Claims (1)
- 【特許請求の範囲】 1.ヒト大腿骨の頸部(大腿骨頸)(5)の頭部(大腿骨頭)(8)を大腿骨 幹(9)に対し所定の角度(αc)およびその頭部(8)の基端との関係で所定 の高さ(Lc)に正確に切除するのに用いられる切断ガイド器具であって: 切除の間、大腿骨に固定される取付け部材(14)を含む支持部(11)と; 器具使用時において前記支持部(11)により支持され、上記所定の切断角度 (αc)で大腿骨頸(5)に沿って延出する長尺カイドキャリア(12)と; 該ガイドキャリア(12)により支持され、かつ該ガイドキャリア(12)の 軸(D−D)に沿って所定切断高さ(Lc)に相当する切断位置まで移動可能な 切断ガイド(13)と; を具備してなる切断ガイド器具。 2.前記ガイドキャリア(12)が、前記支持部(11)により枢支され、大 腿骨の対称面(M)を横切る方向に延びる枢軸(C−C)の周りを角度的に移動 可能となっていて、これにより大腿骨幹(9)に対する上記切断角度(αc)を 調整し得るようになっている請求項1記載の切断ガイド器具。 3.角度照合部材(40)を更に有し、該角度照合部材(40)に、前記ガイ ドキャリア(12)の枢軸(C−C)に対し横切る方向に延びた比較的長尺の整 合部(44)が設けられ、さらに該角度照合部材(40)は上記ガイドキャリア (12)の枢軸(C−C)の周りに回動可能に前記支持部(11)により枢着さ れていて、整合部(44)が大腿骨幹(9)と平行になる角度をとることができ 、切断ガイド(13)が切断位置にあるとき前記ガイドキャリア(12)と整合 部(44)との角度が上記切断角度(αc)と一致するようになっている請求項 2記載の切断ガイド器具。 4.前記ガイドキャリア(12)および角度照合部材(40)がそれぞれ別に 前記ガイドキャリア(12)の枢軸(C−C)の周りに回動可能に枢着されてい て、これらに上記ガイドキャリア(12)と整合部(44)との角度の値を視覚 的に認識可能とする協同角度指示手段(41、42、46)が設けられている請 求項3記載の切断ガイド器具。 5.前記枢軸(C−C)の周りの前記ガイドキャリア(12)の回動をロック するための切断角度ロック手段(30、31、32)が、さらに設けられている 請求項2ないし4のいずれか一項に記載の切断ガイド器具。 6.前記ガイドキャリア(12)が前記支持部(11)を横ぎり、前記枢軸( C−C)と平行な方向に移動自在となっていて、これにより切断ガイド(13) と大腿骨の頸部(5)との間隔を調整し得るようになっている請求項1ないし5 のいずれか一項に記載の切断ガイド器具。 7.切断ガイドロック手段(57)をさらに具備し、これにより切断ガイド( 13)の前記ガイドキャリア(12)の軸に沿う移動をロックするようにした請 求項1ないし6のいずれか一項に記載の切断ガイド器具。 8.切断ガイド(13)が前記ガイドキャリア(12)の軸(D−D)を中心 として回動自在になっている請求項1ないし7のいずれか一項に記載の切断ガイ ド器具。 9.切断ガイド(13)が、その切断位置において、除去されるべき大腿骨の 一部に固定されるようになっている請求項1ないし8のいずれか一項に記載の切 断ガイド器具。 10.切断ガイド(13)に、釘状物(58)などの固定手段を受入れるため の1以上のチャンネル(51)が設けられ、これにより切断ガイド(13)を固 定し得るようになっている請求項9に記載の切断ガイド器具。 11.支持部(11)の取付け部材(14)が釘状物(14)をなし、これに より大腿骨に一時的に固定されるようになっている請求項1ないし10のいずれ か一項に記載の切断ガイド器具。 12.釘状物(14)が大腿骨頸の上部(転子窩)に、大腿骨幹(9)に向か う方向に導入されるようになっていて、この釘状物(14)の長さが大腿骨幹( 9)に案内されつつその内部に到達し得る長さとなっている請求項11に記載の 切断ガイド器具。 13.支持部(11)がさらにカンチレバー(22)を有し、これがその第1 端部(25)において取付け部材(14)に着脱自在に接続され、第2端部(2 6)において前記ガイドキャリア(12)を支持している請求項1ないし12の いずれか一項に記載の切断ガイド器具。 14.支持部(11)がさらに例えばロッド状の頸部整合部材(19)を有し 、これが大腿骨頸の対称面(M)と平行に整合されるようになっていて、前記ガ イドキャリア(12)を前記対称面(M)と平行な方向に配向し得るようにした 請求項1ないし13のいずれか一項に記載の切断ガイド器具。 15.前記ガイドキャリア(12)にさらに接合アーム(53)が設けられ、 該接合アーム(53)がガイドキャリア(12)からそれを横切るようにして延 び、かつ頭部(8)の基端と当接するようになっていて、前記ガイドキャリア( 12)および切断ガイド(13)が、ガイドキャリア(12)の軸(D−D)に 沿う接合アームと切断ガイド(13)との間隔を視覚的に認識させるための共に 動く位置指示手段(54、55、56)を具備している請求項1ないし14のい ずれか一項に記載の切断ガイド器具。 16.前記ガイドキャリア(12)が支持部(11)との関係においてその長 手方向に移動自在となっていて、これにより前記接合アーム(53)を頭部(8 )の基端と当接するようにした請求項15に記載の切断ガイド器具。 17.切断ガイド(13)に、切断刃あるいはその類似物を受入れ、案内する ための切断スリット(50)が設けられている請求項1ないし16のいずれか一 項に記載の切断ガイド器具。 18.ヒト大腿骨の頸部(大腿骨頸)(5)の頭部(大腿骨頭)(8)を大腿 骨幹(9)に対し所定の角度(αc)およびその頭部(8)の基端との関係で所 定の高さ(Lc)に正確に切除するための方法であって; 大腿骨幹(9)に対し前記所定切断角度(αc)で大腿骨頸(5)に沿って長 尺ガイドキャリア(12)を整合させる工程と、 前記ガイドキャリア(12)の軸(D−D)により支持され、該軸に沿って移 動可能な切断ガイド(13)を設ける工程と、 該切断ガイド(13)を前記ガイドキャリア(12)に沿って前記所定切断高 さ(Lc)に相当する切断位置まで移動させる工程と、 からなることを特徴とする切除方法。 19.前記ガイドキャリア(12)を支持する支持部(11)を設け、該支持 部(11)を前記切除の間において大腿骨に固定する工程をさらに含む請求項1 8に記載の切除方法。 20.前記ガイドキャリア(12)が、前記支持部(11)により枢支され、 大腿骨の対称面(M)を横切る方向に延びる枢軸(C−C)の周りを角度的に移 動可能となっていて、大腿骨に前記支持部(11)を固定する工程の後、ガイド キャリア(12)を該枢軸(C−C)を中心として回動させて大腿骨幹(9)に 対する所定切断角度(αc)に調整する工程をさらに含む請求項19に記載の切 除方法。 21.前記のガイドキャリア(12)を該枢軸(C−C)を中心として回動さ せて所定切断角度(αc)に調整する工程が、角度照合部材として該枢軸(C− C)を中心として回動自在に前記支持部(11)に枢着された比較的長尺の整合 部(44)を大腿骨幹(9)と平行になるように整合させる工程を含み、切断ガ イド(13)が切断位置にあるとき前記ガイドキャリア(12)と整合部(44 )との角度が前記切断角度(αc)と一致するようにした請求項20に記載の切 除方法。 22.前記の比較的長尺の整合部(44)を大腿骨幹(9)と平行になるよう に整合させる工程が、該整合部(44)をを回動させ、その先端部を対応する膝 関節、好ましくは膝蓋と反対側に位置させる工程を含む請求項21に記載の切除 方法。 23.ガイドキャリア(12)を前記切断角度(αc)に調整する工程の後、 ガイドキャリア(12)の前記枢軸(C−C)の周りでの回動をロックする工程 をさらに含む請求項20ないし22のいずれか一項に記載の切除方法。 24.切断ガイド(13)を前記切断位置に移動させる工程の後、切断ガイド (13)をガイドキャリア(12)との関連でロックする工程をさらに含む請求 項18ないし23のいずれか一項に記載の切除方法。 25.切断ガイド(13)を前記切断位置に移動させる工程の後で前記切除の 工程の前に、切断ガイド(13)を切除されるべき大腿骨の部分に固定する工程 をさらに含む請求項18ないし24のいずれか一項に記載の切除方法。 26.支持部(11)を固定する工程が、釘状物(14)を大腿骨に一時的に 固定する工程を含む請求項18ないし25のいずれか一項に記載の切除方法。 27.該釘状物(14)を大腿骨頸の上部(転子窩)に、大腿骨幹(9)に向 かう方向に導入する請求項26に記載の切除方法。 28.該釘状物(14)の長さを大腿骨幹(9)に案内されつつその内部に到 達し得る十分な長さに選択する請求項26または27に記載の切除方法。 29.ガイドキャリア(12)を大腿骨頸(5)に沿って整合させる工程が、 ガイドキャリア(12)を大腿骨頸(5)の対称面(M)と平行になるように整 合させる工程を含む請求項18ないし28のいずれか一項に記載の方法。 30.ヒト大腿骨の頸部(大腿骨頸)の頭部(大腿骨頭)(8)を正確に切除 する場合に用いられる上記請求項1ないし17のいずれか一項に記載の切断ガイ ド器具の使用方法であって; 支持部(11)を取付け部材(14)を用いて大腿骨に固定し; カイドキャリア(12)を大腿骨幹(9)に対し所定切断角度(αc)で大腿 骨頸(5)に沿って整合させ; 切断ガイド(13)をキャリア(12)の軸に沿って所定切断高さ(Lc)に 相当する切断位置まで移動させ; この所定切断角度(αc)および所定切断高さ(Lc)にて、切断ガイド(13 )に案内されながら切断器具を用いて該頭部(8)を切除する; ことからなる切断ガイド器具の使用方法。
