Description

【発明の詳細な説明】 本発明は外科用骨畳め具、より具体的には柔らかい組織
を骨に固定する留め金（ステープル）に関する。外科の
分野では、柔らかい組織を骨に固着し、増殖させるため
に患者の骨に柔らかい組織（例えば、靭帯、筋肉、軟骨
、健）を移殖することがしばしば必要となる。そのよう
に、例えば、けがで骨から剥離された柔らかい組織は再
結合される。その他の手術では、柔らかい組織は、短か
くしたら、又は移動させたりできるように骨から外相的
に分離される。そのような手術例としては、大腿部のみ
ぞの膝蓋骨の側部亜脱臼を抑制するために膝蓋健を平均
に移殖する手術が良（知られている。
横方向のブリッジ部相の裏側」二に歯を保持して柔らか
い組織を留める外科用留め金は公知である。
その例として、す゛チャーズ固定留め金（リチャーズ製
造会社、メンフイス、テネγ−）がある。他には「デパ
ルマ　ステーブルｊ　（’ＤｅＰａ１ｍａ　５ｔａｐｌ
ｅ）（ハウメゾイカ　インコーホレイティド、ルーサフ
オード、ニュージャージ）がある。また、米国特許第４
１．２７８，０９１は骨留め金とその裏側上に多数の歯
を有する組織保持要素との組合せを開示している。
これらのすべての留め金において、留め金のブリッジは
骨に予想される弓状に概ね合うように直線又はわずかに
曲線を描いており、その歯は短い。
従って、これらの先行技術の留め金を、骨に管状又は楕
円形の柔らかい組織を固定するために使用づ−るときは
、その組織はブリッジと骨の表面との間で平扁に圧搾さ
れている。管状組織のこの湾曲を起させる圧力は該組織
に深刻な傷をつけ、予定の治療と骨への固着を遅らせる
。
本発明の目的は組織に不必要な圧迫傷をつけることなく
管状の柔らかい組織を骨に安全に取りつける骨留め金を
提供することを目的とする。
この目的と特徴は適所にあるときにブリッジ部材が骨の
表面の上方で弓状になっている骨留め金によって達成さ
れる。このデザインはブリッジと骨との間の過剰圧力に
より管状組織に与える傷を最小限にする。弓状ブリッジ
の凹形裏側は複数の突き刺すために垂れ下っている山を
有し、組織を取付は位置へ確実に固定する。
本発明は、この発明による骨留め金の好ましい実施例と
して記載されている。この好ましい実施例は第１図に示
した。留め金１は横方向のブリッジ部材３によりその上
方端で接合された２本の概ね平行の脚２を持つ。ブリッ
ジ部材３は脚２と同一平面のアーチを形成している。ブ
リッジ部材３の凹状面４は脚に面している。ブリッジ部
材３は脚に垂直の平面内で脚と接合している。しかし、
望むならば特別用途のために、ブリッジ部材３を角状留
め金として、脚に斜めの平面内で脚と接合していてもよ
い。
ブリッジ部材３は第２図の接合部６で示したように脚２
よりも広（（第１図の平面と垂直の方向へ）、留め金を
骨に打ち込むための押しつぶした頭部９を設けるために
水平の広い平扁中心部を有する。
脚２はブリッジ有材３との接合部６にある上方端から下
方端７に向ってわずかに先細になっている。下方端７は
鋭くとがっている。脚の両側面は脚のとがった下方端７
から離れる方向へ向うのこぎり歯の先端８を有するのこ
ぎり歯状になっている。
ブリッジ部材３の凹状面はそこから延びる複数の歯を持
つ。これらの歯は概ね脚に平行であり、かつ歯の先端が
ブリッジ部材の接合部６で脚に垂直な平面を必然的に形
成するように歯の長さが異なっている。この好ましい留
め金の形状において、歯５はブリッジ部材３の凹状面４
０両側面から２列に延びている。歯はブリッジ部材を横
切る溝を削って作っである。これらの並行にセットした
溝は留め金の平面に平行に削った弧形溝に斜角に削られ
ている２列の歯は一列目の歯が２列目の歯と偏位するよ
うに作られている。そのようにして、歯は平行四辺形の
横断面を有する。この製造方法により、弓状溝がブリッ
ジ部材３の凹状面４を形成する。これは第１．３、及び
４図に示されている。歯の端部は先端に向ってとがった
先細になっている。
本発明の留め金の代表的使用方法は次の通りである。
管状又は楕円形横断面を持つ柔らかい組織片（例えば、
健、靭帯、筋肉、軟骨）を再結合する骨にあてがう。そ
の組織片のいずれかの面、相互から横切って延びる概ね
真直な面の上に留め金の脚を置き、骨に打ち込む。骨が
非常に堅い場合には、案内孔をドリルであけて留め金の
脚の骨への挿入を容易にすることができる。脚が骨に完
全に打ち込まれるにつれて、ブリッジ部材は組織を押し
、麟は組織を通って骨の表面に達し、確実に取りつけら
れる。ブリッジ部材が上方移動を防ぎ、しかも歯が長手
方向の移動を支え、脚が側方向の移動を防ぐのでこの取
付けは安定している。この安定した取付けは、ブリッジ
部材が骨の表面から離れて弓状になっているために、従
来の歯形泊め金で生じるような組織を損傷させる危険又
はブリッジ部材下の過剰圧迫による血管分布の危険を最
少限に止めて行なわれる。仮に、脚が骨にできるだけ深
（打ち込まれても、組織を圧迫しない。留め金はこのよ
うにして使用されて、人工健とか人工靭帯のような人工
組織を取り付ける。
本発明の留め金は、異なる直径と厚みをもつ骨と組織に
使用できる範囲の寸法で作ることができる。代表的な留
め金は長さ２２．９　ｍｍ、直径３．０ｍｍ１弓形のブ
リッジ部材により１５．５　ｍ＋π離間保持された脚を
有する。ブリッジ部材の凹状面は脚との接合部から４．
３　ｍｍ上っている。押しつぶした頭部は該接合部上６
．１．　ｍｍである。歯はブリッジの凹部の下と、脚と
ブリッジの接合部の平面の端とから垂れ下っているので
、歯の長さは区々である。この態様では、その中央部の
最長４．３龍から両側部の最小約１．３　ｉｎまで長さ
の異なる５本の歯が２列になっている。本明細書におけ
る好ましい留め金は上記したように２本の脚を有する。
しかしながら、本発明の範囲はこの態様に限定されない
。本発明による留め金は複数の脚を持つ。例えば、四方
形内に４本の脚を持つ「テーブル　ステーブル」を作っ
てもよい。この形の留め金では、第５図に示したような
平面図となる。管状組織が通過する（第５図の矢印Ｂ）
側の正面図は第１図と同様である。
第５図において、ブリッジ３は各々が四隅にある４本の
脚２（点線で図示）と結合している。ブリッジ３は弓の
全長とほぼ同じ幅（矢印Ｂの方向）である。ブリッジの
アーチが円形である場合には、ブリッジは矢印Ｂと平行
の軸を持つ円筒形の形状となるであろう。押しつぶした
頭部９を設けるためにブリッジの先端中央は矩形の平扁
部である。
本発明は留め金の大きさ又は寸法と歯の並びについて特
別の組合せに制限されないが、上記凹形の下側が、脚が
完全に打ち込まれたときに骨の表面上方に有意義に残存
しているようにブリッジの中央部が十分に高い弓形とな
っていることが重要である。これはほとんどの場合、ブ
リッジ部拐のアーチが留め全全長の約２０％〜約５０％
の範囲の高さの山になっているときに実現される。上記
した代表的ステーゾルでは、アーチの高さはその全長の
（４，３／］、５．５）Ｘ１００−２７．７パーセント
である。

Claims (1)

  1. 【特許請求の範囲】 ■、各端部でとがった脚と結合したブリッジ部材と複数
    のとがった歯とから成り、該脚が概ね同一方向にとがっ
    ていて該ブリッジ部材とほぼ同一平面上にあり、該ブリ
    ッジ部材が該脚に面する凹状面を有するアーチの形状に
    なっており、該複数のとがった歯が該凹状面から概ね該
    脚に平行に延び。 該歯が実質的に該脚より短かく、かつ該アーチがその全
    長の約２０％〜約５０％の高さを有し、それによって該
    凹状面の中央部が該脚が骨に完全に打ち込まれたときに
    骨の表面より実質的に上方に存在するようになっている
    外科用骨留め金。 ２、上記歯の先端がその上方端で上記脚に垂直な平面内
    にそろっている特許請求の範囲第１項に記載の外科用骨
    留め金。 ３　上記歯が上記ブリッジ部材の凹状面に沿って２列（
    （なっている特許請求の範囲第２項に記載の外科用骨留
    め金。 ４、上記歯の一列が他方の列の歯から偏位されている特
    許請求の範囲第３項に記載の外科用骨留め金。 ５、上記歯が上記ズリツジ部拐の平面に平行な一対の第
    −側面及び該第−側面に斜めの一対の第二側面と共に平
    行四辺形を形成する横断面を有する特許請求の範囲第４
    項に記載の外科用骨留め金。 ６、上記ブリッジ部材の外側表面が骨に該第め金を打ち
    込むために押しつぶされた頭部を形成−ｒる平扁部分を
    有する特許請求の範囲第２項に記載の外科用骨留め金。 ７　上記脚がその上方から下方端に向って先細にプよっ
    ている特許請求の範囲第２項に記載の外科用骨留め金。 ８、上記脚がその下方端から離れて方向づけだのこぎり
    歯を持つのこぎり形状になっている特許請求の範囲第７
    項に記載の外科用骨留め金。 ９、上記ブリッジ部側のアーチの高さが核心め金の全長
    の約２８％である特許請求の範囲第６項に記載の外科用
    骨留め金。 １０．上記ブリッジ部材が各端部で第二のとがった脚に
    結合され、すべての脚が概ね同−形状及び同一方向にな
    っている特許請求の範囲第１項に記載の外科用′１−・
    留め金、 １！、上記ブリッジ部羽の幅がその全長にほぼ等しく、
    脚が該ブリッジ部材の各隅の下に配設されている特許請
    求の範囲第１０項に記載の外科用骨留め金。 １２、管状の柔らかい組織を骨にとりつげる方法であっ
    て、柔らかい組織を骨の所望位置へ置き、骨留め金の脚
    が柔らかい組織を所望の取付は位置に及ぶように骨留め
    金を置き、 該骨留め金が該脚と結合する概ねアーチの形状で、該脚
    に面する凹状面を持つ横方向のブリッジ部材を有する形
    状になっており；該ブリッジ部材の凹状面から延びる複
    数のとがった歯を有し、該歯が該脚と概ね平行であって
    、実質的に該脚より短かく；かつ該脚が骨に完全に打ち
    込まれたときに該ブリッジ部材の凹状面の中央部が実質
    的に骨の表面の上方にあるように該留め金の全長の約２
    ０−５０％の高さの該アーチを持ち、 そして該留め金を骨に完全に打ち込む工程から成る外科
    用骨留め金の使用方法。
