Description
本出願は、2006年2月6日に「経皮的椎弓根スクリュー延長部においてロッドを賦形する装置および方法」の表題で出願された米国特許仮出願第60/765,606号の出願日の利得を主張し、この内容は、参照することによって、ここに含まれるものとする。
本発明は、最小侵襲手術における経皮的アクセスを改良する方法および装置に関し、さらに詳細には、体内の目印の付いた箇所に基づいて接続装置を体外で選択および賦形するためのテンプレートを設け、接続装置および器具、特に、このように選択または賦形された装置を1つまたは複数のアクセス通路を通して体内の特定箇所によって画定された位置に経皮的に送達する方法および装置に関する。
ガイドワイヤを経皮的に骨内、例えば、図1に示されるような隣接骨内に設置するステップと、
図2〜図5に示されるように、先端が丸い拡張器およびカニューレを用いて、切開および空洞を拡げるステップと、
図6に示されるように、開創ブレードが取り付けられる多軸スクリューを経皮的に設置するステップと、
図7および図7Aに示されるように、カニューレおよびガイドワイヤを取り外すステップと、
当接部材を設置し、スロット付きカニューレ組立体を形成するステップと、
図8、代替的に、図17に示されるように、溝シミュレーション部材または溝シミュレーションロッドをカニューレ組立体および/または多軸スクリューヘッドに設置し、賦形組立体を形成するステップと、
図9、代替的に、図17に示されるように、賦形組立体を平行関係に配列し、リンクを組立体に設置するステップと、
図10に示されるように溝シミュレーション部材と嵌合するように、または代替的に、図18に示される溝シミュレーションロッドと嵌合するように、固定部材を賦形するステップと、
図11に示されるように、賦形された固定部材を経皮的に取り付けるステップと、
図12、図13および図19に示されるように、固定部材を多軸スクリューに固定するステップと、
図14および図20に示されるように、細長部材を取り外し、その後、外科手術を完了するステップと、
を行うとよい。
以下の列挙された項が、本発明の種々の実施形態を代表している。
1. 1つまたは複数の移植片を賦形または選択し、前記移植片を患者の体内に経皮的に移植する方法において、
第1の延長材の遠位部を、前記1つまたは複数の移植片が通過することになる前記体内の第1の解剖学点の近傍に位置決めするステップと、
第2の延長材の遠位部を、前記1つまたは複数の移植片が通過することになる前記体内の第2の解剖学点の近傍に位置決めするステップと、
体外の第1の突出点および第2の突出点への前記第1の解剖学点および前記第2の解剖学点の空間的変換をもたらすために、前記第2の延長材に対する前記第1の延長材の配向を拘束するステップと、
前記第1の突出点および前記第2の突出点の位置に基づいて、前記1つまたは複数の移植片を賦形または選択するステップと、
第1のカニューレの遠位端を前記体内の前記第1の解剖学点の近傍に位置決めするステップであって、前記カニューレは、近位端および軸を有すると共に、側壁に第1のスロットおよび第2のスロットを有する、ステップと、
前記1つまたは複数の賦形または選択された移植片を、皮膚下において、前記第1のカニューレの中心軸と直交する方向に沿って、前記第1のカニューレの前記スロット内を通過させるステップと、
を含むことを特徴とする、方法。
2. 前記第1の解剖学点は、前記脊椎の第1の椎弓根の近傍に位置し、前記第2の解剖学点は、前記脊椎の第2の椎弓根の近傍に位置し、前記1つまたは複数の移植片を前記脊椎の近傍に移植することをさらに含むことを特徴とする、パラグラフ1に記載の方法。
3. 前記1つまたは複数の移植片は、後方脊椎融合システム用のロッドから構成され、前記1つまたは複数の移植片を前記脊椎の近傍に移植する前記ステップは、前記ロッドを前記脊椎の複数の椎骨に固定し、前記椎骨の相対的な運動を拘束することを含むことを特徴とする、パラグラフ2に記載の方法。
4. 前記第1の延長材の遠位部を前記第1の解剖学点の近傍に位置決めする前記ステップは、前記第1の延長材の前記遠位部を前記第1の椎弓根に移植可能な第1の固定部材のロッド接触面と嵌合させることを含み、前記第2の延長材の遠位部を前記第2の解剖学点の近傍に位置決めする前記ステップは、前記第2の延長材の前記遠位部を前記第2の椎弓根に移植可能な第2の固定部材のロッド接触面と嵌合させることを含むことを特徴とする、パラグラフ3に記載の方法。
5. 前記第1の突出点および前記第2の突出点の位置に基づいて前記1つまたは複数の移植片を賦形または選択する前記ステップは、前記ロッドを前記第1の延長材および前記第2の延長材の近位部によって受けることを含むことを特徴とする、パラグラフ4に記載の方法。
6. 前記第2の延長材に対して前記第1の延長材の配向を拘束する前記ステップは、前記第1の延長材と前記第2の延長材との間の実質的に自由な相対的な平行運動を可能としながら、前記第1の延長材および前記第2の延長材を互いに平行に保持するように、第1のブリッジを前記第1の延長材および前記第2の延長材に連結することを含むことを特徴とする、パラグラフ5に記載の方法。
7. 第3の延長材の遠位端を、前記1つまたは複数の移植片が通過することになる前記体内の第3の解剖学点の近傍に配置するステップと、
体外の第1の突出点、第2の突出点、および第3の突出点への前記第1の解剖学点、前記第2の解剖学点、および第3の解剖学点の空間的な変換をもたらすように、前記第3の延長材に対して前記第2の延長材の配向を拘束するステップと、
をさらに含むことを特徴とする、パラグラフ6に記載の方法。
8. 前記ロッドは、前記第1の解剖学点の近傍の脊椎の第1の椎骨の椎弓根内に移植可能な椎弓根スクリューおよび前記椎弓根スクリューに対して多軸運動可能なケージによって前記脊椎に固定され、前記ケージは、前記1つまたは複数の移植片の少なくとも一部を受けるように構成されていることを特徴とする、パラグラフ3に記載の方法。
9. 前記第1のカニューレの前記遠位端は、前記第1のスロットおよび前記第2のスロットの各々が皮膚下長さの全体に沿って形を崩されずに延在し、前記1つまたは複数の移植片が前記スロットを通って前記椎弓根スクリューの前記ケージ内に入るように構成されていることを特徴とする、パラグラフ8に記載の方法。
10. 前記第1のカニューレの前記遠位端は、連結要素を有し、前記連結要素を用いて、前記椎弓根スクリューに固定されることを特徴とする、パラグラフ8に記載の方法。
11. 前記第1のカニューレは、
第1のブレードと、
前記第1のブレードから離れた第2のブレードと、
を備え、
前記第1のブレードおよび前記第2のブレードは、前記第1のカニューレをもたらすように、経皮的に、かつ実質的に互いに平行に位置決めされることを特徴とする、パラグラフ1に記載の方法。
12. 当接部材が、前記第1のカニューレの少なくとも近位端を包むように配置され、前記患者の外皮膚面に当接して前記第1のカニューレの可変皮膚下長さを画定するために、前記第1のカニューレに沿って前方に移動されることを特徴とする、パラグラフ1に記載の方法。

近位部と、前記1つまたは複数の移植片が通過することになる前記体内の骨の近傍の第1の解剖学点(164)の近傍に位置するように構成された遠位部とを備える第1の延長材(380)と、
近位部と、前記1つまたは複数の移植片が通過することになる前記体内の骨の近傍の第2の解剖学点(166)の近傍に位置するように構成された遠位部とを備える第2の延長材(380)と、
体外の第1の突出点(214)および第2の突出点(216)への前記第1の解剖学点および前記第2の解剖学点の空間的な変換をもたらすために、前記第2の延長材に対する前記第1の延長材の配向を拘束するように構成された第1のブリッジ(250)であって、前記第1の突出点および前記第2の突出点は、前記第1の延長材および前記第2の延長材の前記近位部にそれぞれ位置している、第1のブリッジ(250)と、

- 前記1つまたは複数の移植片の各々は、前記体の脊椎(10)の近傍に移植されるように形作られ、前記第1の解剖学点は、前記脊椎の第1の椎弓根(30)の近傍に位置し、前記第2の解剖学点は、前記脊椎の第2の椎弓根(50)の近傍に位置していることを特徴とする、請求項1に記載のシステム。
- 前記第1の延長材の前記遠位部は、前記第1の椎弓根内に移植可能な第1の固定部材(140、152)のロッド接触面(158)と嵌合するように形作られ、前記第2の延長材の前記遠位部は、前記第2の椎弓根内に移植可能な第2の固定部材(142、152)のロッド接触面(158)と嵌合するように形作られていることを特徴とする、請求項11に記載のシステム。
- 前記第1の延長材および前記第2の延長材の各々の前記近位部は、ロッド(270)を受けるように形作られたロッド接触面(388)を備えていることを特徴とする、請求項12に記載のシステム。
- 前記第1のブリッジは、前記第1の延長材および前記第2の延長材を互いに平行に保持するように構成されていることを特徴とする、請求項1に記載のシステム。
- 前記第1のブリッジは、前記第1の延長材と前記第2の延長材との間の実質的に自由な相対的平行運動を可能にしながら、前記第1の延長材および前記第2の延長材を互いに平行に保持するように構成されていることを特徴とする、請求項1に記載のシステム。
- 前記第1のブリッジは、前記延長材の各々がその軸を中心として回転することを可能にしながら、前記第1の延長材および前記第2の延長材を互いに平行に保持するように構成されていることを特徴とする、請求項1に記載のシステム。

前記第1のスライダおよび前記第2のスライダは、互いに摺動可能に連結されるように構成されていることを特徴とする、請求項14に記載のシステム。 - 前記1つまたは複数の移植片が通過することになる前記体内の第3の解剖学点(168)の近傍に配置されるように構成された遠位部を備える第3の延長材(380)と、前記システムが、体外の第1の突出点(214)、第2の突出点(216)、および第3の突出点(218)への前記第1の解剖学点、前記第2の解剖学点、および前記第3の解剖学点の空間的変換をもたらすように、前記第3の延長材に対する前記第2の延長材の配向を拘束するように構成された第2のブリッジ(252)と、をさらに備えていることを特徴とする、請求項1に記載のシステム。
- 前記第1のカニューレは、前記第1のカニューレの前記中心軸に沿って前記第1の延長材を受けるように形成され且つ寸法決めされていることを特徴とする、請求項1に記載のシステム。
