Description
본 발명은 임상적 우울증, 특히 치매 또는 알코올 및/또는 니코틴 남용과 관련된 우울증의 치료 의도의 약물의 생산을 위한 디옥시페가닌의 용도에 관한 것이다.
국제질병분류(International Classification of Diseases, ICD-10) 및 미국 진단 및 통계 매뉴얼(American Diagnostic and Statistics Manual, DSM-IV)에 따른 단극성 우울증(이전에는 조병-우울증 장애라고 불리던 양극성 우울증과 대비하여)은 자주 동시적인 불안증, 수면장애 및 사회적 탈퇴와 함께 의기소침된 사고방식, 심리적 동인 및 관심의 일반적인 결핍의 조합에 특징이 있는 증후군이다. 질병과 관련된 개인의 고통과 자살의 위험은 별문제로하고, 우울증은 치료비용과 노동을 하지 못함으로 인한 거의 추산할 수 없는 정도의 국가경제전체에 대한 손실, 예를 들어 미국에서는 1990년 약 437억 달러의 추정 손실을 가져온다.
우울증은 가장 빈번하게 발생하는 심리적 질병이다. 유행병에 대한 수백건의 대대적인 연구 결과, 모든 여성의 10-25% 및 모든 남성의 5-12%가 일생에서 적어도 한 번은 우울증을 경험한다는 것을 보여준다. 개발된 국가에서는 인구의 약 5%가 우울증을 겪고 있고, 여기에서, 일반 내과의사를 찾거나 또는 병원에 가는 모든 환자의 15 - 25%가 우울증을 겪고 있다고 가정해야 한다. 세계적으로는 이는 이러한 환자의 약 10%에 적용된다. 우울증은 높고 점진적인 재발비율을 가진 질병이고, 이는 더욱 연속적 증가추세에 있다. 재발 가능성은 첫번째 우울증 에피소우드(episode)후의 50%에서 2번의 에피소우드후의 70%로 증가하고, 3번의 그러한 에피소우드후에 90%로 증가한다. WHO는 2020년에 세계인구에 대하여 5.7%가 우울증에 의해 질병부담이 될 것으로 추정하는데, 이러한 비율은 심장혈관 질병 비율의 바로 뒤를 따르는 것이다. 이러한 정보에 기초하여 어느 때든 1억 5천만의 사람이 임상적 우울증을 겪는다는 것을 산출할 수 있다(Mucke HAM. : Next-Generation CNS Therapeutics. Decision Resources, Inc. [Waltham, Mass., USA], 2001).
다수의 연구에서 입증된 바와 같이, 우울증, 알코올 남용 및 니코틴 남용 사이에는 3가지 방식의 동반이환성(comorbidity)이 있다(예를 들어, J. Hamalainen et al. ; J. Epidemiol. Community Health 2001; 55(8) : 573-576). 우울증의 2개의 필수적 병인 인자인 외상적 경험 및 만성적 스트레스는 알코올과 니코틴에 대한 소비행동의 형성과 상당히 연관되어 있다(H.J. Little, Alcohol Res. Health 2000; 24(4): 215 - 224).
임상적 우울증 치료를 위한 가장 효과적인 약물은, 세로토닌 및 노르에피네프린에 대한 신경 수용체를 차단하고 각각의 뉴론에서 이러한 신경전달물질의 재흡수를 억제함으로써, 우울증의 경우에 그 농도가 감소되는 이들의 시냅틱 내부의 농도를 정상화시키는, 소위 "트리사이클릭스(tricyclics)"-트리사이클릭 화합물이다. 트리사이클릭스는 이들의 사용이 상당한 부작용과 연관되고 특히 심혈관성 타입의 부작용과 연관되어 있지만, 현재 가장 널리 사용되고 있다.
선택적 세로토닌-재흡수 억제자 (selective serotonine-reabsorption inhibitor, SSRIs)는 1980년대 중반경에 사용되기 시작하였다. 클래스의 관점에서, SSRIs는 전형적 트리사이클릭스 만큼 효과적이지 않고, 1-2주의 경과후에야 완전한 활성을 보이기 시작하지만, 이들의 부작용은 상당히 작고 이들의 매우 낮은 급성 독성 때문에 SSRIs로 자살을 행하는 것은 실질적으로 불가능하다. 이러한 이유로 SSRIs는 자주 항우울증으로서 제1선택이 되어왔다.
그러나, 부작용은 별론으로 하고, 트리사이클릭스 및 SSRIs는 우울증 치료에서 상당한 부족한 점을 남긴다: 모든 환자의 약30%가 항우울증제의 2가지 클래스의 어느 것에도 적합한 방식으로 반응하지 않는다 (소위 난치성 우울증). 또한, 알코올 남용 감소의 수단으로서 뿐만 아니라 우울증 수준에서 음주 우울증 환자의 문제를 치료할 수 있는 단일한 활성물질은 없다. 과흡연 우울증 환자에 대하여는 단 하나의 활성제제 (Bupropion, GlaxoSmithKline)가 사용이 가능하지만, 2개의 별개의 상이한 용량의 약제의 형태로만 이용가능하다. 이중 하나 (Wellbutrin®)는 우울증의 치료용으로만 승인되었고, 다른 하나 (Zyban®)는 흡연자의 금단 치료를 뒷받침하는 데에만 한정하여 승인되었다.
트리사이클릭스 및 SSRIs에 대한 대체적인 작용제는 모노-아미노 옥시다아제 억제자(mono-amino oxidase inhibitor, MAOIs)이다. 이들은 약 50년간 공지된 활성제제 클래스이고, 이는 모든 "모노아민 신경전달물질" (즉 도파민을 포함하여)의 분해를 억제하여 뇌에서 이들의 농도를 증가시킨다. 이들 클래스의 초기 활성 제제는 비가역적인 방식으로 부분적으로 모노아민 옥시다아제(A 및 B)의 서브타입 모두를 억제한다. 한편으로는 간 손상의 발생과 또 다른 한편으로는 "치즈 효과" (cheese effect, 예를 들어 치즈와 같은 음식물로 섭취된 티라민(tyramine)의 분해 차단에 의해 야기되는 고혈압 위기) 때문에, MAOI's는 트리사이클릭스보다 뒤떨어졌다.
결론적으로, 여전히 항우울증제에 대한 상당한 필요성은 존재하는데, 특히 알코올 및/또는 니코틴을 남용하는 우울증 환자의 특정 경우에서뿐만 아니라 난치성 우울증의 치료를 위하여, 시판되는 활성물질보다 더욱 적합한 항우울증제에 대한 필요성이 존재한다.
따라서, 본 발명의 목적은 치매 치료용 약물, 특히 난치성 치매 치료용 약물로서, 알코올 및/또는 니코틴을 남용하는 우울증 환자의 특정한 경우에서, 시판되는 활성 작용제보다 더욱 적합하고 상기 언급한 단점을 가지지 않는 약물을 제공하는 것이다.
디옥시페가닌(deoxypeganine, 1,2,3,9-테트라하이드로피롤로[2,1-b]퀴나졸린)은, 남가새과(zygophyllacea family)의 식물에 존재하는 분자식이 C11H12N2
인 알칼로이드이다. 디옥시페가닌은, 바람직하게는 시리아 루타(Syrian rue, 페가눔 하말라(Peganum harmala))로부터 정제에 의해 얻거나, 합성에 의해 얻는다. 이는, 문헌, 특히 특허 명세서로부터 약리학적 분야에서 공지되어 있다.
DE-A 199 06 978, 즉 WO 00/48582는 약물중독 및 약물의존성 치료를 위한 디옥시페가닌에 기초한 약물을 기술한다.
DE-A 199 06 975, 즉 WO 00/48599는 알츠하이머 치매 치료를 위한 디옥시페가닌의 용도를 기술한다.
DE-A 199 06 979, 즉 WO 00/48445는 니코틴 의존성의 치료를 위한 디옥시페가닌에 기초한 약물을 기술한다.
약학적 성질에 기초하면, 디옥시페가닌은 가역적으로 작용하는 콜린에스터라아제 억제자 그룹에 포함된다. 디옥시페가닌이 아세틸콜린에스터라아제 뿐만 아니라 모노-아미노 옥시다아제를 억제한다는 사실은 상기 문헌으로부터 일반적인 사실로서 알 수 있지만, 상기 문헌에서는 어떠한 방식으로도 모노-아미노 옥시다아제 A 및 B의 두가지 서브타입을 구별하지는 않는다. 무엇보다도, 모노-아미노 옥시다아제 억제는 디옥시페가닌의 아세틸콜린에스터라아제 억제를 강화하기 위한 의도의 상보적 작용으로서만 항상 기술되어 왔고, 후자의 억제가 가장 중요한 것으로 여겨졌는데, 예를 들어, 아세틸콜린에스터라아제 및 모노-아미노 옥시다아제의 동시 억제의 이점은 - 중량 유닛에 대하여 - 청구된 각각의 출원에 대하여 (원형적인 잠재 콜린에스터라아제 억제자 피소스티그민과 비교하여) 콜린에스터라아제의 낮은 억제를 보상한다는 것이 명시적으로 언급되었다. 마지막으로, 이러한 문헌 어느 것도 응용 가능한 분야로서 우울증에 대하여는 다루지 않았다.
추가적인 약리학적 연구 과정에서, 놀랍게도, 상기 언급한 문헌에서 기술된 바와 같이 디옥시페가닌이 아세틸콜린에스터라아제를 실제로 억제하지만, 시험관내 양적인 주된 작용은 타입 A의 모노-아미노 옥시다아제(MAO-A)의 선택적 억제에 있고, 반면, 타입 B 효소는 크게 억제되지 않는다는 것을 발견하였다. 언급한 초기 모노-아미노 옥시다아제 억제자의 부작용은 선택적 모노-아미노 옥시다아제 A의 가역적 억제자(reversible inhibitor of mono-amino oxidase A, RIMA)에 의하여 대체로 피할 수 있다.
또한, 적절한 동물 실험에서 디옥시페가닌은 강력한 항우울성 및 정신운동적으로 자극적인 활성을 보이는 것을 발견하였고, 이러한 발견은 상기 이중 발견과 관련되는 것이지만 현재기술의 상황에 있어서는 완전히 놀라운 것이다. 콜린작동성 활성화 동물모델에서 통계학적으로 유효한 효과를 아직 나타내지 않는 투여량에서 여기에서의 최대 작용이 이미 발생한다.
랫트 브레인(Wistar 종)으로부터의 모노-아미노 옥시다아제 A에 관하여 디옥시페가닌의 억제적 작용은 Medvedev 등에 의하여 기술된 방법(Biochem Pharmacol 1994; 47(2): 303-308)에 따라 10nM 내지 10μM의 범위의 농도에서 측정하였고, 포지티브 대조군(positive control)인 클로로기라인(clorogyline)과 비교하였는데, 여기에서 양쪽 경우에서 20mM의 포타슘디하이드로젠 포스페이트 버퍼 pH 7.4내에서 1% 디메틸설폭시드 용액내의 95μM [3H]세로토닌을 기질로 제공하였다. 디옥시페가닌에 대하여는 1.49 μM의 값이 반-최대(semi-maximal)억제작용(억제 농도 50% = IC50)값으로 얻어졌다. 이러한 값은 다른 생체외 시스템에서 아세틸콜린에스터라아제의 억제에 대하여 얻어진 IC50 값 이하로 거의 10의 거듭제곱이다. 반대로, 모노아미노 옥시다아제 B는 10μM 농도에서 15 - 20%의 억제만을 받았다.
생체내에서 이러한 MAO-A-억제 작용이 관련되는지 증명하기 위하여 디옥시페가닌을 랫트에 대한 "강요된 스위밍 테스트(Forced Swimming Test)" (R.D. Porsolt 등, Nature 1977; 266(5604):730-732)로 시험하였다. 이러한 모델은 이미 그들에게 알려진 절망적 상황에 있는 동물을 보여주는 "행동적 절망"이라고 불리우는 행동에 기초한다: 빠져나올 수 없는, 물로 채워진 용기내에 이들을 두게되면, 이들은 일정기간후에 탈출하려는 시도를 버리고 가장 필수적인 수영동작만을 행하게 된다. 동물이 이러한 정신 운동학적 불활성화에 빠지기까지 탈출하려고 시도하면서 보낸 시간(우울증에 대한 대리자(surrogate)로서 간주됨)을 측정한다; 연장된 활성은 항우울증 작용에 대응한다.
구체적인 경우에 있어서, 약 6주된 50마리의 수컷 랫트(Sprague Dawley 종; Charles River UK Ltd.로부터 입수함)를 10마리씩 7개의 그룹으로 나누었다. 제1일 습관화 단계 종료후, 각 동물을 약 25°의 물로 15cm 높이로 채워진 실린더형 용기내에 두었다; 이 후, 각각 1시간 후, 19시간 후, 23 시간 후에, (그룹에 따라)네가티브 대조군(물), 15 mg/kg 포지티브 대조군 (이미프라민(imipramine)ㆍHCl), 또는 1.0, 2.5, 7.5, 15.0 또는 22.5 mg/kg체중의 용량으로 디옥시페가닌ㆍHCl의 3번의 경구투여를 하였다. 위장튜브의 도움으로 투여되는 부피는 각각의 경우에서 5ml/kg체중 이었다. 3번째의 처리 후 1시간 후에, 각각의 동물을 정확히 5분간 용기내에 두고 최소 행동성에서 소요되는 시간을 측정하였다.
예비테스트의 결과에 따라 15 mg/kg의 언급된 복용량에 대하여 이러한 시스템에서 최대적으로 달성가능한 행동을 정의하는 잠재적 트리사이클릭 항우울증제 이미프라민은 물과 비교하여 최소 행동성에서 소요되는 시간을 56.5%에서 58.9%로 낮추었다. 1 mg/kg의 복용량의 디옥시페가닌은 아직 효과적이지는 않은 반면, 2.5 mg/kg에서는 부분적으로만 효과적이었고, 7.5 mg/kg부터의 모든 농도는 평균 41.4% 내지 44.1%의 범위의 감소를 나타내었다. 이러한 평탄역의 모든 값은 p<0.01의 수준에서 통계학적으로 의미가 있었다. 따라서, 모든 농도에 대하여 디옥시페가닌으로 처리된 효과는 실제로 이러한 시스템에서 가능한 최대치 이하로 유지되었지만, 포지티브 대조군에 대하여 사용된 투여량(7.5 mg/kg)의 절반에서 이미 이 물질에 대한 최대값이 도달되었다. (표 1 참조).

이러한 결과는 원칙적으로 항우울증 작용을 가지지 않는 정신 운동학적으로 활성적인 물질에 의하여도 얻어질 수 있기 때문에, 디옥시페가닌의 효과도 동일한 복용량 범위로하여 소위 오픈 필드 패러다임내에서 측정하였다. 여기에서 랫트를 개방된 밝은 영역에 두게 되면 랫트에게는 스트레스이기 때문에 가능하면 이러한 상황을 랫트가 회피할 것이라는 사실을 이용한다. Sprague-Dawley 랫트를 사용하여 수행되는 것과 같은, 2.5 내지 22.5 mg/kg의 디옥시페가닌 매일의 복용량으로 2주간 연속 처리를 포함하는 이러한 실험은 정신운동학적 활성화의 어떠한 징조를 표시하지 않지만, Porsolt 테스트의 결과(7.5 mg/kg p.o.에서 항우울증제 이미프라민에 필적하는 최대 작용활성)는 의미있는 것으로 해석되어야 한다.
이는, 추가 테스트 시스템이 - 여기에서 디옥시페가닌의 아세틸콜린에스터라아제-억제 작용을 이용하여 부분적으로 손상된 중추 콜린작동성 경로를 갖는 랫트에서의 콜린작동성 기억 결함이 보상된다 - 7.5 mg/kg의 경구 복용량에서는 통계학적으로 의미있는 것은 아니고, 22.5 mg/kg에서는 이의 최대값에 아직 도달하지 않는 동일한 복용량 범위에서의 분명한 선형 의존성을 보이기 때문에 더욱 놀랍다.
이것으로부터, 스위밍 테스트에서 관찰될 수 있는 효과가, 현재기술의 상태에 따라 고려될 수 없는, 콜린작동성 시스템의 최대 활성화에 필요한 복용량의 부분에서 이의 최대치에 도달한다는 것을 추론할 수 있다.
따라서, 우울증의 인정된 동물모델에서, 디옥시페가닌은 즉 다른 동일한 조건하의 콜린작동성 보상 행동모델에서 절대적으로 여전히 차선적 작용을 나타내는 복용량에서 이미 최대정도로의 각각 항우울성, 정신 운동학적으로 활성적 작용을 갖는다.
따라서, 디옥시페가닌은 항우울증제로서 잠재적으로 적합하다.
디옥시페가닌의 투여는 경구적 또는 비경구적이 될 수 있다. 경구적 투여에 대하여는, 테이블릿, 캡슐, 코팅 테이블릿, 마름모꼴정제(lozenge)와 같은 공지된 투여형태가 사용될 수 있다.
또한, 예를 들면 음용 용액과 같은 액체 또는 반액체 복용형태가 적합한데, 이때 상기 제제는 용액이나 현탁액의 형태로 존재하게 된다. 사용가능한 용매 또는 현탁제는 물, 수성 매질 또는 약리학적으로 허용가능한 오일 (식물성 오일 또는 광물성 오일)이다. 디옥시페가닌 함유 약물은 바람직하게는 연장된 기간에 걸쳐 조절되는 방식으로 이러한 제제를 체내로 전달할 수 있는 저장소(depot) 약물로 제형화된다.
또한, 본 발명의 디옥시페가닌은 직장으로 투여될 수 있고(즉, 좌약으로 도입), 흡입하여 투여될 수 있고(정해진 농도와 크기 분포의 입자를 가진 에어로졸로 흡입), 경피적으로 투여될 수 있고(활성작용제-함유 패치, 바르는약 용액, 겔 등), 경점막적으로 투여될 수 있고(구강 및 코 점막을 통하여 흡수되는 관점에서, 활성작용제가 타액에 용해되어 구강공동내로 방출되거나, 스프레이 용액에 의하여 코안으로 도입 등), 이식된 용기를 통하여 투여될 수 있고(수동 삼투적으로 또는 미니펌프수단에 의해 조절되는 등의), 정맥내, 근육내 또는 피하 주사에 의해 투여될 수 있으며, 대뇌실내적으로 투여될 수 있다.
비경구적 투여와 관련하여, 본 발명의 디옥시페가닌 투여를 위한 경피적 또는 경점막적 복용형태, 특히 DE-A 199 06 977에서 디옥시페가닌을 위하여 특별히 기술된 바와 같은 접착성 경피 치료 시스템(작용제 플라스터)를 사용하는 것이 가 능하고, 이는 특별한 이점을 가진다. 이를 통하여 치료되는 환자에 대하여 피부를 통하여 작용제를 연장된 기간에 걸쳐 조절되는 방식으로 전달하는 것이 가능하다.
본 발명에 따르면, 디옥시페가닌은 치료를 위하여 유리 염기(free base)형태 및 산부가염(acid addition salt)의 형태로 사용되는 것이 가능하다; 바람직한 염은 디옥시페가닌 염산염(deoxypeganine hydrochloride)과 디옥시페가닌 브롬화수소산염(deoxypeganine hydrobromide)이다. 또한, 예를 들어 구연산염(citrate), 주석산염(tartrate) 또는 아세트산염과 같은 약리학적으로 허용가능한 다른 염을 사용하는 것이 가능하다.
디옥시페가닌을 투여하기 위하여 본 발명에 따라 사용되는 약제학적 제제는 다음의 부가제중 하나 이상을 포함할 수 있다:
- 항산화제, 상조제(synergist), 안정화제;
- 보존제(preservatives);
- 맛 교정제(taste corrigent);
- 색소;
- 용매, 가용화제;
- 계면 활성제(유상제, 가용화제, 습윤제, 소포제(antifoams)) ;
- 점도 및 농도(consistency)에 영향을 주는 제제, 겔 형성제;
- 흡수 촉진제;
- 흡착제, 휴멕턴트(humectant), 글리던트(glidant);
- 붕해(disintegration) 및 용해(dissolution)에 영향을 주는 작용제, 필러( 증량제(extender)), 펩타이저(peptizer);
- 방출 지연제
상기 목록은 한정적인 것이 아니다; 적합하고 생리학적으로 허용가능한 물질이 당업자에게 공지되어 있다.
디옥시페가닌은, 각 경우에서 유리 디옥시페가닌으로 계산하여, 바람직하게는 0.1 내지 90중량%의 비율, 특히 바람직하게는 2 내지 20중량%의 비율의 작용제를 포함하는 약제학적 제제로 투여된다. 본 발명에서 사용된 디옥시페가닌-함유 약리학적 제제는 당업자에게 공지된 양으로 불활성 성분, 첨가제(excipient), 부형약(vehicle) 및/또는 안정화제와 같은 부가제를 추가적으로 함유할 수 있다.
하루 투여량은 바람직하게는 0.1 내지 100mg, 특히 10 내지 50mg의 범위이다. 이는 개인의 요구에 따라 적절하게 조절해야 한다.
