Abstract

본 발명은 다양한 타입의 피부 부상으로부터 기인하는 피부 흉터를 감소시키는 신규한 방법에 관한 것이다. 구체적으로, 본 발명은 상처 드레싱 및 흉터 관리 제품에 관한 것으로서, 이는 피부 흉터발생을 감소 및/또는 예방하기 위한 활성 약학적 성분이 혼입되어 있다.

Description

활성 성분 상처케어, 및 이의 흉터 관리 제품, 제조 방법 및 유용한 물품
본 발명은 신규한 상처 드레싱 (wound dressings) 및 흉터 관리 제품 (scar management products)에 관한 것이다. 구체적으로, 본 발명은 활성 약학적 성분 (active pharmaceutical ingredient: API)이 혼입된 상처 드레싱, 및 API를 또한 함유하는 흉터 관리 제품에 관한 것이다. 각각의 경우에, 상기 API는 상처 또는 흉터 부위로 전달되어 피부 흉터발생을 감소 및 예방한다.
섬유아세포 (fibroblast cells)가 흉터 형성의 주요 원인이라는 것은 잘 알려져 있다. 이러한 세포의 활성을 감소시키면 부상 또는 수술로 인한 피부 흉터발생을 상당히 감소시킬 가능성이 있다.
펜실베니아주 앨런타운에 소재한 Bio Med Sciences, Inc.는 Silon-TSR® 및 Rylon® 브랜드의 상처 드레싱을 제조 및 판매한다. 또한 Bio Med Sciences, Inc.는 패치 및 부목 재료를 포함한, Silon® 및 Oleeva® 브랜드의 흉터 관리 제품을 제조 및 판매한다.
피르페니돈 (Pirfenidone)은 섬유아세포가 폐 조직에 축적되어 콜라겐 및 기타 단백질을 분비하는 근섬유아세포로 분화되는 질병인 폐 섬유증 (pulmonary fibrosis)을 치료하기 위한 약물로서, 현재 미국 FDA (U.S. Food and Drug Administration)가 승인한 약물이다. 이와 동일한 생물생리학적 기전이 피부 상처에서 흉터 형성을 유도하는 것으로 알려져 있다. 다른 문헌에는 국한성 경피증 (localized scleroderma)을 개선하기 위해 국소로 적용되는 8% 피르페니돈 겔의 용도가 기재되어 있다. 보다 최근에 비후성 흉터발생 (hypertrophic scarring)을 감소시키기 위해 피부 흉터 부위에 8% 피르페니돈 로션을 국소 적용하는 것이 개시되었다.
실리콘 폴리머는 다양한 API의 국소 또는 경피 전달에서 기능하는 것으로 잘 알려져 있다.
선행기술을 개선하기 위한 노력으로, 본 발명자들은 피부 흉터발생을 감소 및 예방하기 위해, 피르페니돈이 개방 상처 (open wound)로 전달 또는 폐쇄 상처 부위 (closed wound site)로 경피 전달되는, 피르페니돈을 함유하는 상처 드레싱 및 흉터 관리 제품을 모두 제조하였다.
도 1은 본 발명에 따라 제조된 바람직한 상처 드레싱 또는 바람직한 흉터 관리 드레싱의 부분 단면도의 개략도를 나타내며, 여기서 참조 번호 (15)는 상기 드레싱의 상처 또는 피부 접촉면 또는 표면을 포함하는 드레싱의 층을 나타내고, 참조 번호 (13)은 일반적으로 도 1에 도시된 드레싱의 나머지 부분을 나타낸다.
바람직한 일 구체예에서, 본 발명은 도 1에 예시된 바와 같이, 드레싱 (11)을 포함하고, 이는 피부 흉터발생을 감소 및/또는 예방하기 위해, 개방 상처에 사용하기 위한 상처 드레싱 또는 폐쇄 상처 부위에 사용하기 위한 흉터 관리 드레싱일 수 있다. 도 1에 도시된 본 발명의 바람직한 구체예에서, 상기 드레싱 (11)은 상기 드레싱 (11)의 상처 또는 피부 접촉면 또는 표면을 포함하는 층 (15)을 가지며, API (바람직하게는, 피르페니돈)가 상기 층 (15)에 혼입되어, 상처 또는 흉터 부위로 전달되어 피부 흉터발생을 감소 및/또는 예방한다.
하기 실시예는 이들 디자인, 구성 및 공정의 변형이 당업계의 통상의 기술자에게 자명할 것이기 때문에 제한적인 것으로 의도되지 않는다. 마찬가지로, 본 발명의 동일한 개념을 달성하기 위해 다른 물질 및 API가 사용될 수 있다고 여겨진다. 하기 실시예는 본 발명의 다양한 바람직한 구체예 및 본 발명의 이러한 바람직한 구체예를 제조하는 바람직한 방법을 예시한다. 이들 실시예에서, 상기 드레싱 (11)의 상처/피부 접촉면 또는 표면을 포함하는 드레싱 (11)의 층 (15)을 제조하기 위해 사용되는 실리콘 제제 (silicone formulation)로 3 중량%의 피르페니돈을 혼합하였다. 그러나, 상기 드레싱의 상처/피부 접촉면 또는 표면을 포함하는 드레싱의 층인, 드레싱 (11)의 층 (15)을 제조하는데 사용되는 실리콘 제제로 피르페니돈을 0.5 중량% 내지 20 중량% 혼합함으로써, 피르페니돈의 0.5 중량% 내지 20 중량%의 바람직한 범위가 본 발명의 목적을 위해 사용될 수 있다. 또한, 본 발명은 수많은 상처 드레싱 및 흉터 관리 방법에 적용될 수 있다.
실시예 1:
3 중량%의 피르페니돈을 Silon-TSR 상처 드레싱의 상처 접촉면의 실리콘 제제로 혼합하였고, 이는 Dillon의 미국특허 제4,832,009호에 기재된 바와 같이 제조되며, 이는 본원에 참조로 통합된다.
실시예 2:

3 중량%의 피르페니돈을 Silon-TEX 압박복 테스타일 재료 (compression garment textile material)의 피부 접촉면의 실리콘 제제로 혼합하였고, 이는 Dillon의 미국특허 제8,084,051호에 기재된 바와 같이 제조되며, 이는 본원에 참조로 통합된다.

Claims (23)

  1. 피부 흉터발생을 감소 및/또는 예방하기 위한 흉터 관리 제품 (scar management product)으로서,
    피부 접촉면을 갖는 피부 접촉층 (skin contacting layer), 및
    상기 피부 접촉층에 혼입되어 섬유아세포 활성을 감소시키는 항섬유화 활성 약학적 성분 (antifibrotic active pharmaceutical ingredient)을 포함하는 것인 흉터 관리 제품.
  2. 청구항 22에 있어서,
    상기 피부 접촉층은 피르페니돈 (pirfenidone)을 0.5 중량% 내지 20 중량% 포함하는 것인 흉터 관리 제품.
  3. 청구항 22에 있어서,
    상기 피부 접촉층은 피르페니돈을 3 중량% 포함하는 것인 흉터 관리 제품.
  4. 청구항 22에 있어서,
    상기 피부 접촉층은 실리콘 제제 (silicone formulation)를 포함하는 것인 흉터 관리 제품.
  5. 청구항 4에 있어서,
    상기 실리콘 제제는 피르페니돈을 0.5 중량% 내지 20 중량% 포함하는 것인 흉터 관리 제품.
  6. 청구항 4에 있어서,
    상기 실리콘 제제는 피르페니돈을 3 중량% 포함하는 것인 흉터 관리 제품.
  7. 청구항 4, 5 또는 6 중 어느 항에 있어서,
    상기 실리콘 제제는 실리콘인 것인 흉터 관리 제품.
  8. 청구항 4, 5 또는 6 중 어느 항에 있어서,
    상기 실리콘 제제는 폴리유기실록산 (polyorganosiloxane)인 것인 흉터 관리 제품.
  9. 청구항 4, 5 또는 6 중 어느 항에 있어서,
    상기 실리콘 제제는 폴리디메틸실록산인 것인 흉터 관리 제품.
  10. 청구항 1에 있어서,
    상기 피부 접촉층 상에 위치하는 상기 흉터 관리 제품의 나머지 부분을 추가로 포함하고,
    상기 나머지 부분은 텍스타일 패브릭 (textile fabric), 폼 (foam), 부직포 필름 (non-woven film), 메쉬 (mesh), 열가소성 물질 (thermoplastic material), 또는 텍스타일 패브릭 이외의 물질을 포함하는 것인 흉터 관리 제품.
  11. 흉터 관리 제품을 제조하는 방법으로서,
    피르페니돈을 실리콘 제제로 혼합하여 이들의 혼합물을 형성하는 단계로서, 상기 실리콘 제제는 폴리유기실록산, 실리콘, 실리콘 겔, 실리콘 엘라스토머, 또는 폴리디메틸실록산을 포함하는 것인 단계,
    상기 혼합물을 캐리어 상에 적용하여 그 위에 혼합물 층을 형성하는 단계, 및
    상기 혼합물 층을 경화시키는 단계를 포함하는 것인 방법.
  12. 청구항 11에 있어서,
    상기 경화 단계 전에, 상기 혼합물 층에 팽창된 폴리테트라플루오로에틸렌 (expanded polytetrafluoroethylene)을 적용하는 단계를 추가로 포함하는 것인 방법.
  13. 청구항 12에 있어서,
    상기 실리콘 제제는 실리콘이고,
    상기 혼합물 층 및 상기 팽창된 폴리테트라플루오로에틸렌은 실리콘 및 폴리테트라플루오로에틸렌의 상호침투 폴리머 네트워크 (interpenetrating polymer network)를 형성하며, 상기 상호침투 폴리머 네트워크는 제1 면 및 제2 면을 갖고, 상기 제1 면은 상기 피부 접촉층의 피부 접촉면을 형성하며,
    상기 상호침투 폴리머 네트워크의 제2 면에 열가소성 부목 재료를 적층하는 단계를 추가로 포함하는 것인 방법.
  14. 청구항 11에 있어서,
    상기 경화 단계 전에, 상기 혼합물 층에 천공된 메쉬 층 (apertured mesh layer)을 적용하는 단계를 추가로 포함하는 것인 방법.
  15. 청구항 11에 있어서,
    상기 경화 단계 전에,
    상기 혼합물 층에 미세다공성 폴리머 시트막 (microporous polymer sheeting membrane)을 적용하여, 상기 혼합물 층을 상기 미세다공성 폴리머 시트막에 주입시키거나 또는 이를 유발하는 단계, 및
    상기 주입된 미세다공성 폴리머 시트 재료의 원위면에 백킹 재료 (backing material)를 적용하는 단계로서, 상기 백킹 재료는 상기 미세다공성 폴리머 시트막을 통과한 혼합물과 접촉하는 단계를 추가로 포함하는 것인 방법.
  16. 청구항 15에 있어서,
    상기 백킹 재료는 텍스타일 패브릭, 폼, 부직포 필름, 또는 텍스타일 패브릭 이외의 재료를 포함하는 것인 방법.
  17. 청구항 11에 있어서,
    상기 경화 단계 전에, 미세다공성 폴리머 시트막 및 백킹 재료의 적층을 상기 혼합물 층에 적용하여, 상기 혼합물 층을 상기 미세다공성 폴리머 시트막에 주입시키거나 또는 이를 유발하는 단계를 추가로 포함하는 것인 방법.
  18. 청구항 17에 있어서,
    상기 백킹 재료는 텍스타일 패브릭, 폼, 부직포 필름, 또는 텍스타일 패브릭 이외의 재료를 포함하는 것인 방법.
  19. 흉터 관리 제품을 제조하는 방법으로서,
    피르페니돈을 실리콘 제제로 혼합하여 이들의 혼합물을 형성하는 단계,
    상기 혼합물을 텍스타일 패브릭 상에 적용하여, 상기 혼합물이 적용된 텍스타일 패브릭을 형성하는 단계, 및
    상기 혼합물이 적용된 텍스타일 패브릭을 흉터 관리 제품으로 제조하는 단계를 포함하는 방법.
  20. 청구항 19에 있어서,
    상기 흉터 관리 제품은 의복 (garment)인 것인 방법.
  21. 피부 흉터를 감소시키는 방법으로서,
    청구항 1 내지 11, 22 또는 23 중 어느 항의 흉터 관리 제품을 제공하는 단계,
    상기 흉터 관리 제품의 피부 접촉층의 피부 접촉면을 폐쇄 상처 (closed wound) 부위와 접촉하도록 그 위에 적용하는 단계, 및
    상기 흉터 관리 제품을 상기 폐쇄 상처 부위 상에 접촉시켜서, 피부 흉터발생을 감소 및/또는 예방하는데 효과적인 양의 시간 동안 유지시키는 단계를 포함하는 것인 방법.
  22. 청구항 1에 있어서,
    상기 항섬유화 활성 약학적 성분은 피르페니돈인 것인 흉터 관리 제품.
  23. 청구항 10에 있어서,
    상기 흉터 관리 제품의 나머지 부분은 백킹층 (backing layer)을 포함하는 것인 흉터 관리 제품.
