Abstract

Description

현재, 일반적으로 처방은 상당한 정도의 자동화의 도움없이 대개 소매 약국에서 대부분 약사에 의해 직접 수작업으로 수행된다. 현재의 처방순서는 처방목록을 프린트하며, 지시목록이나 처방이 완료되었을 때의 비용입력이 수행되는 것 뿐만 아니라 환자의 이력을 점검하는 것과 같은 업무흐름 작업이 다양하게 수행되는 컴퓨터 시스템에 의해 이루어진다. 전형적으로 처방전은 단지 약사, 기사, 점원, 및 처방지시 기재에서부터 약제비용까지의 모든 잔일을 수행하는 회계원과 같은 단지 몇 사람에 의해 손으로 기입된다.
현재의 소매 약국 시스템의 단점은 대부분의 소매 약국들은 조제자동화 장비를 아무도 사용하지 않거나 극소수가 사용한다는 것이다. 또한 품질 관리를 위해 처방이나 약제에 관한 영상을 디스플레이하는 컴퓨터를 사용하지 않는 것이다. 그러지 않는 대신 환자의 의약처방의 처음에서 끝까지 필요한 작업량을 처리하는 것을 인력에 의존하는 것이다.
<종래기술의 설명>
의약처방 수행단계를 부분적으로 자동화한 다양한 형태의 약국 시스템은 종래에도 발표되었다. 예를 들면, 샤아후트(Charhut)의 미국특허 제5,208,762호에는 약품조제를 위한 방법 및 장치가 개시되어 있다.
이 시스템은 약사가 환자의 신원과 처방을 입력하는 컴퓨터 단말; 환자순서에 따라 하나 또는 그 이상의 처방을 갖는 약병에 자동적으로 채우고, 라벨하고, 닫고, 분류하는 일련의 기계들; 및 분류 컨베이어를 이용하여 약사가 수작업으로 환자목록을 보게 되는 픽업(Pick-up) 스테이션으로 이송시키는 환자 처방전 이송수단;을 제공한다. 이 시스템은 통신판매용 처방전을 수행하거나 외래 픽업에 사용되지만, 처방을 수행하는 부분을 수반하는 설비의 크기나 설비가 차지하는 공간 때문에 소매 약국용으로는 사용될 수 없다.
하버손(Harvorson)의 미국특허 제4,847,764호에는 보건소, 병원 또는 메디컬 클리닉(Medical Clinic) 등의 의료기관에서의 의약조제를 위한 약국 시스템을 개시하고 있다. 상기 조제 시스템은 키보드, 프린터, 및 조제 인터페이스를 구비한 디지털 컴퓨터와 같은 하드웨어를 구비한다. 다른 조제 하드웨어는 키보드가 있는 비디오 디스플레이 장치를 구비한 전자기계적인 조제기들을 구비한다.
필수적으로, 컴퓨터와 그와 관련된 하드웨어 및 소프트웨어는 주어진 처방대로 조제를 수행하기 위하여 의사 및 전자조제기에 기술된 처방전을 수행하기 위하여 환자에 관한 중요한 모든 정보를 기록하고 있으며, 전체 의료시스템을 모니터하기 위해 저장된 기록을 출력할 수 있도록 되어 있다.
이 시스템은 주로 소매 약국에서가 아닌 병원에 설치되어서 사용된다.
바움(Baum)의 미국특허 제4,918,604호에 의하면 우편 서비스 약국 또는 소매 약국에서 처방약을 조제하기 위한 약 라벨링 및 조제충전 시스템이 개시되어 있다. 이 시스템은 키보드 터미널에 의해 제어되는 메인 프레임(Main Frame) 컴퓨터를 구비하고, 그 컴퓨터는 처방라벨들에 입력된 각각의 처방을 조함하여 수행하는 프로그램을 내장하고 있으며, 상기 처방라벨들은 해당 라벨에 기록된 해당 약품정보, 처방지시 및 환자명, 의사명 및 약사가 조제하는데 필요한 기타 생명징후 데이터와 같은 처방정보를 가지고 있다. 완료된 각각의 처방은 컴퓨터에 의해 바코드 번호로 지정하는데, 상기 처방에 고유의 번호이다.
또한 전술한 시스템은 약사를 위한 마이크로 컴퓨터를 구비하고 있고, 디스플레이 스크린과, 컬러 그래픽을 프린트할 수 있는 컬러프린터와, 처방약의 그래픽을 저장한 디스크 파일 및 바코드 판독기를 역시 구비하고 있는데, 상기 바코드 판독기는 처방정보를 확인하기 우하여 사용된다. 이 시스템은 다른 조제장비나, 컴퓨터 영상 포착이나, 약 영상 디스플레이 점검용 하드웨어 및 그에 관련된 소프트웨어를 사용하지 않는다.

본 발명의 다른 목적은, 환자에게 의사가 최초에 처방한 전자영상을 포착하도록 디자인되고, 작업자가 상기 웨크스테이션에서 특정 처방과정에서의 변수를 설정할 수 있도록 하는 영상용 워크스테이션을 제공하는 데 있다. 이것은 수행과정에서 최초에 기록된 처방의 정확성을 유지하는 영상용 시스템을 사용함으로써 문서작업을 상당히 줄인다.

본 발명은 처방기입에서부터 지불에 이르기까지 설정하는 소매 약국의 의약조제 수행과정을 자동화하는 하드웨어와 소프트웨어를 구비하는 통합된 소매 약국 시스템에 관한 것이다. 이 시스템은 영상화, 충전 및 검사하는 기능을 제공하는 적어도 세 개의 워크스테이션을 구비하고 있다.

본 발명은 의약처방 수행과정을 자동화하는 소매 약국 시스템을 위한 장치를 제공한다. 약국 시스템은 적어도 중앙집중식 데이터베이스로부터 컴퓨터 지시에 의해 작동하는 세 개의 상호연관되고, 필수적이며, 또한 기본적인 워크스테이션들을 구비한다. 이들 워크스테이션들은 처방수행 과정에서 영상화하고, 충전하고, 점검하는 기능을 수행한다.

충전용 워크스테이션은 약조제부의 구성품을 제어하도록 되어 있다. 약조제부는 복수의 계수기와 이동가능한 선반을 갖는 복수 개의 수직 카루셀을 구비할 수 있다. 또한 충전용 워크스테이션은 약분배부의 다른 형태를 갖는다. 충전용 워크스테이션은 영상용 워크스테이션을 이루는 제 1 컴퓨터에 연결된 제 2 컴퓨터와, 조제된 의약품을 고객용 약병에 계수, 조제, 포장하는 조제장치를 더 구비하는데, 상기 제 2 컴퓨터는 다양한 조제설비를 제어하도록 이루어진다.
점검용 워크스테이션은 완료된 의약처방을 최종 품질검사하는 약사를 보조하도록 사용된다. 이 스테이션에서 상기 컴퓨터는 의사의 최초 영상과 조제된 약품영상 모두를 비교하기 위해 디스플레이한다. 점검용 워크스테이션은 충전용 워크스테이션의 제 2 컴퓨터에 전기적으로 연결된 제 3 컴퓨터를 구비하고, 게다가 주어진 약병에 부착된 바코드 라벨을 스캐닝하는 스캐너 및 최초 의약처방에 대한 수치화된 영상을 디스플레이하고, 특정 조제약품의 수치화된 영상을 디스플레이하는 디스플레이를 구비한다. 최종 품질검사 단계에서는 처방된 약품의 수치화된 영상과 특정 약병에 투입되어 있는 처방된 약품간을 비교하는 제 1 영상 비교와, 고객에게 제공되기 전에 최초 의약처방의 수치화된 영상과, 특정 약병에 투입되어 있는 수치화된 영상을 비교하는 제 2 영상 비교를 수행하는 약사를 필요로 한다.
그때 약사는 처방이 적절하게 이루어짐을 나타내는 약사의 개인 바코드에 의한 전자원드(Electronic wand)를 사용하여 종료한다.

본 발명의 바람직한 실시예에 따른 약국 시스템(10)과 그의 부속부분들은 도1에서 도6을 통해 자세히 개시되어 있다. 약국 시스템(10)은 하드웨어 요소들과 그와 관련된 적어도 세 개의 상호연관되고 필수적인 워크스테이션들에 사용되는 소프트웨어를 구비하고, 워크스테이션들은 영상용 워크스테이션(100), 충전용 워크스테이션(200) 및 점검용 워크스테이션(300)을 포함한다. 이러한 워크 스테이션들(100, 200, 300)은 중앙집중화된 데이터베이스로부터 컴퓨터에 제어되고, 도1과 도2에서 보는 바와 같이 약국 시스템(10)의 자동화된 의약처방 수행과정을 영상화하고, 충전하고, 점검하는 기능을 수행한다.

처방전(222)은 처방 및 충전을 위한 생명징후 자료를 포함하는 적어도 20개 분야의 정보로 이루어진다. 처방전(222) 상의 정보중에서 가장 중요하고 관련있는 분야는 환자명, 처방번호, 약품명, 의사명, 그리고 엔디시 번호이다.

충전용 워크스테이션(200)에는 충전용 워크스테이션 영역(202)과 점검용 워크스테이션 영역(302)에 근접되게 포장용 워크스테이션(280)이 구비된다. 포장용 워크스테이션(280)은 포장보지부(284)와 포장용백(Tote Bag)(290)이 구비되어 있는 포장용 워크스테이션 영역(282)이 구비된다. 포장보지부(284)는 충전된 처방약병(232)과 처방전(222)를 보관하고, 피크라이트(288)가 구비되어 있는 복수 개의 대형 커비홀 슬롯(286)을 가지고 있다. 또한, 포장용 워크스테이션(280)은 트랜잭션 데이터 기록부(220) 내에 저장되어 있는 주민번호를 기초로 하여 온(On) 되도록 교대로 피크라이트(288)를 트리거하는 바코드를 스캐닝(18) 용으로 사용되는 이동용 바코드 스캐너원드(292)를 구비하고 있다. 포장용 워크스테이션(280)의 기능은 점검용 워크스테이션 영역(302)에서 한 명 또는 가족 단위의 고객에 대해 최종 검사하는 약사에게 처방전을 건네주기 전에 충전 기사나 판매원에 의해 약국 시스템(10)의 의약처방(12)을 완성하고, 모으고, 포장하는 것이다.

점검용 워크스테이션(300)에서, 등록된 약사는 적절한 약품(230)과 처방된 의약처방(12) 양이 정확하게 조제되었는지를 검사한다. 최초에 쓰여진 의약처방의 수치화된 영상(14)과 조제된 의약품(16)의 영상을 이용하고 상기 영상들을 고객에게 건네지기 전의 실제 처방된 의약품(230)과 비교함으로써 검사가 수행된다.

<본 발명에 의한 운용방법>
운용에 있어서, 수행되는 처방의 흐름경로는 영상용 워크스테이션으로부터 충전용 워크스테이션 그리고 점검용 워크스테이션으로 이루어진다. 약국 시스템(10)의 조제과정은 고객이 의사의 의약처방(12)을 판매원 또는 약사에게 제공될 때부터 비롯된다. 의약처방(12)으로부터 얻어진 정보는 트랜잭션 데이터 기록(220)과 처방전(222)가 기재된 정보부분을 이루는 데이터베이스(110)로 입력된다.
또한, 고객은 구두(口頭)로 판매원이나 약사와 같은 약국직원에게 기타 개인정보를 제공하고, 약국직원은 수작업에 의해 개인정보를 호스트 컴퓨터(104)의 키보드(106)로 데이터베이스(110)에 입력한다. 이러한 정보는 트랜잭션 데이터 기록(220) 및 처방전(222)의 필요한 부분의 잔여부분을 채우도록 사용된다.
라벨프린터(114)는 바코드가 기재된 약병라벨(118)과, 필요한 정부법률, 백라벨(224), 및 약안내 라벨(226)이 표시된 라벨을 포함하는 처방전(222)를 프린트한다. 동시에 트랜잭션 데이터 기록(220)은 직렬데이터 링크(216)를 통하여 저장용 데이터베이스(130)에 전송된다. 트랜잭션 데이터 기록(220) 및 처방전(222)에 대한 정보는 데이터베이스(110)를 통해 프린터(114)로 제공된다.
그후 판매원 및 약사는 수동파지식 스캐너원드(128)로 처방(12)에 의한 라벨(118)의 바코드(18)를 스캔한다. 그리고 판매원 및 약사는 스캐너 수신기(126)로 처방전(12)을 스캔하여 바코드(18)에 포함된 처방번호를 최초 의약처방(12)의 영상(14)과 결합한다. 의약처방(12) 영상(14)은 데이터베이스(130)에 저장된다. 최초의 처방전(12)은 나중에 판매원에 의해 정부기관을 위한 영구기록으로 보관된다.
저장된 처방 영상(14)은 점검용 워크스테이션(300)에서 품질검사용으로 사용된다. 정제, 알약, 캡슐 및 동류와 같은 형태의 약품(230)을 보여주는 의약품의 시각영상(16)들은 편집되어서 소프트웨어 프로그램(20)에 입력되고, 데이터베이스(130)에 저장되며, 약조제과정의 점검용 워크스테이션(300)에서 품질검사용으로 사용된다. 처방전(222)는 수작업으로 조제과정을 위해 충전용 워크스테이션(200)에 이송된다.

의약처방(12)으로부터 얻어진 라벨(118)에 형성된 바코드(18)에는 저장부(130)에 저장되는 트랜잭션 데이터 기록(220)의 인덱스로 제공되는 처방번호가 형성되어 있다. 저장용 데이터베이스(130)의 정보는 약조제부(240, 250, 260, 270)가 올바른 처방약(230)을 분배하기 용이하도록 결정하는데 사용된다. 그리하여, 약사가 라벨을 스캔(18)할 때 필요한 정보가, 적절한 약조제부(240, 250, 260, 270)를 움직이도록 하는 제 2 컴퓨터(204)에 제공된다.

  1. a) 조제된 의약품과 고객정보에 대한 최초 의약처방 기입자료를 수신하고, 조제수행자료의 기록을 제공하는 호스트 컴퓨터와,
    상기 호스트 컴퓨터에 전자적으로 연결되고, 상기 최초 의약처방의 수치화된 영상을 형성하는 영상 스캐너를 구비하는 제 1 컴퓨터와,
    조제된 의약품이 수용되는 약병에 부착되는 처방라벨을 프린트하는 제 1 프린터와,
    상기 약병에 부착되는 바코드 라벨을 프린트하는 제 2 프린터 및
    상기 조제수행자료의 기록을 상기 호스트 컴퓨터로부터 상기 제 1 컴퓨터로 전송하는 전송수단으로 이루어지는 영상용 워크스테이션;
    b) 상기 영상용 워크스테이션을 이루는 상기 제 1 컴퓨터에 연결된
    제 2 컴퓨터와,
    상기 조제된 의약품을 고객용 상기 약병에 계수, 조제, 포장하는 조제수단 및
    상기 조제수단을 제어하는 상기 제 2 컴퓨터를 구비하는 충전용 워크스테이션;
    및
    c) 상기 충전용 워크스테이션의 상기 제 2 컴퓨터에 전기적으로 연결되고,
    상기 약병에 부착된 상기 바코드 라벨을 스캐닝하는 스캐너와,
    상기 최초 의약처방에 대한 상기 수치화된 영상을 디스플레이하고, 상기 조제된 의약품과 상기 약병에 투입된 상기 조제된 의약품을 제 1 영상비교를 참작하도록 상기 조제된 약품의 수치화된 영상을 디스플레이하는 디스플레이를 구비하는 제 3 컴퓨터와,
    상기 최초 의약조제에 대한 상기 수치화된 영상과 고객에게 제공되기 전에 상기 약병에 투입된 상기 조제된 의약품을 비교하는 제 2 영상비교를 구비하는 점검용 워크스테이션;
    을 구비하여 고객을 위한 의약처방 수행단계를 자동화하는 약국 시스템.
  2. 제 1 항에 있어서,
    상기 고객이 사용하는 용도로 상기 조제된 약품을 포장하는 다수개의 홀딩(Holding) 선반을 구비하는 홀딩 스테이션을 갖는 포장용 워크스테이션을 더 구비함을 특징으로 하는 상기 약국 시스템

    상기 바코드 라벨을 스캐닝하는 포터블 바코드 스캐너를 더 구비함을 특징으로 하는 상기 약국 시스템.
  5. 제 1 항에 있어서, 상기 영상 스캐너는,
    키보드, 스캐너 스크린, 스캐너 수신기, 및 상기 최초 의약처방의 수치화된 영상을 보존하는 데이터베이스 저장수단을 구비함을 특징으로 하는 상기 약국 시스템.
  6. 제 2 항에 있어서, 상기 점검용 워크스테이션은,
    상기 점검용 워크스테이션에서 상기 고객에게 전달될 때까지 완료된 처방전과 진행중인 처방전을 취급하기 위한 다수개의 선반을 구비하는 윌-콜 워크스테이션을 더 구비함을 특징으로 하는 상기 약국 시스템.
  7. 제 1 항에 있어서, 상기 충전용 워크스테이션은,
    상기 바코드 라벨을 스캐닝하고, 그에 의한 응답으로 미리 선택된 조제유닛을 가동시키는 포터블 바코드 스캐너를 더 구비함을 특징으로 하는 상기 약국 시스템.
  8. 제 1 항에 있어서, 상기 조제수단은,
    다수 개의 조제 카루셀(Carousel)로 이루어지며,
    상기 다수개의 조제 카루셀에 포함되는 약품병을 보관하는 다수 개의 이동가능한 선반과,
    상기 조제 카루셀에 전기적으로 연결된 다수 개의 계수장치 및
    상기 조제 카루셀에 전기적으로 연결된 다수 개의 중량계로 이루어짐을 특징으로 하는 상기 약국 시스템.
  9. 제 1 항에 있어서, 상기 조제수단은,
    상기 약병으로 약이 자동적으로 계수되고 조제되도록 상기 조제수단에 각각 내부 카운터와 활강로를 구비하는 다수개의 디스펜서 홀딩 셀(Dispenser Holding Cell)을 구비함을 특징으로 하는 상기 약국 시스템.
  10. 제 1 항에 있어서, 상기 조제수단은,
    상기 약병에 상기 약품을 자동적으로 카운트하고 조제하기 위해 상기 조제수단에 각각 활강로가 형성된 다수 개의 카세트 카운터를 갖는 다수개의 디스펜서 홀딩 카세트를 구비함을 특징으로 하는 상기 약국 시스템.
  11. 고객을 위한 의약 처방수행 과정을 자동화하기 위하여
    a) 호스트 컴퓨터를 구비한 영상용 워크스테이션에서,
    상기 호스트 컴퓨터가 처방된 약품 및 고객정보와 처방 트랜잭션 데이터 기록을 제공하기 위해 최초 의약처방을 수행하는 단계;
    영상 스캐너를 이용하여 상기 최초 의약처방의 수치화된 영상을 형성하는 단계;
    조제된 약품을 받기 위해 약병에 부착되는 처방라벨을 프린트하는 단계; 및
    상기 약병에 부착되는 바코드 라벨을 프린트하는 단계;
    b) 상기 영상용 워크스테이션에 인접되게 위치한 충전용 워크스테이션에서,
    계수하고, 조제하고, 그리고 상기 포장된 약품을 상기 고객용 상기 약병에 포장하는 단계;
    및
    c) 상기 충전용 워크스테이션에 인접되게 위치한 점검용 워크스테이션에서,
    상기 약병에 부착된 상기 바코드 라벨을 스캐닝하고, 상기 최초 의약처방의 상기 수치화된 영상을 디스플레이하는 단계; 및
    상기 조제된 약품의 상기 수치화된 영상과 상기 약병에 투입된 상기 조제된 약품을 제 1 영상비교하고, 상기 최초 의약처방된 상기 수치화된 영상과 상기 약병에 투입된 상기 조제된 약품을 고객에게 제공하기 전에 제 2 영상비교하도록 상기 조제된 약품의 수치화된 영상을 디스플레이하는 단계;
    를 구비하여 이루어짐을 특징으로 하는 약국 시스템.
  12. 제 11 항에 있어서,
    백 라벨(Bag Label)과 약품정보 라벨을 프린트하는 단계를 더 구비함을 특징으로 하는 상기 약국 시스템.
  13. 제 11 항에 있어서,
    이동하고, 라벨링(Labeling)하고, 배깅(Bagging)하고, 그리고 포장용 워크스테이션에 상기 조제된 약품을 홀딩하는 단계를 더 구비함을 특징으로 하는 상기 약국 시스템.
