Abstract

Description
(1)発明の分野
本発明は、心筋画像化を受けている哺乳類にレガデノソン(regadenoson)(アデノシンA2Aレセプターアゴニスト)の用量を投与することにより達成される心筋画像化方法に関する。

(2)技術の説明
心筋灌流画像化(MPI)は、冠状動脈疾患の検出および特徴決定に役立つ診断技法である。灌流画像化は、血流の不充分な領域を確認するために、物質(例えば、放射性核種)を使用する。MPIにおいて、血流は安静時に測定され、そしてその結果は、踏み台上での運動(心臓ストレステスト)中に測定される血流と比較される(このような運動は、血流を刺激するために必要である)。残念なことに、医学的状態(例えば、末梢血管疾患、関節炎など)に起因して、多くの患者は、充分な血流を提供するのに必要なレベルで運動することができない。

従って、心臓血流(CBF)を短い期間の間増加させる薬剤は、大きな利点があり、特に、末梢血管拡張を引き起こさないものは大きな利点がある。血管拡張剤(例えば、ジピリダモール)は、放射性核種を用いた画像化の前に患者においてこの目的で使われている。ジピリダモールは、長期作用性の化合物であり、長期にわたる副作用を逆転させるために解毒剤がしばしば必要になる。ジピリダモールは、(レガデノソンのような)ボーラスよりむしろ輸液である。ジピリダモールはまた、アデノシンレセプターについて非選択的であり、体重を基準とした投薬を必要とする。
天然に存在するヌクレオシドであるアデノシンもまた、血管拡張剤として有用である。アデノシンは、サブタイプA1、A2A、A2BおよびA3と特徴付けられるアデノシンレセプターファミリーと相互作用することによって、その生物学的作用を発揮する。Adenoscan(登録商標)は、天然に存在するアデノシンの製剤である。Adenoscan(登録商標)は、放射性タリウム−201を使用している灌流研究中のアジュバントとして市販されている。しかしながら、その使用は、副作用(例えば、潮紅、胸部不快感、深く呼吸したいという切迫感、頭痛、のど、首および顎の痛み)に起因して制限される。アデノシンのこれらの副作用は、A2A以外の他のアデノシンレセプターサブタイプの活性化に起因し、これは、アデノシンの気管支収縮へと末梢血管拡張作用を媒介する。さらに、アデノシンの短い半減期は手順中の持続した注入を必要とし、このことは、その使用をさらに複雑にする。Adenoscan(登録商標)は、第二度または第三度ブロック、洞房結節疾患、気管支収縮または気管支痙攣肺疾患を有する患者を含めた多くの患者において、そして、薬物に対する既知の過敏症を有する患者において、禁忌である。
A2Aアデノシンレセプターについての他の強力かつ選択的なアゴニストは公知である。例えば、MRE−0470(Medco)は、アデノシンの強力かつ選択的な誘導体であるアデノシンA2Aレセプターアゴニストである。WRC−0470(Medco)は、画像化においてアジュバントとして使用されるアデノシンA2Aアゴニストである。一般に、これらのような化合物は、高いA2Aレセプター親和性を、その結果、長い作用持続時間を有する。長い作用持続時間は、画像化において望ましくなく、おそらく副作用の持続期間を長期化させ得る。

(好適な実施形態の説明)
強力なA2Aアゴニストは、画像化剤を投与する前または画像化剤と同時のいずれかで添加される場合、心臓画像化における補助物として有用である。適切な画像化剤としては、201タリウムまたは99mテクネチウム−セスタミビ(Sestamibi)、99mTc−テボロキシム(teboroxime)およびテクネチウム−99m(III)が挙げられるが、これらに限定されない。
強力なA2Aアゴニストは、画像化剤を投与する前または画像化剤と同時のいずれかで添加される場合、心臓画像化における補助物として有用である。適切な画像化剤としては、201タリウムまたは99mテクネチウム−セスタミビ(Sestamibi)、99mTc−テボロキシム(teboroxime)およびテクネチウム−99m(III)が挙げられるが、これらに限定されない。
CBFを上昇させるが、末梢の血流を顕著には上昇させない、新規でかつ強力なA2Aアゴニストが確認された。1つの特に有用なA2Aアゴニストはレガデノソンである。レガデノソンは、文献において、CVT−3146または(1−{9−[(4S,2R,3R,5R)−3,4−ジヒドロキシ−5−(ヒドロキシメチル)オキソラン−2−イル]−6−アミノプリン−2−イル}ピラゾール−4−イル)−N−メチルカルボキサミドともいわれ、以下の式を有する:
レガデノソンは、当該分野で公知である薬学的投与方法により投与され得る。レガデノソンが静脈内投与されることが好ましい。レガデノソンが一用量のi.v.において投与されることがより好ましい。用語「一用量」とは、一般に、治療的な量のレガデノソンの、迅速に投与される一用量をいう。用語「一用量」は、(例えば、連続した静脈内注入によって)長期間にわたって投与される用量を包含しない。
レガデノソンは代表的に、使用の前に薬学的組成物に組み込まれる。用語「薬学的組成物」とは、一緒になって溶液または懸濁液を形成する、レガデノソンと少なくとも一つの液体キャリアとの組合せをいう。使用の前に適切な液体キャリアの添加によって凍結乾燥粉末を再構成することが意図される限り、本発明の組成物を含む凍結乾燥粉末は、「薬学的組成物」の範囲内にある。適切な液体キャリアの例としては、水、蒸留水、脱イオン水、食塩水、緩衝溶液、正常等張生理食塩水溶液、デキストロース水溶液およびそれらの組み合わせが挙げられるがこれらに限定されない。このような薬学的組成物は一般に、注入に適している。
用語「緩衝溶液」または「緩衝液」は、本明細書で用いられる場合、弱酸およびその共役弱塩基の両方を含んでいる溶液をいう。緩衝溶液は、pH変化に耐えるために、本発明の薬学的組成物において使われる。有用な緩衝溶液の非限定的な例は、重炭酸ナトリウムおよびリン酸ナトリウムを含む溶液である。
本発明の化合物を含む薬学的化合物および/またはその誘導体は、非経口投与のための溶液または凍結乾燥粉末として処方され得る。粉末は、使用の前に、適切な希釈剤または他の薬学的に受容可能なキャリアの添加によって再構成され得る。液体状態において使われる場合、本発明の化合物は好ましくは、緩衝された、等張性の水溶液に組み込まれる。適切な希釈剤の例は、正常等張性生理食塩水、標準5%デキストロース水溶液および緩衝化酢酸ナトリウム溶液または緩衝化酢酸アンモニウム溶液である。このような液状処方物は非経口投与に適しているが、経口投与のために使われてもよい。ポリビニルピロリジノン、ゼラチン、ヒドロキシセルロース、アカシア、ポリエチレングリコール、マンニトール、塩化ナトリウム、クエン酸ナトリウムまたは当業者に公知の他の任意の賦形剤のような賦形剤を本発明の化合物を含む薬学的組成物に添加することが望ましい場合がある。
レガデノソンを含む薬学的組成物は、調製されて、その後、投与され得、その間には貯蔵期間があっても無くてもよい。本発明の薬学的組成物を処方する場合に考慮されるさまざまな特性としては、製品の貯蔵寿命、レガデノソンの溶解度、組成物のpH、静脈刺激、溶血、貯蔵条件(例えば、薬学的組成物が室温で保存されるか、または他の何らかの温度で保存されるか)および殺菌手順に耐える能力が挙げられるがこれらに限定されない。
所望の薬学的組成物特性を達成する1つの方法は、薬学的組成物に共溶媒を含めることである。共溶媒は、薬学的組成物に所望の特性を与える、溶液中の任意の液体または化合物から選択され得る。有用な共溶媒の例としては、メチルボロン酸、ホウ酸緩衝液、プロピレングリコールまたはポリエチレングリコールが挙げられるがこれらに限定されない。薬学的組成物中の共溶媒の量は、選択されたA2Aレセプターアゴニストの特性(例えば、溶解度および安定性)に依存する。共溶媒を含んでいる薬学的組成物の例は、米国特許公開番号第2005/0020915号に見出され得る。米国特許公開番号第2005/0020915号の明細書は、その全体が本明細書中に参考として援用される。
レガデノソンは、約50マイクログラム/mLの水への溶解度を有する。従って、所望の重量の量のレガデノソンが受容可能な体積において投与され得る限り、レガデノソンは、水に溶解されて投与され得る。例えば、約400マイクログラムという好適な用量は、8mLの水において投与され得る。この体積が投与目的には大きすぎる場合、あるいは、薬学的組成物が室温(RT)以外で保存される場合、この組成物中でのレガデノソンの溶解度を上昇させるため、および/または得られる薬学的組成物に他の改善された特性(例えば、改善された安定性および貯蔵特性)を提供するために、さらなる成分をこの組成物に添加し得る。
レガデノソンを含む本発明の薬学的組成物は、約1ミリグラム/mlまでのレガデノソンを含み得る。レガデノソンを含む薬学的組成物が、約50/mL〜約250マイクログラム/mLの、そして好ましくは約50〜150マイクログラム/mLのレガデノソンを含むことが好ましい。
溶解度および貯蔵特性を改善するために、レガデノソンは、メチルボロン酸(MBA)共溶媒を含んでいる薬学的組成物において投与され得る。メチルボロン酸は、アゴニストの溶解度および貯蔵寿命を改善するために、薬学的組成物に添加される。MBAは、得られる組成物のpHを上昇させる。MBAを含んでいる薬学的組成物におけるレガデノソンの溶解度は、この組成物のpHが中性に向かって低下するにつれて、減少する傾向がある。従って、レガデノソンについては、MBA含有組成物の最適なpHは、約8.5〜約10であり、約9.1〜約9.4のpHが好ましく、そして約9.3のpHが最も好ましい。これは、約50mg/mL〜約250mg/mLのMBAを含んでいる組成物に対応する。MBAに代わるものとして、レガデノソンは、ホウ酸緩衝液と組み合わされ得る。代表的に、ホウ酸緩衝液は、酸または塩基を用いて所望のpH(例えば、9.3のpH)に調整されたホウ酸ナトリウム水溶液からなる。
MBAを含んでいる薬学的組成物は、貯蔵の問題を受け得る。すなわち、特定のI型ガラス容器に包装された場合にMBAによって離層が生じ得る。この問題は、MBAを含んでいる薬学的組成物をプラスチック容器中に、またはより抵抗性のI型ガラス容器中に保存することにより解決され得る。
中性により近いpHを有するレガデノソン含有薬学的組成物が所望される場合、代替法は、レガデノソンをプロピレングリコール(PG)共溶媒と組み合わせることである。レガデノソンを使用する場合、組成物中で使用されるPG量は、約5%体積%から約25体積%までの範囲にわたり得、約8体積%〜約20体積%の範囲がより好ましい。PGに対する代替物は、ポリエチレングリコール−PEGである。好適なPEGは、約200〜400の平均分子量を有する。
好ましくは、PGまたはPEGを含むレガデノソン組成物は、約6〜約8のpHを有し、約7のpHが好ましい。組成物のpHを所望の値に調整し得る任意の生理的に受容可能な緩衝剤が用いられ得る。このような緩衝剤の例としては、二塩基性リン酸ナトリウム、二塩基性リン酸ナトリウム脱水物および一塩基性リン酸ナトリウム一水和物が挙げられるが、これらに限定されない。EDTAおよびジメチルアセトアミドのような必要に応じたさらなる成分は、同様に組成物において使用され得る。
本発明の薬学的組成物は、一つ以上の抗酸化剤(例えば、ブチル化ヒドロキシアニソール(BHA))を含み得る。
レガデノソンは、投与される場合、迅速な作用開始および短い作用持続時間を有する。レガデノソンは、単一のボーラス静脈内(i.v.)注射において非常に少ない量で投与される場合、非常に有用である。レガデノソンは、10μg程度のごく少量で、そして2000μg以上の極めて多量で投与され得る。最適な用量は、10μg程度のごく少量および約1000μg以上の極めて多量のレガデノソンを含み得る。より好ましくは、最適な用量は、約100μg〜約500μgのレガデノソンの範囲にわたる。レガデノソンが約300μg、約400μg、約500μg、約600μgおよび約700μgから選択される量の単一のボーラス注射において投与されることが好ましい。代表的には約140μg/kg/分の速度の静脈内注入によって連続的に投与されるアデノシンと比較した場合、これらの量は予想外に少ない。アデノシンとは異なり、同じ投与量のレガデノソンは、患者の体重に関係なくヒト患者に投与され得る。このように、心筋画像化のための静脈内ボーラスによる単一の均一量のレガデノソンの投与は、時間および体重に依存したアデノシンの投与よりも劇的に単純であり、かつ誤りの傾向が少ない。しかしながら、ヒト患者に投与されるレガデノソンの用量は、体重によって決定されてもよい。代表的に、体重に基づく用量は、約0.05μg/kg〜約60μg/kgの範囲にわたり、そしてより好ましくは、約0.1〜約30μg/kgの範囲にわたる。レガデノソンは特に、立位患者においては10μg/kgまでの量、そして仰臥患者においては20μg/kgまでの量において投与される場合、一般に充分許容される。
代替の実施形態では、レガデノソンは、経口で、静脈内で、表皮を通して、または当該分野で公知の他の任意の治療剤投与手段によって投与され得、ボーラス静脈内投与が好ましい。1つの実施形態では、ボーラス投与は、60秒以下において行われる。なお他の実施形態において、ボーラス投与は、30秒以下において、そしてより好ましくは約20秒以下において、または約10秒以下において行われる。
レガデノソンの薬物動態は、以下の実施例においてさらに詳細に開示される。
(実施例1)
本研究の目的は、健常なヒト被験体におけるレガデノソンの薬物動態(PK)、薬力学(PD)および最大許容用量を調査することであった。

36人の健常な男性被験体が本研究に含まれた。被験体は、0.1μg/kg〜30μg/kgの範囲にわたる1静脈内ボーラス用量のレガデノソンを受けた。本実施例において、ならびに下記の実施例2および3において投与されたレガデノソン投与量は、上記で考察した好適な成分を含んでいる中性のpH用量であった。レガデノソンの濃度を、様々な時点で収集された血漿サンプルにおいて、および薬物投与後24時間にわたって収集された尿サンプルにおいて決定した。ECG、血圧(BP)および心拍数(HR)を、投与後24時間まで記録した。有害な事象(AE)を、服用後24時間にわたってモニタリングし、そして7日後に電話を介してモニタリングした。3区画PKモデルを血漿濃度−時間に適用する際およびミカエリス−メンテンモデルを心拍数の時間経過に適用する際に、集団アプローチを利用した。PKモデルパラメータおよびPDモデルパラメータに対する様々な共変量の潜在的影響を調査した。
クリアランス(CL)の集団値を40.6L/hであると見積もり、腎臓クリアランスは総クリアランスの57%を占めると見積もった。レガデノソンの分布体積を83.3Lであると見積もった。このモデルにより、HRのベースラインが62bpmであり、最大増加が76bpmであると見積もった。HRの最大の半分の増加を引き起こすレガデノソン濃度(効力)は、12.4ng/mLであると見積もられた。共変量(例えば、ボディーマスインデックス、体重、年齢および伸長)は、PKパラメータにもPDパラメータにも影響を及ぼさなかった。有害事象は、一般に軽度から中度の、迅速に発症し、短い持続期間で、医療的介入が何も必要でないものであった。それらは、腹部不快感、胸部圧/窮屈感、めまい感、呼吸困難、潮紅、頭痛、過換気、悪心、動悸および嘔吐を含んでおり、そして用量のレベルによって増加した。最大許容量は仰臥位において20μg/kgであり、そして立位において10μg/kgであり、用量を制限する失神または失神に近いものが立位の被験体において観察された。
本実施例は、レガデノソンが健常な男性被験体において十分に許容されることを実証する。PKモデルパラメータおよびPDモデルパラメータに対して共変量によるいかなる重大な影響もないことは、レガデノソンについての単位ベースの投薬を示唆する。
(実施例2)

PKデータは、3区画モデルによって、最もよく記載された。クリアランスおよび分布体積の集団値は、それぞれ、29.9L/hおよび68.1Lであると見積もられた。APVデータのPDモデルは、仮想効果区画を含んでいた。APVにおけるベースラインおよび最大の増加を、このデータに基づいて、29.9ng/mLの効力(最大の半分の効果を引き起こすレガデノソン濃度)において、16.5cm/秒および105cm/秒であると見積もった。このモデルにより、血漿から効果部位への分布速度定数についての小さな値(4分−1)が見積もられた。これは、効果のかなり迅速な開始を示す。ミカエリス−メンテンモデルによりHRデータが最良に適合し、HRにおけるベースラインおよび最大の増加についての推定値は67bpmおよび41bpmであり、そして27.5ng/mLの効力であった。共変数(例えば、ボディーマスインデックス、体重、年齢および伸長)は、PKパラメータにもPDパラメータにも重大な影響を及ぼさなかった。AEは、被験体の半分(n=17)より少数の人について報告された;3人以上の被験体について報告される事象は、胸部不快感(n=3)、頻脈(n=4)およびカテーテル部位での出血(n=3)であった。

Claims (11)
- 末梢血管拡張をほとんど有さずに冠状血管拡張を生じる方法であって、平均冠状血管ピーク流速を少なくとも約16.5cm/秒上げるのに充分である量の、レガデノソンおよび少なくとも一つの薬学的賦形剤を含む一用量の薬学的組成物をヒトに投与する工程を包含する、方法。
- 前記一用量の薬学的組成物が、静脈内ボーラスにより投与される、請求項1に記載の方法。
- 前記一用量の薬学的組成物が、約10秒間〜約20秒間において投与される、請求項1に記載の方法。
- 投与される前記一用量の薬学的組成物の量が、平均冠状血管ピーク流速を約16.5cm/秒から約77.0cm/秒までの範囲の量上昇させるに充分である、請求項1に記載の方法。
- 前記一用量の薬学的組成物が、約10マイクログラム〜約500マイクログラムのレガデノソンを含む、請求項1に記載の方法。
- 前記一用量の薬学的組成物が、約0.05μg/kg〜約60μg/kgの範囲の量のレガデノソンを含む、請求項1に記載の方法。
- 前記ヒトへの前記一用量の薬学的組成物の投与後に、該ヒトの心筋灌流画像化が行われる、請求項1に記載の方法。
- 前記ヒトが前記一用量の薬学的組成物を受ける前、前記一用量の薬学的組成物の投与と同時、または、前記ヒトに前記一用量の薬学的組成物を投与した後からなる群より選択される時点で該ヒトに少なくとも一つの放射性核種を投与する工程をさらに包含する、請求項7に記載の方法。
- 前記放射性核種と前記一用量の薬学的組成物とが前記ヒトに別々に投与される、請求項7に記載の方法。
- 前記放射性核種と前記一用量の薬学的組成物とが前記ヒトに同時に投与される、請求項7に記載の方法。
- 心筋検査が、前記一用量の薬学的組成物が前記ヒトに投与された時点から約1分間以後に始まる、請求項7に記載の方法。
