백분율을 나타내는 번호체계로 표시되며, 상표명인 플루로닉(pluronic)으로도 지칭된다. 예를 들면 Poloxamer 407과 상표명인 Pluronic F-127은 상호호환적으로 사용 가능하다.

상기 폴록사머는 계면활성제의 한 종류이며, 수성환경에서 미셀(micelle)을 형성해 물질을 로딩할 수 있다. 폴록사머로 구성된 하이드로겔은 매트릭스를 형성하여 물질을 국부적, 지속적으로 방출하는 전달체로 사용될 수 있다. 상기 폴록사머 하이드로겔은 폴록사머 간의 가교결합을 포함하는 것일 수 있다. 상기 폴록사머는 다양한 분자량 및 비율로 사용 가능하다.
일 실시예에 있어서, 상기 조성물은 공지된 항암제를 더 포함할 수 있다. 예를 들어, 세포독성항암제, 표적항암제 및 면역항암제에서 선택된 하나 이상을 포함할 수 있다.
세포독성항암제는 암세포의 대사경로에 개입하여 DNA 또는 RNA의 합성과 분열을 억제하거나, 또는 DNA 자체에 손상을 주어 세포의 사멸을 유도하는 항암제일 수 있다. 세포독성 항암제는 화학항암제, 화학약물 항암제를 의미할 수 있다. 예를 들어, 알킬화제, 백금화합물, 대사길항제, 식물유래 알칼로이드 등일 수 있다. 화학약물에 의해 항암효과를 제공하는 것이라면 이에 제한되는 것은 아니다.
예를 들어, 알킬화제는 다른 화합물에 알킬기 R-CH2를 도입시킬 수 있는 물질을 의미한다. 예를 들어, 알킬화제는 시클로포스파미드, 이포스파미드, 벤다무스틴 등일 수 있다.

표적항암제는 암 세포나 암 조직에서 특이적으로 변화되는 단백질이나 유전자를 표적으로 하여, 암의 성장 및 발생에 관여하는 분자 활동을 방해함으로써 항암 효과를 나타낼 수 있다. 예를 들어, 티로신 키나아제 억제제, PARP 억제제(poly-ADP ribose polymerase 억제제), CDK4/6 억제제(cyclin-dependent kinases 4/6 inhibitor), 항체-약물 접합체 (antibody-drug) 등일 수 있다.
면역항암제는 암 자체를 공격하는 기존 항암제와는 달리 인공면역 단백질을 체내에 주입하여 면역체계를 자극함으로써 면역세포가 선택적으로 암세포만을 공격하도록 유도하는 치료제일 수 있다. 예를 들어, 상기 면역항암제는 수동면역치료에 적용되는 면역체크포인트억제제, 면역세포치료제, 치료용 항체 등이 있고, 능동면역치료에 적용되는 암치료 백신, 면역조절제 등이 있다. 다만, 이에 제한되는 것은 아니다.

일부 실시예에 있어서, 상기 조성물은 서방성 제제일 수 있다.
혈관내피세포에 노출되는 시간이 길수록 혈관내피세포의 사멸효과가 더 크게 나타날 수 있다. 상기 조성물을 서방형 제제로 제공하기 위해, 상기 조성물은 카프르산 또는 이의 생리학적 허용 가능한 염, 및 폴록사머를 더 포함할 수 있다.
일 실시예에 있어서, 상기 조성물에 베바시주맙을 추가할 수 있고, 서방형으로 제공할 수 있다. 이에 암세포의 신생혈관생성을 억제하므로 종양내에 분포한 혈관을 효과적으로 소멸시킬 수 있다.
일부 실시예에 따른 조성물은 상기 종양 치료와 관련하여 동일 또는 유사한 기능을 나타내는 유효성분을 1종 이상 함유할 수 있다.
일부 실시예에 따른 조성물은 유효성분의 용해성 및/또는 흡수성을 유지하거나 증가시키는 화합물을 추가로 함유할 수 있다.
일 실시예에 있어서, 상기 종양은 고형암을 포함할 수 있다.

상기 조성물의 제형은 경구용 또는 비경구용 제형으로 제조할 수 있다. 예를 들면 상기 제형은 구강(oral), 직장(rectal), 비강(nasal), 국소(topical; 볼 및 혀 밑을 포함), 피하, 질(vaginal) 또는 비경구(parenteral; 근육내, 피하 및 정맥내를 포함) 투여에 적당한 제형일 수 있다. 또는 흡입(inhalation) 또는 주입(insufflation)에 의한 투여에 적당한 제형일 수 있다.
상기 조성물의 제형은 주사제일 수 있다. 생체 환경, 혈액 등에서 침전을 형성하지 않는 것으로서, 얇은 주사 바늘로도 투여가 가능하다.
상기 조성물의 제형은 주사제 제형인 경우 바람직하다.
일 실시예에 있어서, 상기 조성물은 종양 주위에 국소주사로 투여될 수 있다.
도 1을 참조하면, 예를 들면 내시경을 이용하여 암 저부에 인젝터(injector)를 통해 조성물이 전달될 수 있다. 구체적으로, 암의 저부(basal layer)를 향하여 암 바로 옆 정상위벽의 점막하층(submucosa zone)에 조성물을 주입할 수 있다. 암 조직에 혈액을 공급하는 혈관의 혈관내피세포로 전달되는 약물에 의해 혈관내피세포의 사멸효과 또는 신생혈관의 합성 억제 효과가 나타날 수 있다.
상기 국소주사는 일반적인 항암제의 정맥 투여와 비교하여 종양 주위 점막하층뿐만 아니라, 주위 임파선에도 많은 양의 약물을 전달할 수 있고, 전신투여와 비교하여 항암제의 부작용을 줄일 수 있다.
예시적인 실시예에 따른 상기 조성물은 약학적으로 유효한 양으로 투여될 수 있다. 유효용량 수준은 환자의 질환의 종류, 중증도, 약물의 활성, 약물에 대한 민감도, 투여시간, 투여 경로 및 배출 비율, 치료기간, 동시 사용되는 약물을 포함한 요소 및 기타 의학 분야에 잘 알려진 요소에 따라 결정될 수 있다.
예시적인 실시예에 따른 고형암 국소 투여제는 상기 조성물을 포함할 수 있다. 고형암의 구체적인 예시는 상술한 바와 동일하다.
일부 실시예에 있어서, 상기 조성물은 개별 치료제로 투여하거나 다른 치료제와 병용하여 투여될 수 있다. 항암 병용 제제에 포함되는 성분은 순차적 또는 동시에 투여될 수 있으며, 단일 또는 다중 투여될 수 있다. 상기한 요소들을 모두 고려하여 부작용 없이 최소한의 양으로 최대 효과를 얻을 수 있는 양을 투여하는 것이 중요하며, 이는 당업자에 의해 용이하게 결정될 수 있다.
예를 들어, 상기 조성물의 투여량은 환자의 체중, 연령, 성별, 건강상태, 식이, 투여시간, 투여방법, 배설률 및 질환의 중증도 등에 따라 그 범위가 매우 다양하며, 적정한 투여량은 예를 들면 환자의 체내에 축적된 약물의 양 및/또는 사용되는 본 발명의 전달체의 구체적 효능정도에 따라 달라질 수 있다. 예를 들면 체중 1 kg당 0.01㎍ 내지 1g 일 수 있으며, 일별, 주별, 월별 또는 연별의 단위 기간으로, 단위 기간 당 일회 내지 수회 나누어 투여될 수 있으며, 또는 인퓨전 펌프를 이용하여 장기간 연속적으로 투여될 수 있다. 반복투여 횟수는 약물이 체내 머무는 시간, 체내 약물 농도 등을 고려하여 결정된다. 질환 치료 경과에 따라 치료가 된 후라도, 재발을 위해 조성물이 투여될 수 있다.
이하, 본 발명을 구체적으로 설명하기 위해 실시예를 들어 상세하게 설명하기로 한다.
<실시예>
실시예 1:5-FU의 위암조직 사멸 작용기전(종양조직내의 신생혈관의 내피세포 독성) 확인
1) 5-FU에 의한 내피세포의 트롬보스폰딘-1(TSP-1)의 발현 증가
TSP은 여러 기능을 갖는 고분자로서 아형 중 TPS-1는 혈관형성 억제인자 역할을 한다. 이하, 5-FU의 농도에 따른 TSP-1의 발현의 변화를 확인하고자 한다.
HUVEC(human umbilical vein endothelial cell) 세포주에서 TSP-1 단백질의 발현을 확인하고자 웨스턴 블랏을 진행하였다.
배양접시에 키운 HUVEC 세포를 50mM Tris (pH 8.0), 150mM NaCl, 1% Nonidet p-40, 0.5% sodium deoxycholate(SDC), 0.1% sodium dodecyl sulfate(SDS), 1X Protease inhibitor cocktail을 함유한 완충액으로 세포를 용해시킨 후, 30분 동안 얼음으로 냉각시키고 14,000g을 10분 동안 원심 분리한 후 상층액을 분리하였다.
정량은 bicinchoninic acid protein assay kit (Pierce Chemical, Rockford, IL, USA)를 이용하였으며, 30㎍의 단백질용액을 10% SDS-polyacrylamide 겔에 전기영동 한 후 니트로셀루로스막에 이동시켰다. 전기영동된 막을 차단우유로 1시간 반응시킨 후, 면역 염색에 사용하였던 항체와 동일한 TSP-1 항체를 실온에서 1시간 반응시켰다. 0.1% Tween 20을 함유한 Tris 완충액에 15분 간격으로 세 번 세척한 후, 1차 항체에 대한 2차 항체에 실온에서 1시간 반응시키고, 화학발광제(Amersham Life Science, Arlington Heights, IL, USA)를 이용하여 검출하였다. 결과는 도 2에 나타내었다.
HUVEC에서 추출한 단백질을 양성 대조군으로 사용하였다. 5-FU의 농도와 적용시간에 따른 HUVEC세포의 TSP-1 발현 결과를 도 3에 나타내었다. 도 3에서 5-FU의 적용시간이 길수록, 5-FU의 약물농도가 높을수록 발현되는 TSP-1의 양이 증가되는 것을 확인할 수 있었다.
2) 5-FU에 의한 HUVEC의 LDH 방출 연구
일반적으로 TSP-1,2의 발현이 증가되면 endothelial cell의 apoptosis가 발생한다고 알려져 있다. 5-FU를 투여하였을 때 endothelial cell의 apoptosis가 증가하는지 확인하기 위해서 HUVEC의 LDH 방출연구를 실시하였다. 세포독성은 세포막 손상으로 인해 방출되는 LDH의 양으로 측정하였다. 방출된 LDH는 제조사의 프로토콜에 따라 LDH assay kit(EZ-LDH1000; DoGenBio, Seoul, Korea)로 정량하였다.
HUVEC 5×104 cells/well을 96-well 폴리스티렌 플레이트에 접종하고 37℃에서 24시간 동안 배양하였다. LDH 기질을 각 well에 첨가한 후, 37℃에서 추가로 10분 동안 배양하였다. 플레이트를 600g에서 5분 동안 원심분리하였다. 상층액(10 ㎕)을 배양 플레이트의 well에서 분석 플레이트로 옮겼다. LDH 반응 혼합물 시약(100 ㎕)을 연속적으로 분석 플레이트의 각 well에 있는 상등액에 연속적으로 첨가하였다. 반응 30분 후 마이크로플레이트 리더(Model 680; Bio-Rad)를 이용하여 450 nm 파장에서 흡광도를 측정하였다. HUVEC 세포에 대한 5-FU의 독성은 방출된 LDH의 양에 의해 결정되었다. HUVEC의 5-FU 농도 및 노출시간에 따른 LDH의 방출정도 측정한 결과를 도 4에 나타내었다.
도 4를 살펴보면, 방출된 LDH의 양은 처리 시간에 비례하여 1mM, 5mM 및 10mM 5-FU로 처리한 후 증가하였다 (P <0.05).
3) 5-FU의 pH에 따른 HUVEC 독성의 변화
HUVEC의 5-FU의 pH에 따른 HUVEC에 대한 세포사멸효과를 알아보기 위해 MTS cell proliferation assay를 진행하였다. HUVEC에 대한 다양한 pH상태의 5μM 5-FU의 세포독성은 CellTiter 96 AQueous One Solution Cell Proliferation Assay(Promega Corp., Madison, WI, USA)를 사용하여 측정하였다.
HUVEC 세포를 96-well의 플레이트에 5x103 cells/well의 밀도로 심고(seeding) 24시간 동안 배양하였다. 그 다음, 세포를 다양한 pH(6.4, 7.4, 8.4 및 9.0)에서 5-FU를 함유하는 DMEM 배지와 함께 24시간 동안 배양하였다. 그런 다음, 세포를 PBS로 2회 헹구고, 100㎕의 새로운 성장 DMEM 배지를 첨가하고, 세포를 추가로 48시간 동안 더 배양하였다. 배지를 100㎕의 새로운 성장 DMEM 배지로 교체한 후 20㎕의 3-(4,5-dimethylthiazol-2-yl)-5-(3-carboxymethoxyphenyl)-2-(4-sulfophenyl)-2H tetrazolium (MTS)(Promega Corp., Madison, WI, USA)를 첨가하였다. 추가 2시간 배양 후, BIO-RAD 모델 680 마이크로플레이트 판독기(Spark, TECAN, CA, USA)를 사용하여 490nm에서 흡광도를 측정하였다. HUVEC의 5-FU의 pH에 따른 MTS cell proliferation assay의 결과를 도 5에 나타내었다.
도 5를 살펴보면, 산성 또는 중성상태의 5-FU에 비해 약물의 알칼리가 강할수록 세포독성이 크게 나타났다.
4) 5-FU 5mg 일반형 누드마우스 주입 후 위암종양 내부의 CD31과 VEGF 염색
5-FU를 국소 주사한 위암종괴 내부의 혈관조직의 변화를 관찰하기 위하여 혈관염색을 위한 CD31과 VEGF면역조직화학적 검사를 시행하였다.
대조군은 누드마우스 등쪽에 위암조직을 이식 후 1cmx1cm 크기가 되었을 때 절제하여 사용하였으며, 5-FU 주입 한 위암종괴는 누드마우스 등쪽에 위암조직 이식 후 1cmx1cm 크기가 되었을 때 5-FU 5 mg을 종괴의 저부(base)쪽으로 주입한 뒤 일주일 후에 위암종괴를 절제하여 면역조직화학 염색을 실시하였다. 면역조직화학적 검사법은 다음과 같다.
보관된 포르말린 고정 파리핀 포매 위암 조직 중에서 조직미세배열(tissue microarray; TMA) 블록을 만들어 3.5㎛ 두께로 박절한 후 슬라이드에 일정 방향으로 부착시켜 건조시켰다. 면역화학 반응성을 증가시키기 위하여 10 mM/L citrate buffer (pH 6.0)에 담근 슬라이드를 전자렌지에서 15분간 microwave 처리하였다. 3'DW (3차 증류수)로 5분간 세척한 후 조직내 혈구에 존재하는 내인성 peroxidase 활성을 줄이기 위해 3% H2O2 MeOH 용액으로 실온에서 15분간 반응시켰다. 3'DW로 3분간 2회 세척한 후 비특이성 결합을 막기위해 우혈청알부민을 포함한 blocking 항체를 이용하여 실온에서 30분간 반응시키고 과도한 blocking 항체를 제거한 다음 1:100 희석한 VEGF 항체(sc-7269, Santa Cruz Biotechnology)를 실온에서 60분간 반응시켰다. TBST (Tris-buffered saline with 0.1% Tween® 20 Detergent)로 3분간 2회 세척한 다음 Detection kit Reagent1(HRP Polymer-anti-mouse/rabbit IgG)를 실온에서 15분간 반응시키고 TBST로 3분간 2회 세척했다. Detection kit Reagent 2 mixture로 실온에서 1분간 반응시키고 3'DW 로 1-2분간 세척하였다. 대조 염색은 Harris hematoxylin을 이용하였고 매 염색시 평활근의 혈관내피세포를 양성 대조로 삼았다.
신생혈관 형성을 평가하기 위하여 혈관내피세포의 표지자인 CD31에 대한 항체 (PECOM-1, sc-376764, Santa Cruz Biotechnology)를 VEGF와 같은 방법으로 시행하였다.
결과 분석은 면역염색 결과 조직 전체 면적 대비 CD31 혈관염색 및 VEGF로 염색된 암세포 면적의 비율을 계산하여 대조군과 5-FU 국소 주입군을 서로 비교하였다. 결과를 표 1에 기재하였다.
도 6은 대조군(왼쪽)과 5-FU(오른쪽) 주입 1주 후에 위암종괴를 CD31로 염색한 결과이다. 도 7은 대조군(왼쪽)과 5-FU(오른쪽) 주입 1주 후에 위암종괴를 VEGF로 염색한 결과이다.

도 6을 참조하면, 면역화학 염색 결과 CD31은 혈관내피세포의 세포질이 갈색으로 염색되었다. 5-FU 주입군의 종괴 내부에 대조군과 비교했을 때 괴사된 조직의 분포가 증가되어 나타났으며, CD 31의 염색된 면적 비율도 낮게 나타났다.
도 7을 참조하면, VEGF는 주로 암세포의 세포질에 과립상으로 국소적으로 염색되었으며, 대조군과 비교했을 때 5-FU 주입군에서 염색면적이 작게 확인되었다. 따라서, 5-FU 주입된 암종괴 내부에 대조군과 비교하여 혈관분포가 감소하였고, VEGF 분비 정도도 감소된 것으로 나타났다. 이는 5-FU 주입이 혈관내피세포에 손상을 유도하여 혈관분포가 감소하였고, 신생혈관의 재생도 방해하여 암종괴 괴사가 유도되어 종괴크기의 감소를 초래했다고 볼 수 있다.
표 1을 참조하면, 5-FU 투여 후 혈관부분을 나타내는 CD31 및 VEGF의 염색된 면적이 대조군에 비해 작게 나타났다.
5) 누드마우스 등에 사람위암세포를 이종이식 후 항암약물의 반응을 보는 in-vivo 실험
실험에 사용할 동물은 오리엔트에서 생산하는 생후 5주된 체중이 평균 30gram 내외의 male nude mouse(Crj:BALB/c-nu/nu mice, male) 6 마리이며, 연구소에서 일주일간 검수기간을 거친 후 사용하였다. 누드마우스 한 마리당 사람의 위암세포 5x106cells/100㎕(PBS)을 등쪽 피하지방층에 이식하였다. 종양의 크기를 주기적으로 측정하여 직경 1cm에 이르게 되면 약물 투여실험을 실시하였다.
실험동물은 두 군으로 나눈다. 한 군은 대조군으로 생리식염수만 투여를 받게되며, 다른 한군은 5mg pH 8.4의 5-FU 약물을 종양 바로 옆에 0.2 cc, 일주일 간격으로 3번 주입한다. 한달 뒤 등쪽 표피에 자라는 종양의 장경과 단경을 측정하여 약물투여 전, 후의 크기를 mean tumor volume=(장경×단경2)/2(mm3)로 측정하여 비교분석하였다. 결과는 도 8 및 도 9에 나타내었다.
도 8에서는 종양의 크기는 주사 전에 비해 약물 처리후 4주 후에 감소하였다 (P<0.05).
도 9에서는 PBS를 투여한 대조군에 비해, 알칼리성의 5-FU를 투여한 경우 종양의 크기가 감소하였다(p<0.05).
6) 5-FU 피하조직에 주입 후 피하 근육층에 나타나는 병리현상 관찰
pH 8.4의 5-FU 5mg을 종양주변에 주사했을 때 주변조직에 나타나는 병리현상을 관찰하기위해 누드마우스에 약물을 국소 주입 후 24시간 지난 뒤 근육을 포함하여 조직을 채취하여 H&E 염색을 한 뒤 근육층에 나타나는 degeneration, necrosis, inflammation and fibrosis 여부를 관찰하였다. 각각의 병리변화의 정도는 0 = absent, 1= mild, 2 = moderate, 3 = severe로 수치화 하여, 도 10a 및 도 10b에 나타내었다. 도 10a는 40배율, 도 10b는 100배율로 확대한 것이다.
도 10a 및 도 10b를 살펴보면, 5-FU을 주입한 부위의 피하근육층의 병리적 변화를 관찰한 결과 어떠한 병리적 변화도 나타나지 않았다(score = 0).
실시예 2: L-NMMA의 위암 조직 사멸 촉진기전 확인
1) L-NMMA의 농도에 따른 HUVEC 독성의 변화
5-FU와 L-NMMA 농도에 따른 HUVEC에 대한 세포사멸효과를 알아보기 위해 MTS cell proliferation assay를 시행하였다.
HUVEC에 대한 다양한 L-NMMA 농도에서의 5μM 5-FU의 세포독성은 CellTiter 96 AQueous One Solution Cell Proliferation Assay를 사용하여 측정하였다. HUVEC 세포를 96-well의 플레이트에 5x103 cells/well의 밀도로 심고(seeding) 24시간 동안 배양하였다. 그 다음, 세포를 다양한 L-NMMA 농도에서 DMEM 배지와 함께 24시간 동안 배양하였다. 그런 다음, 세포를 PBS로 2회 헹구고, 100㎕의 새로운 성장 DMEM 배지를 첨가하고, 세포를 추가로 48시간 동안 더 배양하였다. 배지를 100 ㎕의 새로운 성장 DMEM 배지로 교체한 후 20㎕의 3-(4,5-dimethylthiazol-2-yl)-5-(3-carboxymethoxyphenyl)-2-(4-sulfophenyl)-2H tetrazolium (MTS)를 첨가하였다. 추가 2시간 배양 후, BIO-RAD 모델 680 마이크로플레이트 판독기(Spark, TECAN, CA, USA)를 사용하여 490nm에서 흡광도를 측정하였다.
결과는 도 11에 나타내었다. 도 11을 참조하면, L-NMMA 단독으로 투여시 세포생존율의 감소를 나타냈으며, 5-FU와 같이 투여시 추가적인 세포사멸효과를 확인하였다.
2) 누드마우스 등에 사람위암세포를 이종이식 후 항암약물의 반응을 보는 in-vivo 실험
실험에 사용할 동물은 오리엔트에서 생산하는 생후 5주된 체중이 평균 30g 내외의 male nude mouse(Crj: BALB/c-nu/nu mice, male) 2 마리이며, 연구소에서 일주일간 검수기간을 거친 후 사용하였다. 누드마우스 한 마리당 사람의 위암세포 5x106cells/100㎕(PBS)을 등쪽 피하지방층에 이식하였다. 종양의 크기를 주기적으로 측정하여 직경 1cm에 이르게 되면 약물 투여실험을 실시하였다 실험동물은 두 군으로 나눈다. 한 군은 대조군으로 생리식염수만 투여를 받게되며, 다른 한군은 5mg pH 8.4의 5-FU 약물과 L-NMMA 0.1mg을 종양 바로 옆에 0.2cc, 일주일 간격으로 3번 주입한다. 한달 뒤 등쪽 표피에 자라는 종양의 장경과 단경을 측정하여 약물투여 전, 후의 크기를 mean tumor volume = (장경Х단경2)/2(mm3)로 측정하여 비교분석하였다. 결과는 도 12, 도 13a 및 도 13b에 나타내었다.
도 12, 도 13a 및 도 13b를 참조하면, 5-FU과 L-NMMA 주입 후 종양의 크기가 현저하게 감소하였다. 5-FU만 주입한 경우보다 더 크게 감소하였다.
실시예 3: 베바시주맙의 위암조직 사멸효과 확인
1) 누드마우스 등에 사람위암세포를 이종이식 후 항암약물(베바시주맙)의 반응을 보는 in-vivo 실험
실험에 사용할 동물은 오리엔트에서 생산하는 생후 5주된 체중이 평균 30g 내외의 male nude mouse(Crj:BALB/c-nu/nu mice, male) 3 마리이며, 연구소에서 일주일간 검수기간을 거친 후 사용하였다. 누드마우스 한 마리당 사람의 위암세포 5x106cells/100㎕(PBS)을 등쪽 피하지방층에 이식하였다. 종양의 크기를 주기적으로 측정하여 직경 1cm에 이르게 되면 약물 투여실험을 실시하였다.
실험동물은 1mg 베바시주맙 약물을 종양 바로 옆에 0.2 cc, 일주일 간격으로 3번 주입한다. 한달 뒤 등쪽 표피에 자라는 종양의 장경과 단경을 측정하여 약물투여 전, 후의 크기를 mean tumor volume=(장경Х단경2)/2(mm3)로 측정하여 비교분석하였다. 결과는 도 14에 나타내었다.
도 14를 참조하면, 종양의 크기는 주사 전에 비해 약물 처리후 4주 후에 거의 변하지 않았다 (P> 0.05). 종양의 크기는 이후 두달동안 변하지 않았으며 석달째가 되서야 크기가 증가하기 시작하였다.
2) 누드마우스 등에 사람위암세포를 이종이식 후 항암약물(알칼리성 5-FU 및 베바시주맙)의 반응을 보는 in-vivo 실험
실험에 사용할 동물은 오리엔트에서 생산하는 생후 5주된 체중이 평균 30g 내외의 male nude mouse(Crj:BALB/c-nu/nu mice, male) 3 마리이며, 연구소에서 일주일간 검수기간을 거친 후 사용하였다. 누드마우스 한 마리당 사람의 위암세포 5x106cells/100㎕(PBS)을 등쪽 피하지방층에 이식하였다. 종양의 크기를 주기적으로 측정하여 직경 1cm에 이르게 되면 약물 투여실험을 실시하였다.
실험동물은 0.2mg 베바시주맙과 pH 8.4의 5-FU 5mg을 종양 바로 옆에 0.2cc, 일주일 간격으로 3번 주입한다. 한달 뒤 등쪽 표피에 자라는 종양의 장경과 단경을 측정하여 약물투여 전, 후의 크기를 mean tumor volume=(장경Х단경2)/2(mm3)로 측정하여 비교분석하였다. 결과는 도 15 및 도 16에 나타내었다.
도 15 및 도 16을 참조하면, 종양의 크기는 주사 전에 비해 약물 처리후 4주 후에 감소하였다(P<0.05). 종양의 소멸되는 패턴에 특이점 이 있었다. 전체적인 종양의 크기는 유지하다가 가운데부터 괴사가 진행되어 주변으로 퍼져나가는 결과를 확인하였다.
실시예 4: 서방형 5-FU sol-gel의 국소주입을 이용한 위암의 치료
1) 5-FU-sol-gel 제조를 위한 Pluronic F-127(P407)의 농도결정
온도 및 점도에 따른 P407의 점성을 확인하기 위해, P407의 농도를 10%, 20%, 및 30%로 하고, 온도는 약 24℃ 내지 38℃로 승온하면서 Brookfield 점도계로 점도를 측정하였다. 결과는 도 17에 나타내었다.
도 17을 참조하면, 37℃에서 gel화 되기 위해서는 P407농도가 30%를 만족하여야 함을 확인하였다. 또한30% 이상의 농도에서 점도가 지나치게 높아 약물의 국소주사가 어려워지기 때문에 30%가 가장 적절함을 확인하였다.
2) 5-FU-sol-gel의 약물 방출실험 (UV-VIS spectrophotometer)
5ml PBS와 0.76ml gel (2mm 두께, 30% P407, 4.3mg 5-FU)로 약물방출 실험을 진행하였다. 37℃의 환경에서 1, 2, 3, 4, 5, 6, 9, 12, 24시간 째의 변화를 관찰하였다. 결과를 도 18에 나타내었다.
도 18을 참조하면, 12시간이 경과했을 때, 약물이 완전히 방출되는 것을 확인할 수 있다.
3) 누드마우스 등에 사람위암세포를 이종이식 후 항암약물의 반응을 보는 in-vivo 실험
실험에 사용할 동물은 오리엔트에서 생산하는 생후 5주된 체중이 평균 30g 내외의 male nude mouse(Crj:BALB/c-nu/nu mice, male) 5 마리이며, 연구소에서 일주일간 검수기간을 거친 후 사용하였다. 누드마우스 한 마리당 사람의 위암세포 5x106cells/100㎕(PBS)을 등쪽 피하지방층에 이식하였다. 종양의 크기를 주기적으로 측정하여 직경 1cm에 이르게 되면 약물 투여실험을 실시한다. 30% P407 및 2.5% pH 8.4 5-FU 약물을 종양 바로 옆에 0.2 cc (5-FU 5mg), 일주일 간격으로 3번 주입한다. 한달 뒤 등쪽 표피에 자라는 종양의 장경과 단경을 측정하여 약물투여 전, 후의 크기를 mean tumor volume=(장경×단경2)/2(mm3)로 측정하여 비교분석한다. 결과는 도 19 및 도 20에 나타내었다.
도 19 및 도 20을 참조하면, 위암 종양 하부에 투여하였을 때 악성종양이 거의 사멸하는 것을 확인할 수 있었다. 또한, 약물 투여 전, 후의 종양 부피를 비교해 볼 때, 종양 크기가 감소함을 확인하였다(P<0.05).
Claims (15)
- 알칼리성 5-플루오로우라실 및 니트릭 옥사이드 생성억제제를 포함하는, 종양 혈관 파괴용 약학 조성물.
- 청구항 1에 있어서, 상기 니트릭 옥사이드 생성억제제는 L-NMMA(NG- Monomethyl-L-arginine), L-NAME(NG-Nitro-L-arginine methyl ester), L-NA(Nitro Arginine) 및 7NI(Nitroindazole)에서 선택되는 하나 이상을 포함하는, 종양 혈관 파괴용 약학 조성물.
- 청구항 1에 있어서, 상기 니트릭 옥사이드 생성억제제는 L-NMMA(NG- Monomethyl-L-arginine)을 포함하는, 종양 혈관 파괴용 약학 조성물.
- 청구항 1에 있어서, 상기 알칼리성은 pH 8 내지 9인, 종양 혈관 파괴용 약학 조성물.
- 청구항 1에 있어서, 상기 알칼리성 5-플루오로우라실은 알칼리 용매에 5-플루오로우라실이 용해된 형태인, 종양 혈관 파괴용 약학 조성물.
- 청구항 1에 있어서, 상기 알칼리성 5-플루오로우라실의 농도는 전체 조성물에서 0.1 내지 600 mM인, 종양 혈관 파괴용 약학 조성물.
- 청구항 1에 있어서, 상기 알칼리성 5-플루오로우라실은 종양 혈관내피세포에서 트롬보스폰딘-1 단백질 발현을 증가시켜 혈관내피세포를 사멸시키는, 종양 혈관 파괴용 약학 조성물.
- 청구항 1에 있어서, 상기 니트릭 옥사이드 생성억제제의 농도는 전체 조성물에서 0.1 내지 300 mM인, 종양 혈관 파괴용 약학 조성물.
- 청구항 1에 있어서, 상기 니트릭 옥사이드 생성억제제는 종양 혈관을 수축시키고, 5-플루오로우라실에 의해 손상된 종양 혈관 내부에 혈전증 발생을 촉진하여 혈관을 폐쇄시키는, 종양 혈관 파괴용 약학 조성물.
- 청구항 1에 있어서, 베바시주맙, 카프르산 또는 이의 생리학적 허용 가능한 염, 및 폴록사머에서 선택된 하나 이상을 더 포함하는, 종양 혈관 파괴용 약학 조성물.
- 청구항 1에 있어서, 세포독성항암제, 표적항암제 및 면역항암제에서 선택되는 하나 이상을 더 포함하는, 종양 혈관 파괴용 약학 조성물.
- 청구항 1에 있어서, 상기 종양은 고형암을 포함하는, 종양 혈관 파괴용 약학 조성물.
- 청구항 1에 있어서, 종양 주위에 국소주사로 투여되는, 종양 혈관 파괴용 약학 조성물.
- 청구항 13에 있어서, 상기 종양 주위는 종양 혈관이 위치하는 점막하층인, 종양 혈관 파괴용 약학 조성물.
- 청구항 1에 따른 종양 혈관 파괴용 약학 조성물을 포함하는, 고형암 국소 투여제.
