Description

【発明の詳細な説明】 本発明は移植により関節軟骨の欠陥と骨の欠陥とを修復
するに使用するための組）ａ物に関するものである。本
発明の組成物はそうした修復用に使用できる。それらは
、それとして、または天然または人工骨をキャリヤとし
て一諸にして、ゲルの形で提供される。以下の記述は主
として関節軟骨の修復に関連しているが、これは最も問
題であり繊細な生物学構造であるからである。本発明の
新規な組成物は、軟骨形成誘因因子の影響により潜在的
に軟骨細胞に変換されうるところの萌芽性の軟骨細胞な
いし成る間葉繊細胞を組合せて含んで居り、細胞は適量
の線維素片、トロンビン、及びアンチプロテアーゼを組
合せて膠として役立つように作られている。
結果としてできるゲルは組織培養貯蔵条件下に長期間に
渉って貯蔵でき、ト匈送でき、容易に取扱いできる。
本発明の好ましい態様によれば、細胞外基質（ＥＣＭ　
）　、または、ＳＭ　、　ＦＧＦ　、　ＣＧＦ　、　Ｂ
ＤＧＦの如き成る生長ファクターまたはこれらの何れか
の４ｒａ合せをそうしたゲル中に内蔵させられうる。
骨の成る欠陥の修復のためには、そうしたゲルで満して
適当な形状にした骨部祠を含む夢植物を使用しうる。
本発明の他の更に別の特徴は今後明かとなるであろう。
関節軟骨が外傷、感染ないし退化性操作によって損ぜら
れると、そうした損傷は一般に治し得す、改良さえもで
きない。これまで、骨軟骨植接にたよることや、種々の
形の補綴を用意することに種々の試みが行われて来たが
、長期的結果は貧弱であり落胆すべきものであった。軟
骨移□植の源泉として培養した軟骨細胞を使用する試み
も報告されたが移植物と隣接する軟骨との一体化は一般
に不満足であった。
本発明によれば、直ちに使用可能な組成物が提供され、
萌芽性の軟骨細胞または、間葉織起源の細胞、何等かの
ｉｆｓ［のもので、軟骨形成誘因ファクターの影Ｕによ
り、試Ｐ倹管内または生体内で軟骨細胞へ変換されつる
もので、゛ゲル″を形成する適当な生物学的媒体中で大
人の関節軟骨及び骨への損傷の修理に対して、損傷の場
所への移植により行・うのに使用しうるものが提案され
ている。また、そうした移植可能材料の生産用の操作も
また提案されている。
また、軟骨の欠陥の修理に使用する組成物も提案されて
いる。その８Ｍ物は、適当な萌芽性の組織の機械的破砕
と共にトリプシン化による萌芽性軟骨細胞（若さを留め
た軟骨細胞）の遊離で作り、ついで適当な媒質上で培養
し、細胞を収穫し、それらを線維素片、アンチプロテア
ーゼ及びトロンビンを加えてその結果として、培養器中
で限られた時間の期間貯蔵ができるゲルを得、または、
収穫された細胞が望むゲルへ使用する前に深冷及び氷解
によって長期間保存ざｎうるようになるようなゲルを得
る。ゲルは、欠陥を満すようにトロンビン溶液を噴霧し
た移植の場所に適用するｔｊｉｊに線維素片の溶液中に
浸されている。動物実験（鳥類及び哺乳類種につき）に
於て、軟骨と骨との欠陥は２乃至１２ケ月の４ｖ］川］
後の検証したとところでは、切筋損傷の侶れた修復を示
して居て、正しく満されたことが示された。
本発ＥＩ＋］の組成物は、外傷ないし老齢のせいでの人
ＩＬＪ関変軟骨または骨の損傷の修復に価値がある。そ
うした損傷は４ｆｉ々の型の軟骨及び骨で、接合部の退
化性傷吾や破砕を含んでいる。
本発明の好ましい態様によれば、本発明の新規な処方は
、以下に於て開示される如くに特殊技法で調製される萌
芽性軟骨細胞の或：（１の細、抱外ｉ、質（ＥＣＭ　）
を含んでいる。
ＥＣＭを設けることで、新規なａＫ物が植付けられた時
に軟骨細胞の成長を高めるが、これらが望む天然環境に
似ている事が最も確からしい。
軟骨細胞は、移植される材料が適用きるべき種の適当な
萌芽性置端ｍ織から得られる。異質形成細胞は、ＨＬＡ
タイプ化にたよる必要なしに全く満足な結果を与える。
人間での使用に対しては、人間萌芽性軟骨細胞が培呑さ
れた。
毎単位容積当りの軟骨細胞数は成る値を超えないもので
あるべきである。それでないと、ＩＢ胞の類壊死が生じ
、より劣る結果が得られる。
軟骨細胞濃度の代表的値は、ゲルの毎ミリリットル当り
約１０００００から５０００００である。毎ミリリット
ル当り約５から５０単位のトロンビンと約２５から８０
　Ｔ４　／　ｎｆ　、ｆｆＡ　ｆ、５１．　％原が使用
される。一般にプロテアーゼ抑制剤を、ゲルの速い溶解
を防ぐか、抑えるため組成物へ加える。天然または合成
プロテアーゼ抑制剤を使ってもよい。適切なプロテアー
ゼ抑制剤は、ゲルの約１０から２０■／　ｍｅの量で使
用されるε−アミ／カプロイン酸、ゲルの約１から２ｍ
’ｉ　／　７の量で使用されるトランエキゼミン酸の如
きである。アンチ・トリプシン（チキン卵白、シグマ、
’　ｆｆ５　ｉｌｌ　ｉ５　）の如き天然プロテアーゼ
抑制剤を使ってもよい。ゲルの遅い形成が望まれる時は

ビンが使用され、ゲルの速い硬化が望まれる時は２０か
ら５０単位毎７の程度の量のトロンビンが使用される。
上に加えて、ゲル中に、局部架（例えばＥＣＭ、（。
ＢＭＰ　）またはホルモン性、例えば、ソマトメジン（
ＳｉＯ状ペプチード；軟骨成長ファクター（ＣＧＦ　）
　；などの一つ以上の成長ファクターを内−戊させると

Ｆ−１２プラス１０％子牛脂児血清で作った組成物は、
７均３７℃でＣＯ２培養器内に二、三週の間メ１？蔵で
きる。本発明によるゲルは、直ちに使用でき、下記に示
す如くに、損傷された、または、欠陥個所に直接に適用
しうる。
成る修復に対しては、トロンビン、アンチグロテアーゼ
、線維葉片及び一つ以上の成長ファクターおよび／また
は上に鵡（定された型の細胞の小濃度を伴うか、または
、そうした細胞なしでさえも、ホルモンから成るゲルで
満した骨構造（天然または人工）から＋ｍる移イ１）を
利用することが推奨される。後者の場合、棋ｌｌからの
細胞が移植内に徐々に滲透し、環」〕）と移植との間に
凝集牲一体物を形成する。

、日（６−１０日）　４１１ｉ除きさせる。この生ぎ１
１月１■匂の間、線維芽問胞の大抵のものは死に去り、
秋骨約胞九１殖が巳る。炊・く寒天板からの細８．４は
更にスピンナー曇内のＦ　−ｔ　２の、ニー！：、濁培
養内にもりされ、更に３１０Ｌ−１の明ｉｊＪ生長させ
られる。スピンナーｌ云内の、犬長ずた件は再び線惟芽
細胞よりも款骨絢゛個を好んで支持する。スピンナー９
からの細胞は遠心分に９により採取され、直」灰に映用
されるか、または、液体膝紫中にて長明間冷凍保存され
る（２０％胎児子牛血清、１０％ジメチルスルフォキサ
イド（ＩＩＭＳＯ）及び７ｏ％ｒ−１２）。
出つ：り上った軟骨ゴ硼ｉＩ凪のペレットは、４１・色
素、原（ｂ　Ｏｘ７ｇ７　ｍｅ　’）または回前なる他
の血１管凝精蛋白及ヒｌ−！Ｊプシン抑制剤（５０μ／
ｍ（＋シグマ４９．　！ＩＩ　）式たは他のアンチグロ
テアーゼを含む小容遺の燐酸塩緩衝塩水中に再懸濁され
る。
細胞（培養の期間と大きさとに従って涙度が１−５Ｘ１
０５細胞／ｍｌの［ｔ、１に渉っている）と、線維素片
とトリプシン抑制剤とを含んでいる最後の溶液を溶液Ａ
とする。ミクロテスト板（蓋付き９５ＦＮｕｎｃデンマ
二り）才たは、炙岐井戸に、トロンビン溶液（溶液Ｂ）
　（４０ｍＭｃａｃ１２のＩ　Ｕ　／　３０　ｍｅ　）
の３０マイクロリツトルを各井戸へ加え（底及び側壁上
に平均に分布して）、それから溶液Ａを９０マイクロリ
ットル加え、そして混合物を固化（ゲル化）させる。
需要に従って、最後の母は貧化出来、かくて、溶液Ａと
Ｂの比を３：１（ｖ／ｖ）に保つ。Ｆ−１２の０．２　
ｔｒ＝ｅで蔽ったゲル（Ｊそれから或期間（約１４日）
、５−１０％Ｃ０２を空気に入れて平衡化した培養器中
か、または冷凍保存されて、保たれることができる。
移植では、損傷した場所をトロンビン溶液で繊細に噴霧
し、他方、移植に先立つゲルは線維素片とトリプシン抑

μｇ／　ｇに）溶液中に浸漬し、そし・て試料を損傷し
た場所に圧入し欠陥を滑らかに読す。
萌芽性軟骨細胞の代りに、萌芽性軟骨細胞（段階２４）
また：ま青嵐・痒辿胞を使ってもよい。
加うるに、トラップされた非差異化間葉繊細胞をつけた
１１ｉｊ何なる成人運結狙゛雁を使用してもよく、培、
連中に細胞を発生し、それが最炭に自己差Ａ化により軟
骨細胞に変形され、または軟骨ｊｌｉ　ｌ泡ファクター
により指向される。
先天性移毛に対しては、受１四トドの欣人徂織内にトラ
ップされた同質江聞葉獄型１．！■胞で、皮膚、１．λ
堆芽′酊珂包、生倹により得られる骨髄、’：’ｉｌｌ
胞の叩きを・吏ってもよ（、これらは、培養中をこ細、
泡を生じ、それらが軟骨？ｉＩｌ胞に変セ１さｉする。
スピンナー壜（１４−２１日培；詭中）からの萌芽性ひ
よこ軟骨細胞を、２Ｘ１０５ａ胞／３５心皿のｍ期濃度
で板にし、フィブロネクチンを塗布した皿上にアスコル
ビン酸（５０μｆ／ｍｌ）で補足したＦ−１２媒質中に
維持した。培養らは６日以内に合流になり、それから！
柴３”ｔを更新し、また培養を更に追加の６日間培養し
た。三組の培養の組はトリプシン化され、Ｃａｎ１ｔｅ
ｒ計数器（モデルエ蟇Ｄ）により細胞計数を調べる為に
使用した。末９；１ｔでの細胞計数はｌＸ１０６ｉ：Ｑ
胞／板の周りの範囲であった。総ての他の板をそれから
燐酸緩箭塩水（ＰＢＳ　”ｌで洗浄し、２０分間蒸溜水
中で２０　ｍ　Ｍ　Ｎ　Ｈ４０Ｈに：：ｉ、ｒ　した。
そして３回ＰＢＳで再洗滌Ｉ７、溜水水で２　［ｉｉ　
；ニアと１゛、４洗滌し、細胞体質ないし核が、皿に倹
７′ＩデシたもとのままのＥＣＭと組合されて認められ
ることがないようにした。ＥＣＭはゴムポリスマンで（
：足長して洟δ霜真空乾燥した。乾燥粉末化ＥＣＭの収
ＡＸは０．３−１．０即／板の間の範囲であった。
ＥＣＭは５乃至２０巧毎リツトルのがで混ぜ合せた。最
良の結果は約１０πｉ’　／　ｆｎｅで得られた。
軟骨細胞ゲルを種々の坪（鳥類及び咄乳頬）内の移植と
して実験し、巨視的篤察、相１”；Ｉ’に学的断面及び
生化学試験により採集したデータは、移植の場所に於て
、二三ケ月以内に、欠陥は活性の増殖する軟骨細胞で正
しく満され（関節軟骨の表面を含め゛Ｃ，環境にシ：ｊ
歪されてう、特徴的な（表現型の）代謝性体質と古いし
・“−接軟骨組織と積胆った井戸（線維軟骨、才たは他
の軟かい組織かヘリに無い）とを表しでいた。

Claims (1)

  1. 【特許請求の範囲】 １、　移植によって関節の軟骨および骨の欠陥を修復す
    るためのものであって、それのＨ＝物は、その組成物が
    移植されるべき楢の萌芽性軟骨細胞または間葉繊細胞で
    、線維累原、トロンビン、−及び天然ないし化学的プロ
    テアーゼ防止剤の成る量とから成る生物学的膠による軟
    骨形成誘因因子により軟骨細胞に変換されうるものを含
    んでいるところの、組成物。 ２、　毎ミリリットルごとに、１０００００がら５００
    ０００個の軟骨細胞または間Ｍ繊細路を含んでいるとこ
    ろの、特許請求の範囲ｊＨＩ項記載の組成物。 ３．５乃至２０〜／ｌｎｌの細胞外基質を含んでいると
    ころの、特許請求の範囲第１項記載の組成物。 ４、局部的生長ないしホルモン生長因子剤を含んでいる
    ところの、特許請求の範囲第１項記載の組成物。 ５、局部的生長因子が軟骨起源生長因子（ＣＤＦ）また
    は骨起源生長因子（ＢＤＧＦ　）であるところの、特許
    請求の範囲第４項記載の組成物。 ６、　ホルモン性成長因子が血清または下垂体からのン
    マトメジンであるところの、特許請求の範囲第４項記載
    の組成物。 ７、細胞が軟骨細胞または未分化間葉織捕捉細飽を含む
    結ケ組織から導出された細胞、培養中に軟骨細胞になる
    ように増殖し変換されるよう誘導される細胞であるとこ
    ろの、特許請求の範囲第１項記載の組成物。 ８、　ゲルが、適当な形状にしｌた親水化された（キー
    ル骨）または人工的骨中に包埋されているところのζ特
    許請求の範囲第７項記載の組成物。 ９、　特許請求の範囲第１項記載の組成物を欠陥組織内
    に植付けることを含むところの、管端軟骨及び骨修復の
    操作。 １０、細胞外基ｉ　（ＦＣ！　＞または適当な局部的な
    いしホルモン性成長型因子で、特許請求の範囲第１項記
    載の細胞の少量を含むか、またはそうした細胞を欠いて
    いるものと共に、トロンビン、アンチプロテアーゼ及び
    線維素片とから成る生物学的ゲルで含浸された適当に形
    状づけられた骨構造を含むところの、軟骨ないし骨修復
    用の移植。
