Abstract
본 발명은 다발성 경화증 및 T 세포 매개된 자기면역 질환의 치료방법에 관한 것이다. 본 발명의 방법은 환자에게 접촉 의존성 헬퍼 이펙터 작용을 중개하는 T 세포의 표면상에 수용체의 길항제, 예를 들어 항-gp39 항체를 치료학적 유효량으로 투여함을 포함한다.
Description
자기면역 질환은 자기 조직에 대한 면역 시스템 공격이 특징이다. 자기면역 질환은 통상 자기 항원에 대한 면역 시스템의 허용력의 파괴에 기인한다. 다양한 자기면역 질환에서 면역 시스템이 인식하는 특이 항원은 전신적으로 존재할 수 있거나 기관 특이적일 수 있다. 예를 들면, 전신성 홍반성 루푸스(SLE)는 기관 특이적이지 않은 DNA, 리보핵산단백질, 히스톤 및 다른 분자에 대한 항체의 존재가 특징이다. 이러한 자기면역 질환에는 췌장내의 랑게르한스 섬의 β세포를 생성하는 인슐린이 파괴되는 제 1형 당뇨병이 포함된다.
몇몇 자기면역 질환에서, 조직 파괴는 주로 고 수준의 항체의 발생으로 일어난다. 이러한 질환에는 류머티스성 관절염이 포함되는데, 이는 관절 연골의 파괴 및 활막의 염증이 특징이다. 류머티스성 관절염이 있는 환자는 IgG 및 IgG 분자의 Fc 부분에 대한 항체의 연합에 의해 형성된 관절내의 면역 복합체가 축적된다. 이들 면역 복합체는 보체 캐스케이드를 활성화시키고, 이에 따라 조직이 손상된다. 진행성 근육 약화 질환인 중증근무력증은 신경근 접합부의 운동종판내의 아세틸콜린 수용체에 반응성인 자기항체의 생성에 의해 야기된다.
다른 자기면역 질환에서, 조직 파괴는 주로 자기항체의 생성에 의해 매개되기 보다는 자기 반응성 T 림프구에 의해 매개되는 것으로 보인다. 예를 들면, 뇌 및 척추의 탈수초를 특징으로 하는 다발성 경화증에 대한 동물 모델인 실험적 알레르기성 뇌척수염을 질환에 걸린 동물로부터 CD4+ T 세포의 전이에 의해 실험을 받은 적이 없는 동물에서 유도할 수 있다. 따라서, EAE는 β세포 매개된 자기면역 질환보다는 T 세포 매개된 자기면역 질환을 대표하는 것으로 생각된다(참조: Ben-Num, A. et al.(1981) Eur. J. Immunol. 11, 195).
다발성 경화증(MS)은 뇌와 척수의 공통적인 탈수초 질환이다. 다발성 경화증은 중추 신경계의 상이한 부분에 영향을 미치는 신경 기능장애의 악화와 완화가 특징이다. 이 질환의 증상은 염증성 탈수초의 병소로부터 기인하며, 후에 뇌의 백질, 뇌간 또는 척수에서의 플라크로서 나타나는 반흔을 형성한다. 현재, 다발성 경화증에 이용할 수 있는 명확한 진단 시험은 없으며 진단 및 치료 섭생법은 환자의 증상의 정도 및/또는 신경 기능장애의 악화 시작시의 환자의 연령과 같은 인자를 기준으로 처방된다.
다발성 경화증이 있는 환자는 통상 환자의 증상을 완화시키거나 환자의 질환의 진행을 늦추기 위한 목적으로 스테로이드로 처리한다. 질환의 특정 증상을 치료하기 위해 다른 약물, 예를 들면, 근이완제를 사용하였다. 다발성 경화증에 이용할 수 있는 치료에서의 최근의 발전은 β-인터페론의 투여가 포함된다. β-인터페론은 질환의 진행을 늦추는데 어느 정도 유망한 것으로 보인다. 그러나, 다발성 경화증에 대한 유효한 치료방법이 여전히 요구되고 있다.
발명의요약
본 발명은 다발성 경화증과 같은 T 세포 매개된 자기면역 질환을 치료(치료 및 예방을 포함)하는 방법에 관한 것이다. 이 방법은 피검자에게 접촉 의존성 헬퍼 이펙터 기능을 매개하는 T 세포의 표면상의 수용체의 길항제를 치료 또는 예방 유효량으로 투여하는 것을 포함한다. 바람직한 상태에서 투여된 길항제는 T 세포 수용체 gp39에 특이적으로 결합하는 항체 또는 이의 단편이다.
도 1은 항 gp39 투여가 실험적 알레르기성 뇌척수염 EAE의 발생을 예방함을 보여주는, 항 gp39 항체(흑색 막대) 또는 PBS(회색 막대)를 갖고 PLP-펩티드 75㎍(패널 A) 또는 300㎍(패널 B)을 0일째에 주사한 마우스에서 매일 측정한 DAS 유니트를 도시한 것이다.

본 발명은 다발성 경화증과 같은 T 세포 매개된 자기면역 질환의 치료에 관한 것이다. 다발성 경화증은 접촉 의존성 T 세포 헬퍼 이펙터 기능을 매개하는 T 세포의 표면상의 수용체에 대한 길항제를 투여하여 치료한다.

T 세포 매개된 자기면역 질환
자기면역 질환은 피검자의 면역 시스템이 자기항원에 반응하여 피검자에게 상당한 조직 또는 세포 파괴가 일어나는 질환을 포함한다. 자기항원은 피검자의 면역 시스템이 인식하는 피검자의 항원을 포함한다. 자기항원(autoantigen) 및 자기항원(self-antigen)은 본 명세서에서 호환적으로 사용된다. 본 명세서에서 자기(self) 란 피검자의 일부를 의미하며 분자, 세포 및 기관을 포함한다. 자기항원은 펩티드, 핵산 또는 다른 생물학적 물질일 수 있다. gp39 길항제

면역화에 이어 항혈청을 수득할 수 있고, 필요한 경우, 혈청으로부터 다클로널 항체를 분리할 수 있다. 단일클론성 항체를 생성하기 위해서, 항체 생성세포(림프구)를 면역화된 동물로부터 수집하여 표준 체세포 융합 방법으로 골수종세포와 융합시킴으로써 이들 세포를 불멸화시키고 하이브리도마 세포를 수득한다. 이러한 기술은 본 기술 분야에 널리 공지되어 있다. 예를 들면, 하이브리도마 기술은 원래 코흘러 및 밀슈타인(Kohler and Milstein) (Nature (1975) 256: 495-497) 이 개발한 기술이며, 사람 β세포 하이브리도마 기술 (Kozbar et al., Immunol. Bliss, Inc., pages 77-96), 및 조합 항체 라이브래리의 스크리닝 (Huse et al., Science (1989) 246: 1275)도 있다. 하이브리도마 세포는 단백질 또는 펩티드 및 분리된 단일클론성 항체와 특이 반응성인 항체의 생성을 위해 면역화학적으로 스크리닝할 수 있다.
본 명세서에서 사용되는 항체는 gp39 단백질 또는 이의 펩티드 또는 gp39 융합 단백질과 특이 반응성인 단편을 포함한다. 항체는 통상의 기술을 사용하여 단편화할 수 있고 단편을 완전 항체에 대해 위에서 설명한 바와 동일하게 이용을 위해 스크리닝할 수 있다. 예를 들면, F(ab')2단편은 항체를 펩신으로 처리하여 생성시킬 수 있다. 수득한 F(ab')2단편은 디설파이드 브릿지를 분해하여 Fab' 단편을 생성하도록 처리할 수 있다. 본 발명의 항체는 또한 항 gp39 부분을 갖는 양특이적(bispecific) 및 키메라성 분자를 포함한다.

사람 치료 목적을 위해 gp39 단백질 또는 펩티드와 특이 반응성인 단일클론성 또는 키메라성 항체는 항원 결합 도메인의 가변부, 특히 보존된 구조부의 일부가 사람 기원이며 초가변부만이 비사람 기원인 사람 가변부 키메라에 의해 추가로 인간화될 수 있다. 이러한 변형된 면역글로블린 분자는 본 기술 분야에 공지된 몇몇 기술(참조: Teng et al., Proc. gp39 단백질 또는 펩티드에 대해 반응성인 특이 항체 또는 항체 단편을 생성시키는 또 다른 방법은 gp39 단백질 또는 펩티드를 갖는 박테리아에서 발현되는 발현 라이브래리 암호와 면역글로불린 유전자 또는 이의 일부를 스크리닝하는 것이다. 예를 들면, 완전 Fab 단편, VH 부 및 FV 부는 파아지 발현 라이브래리를 사용하여 박테이아에서 발현시킬 수 있다(참조: Ward et al., Nature, 341: 544-546: (1989); Huse et al., Science, 246:1275-1281(1989); and McCafferty et al., Nature, 348:552-554(1990)). 이러한 라이브래리를, 예를 들면 gp39 펩티드로 스크리닝함으로써 gp39와 반응성인 면역글로블린 단편을 확인할 수 있다. 다르게는, SCID-hu 마우스(Genpharm에서 입수가능)를 사용하여 항체 또는 이의 단편을 생성시킬 수 있다.
본 발명의 방법에 사용하기 위한 적합한 모노크로널 항체 및 사람 gp39 및 마우스 gp39를 포함하는 gp39에 대해 지향적인 단일클론성 항체를 제조하는 방법은 본 명세서에 참고로 인용하는 발명의 명칭이 항 gp39 항체 및 이를 위한 용도 인 PCT 특허원 WO 제 95/106666호에 기술되어 있다. 본 발명의 특히 바람직한 항 사람 gp39 항체는 mAbs 24-31 및 89-76으로서 각각 하이브리도마 24-31 및 89-76에 의해 생성된다. 89-76 및 24-31 항체를 각각 생성하는 89-76 및 24-31 하이브리도마는 1944년 9월 2일자로 미국 매릴랜드주 록빌 파크론 드라이브 소재의 아메리칸 타입 컬쳐 콜렉션(American Type Collection)에 부다페스트 조약의 규정의 따라 기탁되었다. 89-76 하이브리도마는 ATCC 기탁 번호 HB 11713을 부여 받았고 24-31 하이브리도마는 ATCC 기탁번호 HB 11712를 부여받았다.
키메라성 및 인간화된 항체와 같은 재조합 항 gp39 항체를 표준 재조합 DNA 기술에 따라 항 gp39 항체를 암호화하는 핵산(예: DNA)을 조작하여 생성시킬 수 있다. 따라서, 본 발명의 또 다른 양태는 gp39, 특히 사람 gp39와 반응성인 면역글로불린 중쇄 또는 경쇄를 암호화하는 분리된 핵산 분자 또는 이의 일부에 관한 것이다. 면역글로불린 암호화 핵산은 연결된 중쇄 또는 경쇄 불변부(또는 이의 일부)의 존재 또는 부재하에 면역글로불린 경쇄 또는 중쇄 가변부를 암호화할 수 있다. 이러한 핵산은 표준 기술에 의해 항 사람 gp39mAb를 생성하는 세포(예: 하이브리도마)로부터 분리할 수 있다. 예를 들면, 24-31 또는 89-76 mAb를 암호화하는 핵산은cDNA 라이브래리 스크리닝에 의해 24-31 또는 87-76 하이브리도마 각각으로부터 분리할 수 있다. 또한 분리 또는 가능한 추가 조작 후, 항 사람 gp39 mAb를 암호화하는 핵산을 발현 벡터에 삽입시켜 숙주 세포내로 도입함으로써 재조합 형태의 항 사람 gp39 항체의 발현 및 생성을 촉진시킬 수 있다.
B. gp39에 대한 가용성 리간드
T 세포 내성을 유도하는데 사용할 수 있는 다른 gp39 길항제는 가용성 형태의 gp39 리간드이다. 가용성 CD 40과 같은 gp의 1가 가용성 리간드는 gp39를 결합시킴으로써 gp39와 B 세포상의 CD 40의 상호작용을 억제한다. 가용성 gp39 리간드는 화학적 합성 또는 바람직하게는 재조합 DNA 기술, 예를 들면, 리간드의 세포외 도메인(트란스멤브레인 또는 세포질 도메인이 없는)만을 발현시킴으로써 제조할 수 있다. 바람직한 가용성 gp39 리간드는 가용성 CD 40이다. 선택적으로, 가용성 gp39 리간드는 융합 단백질의 형태일 수 있다. 이러한 융합 단백질은 제 2 분자에 부착된 gp39 리간드의 적어도 일부를 포함한다. 예를 들면, CD 40은 면역글로불린(즉, CD 401g 융합 단백질)을 갖는 융합 단백질로서 발현될 수 있다. 하나의 양태에서, CD 401 g 융합 단백질을 형성시키기 위해 면역글로불린 중쇄의 힌지(hinge) CH2및 CH3부(예를 들면, Cγl)에 상응하는 서열의 아미노산 잔기에 결합된 CD 40 분자의 세포외 도메인 부분의 아미노산 잔기를 포함하는 융합 단백질을 생성시킨다(참조: Linsley et al. 본 발명의 길항제는 생체내 약제 투여에 적합한 생체 적합 형태로 피검자에게 투여한다. 생체내 투여에 적합한 생체 적합 형태란 단백질의 치료 효과가 독성 효과를 압도하는 투여되는 길항제의 형태를 의미한다. 피검자란 면역 반응이 유도될 수 있는 생물, 예를 들면 포유동물을 의미한다. 피검자의 예에는 사람, 개, 고양이, 마우스, 래트 및 이의 유전자이식 종이 포함된다. gp39 길항제는 임의의 약리학적 형태로 임의로 약제학적으로 허용되는 담체에 함유시켜 투여할 수 있다. 치료학적 활성량의 길항제의 투여는 목적하는 결과를 달성하는데 필요한 투여량에서의 및 시간 동안의 유효량으로서 정의된다. 예를 들면, 치료학적 활성량의 gp39의 길항제는 개인의 질병상태, 연령, 성별 및 체중과 개인에게서 길항제가 목적하는 반응을 유도해내는 능력에 따라 달라진다. 투여량 섭생법은 최적 치료 반응을 제공하도록 조정할 수 있다. 예를 들면, 하루에 두차례 분할 용량을 투여하거나 치료 상황의 긴급성에 따라 지시 용량을 비례적으로 줄일 수 있다.
활성 화합물(예: 길항제)은 편리한 방법, 예를 들면, 주사(피하, 정맥내 등), 경구투여, 흡입, 경피 투여 또는 직장 투여에 의해 투여할 수 있다. 투여 경로에 따라, 활성 화합물은 이를 불활성화시킬 수 있는 효소, 산 및 다른 자연 조건의 작용으로부터 보호하기 위해 물질로 피복할 수 있다. 바람직한 투여 경로는 정맥내 주사이다.
비경구 투여 외의 다른 방법으로 gp39의 길항제를 투여하기 위해서는 길항제를 이의 불활성화를 방지할 수 있는 물질로 피복하거나 이러한 물질과 함께 투여하는 것이 필요할 수 있다. 예를 들면, 길항제는 적합한 항체 또는 희석제에 함유시켜 개인에게 투여하거나, 효소 억제제와 함께 투여하거나 리포솜과 같은 적합한 캐리어에 함유시켜 투여할 수 있다. 약제학적으로 허용되는 희석제에는 염수 및 완충 수용액이 포함된다. 효소 억제제에는 췌장 트립신 억제제, 디이소프로필플루오로포스페이트(DEP) 및 트라실올이 포함된다. 리포솜에는 수중유중수 에멀젼 및 통상의 리포솜이 포함된다(참조: Strejan et al., (1994) J. Neuroimmunol 7: 27).
활성 화합물은 또한 비경구적으로 또는 복강내 투여할 수 있다. 글리세롤, 액체 폴리에틸렌 글리콜 및 이들의 혼합물 속의 또는 오일 속의 분산액도 제조할 수 있다. 일반 저장 및 사용 조건하에서는, 이들 제제는 미생물의 성장을 방지하기 위한 방부제를 함유할 수 있다.
주사용으로 적합한 약제학적 조성물에는 멸균 수용액(가용성인 경우) 또는 멸균 수성 분산액 및 멸균 주사액 또는 분산액의 임시 제조용 멸균 분말이 포함된다. 모든 경우에 있어서, 조성물은 멸균되어야 하며 용이한 주사성이 존재하는 정도로 유동성이어야 한다. 이는 제조 및 저장 조건하에 안정해야 하며 박테리아 및 진균과 같은 미생물의 오염 작용에 내성을 나타내어 보존되어야 한다. 담체는, 예를 들면 물, 에탄올, 폴리올(예: 글리세롤, 프로필렌 글리콜 및 액체 폴리에틸렌 글리콜 등) 및 이들의 적합한 혼합물을 함유하는 분산매 또는 용매일 수 있다. 적당한 유동성은, 예를 들면, 레시틴과 같은 피복물질의 사용에 의해, 분산액의 경우 필요한 입자 크기의 유지에 의해서 및 계면활성제의 사용에 의해 유지시킬 수 있다. 미생물의 작용 방지는, 예를 들면, 파라벤, 클로로부탄올, 페놀, 아스코르브산, 티메로살 등의 다양한 항박테리아 및 항진균제에 의해 달성할 수 있다. 많은 경우에 있어서, 등장제, 예를 들면, 슈가, 다가알콜(예: 만니톨, 소르비톨), 염화 나트륨을 조성물에 포함시키는 것이 바람직할 것이다. 주사용 조성물의 장기간 흡수는 조성물에 흡수 지연제, 예를 들면 알루미늄 모노스테아레이트 및 젤라틴을 포함시켜 달성할 수 있다.
멸균 주사액은 필요에 따라 위에서 열거한 성분 중의 하나 또는 성분들의 배합물과 함께 적합한 용매에 필요량의 활성 화합물(예: gp39의 길항제)를 혼입시킨 다음 여과 멸균시킴으로써 제조할 수 있다. 일반적으로, 분산액은 활성 화합물을 기초 분산액 매질 및 위에서 열거한 성분들로부터 선택된 다른 성분들을 함유하는 멸균 비히클에 혼입시켜 제조한다. 멸균 주사액의 제조를 위한 멸균 분말의 경우에, 바람직한 제조 방법은 진공 건조 및 동결 건조로서, 이미 언급한 멸균 여과 용액으로부터 활성 화합물(예: 길항제)과 추가의 필요 성분이 함께 포함된 분말이 수득된다.
위에서 설명한 바와 같이, 활성 화합물이 적합하게 보호되면, 단백질을 예를 들면 불활성 희석제 또는 동화할 수 있는 식용 담체와 함께 경구 투여할 수 있다. 본 명세서에서 약제학적으로 허용되는 담체에는 용매, 분산 매질, 제피제, 항박테리아 및 항진균제, 등장성 흡수 지연제 등이 모두 포함된다. 약제학적 활성 물질용의 이러한 매질 및 첨가제의 용도는 본 기술 분야에 널리 공지되어 있다. 통상적인 매질 또는 첨가제가 활성 화합물과 불혼화성이지 않는 이상, 이들의 치료 조성물에의 사용이 고려된다.
투여 용이성과 용량의 균일성을 위해 비경구 조성물을 용량 단위형으로 제형하는 것이 특히 유리하다. 본 명세서에서 사용된 용량 단위형은 치료받는 포유동물 피검자에게 단위 용량으로서 적합한 물리적으로 이산된 단위를 말한다.
예정된 양의 활성 화합물을 함유하는 각 단위는 목적하는 약제학적 담체와 함께 목적하는 치료 효과를 나타내도록 계획된다. 본 발명의 용량 단위형에 대한 상세한 사항은 (a) 활성 화합물의 독특한 특성 및 달성하고자 하는 특정 치료 효과 및 (b) 개인의 감수성의 치료를 위한 그러한 활성 화합물의 배합 기술에 내재하는 제약에 의해서 및 이에 따라 정해진다.

실시예 1
항 gp39 항체 투여에 의한 EAE 예방
본 실시예는 마우스에게 항 gp39 항체를 투여하는 것이 다발성 경화증에 대한 동물 모델인 실험적 알레르기성 뇌척수염(EAE)의 유도를 예방함을 예증한다.
EAE는 T 세포 매개된 자기면역 질환의 널리 특징화된 모델이다. EAE는 마우스와 같은 질환에 걸리기 쉬운 동물을 염기성 단백질(MBP), 프로테오리피드 단백질(PLP), 수초 올리고펜드로사이트 단백질(MOG) 또는 수초 관련 단백질의 서열을 기초로 하는 합성 펩티드와 함께 백일해 균을 함유하는 애쥬번트로 면역화시켜 동물에서 유도할 수 있다. 면역화후 1 내지 2주 후에 동물은 림프구 및 대식세포를 함유하는 혈관주변 침윤물과 뇌 및 척수에서의 탈수초의 발생이 특징인 뇌척수염이 발병한다. 동물은 급성, 만성 또는 만성 재발성 마비 증상을 보인다. 본 실시예에서, 질환에 걸리기 쉬운 마우스에서의 EAE의 발병시 항 gp39 항체의 투여 효과를 분석한다.
EAE는 질환에 걸리기 쉬운 마우스의 복부 옆구리의 두 부위에 완전 프로이트 애쥬번트 50㎕중의 마이코박테리아 투베르쿨로시스(H37RA, Difco) 25㎕ 및 PBS 50㎕중의 PLP 펩티드 70㎍ 또는 300㎍을 함유하는 에멜젼을 피하 주사(0일)하여 유도한다. 보르데텔라 페르투시스 현탁액 200㎕(PBS 1㎖중의 10.1010)을 펩티드와 동시에 및 2일 후에 정맥내 투여한다. 마우스에게 주사한 PLP 펩티드는 래트 PLP의 아미노산 잔기 139 내지 151에 상응하는 아미노산 서열을 갖는다(참조: Dautigny et al., FEBS Lett. 188: 33, 1985). PLP 펩티드는 고체상 합성 방법에 따라 f-moc 보호된 아미노산으로 합성한다. 이 펩티드로 면역화시키면 급성 EAE가 발병하는 데, 이는 임상적으로 및 병리학적으로 전 중추 신경계(CNS) 수초 또는 MBP로 감작하여 유도한 것과 동일하다(참조: Tuohy et al., J. Immunol. 142: 1523,1989; Sobel et al., J. Neuropathol. Exp. Neurol. 49: 468, 1990).
질환의 진행과정에 항 gp-39가 미치는 영향을 측정하기 위해, 위에서 설명한 바와 같이 PLP 펩티드를 0 일째에 주사하고 0, 2 및 4일째에 PBS 200㎕중의 햄스터 항 gp39 Mabs(참조: Noelle et al., Proc. Natl. Acad. Sci USA 89: 6550, 1992) 125㎍ 또는 PBS 200㎕ 중의 보통 햄스터 항체(Serva Feinbiochemical) 125㎍(대조 동물)을 복강내 주사한다. EAE 임상 징후의 중증도를 매일 평가하여 평균 무능 눈금(DAS) 점수에 따라 등급을 매긴다: 등급 0 = 임상적 징후가 없음, 등급 1 = 꼬리 약화, 등급 2 = 뒷 다리의 약한 대부전마비 및 기능장애, 등급 4 = 빈사상태, 등급 5 = EAE로 인해 사망.
제 1도는 PLP 펩티드(패널 A) 70㎍ 또는 PLP 펩티드(패널 B) 300㎍을 주사하고 항 gp39 또는 대조 항체로 치료받은 마우스의 질환 진행 과정을 도시한 것이다. 대조 마우스와 항 gp39 치료를 받은 마우스의 DAS 점수는 질환의 중증도를 반영하는 것으로서 각각 회색 및 흑색 막대로 나타내었다.
결과는 대조 항체를 투여받은 동물은 EAE가 발병된 반면, 항 gp39 항체를 투여받은 동물은 질환이 유도되지 않았음을 보여준다. 대조 항체를 투여받은 동물의 경우, EAE의 제 1 임상적 징후는 11일째에 명확해진다. 이들 동물에서 가장 높은 DAS 점수는 2.33(이는 PLP 펩티드 75㎍을 주사한 동물에게서 15 내지 22일째에 관찰되었다. 도 1, 패널 A, 회색 막대) 및 3.6(이는 PLP 펩티드 300㎍을 주사한 동물에게서 16 내지 23 일째에 관찰되었다. 도 1, 패널 B, 회색 막대). 이에 대해, 항 gp39 단일클론성 항체를 투여받은 동물은 PLP 펩티드 75㎍으로 EAE를 유도한 후 아무런 임상적 징후를 나타내지 않았으며 (도 1, 패널A, 흑색 막대) PLP 펩티드 300㎍으로 질환을 유도한 후 약간의 임상적 징후만이 나타났으나 31일째에 완전히 사라졌다 (도 1, 패널 B, 흑색막대).
이와 같이, 마우스에게 항 gp39 항체를 투여한 결과 이들 마우스에서 EAE의 유도가 완전히 방지되었다.
수초 성분으로 면역화시키거나 수초 성분으로 생체내 활성화시킨 후 수득한 동물로부터 분리한 수초 반응성 T 세포의 양자 이입에 의한 EAE의 유도는 특히 활성화된 T 세포가 유도기 후의 임상적 특성의 발생에 책임이 있음을 나타낸다(참조: Pettinelli; and McFarlin, J. Immunol. 127: 1420, 1979 ; Mokhtarion et al., Nature 309: 356, 1984 ; Veen et al., J. Neuroimmunol. 21 ; 183, 1989). 그러나, 본 명세서에 기술한 실험은 항 gp-39 단일클론성 항체의 투여로 EAE의 발병이 방지됨을 보여준다. 대조 그룹에서, 현저한 항 PLP 펩티드 항체 반응이 각각 저 PLP 펩티드 용량 및 고 PLP 펩티드 용량으로 EAE가 유도된 동물에서 14일(흡광도 1.92) 및 21일(흡광도 2.15) 째에 관찰되었다. 이에 대해, gp39 처리한 동물에서 현저한 항 PLP 펩티드 항체 반응이 14일째에 관찰되었으며 저 PLP 펩티드 용량 및 고 PLP 펩티드 용량을 각각 주사한 동물에서 31일(흡광도 0.928) 및 40(흡광도 1.54) 째에 평탄역 수준에 도달하였다. 항 39 단일클론성 항체 처리한 마우스에서의 현저한 항 PLP 펩티드 항체 반응의 발생은 14일째 까지 연기되었는데, 이는 항 gp39 단일클론성 항체가 항체 생성에 어느 정도 영향을 미침을 나타낸다.
이와 같이, 본 실시예는 항 gp39가 EAE의 발병을 방지함을 보여주며 항 gp39 항체가 다발성 경화증과 같은 T 세포 매개된 자기면역 질환을 치료하는 데 사용될 수 있음을 나타낸다.
실시예 2
항 gp39 항체 투여에 의한 EAE의 역전
실시예 1은 EAE의 유도에 미치는 항 gp39 항체의 억제 효과를 보여주었다. 이와 같이, 질환의 유도시 마우스의 면역화가 질환의 발병을 방지함이 입증되었다. 본 실시예는 질환의 유도 후 항체의 투여가 질환의 역행을 유도함을 보여준다.
본 실시예에서, EAE는 위에서 기술한 대로 제조한 PLP 펩티드 150㎍을 함유하는 에멀젼의 주사에 의해 암컷 SJL/j 마우스 (10내지 12주 성장)에서 유도하였다. 질환의 유도후에 마우스에게 투여한 항 gp39 항체의 효과를 측정하기 위해, 마우스에게 0, 2 및 4일째에, 4,6 및 8일째에 또는 7,9 및 11일 째에 PBS(항 gp39 처리한 마우스) 200㎕중의 항 gp39 단일클론성 항체(Noelle et al., Proc. Natl. Acad. Sci. USA 89: 6550, 1992) 125㎍ 또는 PBS 200㎕ 만(대조 마우스)을 복강내 주사한다. 결과는 억제 %로서 나타내었으며 항 gp39 처리한 동물과 대조 동물에서의 매일 DAS 점수(12 일부터 28까지)의 총 점수를 비교한 것이다.
도 2에 제시되어 있는 결과를 PLP 펩티드를 마우스에게 투여한 지 7일이나 지난 후의 제 1용량의 투여가 질환을 60% 넘게 억제함을 나타낸다. 이와 같이, 항 gp39 항체는 EAE를 역행 또는 억제할 수 있다.
결과는 또한 마우스에서 질환을 유도한 후 7일이 지나서 항체의 제 1 용량을 투여할지라도 질환의 발병이 현저하게 억제되고 항 gp39 처리가 질환을 유도한 직후 항체의 제 1 용량을 투여시 어느 정도 더 효과적임을 나타낸다.
이와 같이, 마우스에게 항 gp39 항체를 투여하면 질환의 유도시 EAE의 발병으로부터 마우스를 보호하며 EAE를 갖는 마우스에서 질환이 억제된다.
실시예 3
gp39 처리한 마우스의 비장 세포 이식후 EAE의 억제
EAE로부터 회복되고(Pesoa et al., J. Neuroimmunol. 7: 131, 1984) 수초 성분의 경구 투여(Lider et al., J. Immunol. 142: 748, 1989 ; Hafler et al., Ann. NY Acad. Sci. 636: 251, 1991)후 루이스 래트에서 조절 억제 T 세포가 검출되었다. 카르푸스 및 스완보르그(Karpus and Suanborg; J. Immunol. 143: 3492, 1989)에 의해 EAE로부터 회복한 래트로부터 분리한 CD 4+억제 T 세포가 림포카인 생성의 차별 억제에 의해 EAE T 이펙터 세포를 감소 조절할 수 있는 것으로 판단되었다. 이에 대해, MBP의 경구 투여에 의한 루이스 래트에서의 EAE의 억제는 CD 8+T 세포에 의해 매개되었다(Miller et al., J. W. H. Freeman and Company, 1980). 개별 비장 세포(500㎕)를 실험을 받은 적이 없는 5 Gy 조사된 수용 암컷 SLJ/j 마우스(10 내지 12 주 성장)에게 정맥내 주사한다. 세포를 이식한 지 2일 후에 실시예 1의 절차에 따라 마우스에게 PLP 펩티드 150㎍을 복강내 주사하여 챌린지시킨 후 DAS 점수를 측정한다.
도 3은 동물의 DAS 점수를 나타낸다. 결과는 최초 PLP 펩티드 및 항 gp39를 주사한 동물로부터의 비장 세포를 이식 받은 마우스가 질환의 발병으로부터 보호된 반면 최초 PLP 펩티드 만을 주사한 동물로부터의 비장 세포를 이식받은 마우스는 EAE가 발병됨을 입증한다. 또한, 항체의 측정 반감기가 12일임을 고려할 때, 마우스에서 이식받은 비장 세포에 항체가 존재하지 않을 것으로 예상할 수 있다. 따라서, PLP 펩티드 및 항 gp39 항체를 투여받은 동물로부터의 공여 비장 세포에 의해 부여되는 보호 효과는 항 gp39 항체의 존재에 의해 설명할 수 없다. 이들 마우스의 DAS 점수는 대부분 이식된 비장 세포 현탁액 속의 T 억제 세포 집단의 존재로 인한 것이며 T 억제 세포 집단이 T 이펙터 세포 집단을 효과적으로 지배함을 나타낸다.
실시예 4
gp39 양성 Th 세포의 검출
본 실시예는 다발성 경화증이 있는 사람 피검자의 중추 신경계에서 gp39 양성 세포의 존재를 입증한다.
사람 검시 중추 신경계(DNS) 조직을 네덜란드 암스테르담 소재의 네덜란드 브레인 뱅크(Netherlands brain bank)로부터 입수한다. gp39 양성 세포를 본 기술 분야에 공지된 방법에 따라 CD 40-Ig 융합 단백질로 검출한다. MS 환자 및 알쯔하이머 환자의 CNS 조직 부분을 CD 40-Ig로 염색한다. 본 실시예에서, 항 MBP 항체 형성 세포를 미리 검출한 MS 환자의 CNS 조직만을 사용한다. 염색의 결과는 MS 환자의 관측 대뇌 부분에 gp39 양성 세포의 존재를 보여주지만, 알쯔하이머 환자의 관측 대뇌 부분에서는 gp39 양성 세포가 검출되지 않았다. 이와 같이, gp39 양성 세포는 MS 환자의 CNS 조직 부분에서만 검출되었다. 다발성 경화증 환자의 CNS 조직에서의 gp39 양성 세포의 존재와 함께 다발성 경화증 환자의 CNS 조직에서만 존재하는 항 MBP 항체 형성 세포의 검출은 이러한 세포가 대조 CNS 조직에서는 존재하지 않고 다발성 경화증 환자의 병리학적 영향을 받은 CNS 조직에서 일정한 역할을 한다는 것을 나타낸다.
본 연구에서, 본 발명자들은 항 gp39 Mab의 투여에 의한 B 세포 활성화의 억제가 EAE가 유도되는 항원의 용량 및 EAE 유도와 항 gp39 Mabs 투여 사이의 시간에 따라 EAE 발병을 완전 예방할 수 있음을 밝혀내었다. EAE 유도 및 발병에 책임이 있는 정확한 메카니즘은 분명하지 않지만 이들 자료는 항 gp 39 항체가 자기 면역 질환의 치료에 사용될 수 있음을 나타낸다.

Claims (11)
- 피검자에게 접촉 의존성 헬퍼 이펙터 기능을 매개하는 T 세포의 표면상의 수용체의 길항제를 치료학적 유효량으로 투여함을 포함하여 T 세포 매개된 자기면역 질환을 치료하는 방법.
- 제 1항에 있어서, T 세포 매개된 자기면역 질환이 다발성 경화증, EAE, 제 I형 당뇨병, 난소염 및 갑상선염으로 이루어지는 군으로부터 선택되는 방법.
- 제 1항에 있어서, T 세포 매개된 자기면역 질환이 다발성 경화증인 방법.
- 제 1항에 있어서, 접촉 의존성 헬퍼 이펙터 기능을 매개하는 T 세포의 표면상의 수용체가 gp39 인 방법.
- 제 4항에 있어서, 길항제가 항 gp39 항체인 방법.
- 제 5항에 있어서, 항 gp39 항체가 단일클론성 항체인 방법.
