Abstract

Description
본 발명은 암과 감염성 질병을 치료하기 위한 방법들에 관한 것이다.
톨 유사 수용체(toll-like receptor)는 박테리아와 바이러스 병원체중 가장 일반적인 패턴들을 인식하기 위한 것이다. 그것의 활성화는 선천성 및 적응성 면역 반응의 보강을 가져오게 된다. 항원체의 위치에 대한 면역 시스템의 수용체 세포들은 유효한 면역 반응의 주요한 단계이다. 그 이유는 면역화는 다른 형태의 보조약(adjuvants)을 포함하고 있기 때문이다. 대부분의 종양들은 종양-특이성 항원들을 발현하고 있지만, 그 종양들은 면역 인식을 회피하는 수 많은 메카니즘을 이용하고 있다.

본 발명은 제 1의 핵산과 제 2의 핵산으로 구성된 벡터에 관한 것으로서, 상기 제 1의 핵산은 톨 유사 수용체를 인코드하며, 상기 제 2의 핵산은 톨 유사 수용체 효능제(agonist)를 인코드 한다. 상기 제 1의 핵산은 톨 유사 수용체의 분비 형태를 위해 인코드 한다. 상기 제 2의 핵산은 플라젤린의 분비 형태가 된다. 톨 유사 수용체는 플라젤린이 될 수 있다. 벡터는 포유류 발현 벡터이다. 상기 벡터는 아데노바이러스, 렌티바이러스(lentivirus) 또는 리포솜으로부터 발현된다. 플라젤린의 분비 형태는 CBLB502S가 된다. 톨 유사 수용체는 TLR-5이다.
본 발명은 약제가 필요한 포유류에 대해 약제를 투여하여 포유류내의 암을 치료하는 방법에 관한 것으로서, 제 1의 핵산과 제 2의 핵산으로 구성된 벡터를 포함하며, 상기 제 1의 핵산은 톨 유사 수용체를 인코드하며, 상기 제 2의 핵산은 톨 유사 수용체 효능제(agonist)를 인코드 한다. 상기 암은 종양이 된다. 상기 종양은, 전립선, 가슴, 결장, 식도, 위, 폐, 췌장, 신장, 갑상선, 난소, 목구멍 또는 경부(cervix)로 구성되는 그룹으로부터 시작된다. 상기 종양은 육종(sarcomas), 멜레노마(melenomas), 백혈병과 림프종으로 구성되는 그룹으로부터 시작된다. 상기 약제는 포유류의 상기 종양에서 인 트랜스(in trans) 형태로 또는 외부에서 투여된다. 상기 약제는 포유류의 상기 종양의 내부로 직접 투여된다. 상기 약제는 면역 촉진제와 함께 투여된다. 상기 면역 촉진제는 성장 호르몬, 프로락틴과 비타민 D로 구성되는 그룹으로부터 선택된다. 상기 성장 호르몬은 소마토트로핀(somatotrophin)이 된다. 상기 약제는 사이토킨(cytokin)과 함께 투여된다. 상기 사이토킨은 줄기 세포 인자가 된다.

TLR은 Gram-포지티브, Gram-네가티브 박테리아, 균류와 바이러스를 포함하는 병원체로부터 유도되는 보존성 분자 산출물(conserved molecular product)이나, 병원체-관련 분자 패턴(PAMPs)으로 총체적으로 언급되는, 호스트 분자와는 구별되는 분자들을 인식한다. TLR은 또한 총체적으로 손상-관련 분자 패턴(DAMPs)으로 언급되는 손상되거나 또는 죽은 세포로부터 방출되는 내인성(endogenous) 분자들을 인식할 수 있다. PAMP 또는 DAMP는 다음에 추가로 기술되는 TLR 효능제(agonist)가 된다. 플라젤린은, TLR을 결합하고 NF-κB 활동의 활성화와 같은, TLR-중재에 의한 활성화를 유도하는 플라젤린의 프래그먼트, 변종, 아나로그, 호모로지(homology) 또는 그 유도체가 될 수 있다. 플라젤린의 프래그먼트, 변종, 아나로그, 호모로지(homology) 또는 유도체는 TLR을 결합하고 TLR5-중재에 의한 활동을 유도하는 플라젤린의 아미노산과 최소한 30-99% 동일하다.
플라젤린은 (GenBank 접속 번호 M84972에 의해 인코드된) S.dublin의 대표적인 보기가 되는 살모넬라균으로부터 얻어질 수 있다. 플라젤린과 관련된 폴리펩타이드는 TLR을 결합하고 NF-κB 활동의 활성화와 같은, TLR-중재에 의한 활성화를 유도하는, M84972호의 프래그먼트, 변종, 아나로그, 호모로지(homology) 또는 유도체 또는 그들의 조합이 될 수 있다. 플라젤린의 프래그먼트, 변종, 아나로그, 호모로지(homology) 또는 유도체는 플라젤린의 영역 구조와 TLR5에 의해 인지되는 보존 구조에 근거하는 합리적인 설계에 의해 얻어진다.

플라젤린은 박테리아 플라젤룸(flagellum)의 주요 성분이 될 수 있다. 플라젤린은 3개의 영역들로 구성될 수 있다(도 4). 영역 1(D1)과 영역 2(D2)는 비연속이며 아미노 말단(terminus)과 카르복시 말단내의 나머지(residue)가 헤어핀(hairpin) 구조의 형태에 의해 병렬 배치되어 있는 경우에 형성될 수 있다. D1과 D2영역들을 포함하는 아미노와 카르복시 단말은 최대한 보존되며, 반면에 중간의 고변위(hypervariable) 영역(D3)은 매우 가변적이다. 4. 약제
본 발명은 또한 치료가능한 유효한 TLR과 TLR 효능제 양을 포함하는 약제에 관한 것이다. 약제는 TLR 효능제로부터 TLR을 분리하여 전달할 수 있다. 약제는 벡터가 될 수 있다. 벡터는 TLR을 인코딩하는 제 1핵산과 TLR 효능제를 포함하는 제 2핵산을 포함한다. 벡터는 포유류 세포를 형질 변환(transducing)할 수 있다. 벡터는 TLR 및/또는 강한 프로모터(promoter)를 이용하는 TLR 효능제의 바이-시스트로닉(bi-cistronic) 발현을 실행할 수 있다. 벡터는 강한 프로모터에 의해 제어되는, TLR을 인코딩하는 유전자만을 포함할 수 있다. 벡터는 바이러스 또는 리포솜과 관련되는 벡터 시스템에 의해 포유류 세포내에 전달될 수 있다. 바이러스 벡터 시스템은 아데노바이러스 또는 싸이토메갈로바이러스가 될 수 있다.

약제는 발현을 유도하고 TLR을 동시에 활성화시킴으로써, 내장벽을 통해 대량의 침투 상황과 같이 호스트 면역 시스템에 종양 또는 전염된 세포들을 노출시키는, 약품 조성물이 될 수 있다. 약제는 TLR 효능제와 결합하여 TLR을 발현시키는 약품 조성물이 될 수 있으며, 근육 내부에 대한 투여를 위해 용액 상태로 체계적으로 전달될 수 있다. 약제는, 아데노바이러스 또는 싸이토메갈로바이러스 벡터 시스템과 같은, 동일한 벡터로부터 발현되는, TLR 효능제와 결합하여 TLR을 발현하는 악품 조성물이 될 수 있다. 약제는 포유류 세포의 표면으로 기능성 효능제를 운반하는, 나노-입자의 형태로 발현되는 TLR 효능제와 결합하여 TLR을 발현하는 악품 조성물이 될 수 있다.
약제는, 종래의 기술에서 잘 알려진 방법들을 이용하여 생성되는, 상기 약품 조성물을 포함하는 약학적 제품이다. 약제는 또한 공동인자(coagent)를 포함한다.
벡터는 TLR5를 인코딩하는 제 1핵산과 플라젤린을 포함하는 제 2핵산으로 이루어진다. 벡터는 강한 프로모터를 이용하여 TLR5 및/또는 플라젤린을 발현할 수 있다. 발현 벡터는 TLR 또는 TLR5 및/또는 플라젤린을 인코딩하는 유전 인자의 리더 서열 클로운 업스트림(cloned upstream)을 추가로 포함한다. 발현 벡터는 pCD515에 근거하는 벡터 시스템이 될 수 있다. 발현 벡터는 도 1a에 기재된 바와 같이 pCD515-CMV-hTLR5-EF1-502가 될 수 있다. 발현 벡터는 도 1c에 기재된 바와 같이 pCD515-CMV-Sseap-502가 될 수 있다.

약품 조성물은,
근육 내부 투여를 위해 용액으로 체계적으로 전달되는 TLR 효능제와;
TLR과 같이 아데노바이러스 벡터로부터 발현되는 TLR 효능제;
또는 CBLB502로부터 전달되는, 플라젤린과 같이, 기능성 TLR 효능제를 표면상에 운반하는 나노-입자들의 형태로 발현되는 되어 있는 TLR 효능제와 결합하여 아데노바이러스 발현 기능성 인체 TLR5가 될 수 있다. 나노 입자는 박테리오파지 T7에 근거하거나 또는 생물학적인 활동을 보존하기 위해 완전하게 형성된다. 나노-조성물은 투여량에 의해 영향을 받는, NF-κB-응답성 리포터 활동을 위해 제공되며, 효과적인 면역 방법을 위한 엔도시토시스(endocytosis)에 의해 세포 내재화(internalization)를 초래하게 된다.
a. 투여
여기에 기재된 방법을 이용하는 약제의 투여는, 구강 투여, 비경구 투여, 혀밑 투여, 경피 투여, 직장 투여, 점막 투여, 국소 부분 투여, 흡입, 구강 (buccal)투여, 또는 그들의 조합을 통해 이루어진다. 비경구 투여는 정맥 투여, 동맥 투여, 복막 투여, 피하 투여, 근육내 투여, 수막공간 투여 밑 관절 투여를 포함하나, 이에 한정되지는 않는다. 수의학 목적을 위해, 약제는 정상적인 수의학 관습에 다라 적절하게 허용가능한 조성물로서 투여될 수 있다. 수의사는 투여 요법과 특정한 동물에게 가장 적합한 투여 경로를 용이하게 결정할 수 있다. 약제는 환자, 고양이, 개, 큰 동물 또는 조류에 대해 투여될 수 있다.

b. 조성물
본 방법은 약제의 투여 방법을 포함한다. 여기에서 제공되는 약제는 종래의 방법으로 제조되는 태블릿 또는 정제의 형태로 되어 있다. 예를 들면, 구강 투여를 위한 태블릿과 캡슐은 종래의 부형제(excipient)를 포함하고 있으며 이러한 부형제는 결합제, 필러, 윤활제, 붕괴제(정제 분해물질)와 습식제가 된다. 결합제는 시럽, 아카시아, 젤라틴, 소르비톨, 트래거캔스, 녹말의 점액과 폴리비닐파이롤리돈을 포함하나, 이에 한정되지는 않는다. 필러들(fillers)은 락토제, 설탕, 미정질의 셀룰로스, 옥수수 녹말, 칼슘 인과 소르비톨을 포함하나 이에 한정되지는 않는다. 윤활제는 마그네슘 스테아레이트산, 스테아릭산, 탈크, 폴리에틸렌 글리코과 규소를 포함하나 이에 한정되지는 않는다. 붕괴제는, 감자 녹말과 나트륨 녹말 클리콜레이트(glycollate)가 된다. 습식제는 나트륨 로릴 황산염이 된다. 태블릿은 기존의 방법들에 따라 코팅된다.

여기에서 제공되는 약제는 또한 투입, 종양내 투입 또는 연속 주입에 의한 비경구 투여를 위해 제조되었다. 투입을 위한 조성물은 지용성 또는 수용성 매체내에서 서스펜션, 용액 또는 에멀션의 형태로 되며서스펜딩제, 안정제 및 분산제를 포함하나 이에 한정되지 않는 조성제를 포함한다. 약제는 또한 살균한 그리고 발열 물질이 없는 물을 포함하나 이에 한정되지 않는 적합한 매체와 재구성하기 위한 분말 형태로 제공된다.

C. 투여량
본 방법은 필요한 환자에게 치료가능한 유효한 양의 약제를 투여하는 단계를 포함한다. 치료시에 필효한 치료가능한 유효한 양은 치료되는 질병의 특성, TLR 활동을 활성화시키기 위해 요구되는 시간, 및 환자의 나이/조건에 따라 달라진다. 그러나, 일반적으로, 성인 환자를 위해 이용되는 투여량은 하루에 0.001 mg/kg에서 약 200 mg/kg의 범위에 있다. 그 투여량은 약 1 mg/kg에서 약 100 mg/kg의 범위에 있을 수도 있다. 종래에는, 소망하는 투여 방식이 단일한 투여로 실행되거나, 또는 적절한 구간을 두고 투여되는 다중 투여로 실행되었다. 예를 들면, 하루에 2, 3, 4 또는 그 이상의 서브-투여(sub-dose)가 실행되었다. 다중 투여는 희망하나 또는 필요에 의해 이루어질 수 있다.

5. 암 치료방법

여러 종류의 암들이 본 발명에 따라 치료된다. 상기 암들은, 악성 종양, (가속화되며 전이성 방광암을 포함하는)방광, 가슴, (직장결장 암을 포함하는)결장, 신장, 간, (소규모의 그리고 소규모가 아닌 세포 폐암과 선암종(adenocarcinoma)을 포함하는)폐, 난소, 전립선, 고환, 요생식로, 림프계, 직장, 후두(larynx), (외분비의 췌장 악성 종양을 포함하는)췌장, 식도, 위, 담낭, 경부, 갑상선과 (편평세포암종(squamous cell carcinoma)을 포함하는) 피부; 백혈병, 급성 림프구 백혈병, 급성 림프모구 백혈병, B-세포 림프종, T-세포 림프종, 호지킨스 림프종, 넌-호지킨스(non-Hodgkins lymphoma), 텀모양세포 림프종, 조직구(histiocytic) 림프종, 및 버켓츠 (Burketts) 림프종을 포함하는 림프계의 조혈(hematopoietic) 종양들; 급성 및 만성 골수 백혈병, 골수형성이성질환(myelodysplastic syndrome), 골수 (myeloid) 백혈병, 및 전골수구 백혈병(promyelocytic leukemia)을 포함하는 골수계의 조혈 종양들; 별아교세포종(astrocytoma), 신경아세포종(neuroblastoma), 신경교종(glioma) 및 신경집종(schwannomas)을 포함하는 중추신경계 및 말초신경계의 종양들; 섬유육종(fibrosarcoma), 횡문근육종(rhabdomyoscarcoma) 및 골육종(osteosarcoma)을 포함하는 중간엽 세포(mesenchymal origin)의 종양들; 그리고 흑색종, 이종피부 색소병(xenoderma pigmentosum), 각질가시세포종(keratoacanthoma), 생식세포종(seminoma), 갑상선 소포암(follicular cancer), 기형암종(teratocarcinoma)과 위장관 또는 복부골반강(abdominopelvic cavity)을 포함하는 다른 종양들이 된다.
본 방법은, 면역 촉진제, 싸이토킨 또는 화학적 치료의 사용을 포함하는, 다른 암 치료하는 방법들과 결합될 수 있다. 면역 촉진제는 성장 호르몬, 프로락틴 또는 비타민 D가 된다.
6. 감염된 세포의 치료
여기에서는 약제에 의해 형질 변환된 세포의 동시 전달에 의해 감연성의 질병을 치료하는 방법이 제공된다. 본 방법은 바이러스, 박테리아, 원충 기생충 또는 균류 전염을 치료하는데 이용된다. 본 방법은 시스템에 최소한의 영향을 주면서 감염된 세포들의 TLR 신호처리의 오토크라인(autocrine) 활동을 초래하는 세호내 주입에 의해 감염 질병을 치료하는데 이용된다. 그리하여, 감염 세포들에 대한 선천적인 특정 면역 반응성을 가져오게 한다. 본 방법은 바이러스, 박테리아, 원충 기생충 또는 균류 전염을 치료하기 위해 다른 치료법들과 결합되어 이용된다.
본 방법은 약제를 투여하는 단계를 포함한다. 본 방법은 약제를 포함하는 백신의 투여를 포함하며, 선택된 항원을 발현하는 구조를 포함하는 다른 백신과 결합하여 이용될 수 있다.

벡터의 합성과 종양 세포의 치료
톨 유사 수용체 5(TLR5)와 플라젤린 CBLB502를 발현하기 위해 벡터 구조가 생성되었다. 벡터 pCD515는 이러한 구조에 대한 주요 요소로 사용되었다. 인체의 TLR5의 cDNA 서열과 톨 유사 수용체 효능제의 CBLB502를 인코딩하는 DNA는 알칼라인 포스파타제로부터 유도되는 리더 펩타이드와 각각 결합되어, 세포 외부로 분비하기 위해 소포체(ER)와 골기체를 통해 발현된 단백질의 라우팅을 실행하게 된다.
pCD515-CMV-hTLR5-EF1-502s 벡터 구조는 세포의 표면에서 CBLB502 플라젤린(CBLB502S)의 분비 형태와 톨 유사 수용체5(TLR5)를 발현하고 있다. 이러한 아데노바이러스 벡터는 유효한 합성과 포유류에 의한 분비를 실현하기 위해 CBLB502의 수정을 요구하였다. 아데노바이러스 구조는 알칼라인 포스파타제로부터 유도되는 리더 핵산 서열(Atgctgctgctgctgctgctgctgggcctgaggctacagctctccctgggc)로 구성되며 단절된(truncated) 살모넬라 플라젤린(fliC) 유전자의 복제된 업스트림이다(플라젤린의 분비 가능 형태를 인코드하는 것(예를 들면, CBLB502S). Butdela et al., Science 320:226-230(2008)참조). EF1(연장도 인자: elongation factor 1α) 프로모터는 CBLB502S를 인코딩하는 이러한 카세트(cassette)의 복제된 업스트림이다. TLR5 유전자는 인체로부터 도출되며 도 9에 도시된 바와 같이 아미노산 서열을 가지고 있다. CMV 프로모터는 TLR5 유전 인자의 복제된 업스트림이다. 이러한 구조는 TLR5와 CBLB502S를 동시에 발현하고 있다. 이러한 구조는 도 1a에 도시되어 있다.
pCD515-CMV-hTLR5 발현 벡터는 인체의 TLR5(도 9 참조)의 형태를 발현하기 위해 구성되었다. 아데노바이러스 구조는 hTLR5 카세트의 강한 CMV 프로모터 복제 업스트림을 포함한다. 이러한 구조는 도 1b에 도시되어 있다.
pCVD515-CMV-Sseap-502 발현 벡터는 분비된 플라젤린 CBLB502와 톨 유사체(toll-like)를 발현하기 위해 구성되었다. 아데노바이러스 구조는 리더 서열 SEAP 502 플라젤린(fliC) 유전 인자의 강한 CMV 프로모터 복제 업스트림을 포함한다. 이러한 구조는 도 1c에 도시되어 있다. [복제 정보 필요]

벡터의 합성과 종양 세포의 치료
NF-κB-반응성 GFP를 발현하고 TLR5 상태가 다른 두 개의 리포터 포유류 세포 라인들은 벡터 구조는 pCD515, pCD515-CMV-hTKR5-EF1-502s, pCD515-CMV-hTKR5-502, pCD515-CMV-hTKR5와 pCD515-CMV-Sseap-502에 의해 형질 변경되었다(아래 표 3 참조).
TLR5와 TLR5 효능제 CBLB 502S를 공동 발현하는 벡터는 기존의 TLR중 어느 것도 발현하지 않으며, 단지 TLR5 효능제만으로는 활성화되지 않는 293-널(null) 세포내의 NF-κB 리포터의 발현을 유도하기에 충분하였다. 이러한 실험은 TLR5와 플라젤린 CBLB502S가 트랜스 형태(trans) 또는 시스 형태(cis)로 동작하여 TLR5 신호처리를 활성화시킨다는 것을 입증하였다.
실시예

(pCD515-CMV-hTKR5-EF1-502s)를 가지는 바이-시스트로닉(bi-cistronic) 아데노바이러스의 항종양(antitumor) 효과를 실험하기 위해서, 제어 대상의 비주입 종양(non-injected tumor)이 실험의 종료를 요구하는 크기 제한에 도달하기 까지 종양들이 직경 3-5mm에 이르고 종양 크기가 검사되는 경우에, CT26 생쥐 결장 악성 세포로부터 도출되는 Balb/생쥐의 공통 유전형(syngeneic) 종양들을 성장시키는 두 개의 s.c. 중 한 개에, 10ml의 아데노바이러스 서스펜션(1012-1011 IU/ml)이 주입되었다. 제어 생쥐들에 대해서, 적색 형광 단백질(red fluorescent protein: RFP)을 발현하는 아데노바이러스 벡터가 주입되었다(다시 한번, 한 마리의 생쥐 마다 두 개의 종양중 한 개의 종양에 주입). 대표적인 실험의 결과들이 도 4에 도시되었다. (pCD515-CMV-hTKR5-EF1-502s)로 주입된 종양들의 성장이 거의 완전하게 감소됨에 따라 RFP-발현 아데노바이러스가 주입된 제어 대상 동물의 종양과 비교할 때에 동일한 동물내의 주입되지 않은 종양의 성장이 감소하였다. 이러한 결과는 선천성(시스 효과 : cis effect)과 적응성(트랜스 효과 : trans effect) 면역 반응의 보완성(recruitment)을 나타내는 (i) 강력한 인-시스(in-cis)와 (ii) pCD515-CMV-hTKR5-EF1-502s의 가시적인 인-트랜스 효과를 나타내고 있다.
표 1에 기재된 다른 제어 대상 바이러스들중 홀로 주입된 어느 것도(예를 들면, AD5(제어 대상)와 Ad5(TLR5)) 종양들에 대해서 성장 억제 효과를 나타내지 않았다.
그러므로, TLR5의 강압적이고 정상적인 장소가 아닌 위치에서의 발현(ectopic expression)은, 원래 TLR5가 부족한 종양 세포 형태를 TLR5 촉진에 매우 민감하게 응답하도록 한다. 그러므로, 종양의 면역 허용치를 넘어서게 된다. 이것은 후속의 일반적인 항종양 효과를 가지는 적응성 면역 반응의 효과적인 전개를 향상시키는 선천성 면역 반응의 강력한 장점이 되는 것이다.
Claims (23)
- 제 1핵산과 제 2핵산을 포함하고, 제 1의 핵산은 톨 유사 수용체를 인코드하며, 제 2의 핵산은 톨 유사 수용체 효능제(agonist)를 인코드하는 벡터.

상기 벡터는 아데노바이러스, 렌티바이러스(lentivirus) 또는 리포솜으로부터 발현되는 것을 특징으로 하는 벡터. - 제 1항에 있어서,
상기 제 1의 핵산은 톨 유사 수용체로부터 분비되는 되는 것을 특징으로 하는 벡터. - 제 2항에 있어서,
상기 플라젤린은 플라젤린의 분비 형태가 되는 것을 특징으로 하는 벡터. - 제 7항에 있어서,
상기 플라젤린의 분비 형태는, 도 5에 도시된 TLR5 활성을 위해 중요한 13개의 플라젤린의 보존 아미노산들을 포함하는 것을 특징으로 하는 벡터. - 제 1항에 있어서,
상기 톨 유사 수용체는 TLR-5가 되는 것을 특징으로 하는 벡터. - 제 1항에 있어서,
상기 제 1의 핵산은 도 7에 도시된 서열을 포함하며,
