Claims (1)
- 【特許請求の範囲】 1. 少なくとも1つのロッド(1)と、このロッドに相互連結される骨係留素 子(3、15)とを有し、変形した椎骨に特に適用できる脊椎接骨装置において 、 処置すべき椎骨(L2)に上記ロッド(1)を連結するための少なくとも1つ の連結素子(4)と、この連結素子を上記椎骨に固定するための固定手段とを備 え、この固定手段が、自由な回転運動及び並進運動を許容した状態で当該ロッド を装着できるような寸法を有するリング(8)と、このリングを該ロッドに固定 するための手段(14)と、上記リングから半径方向に延びたアーム(9)とを 有し、上記アーム及び当該リングが一部品ユニットを構成することを特徴とする 脊椎接骨装置。 2. 請求の範囲第1項に記載の装置において、処置すべき椎骨(L2)に固定 するための上記固定手段が上記連結素子(4)の上記アーム(9)を収容するた めのU字状本体(5)を備えた骨係留素子(3)を有することを特徴とする装置 。 3. 請求の範囲第2項ないし第4項のいずれかに記載の装置において、上記骨 係留素子(3)の上記本体が、上記アーム(9)を導入するための横方向開口を 画定する不等長の横方向脚部と、当該アームを固定するための中央の固定素子と を有することを特徴とする装置。 4. 請求の範囲第2項に記載の装置において、上記骨係留素子(3)の上記本 体(5)が、脚部(5A)を貫通する横方向のネジ(19)と、当該U字状本体 (5)内に挿入可能で、上記アーム(9)を収容できる円筒状の固定素子(17 )とを有し、この固定素子が、上記横方向のネジ(19)間に位置し、当該横方 向のネジと一緒に当該アームを該本体(5)に固定できる中央のネジ(18)を 備えたことを特徴とする装置。 5. 請求の範囲第4項に記載の装置において、上記固定素子(17)が、上記 横方向のネジ(19)を通過させる横方向の開口(22)と、上記脚部(5A) の端部間を摺動できるような寸法を有し、上記中央のネジ(18)を通過させる 開口を具備した半径方向のボス(23)とを備え、当該固定素子が上記U字状本 体の入口縁に沿って対応する支持表面(25)に当接するようになった肩部(2 4)を有することを特徴とする装置。 6. 請求の範囲第1項ないし第5項のいずれかに記載の装置において、上記固 定手段が、上記リング(8)の半径方向開口(13)を貫通し上記ロッド(1) に当接する少なくとも1つのネジ(14)から成ることを特徴とする装置。 7. 請求の範囲第1項ないし第6項のいずれかに記載の装置において、上記ア ーム(9)が、円筒状を呈し、上記骨係留ネジ(3)の上記本体(5)に設けた 通路(10)を貫通するようになっていることを特徴とする装置。 8. 請求の範囲第1項に記載の装置において、上記リング(8)が開放してい て溝穴(20)を画定しており、上記固定手段が、当該リングを半径方向に貫通 し上記溝穴(20)の両側で上記ロッドに当接できるネジ(14)により構成さ れていることを特徴とする装置。 9. 請求の範囲第1項ないし第8項のいずれかに記載の装置において、上記ア ーム(9)及び上記ロッド(1)の表面が粗面仕上げ(11、2)されているこ とを特徴とする装置。 10. 請求の範囲第1項に記載の装置において、処置すべき椎骨に固定するた めの上記固定手段が、上記連結素子の上記アーム(9)を収容するためのU字状 本体を有する骨係留フックから成ることを特徴とする装置。 11. 請求の範囲第1項ないし第9項のいずれかに記載の装置において、処置 すべき椎骨に固定するための上記固定手段が、上記連結素子(4)の上記アーム (9)を収容するための閉じた円筒状本体をそれぞれ具備したフック又はネジか ら成ることを特徴とする装置。 12. 請求の範囲第1項ないし第5項のいずれかに記載の装置において、上記 リング(8)がその外周に配置された一連の開口(13)を具備し、これらの開 口のうちの少なくとも1つがクランプネジ(14)を挿入するために外科医が接 近できるようになっていることを特徴とする装置。 13. 請求の範囲第2項に記載の装置において、上記係留素子の上記U字状本 体(5)が上記連結素子(4)の挿入後に上記係留素子(3)に固定されるネジ プラグにより閉じられ、このネジプラグが当該連結素子に半径方向のクランプ力 を作用させることを特徴とする装置。 14. 請求の範囲第1項ないし第9項のいずれかに記載の脊椎接骨装置に使用 する連結素子(4)であって、処置すべき椎骨(L2)に連結素子を固定するた めの固定手段(3)に脊椎接骨ロッド(1)を連結するための連結素子(4)に おいて、 リングを脊椎接骨ロッド(1)に固定するための手段(14)を具備した当該 リング(8)を一端に担持したアーム(9)から成り、上記アームが上記固定手 段と共働することを特徴とする連結素子。 15. 脊椎の変形を矯正するための脊椎固定装置において、 脊柱の長手方向に沿って複数の椎骨間を延びるような寸法を有する長手方向部 材と; 上記複数の椎骨の対応する1つにそれぞれが係合する複数個の骨係留素子と; 少なくとも1つの上記骨係留素子と上記長手方向部材との間に位置した横方向 の連結素子と; から成り、上記連結素子が、 本体であって、上記長手方向部材から離脱することなくこの本体を当該長手方 向部材のまわりで自由に回転させ同長手方向部材に沿って並進運動させるように 、この本体を該長手方向部材に摺動可能に係合させる手段を備えた本体と; 上記本体と一体で、この本体を上記長手方向部材に係合させたときに当該長手 方向部材から離れる方向へ当該本体から外方へ延びるアームと; 上記複数個の骨係留素子の各々に関連し、当該骨係留素子を上記長手方向部材 又は上記横方向の連結素子の上記アームに固定するための固定手段と; を有することを特徴とする脊椎固定装置。 16. 請求の範囲第15項に記載の脊椎固定装置において、上記長手方向部材 が細長いロッドであり;上記横方向の連結素子の上記本体が、上記細長いロッド を摺動可能に貫通収容するような寸法の開口を有することを特徴とする脊椎固定 装置。 17. 請求の範囲第16項に記載の脊椎固定装置において、上記横方向の連結 素子の上記本体が上記ロッドを取り巻くリングであることを特徴とする脊椎固定 装置。 18. 請求の範囲第15項に記載の脊椎固定装置において、上記少なくとも1 つの上記骨係留素子が上記横方向の連結素子の上記アームを収容するためのU字 状本体を有し;少なくとも1つの上記骨係留素子に関連する固定手段が、上記係 留素子の上記U字状本体に挿入可能な固定素子を備え、この固定素子が、この固 定素子を当該U字状本体に挿入したときに上記アームを貫通収容する開口を有す ることを特徴とする脊椎固定装置。 19. 脊椎の変形を矯正する方法において、 患者の変形した脊椎の長さの一部に沿って複数の椎骨間を延びる長手方向の部 材を移植する工程と; 各椎骨に少なくとも1つの骨係留素子を係合させる工程と; 上記各骨係留素子を上記長手方向の部材に連結する連結工程と; 患者の体内において上記長手方向の部材に連結した椎骨で当該長手方向の部材 を屈曲させ脊椎の変形を矯正するように椎骨を操作する工程と; から成ることを特徴とする矯正方法。 20. 請求の範囲第19項に記載の脊椎変形矯正方法において、上記連結工程 が少なくとも1つの上記係留素子を上記長手方向の部材から横方向に偏らせる工 程を有することを特徴とする矯正方法。 21. 請求の範囲第19項に記載の脊椎変形矯正方法において、上記連結工程 が、上記素子と上記長手方向の部材との間の連結を維持した状態で、当該素子と 当該長手方向の部材との間の相対回転及び相対並進を許容するように該各素子を 連結する工程を有することを特徴とする矯正方法。 22. 請求の範囲第19項に記載の脊椎変形矯正方法において、上記長手方向 の部材が強靭で柔軟性のあるロッドであることを特徴とする矯正方法。 23. 脊椎の変形を矯正する方法において、 患者の変形した脊椎の長さの一部に沿って複数の椎骨間を延びる細長いロッド を移植する工程と; 各椎骨に少なくとも1つの骨係留素子を係合させる工程と; 多数の上記骨係留素子を上記細長いロッドに連結する工程と; 患者の体内において上記細長いロッドに連結した椎骨で当該細長いロッドを屈 曲させ脊椎の変形を矯正するように椎骨を操作する工程と; から成ることを特徴とする矯正方法。 24. 脊椎の変形を矯正するための脊椎固定装置において、 脊柱の長さの一部に沿って複数の椎骨間を延びるような寸法を有する細長いロ ッドと; 本体部分と、この本体部分を上記細長いロッドに装着するための手段と、複数 の椎骨のうちの1つの椎骨に係合するような形状の部分とを有する第1の骨係留 素子と; 貫通開口を備えた本体と、この本体と一体で同本体から外方に延びるアームと をそれぞれ有する第1及び第2の連結素子と; 第2の本体部分と、この第2本体部分を上記第1連結素子の上記アームに装着 するための手段と、複数の椎骨のうちの上記1つの椎骨に係合するような形状の 第2部分とを有する第2の骨係留素子と; から成り、 上記第2連結素子が、上記細長いロッドを上記第2連結素子の上記本体の上記 貫通開口を貫通させた状態で、当該細長いロッドに係合しており; 上記第1連結素子が、上記第2連結素子の上記アームを上記第1連結素子の上 記本体の上記貫通開口を貫通させた状態で、当該第2連結素子に係合しているこ とを特徴とする脊椎固定装置。
