Abstract
일반식 (I) 로 나타내는 아실티오우레아 화합물 또는 그 약학적으로 허용되는 염을 유효 성분으로 하는 다른 항종양제의 항종양 효과 증강제.
(식 중, R1 은 치환기를 갖고 있어도 되는 C1 -6 알킬기를 나타내고, 그 치환기로서, 하이드록실기, C3 -10 시클로알킬기, 치환기를 갖고 있어도 되는 C1 -6 알콕시기, 치환기를 갖고 있어도 되는 C1 -6 알킬아미노기, C1 -6 알카노일아미노기, C1 -6 알킬술포닐기, 치환기를 갖고 있어도 되는 C6 -14 방향족 탄화수소기, 치환기를 갖고 있어도 되는 포화 또는 불포화 복소 고리기, 치환기를 갖고 있어도 되는 C1 -6 알킬아미노카르보닐기, 치환기를 갖고 있어도 되는 포화 또는 불포화 복소 고리 카르보닐기 중 어느 하나를 나타내고, R2 는 불소 원자 또는 염소 원자를 나타내며, R3 은 수소 원자, 불소 원자, 또는 염소 원자를 나타낸다.)

Description
본 발명은 항종양의 효과 증강제, 다른 항종양제와 조합하여 이루어지는 항종양약에 관한 것이다.
항종양제의 타겟은 다방면에 걸쳐 있으며, 크게는, 알킬화 약, 백금 화합물, 대사 길항약, 토포이소머라제 저해약, 미소관 저해약, 항종양성 항생 물질, 분자 표적약 등으로 분류된다. 그리고 최근에는, 항종양제를 단독으로 투여하는 것이 아니라, 병용 요법이 널리 실시되기에 이르고 있다.
예를 들어 VEGF 리셉터 키나아제 저해 작용을 갖고, 혈관 저해제인 4-(3-클로로-4-(시클로프로필아미노카르보닐)아미노페녹시)-7-메톡시-6-퀴놀린카르복스아미드와 탁산의 병용 요법이 보고되어 있다 (특허문헌 1).
한편, c-Met 는 Proto-oncogene 의 하나로서 동정된 수용체형 티로신 키나아제이다. 많은 암 (신세포암, 위암, 폐암, 대장암, 췌장암, 난소암, 간세포암, 두경부암, 멜라노머 등) 에 있어서는, c-Met 가 고빈도로 과잉 발현, 변이 또는 전좌하고 있음으로써, 활성화 상태가 항진되고, 세포 증식, 침윤·전이, 종양 형성, 혈관 신생이나 항아포토시스에 기여하고 있는 것이 보고되어 있다 (예를 들어, 비특허문헌 1). 따라서, c-Met 저해 효과를 갖는 화합물은 항종양제로서 유용한 것이 알려져 있다 (특허문헌 2). c-Met 저해제의 하나이며, c-Met 와 AXL 을 포함하는 멀티 키나아제 저해제인 퀴놀린 유도체와 ErbB 저해제의 병용도 보고되어 있다 (특허문헌 3).

그러나, 어떠한 항종양제와 항종양제를 병용했을 경우에, 그들의 항종양 효과가 증강되는 것인지, 혹은 효과가 상쇄되는 것인지에 대해서는 완전히 불분명하다.
본 발명의 과제는, 2 종의 항종양제의 병용에 의해, 그들의 항종양 효과가 증강되는 조합을 제공하는 것에 있다.
그래서 본 발명자는, c-Met 저해제의 1 종에 주목하여, 당해 화합물과 다른 항종양제를 병용했을 경우의 작용을 검토해 온 결과, 하기 식 (I) 로 나타내는 퀴놀린 고리 6 위치 치환기로서 아미노카르보닐기를 갖고, 또한 7 위치 치환기로서 알콕시기를 갖는 아실티오우레아 화합물 또는 그 약학적으로 허용되는 염이 부작용이 경감된 강력한 c-Met 저해제로서 작용하고, 다른 항종양제의 병용에 있어서는, 우수한 항종양 효과 증강 작용에 의해 효과역이나 항종양 스펙트럼을 확대하는 것을 알아내어, 본 발명을 완성시켰다.
즉 본 발명은 이하의 [1] ∼ [8] 에 관련된 것이다.

(식 중, R1 은 치환기를 갖고 있어도 되는 C1-6 알킬기를 나타내고, 그 치환기로서, 하이드록실기, C3-10 시클로알킬기, 치환기를 갖고 있어도 되는 C1-6 알콕시기, 치환기를 갖고 있어도 되는 C1-6 알킬아미노기, C1-6 알카노일아미노기, C1-6 알킬술포닐기, 치환기를 갖고 있어도 되는 C6-14 방향족 탄화수소기, 치환기를 갖고 있어도 되는 포화 또는 불포화 복소 고리기, 치환기를 갖고 있어도 되는 C1-6 알킬아미노카르보닐기, 치환기를 갖고 있어도 되는 포화 또는 불포화 복소 고리 카르보닐기 중 어느 하나를 나타내고, R2 는 불소 원자 또는 염소 원자를 나타내며, R3 은 수소 원자, 불소 원자, 또는 염소 원자를 나타낸다.)
로 나타내는 아실티오우레아 화합물 또는 그 약학적으로 허용되는 염을 유효 성분으로 하는 다른 항종양제의 항종양 효과 증강제.
[2] 상기 식 (I) 로 나타내는 아실티오우레아 화합물 또는 그 약학적으로 허용되는 염과 다른 항종양제를 조합하여 이루어지는 항종양약.
[3] 다른 항종양제의 항종양 효과를 증강하기 위한, 식 (I) 로 나타내는 아실티오우레아 화합물 또는 그 약학적으로 허용되는 염.
[4] 다른 항종양제와, 일제형 (一劑型) 의 제제 형태 또는 별개의 제제 형태로서 투여함으로써 종양을 치료하기 위한, 식 (I) 로 나타내는 아실티오우레아 화합물 또는 그 약학적으로 허용되는 염.
[5] 식 (I) 로 나타내는 아실티오우레아 화합물 또는 그 약학적으로 허용되는 염의, 다른 항종양제의 항종양 효과 증강제 제조를 위한 사용.
[6] 식 (I) 로 나타내는 아실티오우레아 화합물 또는 그 약학적으로 허용되는 염의, 다른 항종양제와 조합하여 이루어지는 항종양약 제조를 위한 사용.
[7] 식 (I) 로 나타내는 아실티오우레아 화합물 또는 그 약학적으로 허용되는 염과 다른 항종양제를 필요한 환자에게 투여하는 것을 특징으로 하는, 다른 항종양제의 항종양 효과 증강 방법.

아실티오우레아 화합물 (I) 또는 그 약학적으로 허용되는 염은 각종 항종양제와 병용함으로써, 이들 항종양제의 효과를 증강시킨다. 즉, 1) 아실티오우레아 화합물 (I) 은 병용하는 항종양제 단제 (單劑) 의 부작용을 거의 증강시키지 않으므로, 병용하는 항종양제의 효과 용량을 감량하는 일 없이, 각각의 약제 단제에 있어서의 최대 효과 발휘 용량끼리에서의 병용이 가능하고, 2) 병용하는 항종양제의 약제 감수성에 상관없이, 그 병용 약제의 항종양 효과를 증강시키고, 또한 3) 그 항종양 효과는 아실티오우레아 화합물 (I) 자신이 항종양 효과를 나타내지 않는 저용량에 있어서도 확인된다. 이상의 결과, 본 발명은 암 치료 효과역의 확대, 치료 효과의 향상 등, 임상 상 유용성이 높은 치료 방법의 제공으로 이어진다.

본 발명의 아실티오우레아 화합물 (I) 또는 그 약학적으로 허용되는 염에 있어서, 어느 구조에 대해 「치환기를 가져도 된다」 라고 하는 경우에는, 당해 구조 상 화학적으로 가능한 위치에 1 개 또는 2 개 이상의 「치환기」 를 갖는 경우가 있는 것을 의미한다.
당해 구조에 존재하는 치환기의 종류, 치환기의 개수, 치환 위치는 특별히 한정되지 않고, 2 개 이상의 치환기가 존재하는 경우에는, 그들은 동일해도 되고 상이해도 된다. 「치환기」 로는, 할로겐 원자, 하이드록실기, 시아노기, 니트로기, C1 -6 알카노일기, C1 -6 알킬기, C3 -10 시클로알킬기, C2 -6 알케닐기, C1 -6 알콕시기, 아미노기, C1 -6 알킬아미노기, C1 -6 알카노일아미노기, C1 -6 알킬아미노카르보닐기, C1 -6 알킬술포닐기, C6 -14 방향족 탄화수소기, 포화 또는 불포화 복소 고리기, 포화 또는 불포화 복소 고리 카르보닐기, 옥소기 등이 예시되고, 상기 치환기가 존재하는 경우, 그 개수는 전형적으로는 1 ∼ 3 개이다.
식 (I) 중, R1 로 나타내는 「치환기를 갖고 있어도 되는 C1 -6 알킬기」 의 「C1-6 알킬기」 는 탄소수 1 ∼ 6 의 직사슬형 또는 분지형 알킬기를 나타내며, 메틸기, 에틸기, n-프로필기, 이소프로필기, n-부틸기, sec-부틸기, tert-부틸기, n-펜틸기, 이소펜틸기, n-헥실기, 이소헥실기가 예시되고, C1 - 4알킬기가 보다 바람직하고, 메틸기, 에틸기, n-프로필기, 이소프로필기, n-부틸기, sec-부틸기가 특히 바람직하다.
식 (I) 중, R1 의 「치환기를 갖고 있어도 되는 C1 -6 알킬기」 의 치환기로서 나타내는 「C3 -10 시클로알킬기」 는 탄소수 3 ∼ 10 의 시클로알킬기를 나타내며, 시클로프로필기, 시클로부틸기, 시클로펜틸기, 시클로헥실기가 예시되고, 시클로헥실기가 보다 바람직하다.

식 (I) 중, R1 의 「치환기를 갖고 있어도 되는 C1 -6 알킬기」 의 치환기로서 나타내는 「치환기를 갖고 있어도 되는 포화 또는 불포화 복소 고리기」 의 「포화 또는 불포화 복소 고리기」 는 산소 원자, 질소 원자, 황 원자 중 어느 하나의 원자를 1 개 또는 2 개 갖는 단고리성 또는 2 고리성의 포화 또는 불포화 복소 고리기를 나타내며, 예를 들어 피롤리디닐기, 피페리디닐기, 피페라지닐기, 모르폴리노기, 티오모르폴리노기, 호모피페리디닐기, 테트라하이드로티에닐기, 이미다졸릴기, 티에닐기, 푸릴기, 피롤릴기, 옥사졸릴기, 이소옥사졸릴기, 티아졸릴기, 이소티아졸릴기, 피라졸리닐기, 트리아졸릴기, 테트라졸릴기, 피리딜기, 피라질기, 피리미디닐기, 피리다질기, 인돌릴기, 이소인돌릴기, 인다졸릴기, 메틸렌디옥시페닐기, 에틸렌디옥시페닐기, 벤조푸라닐기, 디하이드로벤조푸라닐기, 벤조이미다졸릴기, 벤조옥사졸기, 벤조티아졸릴기, 푸리닐기, 퀴놀릴기, 이소퀴놀릴기, 퀴나졸리닐기, 퀴녹살릴기 등을 들 수 있다. 질소 원자 및/또는 산소 원자를 1 ∼ 4 개 갖는 5 ∼ 7 원자의 복소 고리기가 보다 바람직하고, 피롤리디닐기, 모르폴리노기, 디옥소란기, 테트라하이드로피라닐기, 피리딜기, 테트라졸릴기가 특히 바람직하다. 그 포화 또는 불포화 복소 고리기는 추가로 치환기를 갖고 있어도 되며, 치환기로는 C1 -6 알킬기 (특히 메틸기), 옥소기가 바람직하다.
식 (I) 중, R1 의 「치환기를 갖고 있어도 되는 C1 -6 알킬기」 의 치환기로서 나타내는 「치환기를 갖고 있어도 되는 C1 -6 알킬아미노카르보닐기」 의 「C1-6 알킬아미노카르보닐기」 는 상기 C1 -6 알킬아미노기를 갖는 카르보닐기를 나타내며, 에틸아미노카르보닐기, 디메틸아미노기, 메틸부틸아미노기가 보다 바람직하다. 그 C1-6 알킬아미노카르보닐기는 추가로 치환기를 갖고 있어도 되고, 치환기로는 하이드록실기, C1 -6 알콕시기 (특히 메톡시기) 가 바람직하다.
식 (I) 중, R1 의 「치환기를 갖고 있어도 되는 C1-6 알킬기」 의 치환기로서 나타내는 「치환기를 갖고 있어도 되는 포화 또는 불포화 복소 고리 카르보닐기」 의 「포화 또는 불포화 복소 고리 카르보닐기」 는 상기의 포화 또는 불포화 복소 고리기를 갖는 카르보닐기를 나타내며, 질소 원자 및/또는 산소 원자를 1 ∼ 2 개 갖는 5 ∼ 7 원자의 포화 복소 고리 카르보닐기가 보다 바람직하고, 피롤리디닐카르보닐기, 모르폴리노카르보닐기가 특히 바람직하다. 그 포화 또는 불포화 복소 고리 카르보닐기는 추가로 치환기를 갖고 있어도 되고, 치환기로는 할로겐 원자 (특히 불소 원자), 하이드록실기를 갖고 있어도 되는 C1-6 알킬기 (특히 메틸기) 가 바람직하다.
R1 로 나타내는 치환기를 갖고 있어도 되는 C1 -6 알킬기의 C1 -6 알킬기로는, 메틸기, 에틸기, n-프로필기, 이소프로필기, n-부틸기, 또는 sec-부틸기가 바람직하고, 그 알킬기 상의 치환기로는, 하이드록실기, 시클로헥실기, 메톡시기, 에톡시기, 이소프로필옥시기, 디에틸아미노기, 아세틸아미노기, 메틸술포닐기, 페닐기, 피롤리디닐기, 모르폴리노기, 디옥소란기, 테트라하이드로피라닐기, 피리딜기, 트리아졸릴기, 에틸아미노카르보닐기, 디메틸아미노카르보닐기, 메틸부틸아미노카르보닐기, 피롤리디닐카르보닐기, 또는 모르폴리노카르보닐기가 바람직하며, 그 알콕시기는 추가로 치환기로서 하이드록실기를 갖고 있어도 되고, 그 복소 고리기는 추가로 치환기로서 메틸기 또는 옥소기를 갖고 있어도 되고, 그 알킬아미노카르보닐기는 추가로 치환기로서 하이드록실기 또는 메톡시기를 갖고 있어도 되고, 그 복소 고리 카르보닐기는 추가로 치환기로서 불소 원자, 하이드록실기를 갖고 있어도 되는 메틸기를 갖고 있어도 된다.
R1 로 나타내는 치환기를 갖고 있어도 되는 C1 -6 알킬기의 C1 -6 알킬기로는, 메틸기, 에틸기, n-프로필기, 이소프로필기, n-부틸기, 또는 sec-부틸기가 바람직하며, 그 알킬기 상의 치환기로는, 하이드록실기, 메톡시기, 또는 모르폴리노기가 바람직하다.
R1 로는, 메틸기, 메톡시에틸기, 모르폴리노에틸기, 모르폴리노카르보닐메틸기, 2-하이드록시-n-부틸기, 2-하이드록시-2-메틸-n-프로필기, 1-하이드록시-n-부탄-2-일기가 보다 바람직하며, 1-하이드록시-n-부탄-2-일기의 경우에는 S 체가 바람직하다.
R2 의 치환 위치로는 2 위치 또는 3 위치가 바람직하고, 특히 2 위치가 바람직하다. 또, R2 로는, 불소 원자 또는 염소 원자가 바람직하고, 또한 불소 원자가 바람직하다.
R3 의 치환 위치로는 2 위치 또는 4 위치가 바람직하다. 또, R3 으로는, 수소 원자, 불소 원자 또는 염소 원자가 바람직하고, 수소 원자 또는 불소 원자가 보다 바람직하다.
식 (I) 로 나타내는 화합물 중, R1 이 치환기를 갖고 있어도 되는 메틸기, 에틸기, n-프로필기, 이소프로필기, n-부틸기, 또는 sec-부틸기로서, 그 알킬기 상의 치환기가 하이드록실기, 메톡시기, 또는 모르폴리노기이고, R2 가 불소 원자이며, R3 이 수소 원자 또는 불소 원자인 것이 바람직하다.
또한 식 (I) 로 나타내는 화합물의 바람직한 구체예로는, 다음의 화합물을 들 수 있다.
(1) 4-(2-플루오로-4-(3-(2-페닐아세틸)티오우레이도)페녹시)-7-메톡시-N-메틸퀴놀린-6-카르복스아미드 (화합물 (1))
(2) 4-(2-플루오로-4-(3-(2-페닐아세틸)티오우레이도)페녹시)-7-메톡시-N-(2-모르폴리노에틸)퀴놀린-6-카르복스아미드 (화합물 (2))
(3) (S)-4-(2-플루오로-4-(3-(2-(4-플루오로페닐)아세틸)티오우레이도)페녹시)-N-(1-하이드록시부탄-2-일)-7-메톡시퀴놀린-6-카르복스아미드 (화합물 (3))
아실티오우레아 화합물 (I) 의 약학적으로 허용되는 염으로는, 염산, 브롬화수소산, 요오드화수소산, 황산, 질산, 인산 등의 무기산이나, 포름산, 아세트산, 프로피온산, 옥살산, 말론산, 숙신산, 푸마르산, 말레산, 락트산, 말산, 시트르산, 타르타르산, 탄산, 피크르산, 메탄술폰산, 파라톨루엔술폰산, 글루탐산 등의 유기산과의 산 부가염, 나트륨, 칼륨, 마그네슘, 칼슘, 알루미늄 등의 무기 염기나, 메틸아민, 에틸아민, 메글루민, 에탄올아민 등의 유기 염기, 또는 리신, 아르기닌, 오르니틴 등의 염기성 아미노산과의 염이나 암모늄염을 들 수 있다. 또, 아실티오우레아 화합물 (I) 에는, 광학 이성체도 포함되고, 수화물도 포함된다.
본 발명에 의해 항종양 효과 증강 작용을 나타내는 아실티오우레아 화합물 (I) 은 국제 공개 제2009/125597호 팜플렛에 기재된 방법으로 제조할 수 있다.
아실티오우레아 화합물 (I) 은 우수한 c-Met 저해 작용을 갖고, 부작용이 경감된 항종양제인데, 여러 가지 다른 항종양제 (이하, 항종양제 A 라고 한다) 와 병용했을 경우에, 현저한 독성의 악화를 나타내지 않고, 그 항종양제 A 의 항종양 효과를 증강하는 작용을 갖는다.
아실티오우레아 화합물 (I) 에 의해, 그 작용이 증강되는 항종양제 A 는 특별히 한정되지 않지만, 예를 들어 독소루비신, 독실, 에피루비신 등의 항종양성 항생 물질;시클로포스파미드, 니무스틴 등의 알킬화제;시스플라틴, 카르보플라틴, 옥살리플라틴 등의 플라티나 제제;5-플루오로우라실 (5-FU), 테가푸르·기메라실·오테라실칼륨 (TS-1, 일반명 「테가푸르·기메라실·오테라실칼륨 배합제」 (상품명:「티에스원」)), 테가푸르·우라실 (UFT, 일반명 「테가푸르·우라실 배합제」 (상품명:「유에프티」)), 카페시타빈, 독시플루리딘, 5-플루오로-2'-디옥시우리딘 (FdUrd), 겜시타빈, 시타라빈 등의 피리미딘계 대사 길항제;플루다라빈, 클라드리빈, 넬라라빈 등의 푸린계 대사 길항제;페메트렉세드, 메토트렉세이트 등의 엽산 대사 길항제;파클리탁셀 (탁솔, 아브락산 등), 도세탁셀, 이리노테칸 등의 식물 알카로이드계 항종양제;게피티닙, 에를로티닙, 라파티닙, 에베로리무스, 템시롤리무스 등의 저분자 분자 표적약;베바시주맙, 트라스투주맙, 세툭시맙, 리툭시맙 등의 항체 분자 표적약 등을 들 수 있다. 이 중, 피리미딘계 대사 길항제, 엽산 대사 길항제, 식물 알칼로이드계 항종양제, 저분자 분자 표적약이 바람직하다.
아실티오우레아 화합물 (I) 에 의해, 그 작용이 증강되는 항종양제 A 로는, 파클리탁셀 (탁솔, 아브락산 등), 겜시타빈, 라파티닙, 테가푸르·기메라실·오테라실칼륨 배합제, 이리노테칸이 바람직하다.
아실티오우레아 화합물 (I) 이 그 작용이 증강되는 항종양제 A 와 함께 치료할 수 있는 악성 종양으로는 특별히 제한은 없지만, 예를 들어 두경부암, 식도암, 위암, 결장암, 직장암, 간암, 담낭·담관암, 췌장암, 폐암, 유방암, 난소암, 자궁경부암, 자궁체암, 신장암, 방광암, 전립선암, 정소 종양, 골·연부 육종, 백혈병, 악성 임파종, 다발성 골수종, 피부암, 뇌종양 등을 들 수 있다.
아실티오우레아 화합물 (I) 또는 그 약학적으로 허용되는 염과 항종양제 A 를 조합하면, 항종양 효과가 증강된 항종양약이 얻어진다. 이와 같은 새로운 항종양약의 형태로는, 아실티오우레아 화합물 (I) 또는 그 약학적으로 허용되는 염과, 항종양제 A 를 포함하는 일제형의 제제 형태이어도 되고, 아실티오우레아 화합물 (I) 또는 그 약학적으로 허용되는 염을 포함하는 제제와, 항종양제 A 를 포함하는 제제가 별개의 제제 형태이어도 된다. 또 아실티오우레아 화합물 (I) 을 포함하는 조성물의 투여 수단과, 항종양제 A 를 포함하는 조성물의 투여 수단은 동일해도 되고, 상이해도 된다 (예를 들어, 경구 투여와 주사).
아실티오우레아 화합물 (I) 또는 그 약학적으로 허용되는 염을 의약 조성물에 함유시키는 경우, 필요에 따라 약학적 담체와 배합하여, 예방 또는 치료 목적에 따라 각종 투여 형태를 채용 가능하며, 그 형태로는, 예를 들어, 경구제, 주사제, 좌제, 연고제, 첩부제 (貼付劑) 등을 들 수 있지만, 경구제가 바람직하다. 이들의 투여 형태는 각각 당업자에게 공지 관용의 제제 방법에 의해 제조할 수 있다.
약학적 담체는 제제 소재로서 관용의 각종 유기 혹은 무기 담체 물질이 사용되며, 고형 제제에 있어서의 부형제, 결합제, 붕괴제, 활택제, 착색제;액상 제제에 있어서의 용제, 용해 보조제, 현탁화제, 등장화제, 완충제, 무통화제 등으로서 배합된다. 또, 필요에 따라 방부제, 항산화제, 착색제, 감미제, 안정화제 등의 제제 첨가물을 사용할 수도 있다.
경구용 고형 제제를 조제하는 경우에는, 본 발명 화합물에 부형제, 필요에 따라, 결합제, 붕괴제, 활택제, 착색제, 교미·교취제 등을 첨가한 후, 통상적인 방법에 의해 정제, 피복 정제, 과립제, 산제, 캡슐제 등을 제조할 수 있다.
부형제로는, 유당, 백당, D-만니톨, 포도당, 전분, 탄산칼슘, 카올린, 미결정 셀룰로오스, 무수 규산 등을 들 수 있다.
결합제로는, 물, 에탄올, 1-프로판올, 2-프로판올, 단 (單) 시럽, 포도당액, α-전분액, 젤라틴액, D-만니톨, 카르복시메틸셀룰로오스, 하이드록시프로필셀룰로오스, 하이드록시프로필 스타치, 메틸셀룰로오스, 에틸셀룰로오스, 쉘락, 인산칼슘, 폴리비닐피롤리돈 등을 들 수 있다.
붕괴제로는, 건조 전분, 알긴산나트륨, 한천 분말, 탄산수소나트륨, 탄산칼슘, 라우릴황산나트륨, 스테아르산모노글리세리드, 유당 등을 들 수 있다.
활택제로는, 정제 탤크, 스테아르산염나트륨, 스테아르산마그네슘, 붕사, 폴리에틸렌글리콜 등을 들 수 있다.
착색제로는, 산화티탄, 산화철 등을 들 수 있다.
교미·교취제로는 백당, 등피, 시트르산, 타르타르산 등을 들 수 있다.
필요에 따라, 장용성 코팅 또는 효과의 지속을 목적으로 하여, 경구 제제에 공지된 방법에 의해 코팅을 실시할 수도 있다. 이와 같은 코팅제에는 하이드록시프로필메틸셀룰로오스, 에틸셀룰로오스, 하이드록시메틸셀룰로오스, 하이드록시프로필셀룰로오스, 폴리옥시에틸렌글리콜, Tween80 (등록상표) 등을 들 수 있다.
경구용 액체 제제를 조제하는 경우에는, 아실티오우레아 화합물 (I) 에 교미제, 완충제, 안정화제, 교취제 등을 첨가하여 통상적인 방법에 의해 내복액제, 시럽제, 엘릭시르제 등을 제조할 수 있다. 이 경우 교미·교취제로는, 상기에 예시된 것이어도 되고, 완충제로는, 시트르산나트륨 등을, 안정제로는, 트래거캔스, 아라비아 고무, 젤라틴 등을 들 수 있다.
주사제를 조제하는 경우에는, 아실티오우레아 화합물 (I) 에 pH 조절제, 완충제, 안정화제, 등장화제, 국소 마취제 등을 첨가하고, 통상적인 방법에 의해 피하, 근육내 및 정맥내용 주사제를 제조할 수 있다. 이 경우의 pH 조절제 및 완충제로는, 시트르산나트륨, 아세트산나트륨, 인산나트륨 등을 들 수 있다. 안정화제로는, 피로아황산나트륨, EDTA, 티오글리콜산, 티오락트산 등을 들 수 있다. 국소 마취제로는, 염산 프로카인, 염산 리도카인 등을 들 수 있다. 등장화제로는, 염화나트륨, 포도당, D-만니톨, 글리세린 등을 들 수 있다.
좌제를 조제하는 경우에는, 본 발명 화합물에 당업계에 있어서 공지된 제제용 담체, 예를 들어, 폴리에틸렌글리콜, 라놀린, 카카오지, 지방산 트리글리세리드 등을, 추가로 필요에 따라 Tween80 (등록상표) 과 같은 계면 활성제 등을 첨가한 후, 통상적인 방법에 의해 제조할 수 있다.
연고제를 조제하는 경우에는, 본 발명 화합물에 통상적으로 사용되는 기제, 안정제, 습윤제, 보존제 등이 필요에 따라 배합되고, 통상적인 방법에 의해 혼합, 제제화 된다. 기제로는, 유동 파라핀, 백색 바셀린, 백납, 옥틸도데실알코올, 파라핀 등을 들 수 있다. 보존제로는, 파라옥시벤조산메틸, 파라옥시벤조산에틸, 파라옥시벤조산프로필 등을 들 수 있다.
첩부제를 조제하는 경우에는, 통상적인 지지체에 상기 연고, 크림, 겔, 페이스트 등을 통상적인 방법에 의해 도포하면 된다. 지지체로는, 면, 스프, 화학 섬유로 이루어지는 직포, 부직포나 연질 염화비닐, 폴리에틸렌, 폴리우레탄 등의 필름 혹은 발포체 시트가 적당하다.
상기의 각 투여 단위 형태 중에 배합되어야 할 아실티오우레아 화합물 (I) 의 양은 이것을 적용해야 할 환자의 증상에 따라, 혹은 그 제형 등에 따라 일정하지는 않지만, 일반적으로 투여 단위 형태당, 경구제에서는 약 0.05 ∼ 2,000 ㎎, 주사제에서는 약 0.01 ∼ 100 ㎎, 좌제에서는 약 1 ∼ 2,000 ㎎ 정도이다.
또, 상기 투여 형태를 갖는 약제의 1 일당 투여량은 환자의 증상, 체중, 연령, 성별 등에 따라 상이하여 일률적으로는 결정할 수 없지만, 통상적으로 성인 (체중 50 ㎏) 1 일당 약 0.05 ∼ 5,000 ㎎ 정도이며, 0.1 ∼ 2,000 ㎎ 가 바람직하고, 이것을 1 일 1 회 또는 2 ∼ 3 회 정도로 나누어 투여하는 것이 바람직하다.
아실티오우레아 화합물 (I) 또는 그 염을 포함하는 제제와 항종양제 A 를 포함하는 제제가 별개의 제제인 경우에는, 각 제제는 동시에, 또는 하나의 성분 투여 전, 혹은 후의 임의의 시기에 다른 성분을 투여할 수 있다.
아실티오우레아 화합물 (I) 또는 그 약학적으로 허용되는 염과 항종양제 A 의 투여 혹은 배합 비율은 항종양 효과의 증강 효과를 발휘하는 범위이면 특별히 제한되지 않지만, 항종양제 A 1 몰에 대해, 아실티오우레아 화합물 (I) 또는 그 약학적으로 허용되는 염을 0.001 ∼ 100 몰 정도, 바람직하게는 0.005 ∼ 50 몰 정도로 하면 된다.
예를 들어 항종양제 A 가 파클리탁셀 (탁솔, 아브락산 등) 인 경우에는, 파클리탁셀 (탁솔, 아브락산 등) 1 몰에 대해, 아실티오우레아 화합물 (I) 또는 그 약학적으로 허용되는 염을 0.05 ∼ 50 몰 정도이다. 항종양제 A 가 겜시타빈 인 경우에는, 겜시타빈 1 몰에 대해, 아실티오우레아 화합물 (I) 또는 그 약학적으로 허용되는 염을 0.005 ∼ 5 몰 정도이다. 항종양제 A 가 라파티닙인 경우에는, 라파티닙 1 몰에 대해, 아실티오우레아 화합물 (I) 또는 그 약학적으로 허용되는 염을 0.01 ∼ 20 몰 정도이다. 항종양제 A 가 테가푸르·기메라실·오테라실칼륨 배합제인 경우에는, 테가푸르 1 몰에 대해, 아실티오우레아 화합물 (I) 또는 그 약학적으로 허용되는 염을 0.05 ∼ 50 몰 정도이다. 항종양제 A 가 이리노테칸인 경우에는, 이리노테칸 1 몰에 대해, 아실티오우레아 화합물 (I) 또는 그 약학적으로 허용되는 염을 0.05 ∼ 30 몰 정도이다. 항종양제 A 가 페메트렉세드인 경우에는, 페메트렉세드 1 몰에 대해, 아실티오우레아 화합물 (I) 또는 그 약학적으로 허용되는 염을 0.05 ∼ 20 몰 정도이다.
실시예
이하에 실시예, 시험예를 나타내고, 본 발명을 더욱 상세하게 설명하는데, 본 발명은 이들 실시예에 제한되는 것은 아니다. 또 이하의 시험예에서 나타내는, 본 발명 화합물에 의해 항종양 효과가 증강되는 각종 항종양제의 용량 설정에 대해서는, 논문 보고 등에 의해 나타난 최대 내용량 (Maximum tolerated dose;MTD) 혹은 물성 상 투여 가능한 최대 용량을 사용하였다.
항종양제는 최대 약효를 나타내는 용량과 독성 발현 용량이 매우 가까워, 그 약제가 갖는 최대 항종양 효과를 동물 모델에서 평가하기 위해서는 MTD 근방에 있어서 평가하는 것이 일반적이며, 이하의 시험예에 있어서는, MTD 와 최대 효과 발휘 용량을 동일한 의미로 하고 있다.
실시예 1
4-(2-플루오로-4-(3-(2-페닐아세틸)티오우레이도)페녹시)-7-메톡시-N-메틸퀴놀린-6-카르복스아미드 (화합물 (1)) 는 국제 공개 제2009/125597호 팜플렛에 기재된 방법에 의해 합성하였다.
시험예 1 파클리탁셀에 대한 항종양 효과 증강 작용
인간 폐암주 (NCI-H460) 을 생후 5 ∼ 6 주령의 웅성 BALB/cA Jcl-nu 마우스의 우측 흉부에 이식하였다. 종양 이식 후에 종양의 장경 (㎜) 및 단경 (㎜) 을 측정하고, 종양 체적 (tumor volume:TV) 을 산출 후, 각 군의 평균 TV 가 균등해지도록 각 군에 마우스를 할당하고, 이 군 분류 (n = 5) 를 실시한 날을 day 1 로 하였다.
파클리탁셀 (탁솔주, 브리스톨 제약 주식회사) 단독군의 피검액은 파클리탁셀 투여 용량으로서 60 ㎎/㎏/day 가 되도록 조제하였다. 또 화합물 (1) 단독군의 피검액은 200 ㎎/㎏/day 가 되도록 조제하였다. 화합물 (1) 은 day 1 로부터 14 일간 연일 경구 투여하고, 파클리탁셀은 day 1 에 마우스 미정맥으로부터 투여하였다. 병용 투여군에서는, 화합물 (1) 을 200 ㎎/㎏/day 와 파클리탁셀을 60 ㎎/㎏/day 로 투여하였다.
항종양 효과의 지표로서, 각 약제 투여군에서 day 15 에 있어서의 TV 를 측정하고, 하기 식에 의해, day 1 에 대한 상대 종양 체적 (relative tumor volume:RTV), 및 T/C (%) 를 산출하여 항종양 효과를 평가하였다. 병용 효과의 평가 판정은 병용 투여군의 평균 RTV 값이 개개의 단독 투여군의 평균 RTV 값보다 통계학적으로 유의 (Welch's IUT, over all maximum p<0.05) 하게 작은 경우에 병용 효과 있음으로서 판정하였다. 결과를 도 1 및 표 1 에 나타낸다. 도 중, * 표는 단독군에 대해 통계학적 유의차가 확인된 것을 나타낸다.

또 독성의 지표로서 시간 경과적으로 체중 [body weight:BW] 을 측정하고, day 1 에 대한 day 15 까지의 평균 체중 변화율 [body weight change:BWC (%)] 를 하기 식에 의해 산출하였다 (n:2 회/주로 실시하는 체중 측정일이며, 최종 측정일은 최종 평가일인 day 15 에 해당한다). 결과를 도 2 에 나타낸다.

시험예 2 겜시타빈 염산염에 대한 항종양 효과 증강 작용
인간 폐암주 (NCI-H441) 을 생후 5 ∼ 6 주령의 웅성 BALB/cA Jcl-nu 마우스의 우측 흉부에 이식하고, 시험예 1 과 동일하게 사용하였다.
겜시타빈 염산염 (젬자, 일라이릴리사 제조) 단독군의 피검액은 투여량이 240 ㎎/㎏/day 가 되도록 조제하였다. 또 화합물 (1) 의 피검액은 200 ㎎/㎏/day 가 되도록 조제하였다.
화합물 (1) 은 day 1 로부터 14 일간 연일 경구 투여하고, 겜시타빈 염산염은 day 1 및 8 에 마우스 미정맥으로부터 투여하였다. 병용 투여군에서는, 화합물 (1) 을 200 ㎎/㎏/day 와 겜시타빈 염산염을 240 ㎎/㎏/day 로 투여하고, 시험예 1 과 동일하게 평가하였다. 결과를 도 3 및 표 2 에 나타낸다. 도 중, * 표는 각 단독군에 대해 통계학적 유의차가 확인된 것을 나타낸다.
시험예 3 TS-1 에 대한 항종양 효과 증강 작용
인간 위암주 (NUGC-4) 를 생후 5 ∼ 6 주령의 웅성 누드 래트의 우측 흉부에 이식하고, 시험예 1 과 동일하게 사용하였다. TS-1 (티에스원, 타이호 약품 공업사 제조) 단독군의 피검액은 테가푸르 투여량이 18 ㎎/㎏/day 가 되도록 조제하였다. 또 화합물 (1) 의 피검액은 화합물 (1) 250 및 50 ㎎/㎏/day 가 되도록 조제하였다.
화합물 (1) 및 TS-1 을 day 1 로부터 14 일간 연일 경구 투여하였다. 병용 투여군에서는, 화합물 (1) 을 250 및 50 ㎎/㎏/day 와, TS-1 을 18 ㎎/㎏/day 로 투여하고, 시험예 1 과 동일하게 평가하였다. 결과를 도 4 및 표 3 에 나타낸다.
시험예 4 라파티닙에 대한 항종양 효과 증강 작용
인간 위암주 (NCI-N87) 을 생후 5 ∼ 6 주령의 웅성 BALB/cA Jcl-nu 마우스의 우측 흉부에 이식하고, 시험예 1 과 동일하게 사용하였다. 라파티닙 (LC laboratories 사 제조) 단독군의 피검액은 투여량이 100 ㎎/㎏/day 가 되도록 조제하였다. 또 화합물 (1) 의 피검액은 화합물 (1) 200 ㎎/㎏/day 가 되도록 조제하였다.
화합물 (1) 및 라파티닙을 day 1 로부터 14 일간 연일 경구 투여하였다. 병용 투여군에서는, 화합물 (1) 을 200 ㎎/㎏/day 와, 라파티닙을 100 ㎎/㎏/day 로 투여하고, 시험예 1 과 동일하게 평가하였다. 결과를 도 5 및 표 4 에 나타낸다. 도 중, * 표는 각 단독군에 대해 통계학적 유의차가 확인된 것을 나타낸다.
시험예 5 염산 이리노테칸에 대한 항종양 효과 증강 작용
인간 대장암주 (HT-29) 를 생후 5 ∼ 6 주령의 웅성 BALB/cA Jcl-nu 마우스의 우측 흉부에 이식하고, 시험예 1 과 동일하게 사용하였다.
염산 이리노테칸 (캠프토주, 야쿠르트사 제조) 단독군의 피검액은 투여량이 50 ㎎/㎏/day 가 되도록 조제하였다.
화합물 (1) 단독군의 피검액은 50 ㎎/㎏/day 가 되도록 조제하였다. 병용 투여군에서는, 화합물 (1) 을 50 ㎎/㎏/day 와 염산 이리노테칸을 50 ㎎/㎏/day 로 투여하였다. 화합물 (1) 은 day 1 로부터 14 일간 연일 경구 투여하고, 염산 이리노테칸은 day 1, 5, 9 에 마우스 미정맥으로부터 투여하고, 시험예 1 과 동일하게 평가하였다. 결과를 도 6 및 표 5 에 나타낸다. 도 중, * 표는 단독군에 대해 통계학적 유의차가 확인된 것을 나타낸다.
또, 독성의 지표인 시간 경과적인 체중 변화에 대해서도 시험예 1 과 동일하게 평가하였다. 결과를 도 7 에 나타낸다.
시험예 6 파클리탁셀에 대한 항종양 효과 증강 작용
인간 위암주 (NUGC-4) 를 생후 5 ∼ 6 주령의 웅성 BALB/cA Jcl-nu 마우스의 우측 흉부에 이식하고, 시험예 1 과 동일하게 사용하였다.
파클리탁셀 (탁솔주, 브리스톨 제약 주식회사 제조) 단독군의 피검액은 파클리탁셀 투여량으로서 60 ㎎/㎏/day 가 되도록 조제하였다.
화합물 (1) 단독군의 피검액은 12.5 ㎎/㎏/day 가 되도록 조제하였다. 병용 투여군에서는, 화합물 (1) 을 12.5 ㎎/㎏/day 와 파클리탁셀을 60 ㎎/㎏/day 로 투여하였다. 화합물 (1) 은 day 1 로부터 14 일간 연일 경구 투여하고, 파클리탁셀은 day 1 에 마우스 미정맥으로부터 투여하고, 시험예 1 과 동일하게 평가하였다. 결과를 도 8 및 표 6 에 나타낸다. 도 중, * 표는 단독군에 대해 통계학적 유의차가 확인된 것을 나타낸다.
또 독성의 지표인 시간 경과적인 체중 변화에 대해서도, 시험예 1 과 동일하게 평가하였다. 결과를 도 9 에 나타낸다.
도 1, 3, 4, 5, 6 및 8 로부터 분명한 바와 같이, 아실티오우레아 화합물 (I) 또는 그 약학적으로 허용되는 염은 각종 항종양제의 항종양 효과를 현저하게 증강시켰다. 그 효과는 아실티오우레아 화합물 (I) 의 저용량 (항종양 효과를 나타내지 않는 용량, 도 8, 표 6) 인 12.5 ㎎/㎏/day 로부터 확인되고, 누드 마우스에서의 고용량 (최대 효과 발휘 용량) 인 200 ㎎/㎏/day, 및 누드 래트에서의 고용량 (최대 효과 발휘 용량) 인 250 ㎎/㎏/day 에서는, 병용함으로써 현저한 종양의 축소를 유도하였다 (표 1, 2, 3 및 4). 또 병용 투여에 있어서 체중 감소의 악화는 확인되지 않았다 (도 2, 7, 9). 이것으로부터 본 발명 화합물과 각종 항종양제의 병용에 의해 효과역은 적어도 16 배 이상이 확보된 것이 된다.
또 예를 들어 도 1 과 도 8 의 비교에서 나타내는 바와 같이, 파클리탁셀에서는 동일 투여량을 사용해도 종양에 의해 그 항종양 효과 (약제 감수성) 에 차이가 확인되지만, 아실티오우레아 화합물 (I) 과 병용함으로써 모두 효과의 증강이 확인되고 있다. 즉 병용 약제인 파클리탁셀에 대해 감수성이 낮은 종양에 있어서도, 아실티오우레아 화합물 (I) 과 병용함으로써, 파클리탁셀의 종양 증식 억제 효과를 증강하는 것이 기대된다. 이것은 아실티오우레아 화합물 (I) 의 병용에 의해, 다른 항종양제의 항종양 스펙트럼이 확대되는 것을 의미한다.
또한 도 2 에서 나타내는 바와 같이, 아실티오우레아 화합물 (I) 과 각종 항종양제의 병용에 있어서, 고용량인 200 ㎎/㎏/day 를 사용한 경우에 있어서도 항종양제 단독 투여에 비해 현저한 체중 감소의 차가 없는 점에서, 독성을 증강하고 있지 않는 것이 시사되었다.
즉, 아실티오우레아 화합물 (I) 또는 그 약학적으로 허용되는 염과 각종 항종양제의 병용에서는, 현저한 독성의 악화를 나타내지 않고, 병용 효과를 나타내고, 그 치료 효과역이나 항종양 스펙트럼을 확대하는 것이 확인되었다. 또, 아실티오우레아 화합물 (I) 또는 그 염의 약학적으로 허용되는 염의 투여량은 다른 항종양제 1 몰에 대해 0.005 ∼ 50 몰 정도로 하는 것이 좋다.
Claims (40)
- 4-(2-플루오로-4-(3-(2-페닐아세틸)티오우레이도)페녹시)-7-메톡시-N-메틸퀴놀린-6-카르복스아미드 또는 그 약학적으로 허용되는 염을 유효 성분으로 하는 파클리탁셀, 겜시타빈 또는 그 염, 라파티닙 및 테가푸르·기메라실·오테라실칼륨 배합제로부터 선택되는 1 종 이상의 항종양제의 항종양 효과 증강용 약학적 조성물.
- 제 1 항에 있어서,
파클리탁셀이 탁솔 또는 아브락산인 항종양 효과 증강용 약학적 조성물. - 4-(2-플루오로-4-(3-(2-페닐아세틸)티오우레이도)페녹시)-7-메톡시-N-메틸퀴놀린-6-카르복스아미드 또는 그 약학적으로 허용되는 염과 파클리탁셀, 겜시타빈 또는 그 염, 라파티닙 및 테가푸르·기메라실·오테라실칼륨 배합제로부터 선택되는 1 종 이상의 항종양제를 조합하여 이루어지는 항종양 약학적 조성물.
- 제 3 항에 있어서,
파클리탁셀이 탁솔 또는 아브락산인 항종양 약학적 조성물. - 파클리탁셀, 겜시타빈 또는 그 염, 라파티닙 및 테가푸르·기메라실·오테라실칼륨 배합제로부터 선택되는 1 종 이상의 항종양제와, 일제형의 제제 형태 또는 별개의 제제 형태로서 투여함으로써 종양을 치료하기 위한, 4-(2-플루오로-4-(3-(2-페닐아세틸)티오우레이도)페녹시)-7-메톡시-N-메틸퀴놀린-6-카르복스아미드 또는 그 약학적으로 허용되는 염을 포함하는 약학적 조성물.
- 제 5 항에 있어서,
