Description
Claims (1)
- 【特許請求の範囲】 1. それぞれ独立した複数個の縫合糸サポートセグメントと複数個の縫合糸と の組み合わせよりなり、 各縫合糸サポートセグメントは、生物学的に融合性があるとともに不活性の材 質から作られているとともに、上面と、下面と、両側部と、基端部と、先端部と を備え、かつ、上面から下面に延在する少なくとも縫合糸孔を備え、 縫合糸孔は、互いに所定距離(d)だけ離れており、 各縫合糸サポートセグメントは、その周囲が小さくされるべき解剖学的構造体 の組織長(d')に対して1つの縫合糸で個々的に縫合され、その縫合は、縫合 されるところの組織長(d')が上記距離(d)よりも大きくなるように水平マ ットレス(U字状)縫合とされ、 各縫合糸が締め上げられて結ばれたとき、各縫合されたサポートセグメントに より、解剖学的構造体に重なりが生じ、〈d'−d〉に対応する量だけ解剖学的 構造体の周囲が小さくなり、 これにより、解剖学的構造体が正常形態に永久復元されることを特徴とする、 解剖学的構造体を縮小するための器具。 2. 上部分と下部分とを有する脈管構造体の上部周囲及び下部周囲がその上部 分と下部分とで決定される、その脈管構造体の周囲を縮小するための方法であっ て、 以下のa),b),c),d),e)のステップを含み、 a)のステップにおいては、生物学的に融合性がありかつ不活性の材質から構 成した互いに独立した複数個の縫合糸サポートセグメントが準備され、それぞれ 独立した各サポートセグメントは、さらに、上面と、下面と、両側部と、基端部 と、先端部とを有するとともに、上面から下面に向かって延在するとともに所定 距離(d)だけ離れた少なくとも2つの縫合糸孔を有し、 b)のステップにおいては、第1端と第2端とその一端に取り付けられる少な くとも1つの手術用針とを有する複数の縫合糸が準備され、 c)のステップにおいては、その周囲を縮小すべきところの脈管構造体に対し て、各縫合糸サポートセグメントが複数個の縫合糸のうちの1つを用いて個々的 に縫合され、その縫合は、縫合されるべき組織の上記距離(d')が上記距離( d)より大きくなるように、脈管構造体の上記縫合糸の導入ポイントと出口ポイ ント間の脈管構造体の組織長に沿って行われる水平マットレス(U字状)縫合で あり、 d)のステップにおいては、マットレス縫合糸が締め上げられ、 e)のステップにおいては、上記縫合糸サポートセグメントの上面にかつ2つ の上記縫合糸孔間に縫合糸の結び目が形成されるように縫合糸が結ばれ、それに より、各縫合された各縫合糸サポートセグメントによって、脈管構造体内に重な りが形成され、それにより、〈d'−d〉に対応する量だけ脈管構造体の周囲が 縮小することを特徴とする方法。 3. さらに、e),f)のステップを有し、 e)のステップにおいては、脈管構造体に対して縫合した上記縫合糸サポート セグメントを被覆するに十分な所定の寸法を有する、生物学的に融合性がありか つ不活性の安定化材が準備され、この安定化材は上面と下面とを有しており、 f)のステップにおいては、安定化材が複数の縫合糸サポートセグメント及び 脈管構造体の上に固定され、 それにより、縫合された縫合糸サポートセグメント間の間隔が安定化し、かつ 、縫合された縫合糸サポートセグメントが血液に対して直接露出されることが防 止されることを特徴とする請求項2記載の方法。 4. 上記安定化材は、縫合糸を締め上げた後でかつ結ぶ前に、縫合糸サポート セグメントと脈管構造体との上で縫合することにより、安定化材を複数個の縫合 糸サポートセグメントと脈管構造体の上に固定することを特徴とする請求項3記 載の方法。 5. 上記マットレス縫合は以下の、a),b),c),d),e),f),g )の各ステップを含み、 a)のステップにおいては、縫合糸が、安定化材に対して、その上面の導入ポ イントからその下面に通され、 b)のステップにおいては、縫合糸が、縫合糸サポートセグメントの縫合糸孔 に通され、 c)のステップにおいては、脈管構造体が上記導入ポイントと上記出口ポイン ト間の組織長において縫合され、 d)のステップにおいては、縫合糸が、上記縫合糸サポートセグメントの第2 縫合糸孔に通され、 e)のステップにおいては、縫合糸が、上記安定化材に対して、その下面から その上面の出口ポイントに通され、 f)のステップにおいては、上記マットレス縫合糸が締め上げられ、 g)のステップにおいては、縫合糸の結び目が安定化材の上面に着座すべく縫 合されるように、縫合糸が結ばれることを特徴とする請求項3記載の方法。 6. 少なくとも1つの縫合糸サポートセグメントが脈管構造体の下部に縫合さ れ、脈管構造体の下部周囲が縮小されることを特徴とする請求項2記載の方法。 7. 複数の上記縫合糸サポートセグメントが上記脈管構造体の上部周囲及び下 部周囲に縫合され、脈管構造体の周囲が全体的に縮小されることを特徴とする請 求項2記載の方法。 8. 上記各縫合糸サポートセグメントは、所定距離互いに離れた2つの縫合糸 孔を有し、1つの孔がサポートセグメントの基端部の近傍にあり、他の1つの孔 が縫合糸サポートセグメントの先端部の近傍にあることを特徴とする請求項2記 載の方法。 9. 上記各縫合糸サポートセグメントの縫合糸孔は、滑らかに形成されている とともに、合着針と2-0縫合糸を挿入できる直径を有することを特徴とする請求 項2記載の方法。 10. 上記安定化材は織物であることを特徴とする請求項3記載の方法。 11. 上記布地はダクロンであることを特徴とする請求項10記載の方法。 12. 上記安定化材は層状組織であることを特徴とする請求項3記載の方法。 13. 上記組織は、グルタールアルデヒドのようなタンニン剤で処理されてい るか又は未処理の、あるいは、低温保存されているか、あるいは、これと実質的 に同様の処理がなされた自家移植組織、相同器官組織、あるいは異質組織の心膜 であることを特徴とする請求項12記載の方法。 14. 上記各縫合糸サポートセグメントは、長さが約5mmであり、幅寸法が 2〜4mmであることを特徴とする請求項2記載の方法。 15. 上記各縫合糸サポートセグメントは少なくとも約1mmの厚みを有する ことを特徴とする請求項14記載の方法。 16. 上記各縫合糸サポートセグメントはへら状であることを特徴とする請求 項2記載の方法。 17. 上記脈管構造体は心臓弁輪であることを特徴とする請求項2記載の方法 。 18. 上記脈管構造体は僧帽弁であることを特徴とする請求項2記載の方法。 19. 上記脈管構造体は三尖弁であることを特徴とする請求項2記載の方法。 20. 上記脈管構造体は大動脈弁であることを特徴とする請求項2記載の方法 。 21. 上記脈管構造体は肺動脈弁であることを特徴とする請求項2記載の方法 。 22. 解剖学的構造体の周囲を縮小する方法であって、以下のステップa), b),c),d)を有し、 a)のステップにおいては、生物学的に融合性がありかつ不活性の材質よりな り、かつ、それぞれ独立した複数個の縫合糸サポートセグメントが準備され、各 縫合糸サポートセグメントは、さらに、上面と、下面と、両側部と、基端部と、 先端部とを有するとともに、上面から下面に延在してかつ互いに所定距離(d) だけ離れた少なくとも2つの縫合糸孔を有し、 b)のステップにおいては、複数の縫合糸が準備され、 c)のステップにおいては、上記複数の縫合糸の1つを用いて、その周囲が縮 小されるべき解剖学的構造体に対して、各縫合糸サポートセグメントが個々的に 縫合され、実施されるその縫合は、縫合されるべき距離(d')が上記距離(d )より大きくなるように,縫合の導入と導出により決定される解剖学的構造体の 組織長さに沿って行われる水平マットレス(U字状)縫合であり、 d)のステップにおいては、縫合糸の結び目が上記縫合糸サポートセグメント の上面でかつ2つの縫合糸孔間に着座するように、縫合糸が締め上げられて結ば れ、それにより、縫合された各サポートセグメントにより解剖学的構造体の内部 に重なりが生じて、〈d'−d〉に対応する量だけ解剖学的構造体の周囲が縮小 することを特徴とする方法。 23. それぞれ独立した複数個の縫合糸サポートセグメントと複数個の縫合糸 との組み合わせよりなり、 各縫合糸サポートセグメントは、生物学的に融合性があるとともに不活性の材 質から作られているとともに、上面と、下面と、両側部と、基端部と、先端部と を備え、かつ、上面から下面に延在する少なくとも縫合糸孔を備え、 縫合糸孔は、互いに所定距離(d)だけ離れており、 各縫合糸サポートセグメントは、その周囲が縮小されるべき脈管構造体の組織 長さ(d')に対して1つの縫合糸で個々的に縫合され、その縫合は、縫合され るところの組織の距離(d')が上記距離(d)よりも大きくなるように水平マ ットレス(U字状)縫合とされ、 各縫合糸が締め上げられて結ばれたとき、各縫合されたサポートセグメントに より、脈管構造体に重なりが生じ、〈d'−d〉に対応する量だけ脈管構造体の 周囲が小さくなり、 これにより、脈管構造体が正常形態に永久復元されることを特徴とする、脈管 構造体を縮小するための器具。 24. 上記各縫合糸サポートセグメントは、所定距離互いに離れた2つの縫合 糸孔を有し、1つの孔がサポートセグメントの基端部の近傍にあり、他の1つの 孔が縫合糸サポートセグメントの先端部の近傍にあることを特徴とする請求項2 3記載の器具。 25. 上記各縫合糸サポートセグメントの縫合糸孔は、滑らかに形成されてい るとともに、合着針と2-0縫合糸を挿入できる直径を有することを特徴とする請 求項23記載の器具。 26. 上記各縫合糸サポートセグメントは、その長さが約5mmであって、そ の幅寸法が約2〜4mmであることを特徴とする請求項23記載の器具。 27. 上記各縫合糸サポートセグメントは少なくとも約1mmの厚み寸法を有 することを特徴とする請求項26記載の器具。 28. 上記各縫合糸サポートセグメントはへら状であることを特徴とする請求 項23記載の器具。 29. 上記脈管構造体は心臓弁輪であることを特徴とする請求項23記載の器 具。 30. 上記脈管構造体は僧帽弁であることを特徴とする請求項23記載の器具 。 31. 上記脈管構造体は三尖弁であることを特徴とする請求項23記載の器具 。 32. 上記脈管構造体は大動脈弁であることを特徴とする請求項23記載の器 具。 33. 上記脈管構造体は肺動脈弁であることを特徴とする請求項23記載の器 具。
