Abstract

Description
본 발명은 레가데노손(regadenoson)-아데노신 A2A 수용체 작용제-의 투여량을 심근 조영을 수행하는 포유동물에게 투여함으로써 달성되는 심근 조영 방법에 관한 것이다.
심근 관류 조영(MPI)은 관상 동맥 질환의 검출 및 특성화에 유용한 진단 기술이다. 관류 조영은 불충분한 혈류의 부위를 확인하기 위하여 방사성 핵종과 같은 물질을 사용한다. MPI에서, 휴식시의 혈류를 측정하고, 그 결과를 트레드밀 위에서 운동하는 동안 측정된 혈류와 비교하는데(심장 스트레스 시험), 상기 격심한 운동은 혈류를 자극하기 위해 필요한 것이다. 불행하게도, 다수의 환자는 말초 혈관 질환, 관절염 등과 같은 의학적 상태로 인하여, 충분한 혈류의 제공이 필요한 수준의 운동을 하지 못한다.

레가데노손은 종래 기술에서 공지된 약학적 투여 방법에 의해 투여할 수 있다. 레가데노손은 i.v.로 투여하는 것이 바람직하다. 레가데노손은 i.v.로 단일 투여량으로 투여하는 것이 더 바람직하다. 용어 "단일 투여량"은 치료량의 레가데노손의 1회의 신속하게 투여된 투여량을 말한다. 용어 "단일 투여량"은 예를 들어 연속적 i.v. 주입에 의해 연장된 시간에 걸쳐 투여된 투여량 또는 투여량들을 포함하지 않는다.

본 발명의 화합물 및/또는 이의 유도체를 포함하는 약학적 조성물은 비경구 투여용 용액 또는 동결건조된 분말으로서 제형화할 수 있다. 분말은 사용 전에 적절한 담체 또는 기타 약학적 허용 담체를 첨가하여 재구성할 수 있다. 액체 형태로 사용하는 경우, 본 발명의 화합물은 바람직하게는 완충된 등장성 수용액으로 도입한다. 적절한 희석제의 예는 물 및 완충된 나트륨 또는 아세트산암모늄 용액 중 표준 5% 포도당의 노르말 등장성 식염수이다. 상기 액체 제제는 비경구 투여에 적합하나, 경구 투여에도 사용할 수 있다. 부형제, 예컨대 폴리비닐피롤리디논, 젤라틴, 히드록시 셀룰로오스, 아카시아, 폴리에틸렌 글리콜, 만니톨, 염화나트륨, 시트르산나트륨 또는 당업자에게 공지된 임의의 기타 부형제를 본 발명의 화합물을 포함하는 약학적 조성물에 첨가하는 것이 바람직할 것이다.

레가데노손은 약 50 마이크로그램/㎖ 의 수용해도를 갖는다. 따라서, 소정의 중량의 레가데노손을 허용가능한 부피로 투여할 수 있는 한 레가데노손은 물에 용해하여 투여할 수 있다. 예를 들어, 약 400 마이크로그램의 바람직한 투여량을 8 ㎖의 물에 투여할 수 있다. 상기 부피가 투여 목적에 대해 너무 큰 경우, 또는 약학적 조성물이 실온(RT)이 아닌 온도에서 저장되는 경우, 조성물 중 레가데노손의 용해도를 증가시키고/시키거나 개선된 안정성 및 저장 특성과 같은 기타 개선된 특성을 갖는 약학적 조성물을 제공하기 위하여 추가의 성분을 조성물에 첨가할 수 있다.
레가데노손을 포함하는 본 발명의 약학적 조성물은 약 1 밀리그램/㎖ 이하의 레가데노손을 포함할 수 있다. 레가데노손을 포함하는 약학적 조성물이 약 50 ∼ 약 250 마이크로그램/㎖, 더 바람직하게는 약 50 ∼ 150 마이크로그램/㎖ 의 레가데노손을 포함하는 것이 바람직하다.
용해도 및 저장 특성을 개선하기 위하여, 레가데노손은 메틸보론산(MBA) 공용매를 포함하는 약학적 조성물에 투여될 수 있다. 작용제의 용해도 및 반감기를 개선하기 위하여 메틸보론산을 약학적 조성물에 첨가한다. MBA는 생성되는 조성물의 pH를 증가시킨다. MBA를 포함하는 약학적 조성물 중 레가데노손의 용해도는 조성물의 pH가 중성으로 떨어짐에 따라 감소되는 경향이 있다. 따라서, 레가데노손을 갖는, 최적의 MBA 함유 조성물의 pH는 약 8.5 ∼ 10이고, 약 9.1 ∼ 9.4의 pH가 바람직하며, 약 9.3의 pH가 가장 바람직하다. 이는 약 50 ∼ 약 250 ㎎/㎖ 의 MBA를 포함하는 조성물에 해당한다. MBA에 대한 대안으로서, 레가데노손은 보레이트 완충액과 배합할 수 있다. 전형적으로, 보레이트 완충액은 산 또는 염기를 사용하여 소정의 pH, 예컨대 pH 9.3로 조절된 나트륨 보레이트 수용액으로 이루어질 것이다.
MBA를 함유하는 약학적 조성물은 저장 문제가 있다. 즉, MBA는 특정 타입 I 유리 용기에 포장되는 경우 박리를 야기할 수 있다. 상기 문제는 MBA 함유 약학적 조성물을 플라스틱 용기 또는 더 저항성인 타입 I 유리 용기에 저장함으로써 극복할 수 있다.
중성에 가까운 pH를 갖는 레가데노손 함유 약학적 조성물이 필요한 경우, 대안으로 레가데노손을 프로필렌 글리콜(PG) 공용매와 배합한다. 조성물 중 사용되는 PG의 양은 약 5 ∼ 25 부피%의 범위일 수 있고, 레가데노손을 사용하는 경우 약 8 ∼ 약 20 부피%의 범위가 더 바람직하다. PG에 대한 대안은 폴리에틸렌 글리콜 - PEG이다. 바람직한 PEG는 평균 분자량이 약 200 ∼ 400일 것이다.
바람직하게는, PG 또는 PEG를 포함하는 레가데노손 조성물은 pH가 약 6 ∼ 약 8이고, 약 7의 pH가 바람직하다. 조성물의 pH를 소정의 값으로 조절할 수 있는 임의의 생리학적 허용 완충제를 사용할 수 있다. 상기 완충제의 비제한적인 예로서 제이인산나트륨, 제이인산나트륨 탈수화물, 및 제일인산나트륨 일수화물을 들 수 있다. EDTA 및 디메틸아세트아미드와 같은 추가의 임의의 성분도 조성물에 사용할 수 있다.
본 발명의 약학적 조성물은 1 이상의 항산화제, 예컨대 부틸화 히드록시아니솔(BHA)을 포함할 수 있다.
레가데노손은 신속히 작용 개시되며, 투여시 작용의 지속 시간이 짧다. 레가데노손은 단일 일시 정맥내(i.v.) 주사로 매우 소량으로 투여시 매우 유용하다. 레가데노손은 10 ㎍의 소량 및 2000 ㎍ 이상의 다량으로 투여할 수 있다. 최적의 투여량은 10 ㎍의 소량 및 약 1000 ㎍ 이상의 다량의 레가데노손을 포함한다. 더 바람직하게는, 최적의 투여량은 레가데노손의 약 100 ∼ 약 500 ㎍ 범위일 것이다. 레가데노손은 단일 일시 주사로 약 300 ㎍, 약 400 ㎍, 약 500 ㎍, 약 600 ㎍, 및 약 700 ㎍ 중에서 선택되는 양으로 투여하는 것이 바람직하다. 상기 양은 전형적으로 IV 주사에 의해 약 140 ㎍/㎏/분 의 속도로 연속적으로 투여되는 아데노신과 비교시 예기치 못하게 작은 것이다. 아데노신과 달리, 레가데노손은 환자의 체중에 관계없이 인간 환자에게 동일한 투여량으로 투여할 수 있다. 따라서, 심근 조영을 위하여 iv 일시 주사에 의해 레가데노손을 1회 일정량 투여하는 것은 시간 및 체중에 따른 아데노신의 투여보다 훨씬 간편하고 실수가 없다. 그러나, 인간 환자에게 투여되는 레가데노손의 투여량은 체중에 의해 결정될 수 있다. 전형적으로, 체중에 기초한 투여량은 약 0.05 ∼ 약 60 ㎍/㎏, 더 바람직하게는 약 0.1 ∼ 약 30 ㎍/㎏ 의 범위일 것이다. 레가데노손은 특히 기립 환자에게는 10 ㎍/㎏ 이하, 앙와(supine) 환자에게는 20 ㎍/㎏ 이하의 양으로 투여시 일반적으로 잘 용인된다.
대안적 실시양태에서, 레가데노손은 경구, 정맥내, 표피를 통해 또는 치료제 투여에 대해 종래 기술에 공지된 임의의 기타 방법으로 투여될 수 있으며, 일시 i.v. 주사 투여가 바람직하다. 하나의 실시양태에서, 일시 주사 투여는 60초 이하로 한다. 다른 실시양태에서, 일시 주사 투여는 약 30초 이하로 하고, 더 바람직하게는 약 20초 이하 또는 약 10초 이하로 한다.

실시예

본 연구의 목적은 건강한 인간 대상체에 있어서 약동학(PK), 약역학(PD), 및 레가데노손의 최대 용인되는 투여량을 조사하는 것이다.
36명의 건강한 남성 대상체가 연구에 포함된다. 대상체는 0.1 ∼ 30 ㎍/㎏ 범위의 레가데노손의 단일 IV 일시 주사 투여량을 투여받는다. 본 실시예 및 하기 실시예 2 및 3에서, 투여된 레가데노손 투여량은 상기 기재된 바람직한 성분을 포함하는 중성 pH의 투여량이었다. 약물 투여 후 24시간의 기간에 걸쳐 수집한 소변 샘플에서 여러 시간에 수집한 혈장 샘플 중 레가데노손의 농도를 측정하였다. ECG, 혈압(BP), 및 심박수(HR)를 투여후 24시간까지 기록하였다. 투여 후 24시간 동안 및 7일 후 전화를 통해 이상 반응(AE)을 모니터링하였다. 집단 접근방식을 사용하여 3-구획 PK 모델을 혈장 농도-시간에 적용하고, 미카엘리스-멘텐(Michaelis-Menten) 모델을 시간에 따른 심박수에 적용하였다. 각종 공변량이 PK 및 PD 모델 파라미터에 미치는 잠재적인 영향을 조사하였다.
집단 청소율(CL) 값은 40.6 ℓ/h 로 측정되었고, 여기서 신장 청소율이 총 청소율의 57%를 차지한다. 레가데노손의 분포 부피는 83.3 ℓ로 측정되었다. 모델에 의해 HR의 베이스라인 및 최대 증가는 62 및 76 bpm으로 측정되었다. HR의 최대 증가의 1/2을 야기하는 레가데노손의 농도(효능)는 12.4 ng/㎖ 로 측정되었다. 공변량, 예컨대 신체 질량 지수, 체중, 나이, 및 키는 PK 또는 PD 파라미터에 아무런 영향을 미치지 않았다. 이상 반응은 일반적으로 가벼움 내지 중간이었고, 급격히 시작되어 짧게 지속되며, 의료적 개입이 필요없었다. 이는 비정상적 불쾌감, 흉부 압박/긴장, 졸림, 설사, 홍조, 두통, 과호흡, 메스꺼움, 심계 항진, 구토, 및 투여 수준의 증가를 포함한다. 용인되는 최대 투여량은 앙와 자세에서 20 ㎍/㎏ 이고, 기립 자세에서 10 ㎍/㎏ 이며, 기립 자세의 대상체에서 투여량을 제한하는 기절 또는 거의 기절하는 현상이 관측되었다.
본 실시예는 레가데노손이 건강한 남성 대상체에서 잘 용인된다는 점을 증명한다. 공변량이 PK 및 PD 모델 파라미터에 임의의 유의한 영향을 미치지 않는다는 점은 레가데노손을 단위에 기초하여 투여할 수 있음을 보여주는 것이다.

본 연구는 임상적으로 심장 도관술이 지시된 대상체에서 레가데노손의 약동학(PK) 및 약역학(PD)을 조사하는 것이었다.
관상 동맥 조영술이 임상적으로 지시된 36명의 남성 및 여성 대상체를 연구하였다. 대상체는 10 ∼ 500 ㎍ 범위의 단일 IV 일시 주사 투여량의 레가데노손을 투여받았다. 약물 투여 이전 및 이후 다양한 시간에 수집한 혈장 샘플 중 레가데노손의 농도를 측정하였다. ECG, 심장내 도플러 혈류계(flow wire)를 사용하여 측정한 평균 심장 피크 혈류 속도(APV), 혈압(BP), 및 심박수(HR)를 투여 후 3시간까지 지속적으로 모니터링하였다. 이상 반응(AE)의 발생을 투여 후 대략 3시간 동안 및 대략 14일 후 전화를 통해 모니터링하였다. 혈장 농도, APV, 및 HR 데이타에 PK 및 PD 모델을 적용하는 데 있어서, 집단 접근방식을 이용하였다. 각종 공변량이 PK 및 PD 모델 파라미터에 미치는 영향을 조사하였다.
PK 데이타는 3-구획 모델에서 가장 잘 기술된다. 집단 청소율 값 및 분포 부피는 각각 29.9 ℓ/h 및 68.1 ℓ로 측정되었다. APV 데이타의 PD 모델은 가설 효과 부분을 포함하였다. 상기 결과는 레가데노손이 강력하고 잘 용인되는 심장 혈관 확장제라는 점을 증명한다. 공변량이 PK 및 PD 모델 파라미터에 미치는 유의한 영향이 없다는 점은 레가데노손을 단위에 기초하여 투여할 수 있다는 점을 나타낸다.

레가데노손은 심근 관류 조영 동안 관상 동맥 혈관계의 심한 확장의 진행 하에서 선택적 A2-아데노신 수용체 작용제이다. A2A-아데노신 수용체 활성화는 혈소판 응집 및 중성구 활성화의 억제를 야기하는 것으로 보고되어 있다.
본 연구에서, 약물을 더 완벽하게 특성화하기 위하여, 인간 혈소판 및 중성구(막 및 완전 세포), 인간 A2A 수용체를 발현하는 CHO 세포(막 및 완전 세포), 및 래트 뇌 선조체 막 제제 중 레가데노손에 대한 기능적 반응 및 결합에 대한 친화도 및 효능 값을 측정하였다. 비교를 위하여, 참조 A2A 작용제 CGS21680에 대한 반응의 평행 분석을 레가데노손의 각 분석과 함께 수행하였다. 분석 결과는 하기 표 1에 기재되어 있다.

Claims (11)
- 레가데노손(regadenoson) 및 1 이상의 약학적 부형제를 포함하는 약학적 조성물의 단일 투여량을 평균 심장 피크 혈류 속도를 약 16.5 ㎝/초 이상 증가시키기에 충분한 양으로 인간에게 투여하는 것을 포함하는, 말초 혈관 확장을 거의 야기하지 않는 심장 혈관 확장제의 제조 방법.
- 제 1 항에 있어서, 상기 약학적 조성물의 단일 투여량이 iv 일시 주사에 의해 투여되는 방법.
- 제 1 항에 있어서, 상기 약학적 조성물의 단일 투여량이 약 10 ∼ 약 20초로 투여되는 방법.
