【発明の詳細な説明】
【０００１】
【産業上の利用分野】本発明は一般に外科用クリップに
関する。より詳細には、内視鏡外科手術中の縫合結び目
に置き換わるに適切に適合している上記クリップ、並び
に上記クリップを用いて縫合を締め付ける（clamp）方
法、そしてこの方法を達成するための装置に関する。
【０００２】
【従来技術及びその課題】医学および病院のコストが上
昇し続けているため、外科医は、進んだ外科技術を開発
する努力を続けている。外科分野における進歩は、しば
しば、侵入性の低い外科操作を伴うものでありそして患
者の全体的外傷を少なくする手術技術の開発に関するも
のである。このように、病院に居る長さが有意に短縮さ
れ、従って同様に、病院および医学コストを下げること
が可能になる。
【０００３】外科操作の侵入性を低くする、最近行われ
た真に大きな進展の１つは、内視鏡外科手術である。内
視鏡外科手術は、如何なる体の空洞でも目で見て検査し
それを拡大することを可能にする装置である内視鏡の使
用を伴うものである。先端が尖っている装置であるトロ
カールを用いて、体の空洞の壁に穴を開けた後、この内
視鏡をカニューレを通して挿入する。その後、外科医
は、必要に応じ、小さい直径を有する開口部が備わって
いる追加的カニューレを通ってその望まれている体の空
洞の中に入って行くに適合したデザインを有する特別な
装置を用いて、その外科部位で診断および治療操作を行
うことができる。
【０００４】外傷を受けた体の組織を修復もしくは再構
築するために行う必要のある昔からの操作は、縫合であ
る。幸いなことに、カニューレが有する小さい直径の開
口部を通して縫合糸または縫合糸と針との組み合わせを
外科医が取り扱うことを可能にする医学装置が最近設計
された。しかしながら、適当な位置に縫合結び目を結ぶ
ことは、厄介でありそして問題となってきた。
【０００５】従って外科医は、この問題に答えるため、
内視鏡外科手術中に適切な、通常の結び目を結ぶ技術に
対する代替を探求していた。このような代替には、縫合
糸結びの代わりに用いる目的で血管および他の管状部材
をつなぐように設計された、止血用クリップの使用が含
まれる。このような止血用クリップは、例えば米国特許
番号4,418,694および4,476,865に記述されている。カニ
ューレが有する小さい開口部を通して機能するように設
計されているクリップアプライヤー（clip applier）を
用いることで、これらのクリップを容易に取り付けるこ
とができる。不幸なことに、この縫合から上記クリップ
を取り外すに必要な力は、不適当に低い。その結果とし
て、この技術で示されている種類の止血用クリップは、
一般的な内視鏡外科手術が必要としているものに関して
は不適切である。
【０００６】より最近になって、特に、縫合結び目を結
ぶことを置き換える試みが成された。米国特許番号4,66
2,068には、隣接した縫合糸を一緒に融合させると共に
この融合操作を行った後の糸を切断する、多数の機能を
行う装置が開示されている。不幸なことに、上記装置
は、該縫合糸が離れるのを防止するに適当な接着力をそ
の隣接した縫合糸に伝達することはできない。米国特許
番号4,078,731には、縫合結び目を結ぶ必要のない縫合
クリップが記述されている。このクリップは、このクリ
ップに通すべき縫合糸の回りの完全な通路を限定してい
る第一位置の中に取り扱うべきクリップを通すような構
造になっている。このクリップを所望の体組織に隣接さ
せて位置させた後、縫合を締め付ける目的で、このクリ
ップを操作して第二位置に通す。このクリップは、適当
な締め付け力を達成するように設計され得るが、不幸な
ことに、このクリップの全体的デザインは、実施上の困
難さをもたらす可能性のあるデリケートな製造を必要と
している。
【０００７】縫合結び目を結ぶことに対する有効な代替
法を作り出すための従来技術が有する欠点を鑑み、医学
団体が必要としているものは、成功裏に縫合結び目に置
き換わり得るところの、内視鏡技術で用いられるクリッ
プおよびこのクリップの取り付け方法である。更に、上
記クリップに関連させて用いられ得る適切なクリップア
プライヤーと、そのクリップを取り付ける方法が開発さ
れたならば、これはその補助として有益である。
【０００８】
【課題を解決するための手段】１つの面において、本発
明は、外科用結び目を結ぶことなく、縫合糸をしっかり
とつなぎ止める（anchoring）方法である。この方法
は、ａ）各々に、もう一方の足部材の締め付け用内部表
面に向かい合う締め付け用内部表面が備わっている、そ
れらの基部末端で連結している第一および第二足部材を
有するポリマー材料で構成されている外科用クリップを
準備し；ｂ）該第一および第二足部材の該締め付け用内
部表面の間に、縫合糸を置き；ｃ）該クリップの少なく
とも一部に熱をかけることで、該クリップを構成してい
る該ポリマー材料を柔らかくし；ｄ）該クリップを軟化
した状態にしながら、該クリップの該第一および第二足
部材に対して圧縮力をかけることにより、該第一および
第二足部材の該締め付け用内部表面の間で、該縫合糸を
締め付け；そしてｅ）該クリップから該熱および圧縮力
を取り除く；段階を含んでいる。
【０００９】別の面において、本発明は外科用クリップ
である。この外科用クリップは、第一および第二足部材
を含んでいる。各々の足部材には、外側つかみ表面、締
め付け用内部表面、および該外側つかみ表面を該締め付
け用内部表面に連結させている外側末端表面が備わって
いる。足部材各々の該締め付け用内部表面はもう一方の
足部材の締め付け用内部表面に向かい合っている。更
に、該第一足部材にはその中に穴が備わっている。
【００１０】本発明の更に一層の面において、本発明
は、縫合の回りで外科用クリップを締め付けるための外
科用装置であり、ここで、このクリップは、向かい合う
第一および第二足部材を有するポリマー材料で構成され
ている。この装置には、ａ）ベースおよびこのベースに

ーに連結させるための管状シャフト、ｅ）該締め付け用
ジョーの中で該クリップをつかみそして該加熱用要素を
活性化するとき、該クリップを構成しているポリマー材
料が柔らかくなるように該加熱用要素を活性化するため
の第一手段、ｆ）該クリップを構成している該ポリマー
材料が柔らかくなったとき、該クリップの向かい合う該
第一および第二足部材が縫合の回りで閉じるように、該
締め付け用ジョーの向かい合う該第一および第二締め付
け用部材に圧縮力を伝える第二手段、が備わっている。
【００１１】本発明の方法は、簡単なクリップデザイン
を用い、縫合糸が離れるのを防止するに適当な締め付け
力と接着力を有する縫合糸をしっかりとつなぎ止めるか
或は固着させるユニークな方法を提供するものである。
驚くべきことに、この締め付け力と接着力は、縫合結び
目を結ぶ必要性をなくさせるに適当である。優位に、本
発明の方法は内視鏡外科手術に関連して用いられ、従っ
て、縫合結び目を結ぶ必要性をなくさせることによる、
医学の従来技術における多大な進歩を表している。
【００１２】本発明の外科用クリップは、デザインが簡
単であり、製造が容易であると共に、このクリップを取
り付けための広い組み合わせの外科用装置と一緒に用い
られ得る。このクリップを取り付けるための本発明の装
置は、特に、トロカールを通して体の空洞の中で該クリ
ップを操作しそしてそれを位置させることを必要として
いる内視鏡操作中に用いるに適合し得る。
【００１３】本文中に記述する如き発明は、縫合を必要
としている、従って縫合糸を固着させるか或はそれをつ
なぎ止めることを必要としている、如何なる外科操作で
も利用され得る。

【実施例】図１を参照して本発明の外科用クリップ１０
を示す。この外科用クリップには、外側の基部表面１３
においてそれらの基部末端で一緒に連結している第一足
部材１１と第二足部材１２が備わっている。この第一足
部材には、締め付け用内部表面１４、外側つかみ表面１
６、および該締め付け用内部表面を該外側つかみ表面に
表面で連結させている外側末端表面１８が備わってい
る。相当して、該第二足部材には、締め付け用内部表面
１５、外側つかみ表面１７、および該締め付け用内部表
面を該外側つかみ表面に連結させている外側末端表面１

イヤーの締め付け用ジョーの上に位置させた給源からの
熱を、該クリップの別の部分にかける目的で、この穴を

表面と該締め付け用内部表面の間の該第一足部材の中に
位置している。この穴は、製造を容易にする目的で望ま
しくは筒状であり、そして該外側末端表面１８から該第
一足部材を完全に通過して該外側基部表面１３に伸びて
いる。この穴に加熱用要素を挿入したときの該クリップ
の操作を容易にする目的で、この穴は、有利に、該第一

る。本発明の外科用クリップは、体の組織に生適合性を
示すと共に生吸収性を示す如何なるポリマー材料で作ら
れていてもよい。好適な生吸収性ポリマー材料は、グリ
コリッド、ラクチド、パラ−ジオキサノン、トリメチレ
ンカーボネートおよびε−カプロラクトンのホモポリマ
ー類およびコポリマー類である。好適なポリマー材料は
パラ−ジオキサノンの重合で誘導される、と言うのは、
パラ−ジオキサノンから誘導されたポリマーは低い軟化
温度を有しているからである。
【００１６】生吸収性ポリマー材料からこのクリップを
製造するための好適な手段は、上記ポリマーシステムで
通常に用いられている加工条件下で、適当なデザインを
有する鋳型の中に適切なポリマー溶融物を注入すること
である。この鋳型の中で該ポリマー溶融物を冷却させた
後、このクリップのデザイン基準に合致する適当な構造
に成形されたポリマーを、該鋳型から取り出す。次に、
この成形されたクリップを、外科用途に適切なクリップ
にするための通常の方法で滅菌してもよい。
【００１７】図２および３をここに参照して、本発明の
クリップを縫合糸の回りで締め付けることが可能であ
り、従って外科手術中に外科用結び目を結ぶ必要がない
方法を示す。加熱用要素２３は、このクリップの第一足
部材を通って伸びていてもよい。このクリップを通して
該加熱用要素を伸ばすことで、このクリップを柔らかく
する準備ができた後、締め付けることが望まれている縫
合糸を、該クリップの足部材の該締め付け用内部表面の
間に置く。この縫合糸を、該向かい合う内部表面の間の
適当な場所に位置させた後、該加熱用要素を活性化して
もよく、その結果として、この加熱用要素から発生する
熱で、該クリップを構成しているポリマー材料の温度
が、該ポリマー材料の軟化温度を越えるまで上昇する。
この温度はしばしば、このポリマーの温度がこれのガラ
ス転移温度を越えたとき達成される。適当な軟化温度に
到達したら、図３の矢印の方向で示すように、向かい合
う足部材に圧縮力を加えてもよく、その結果として、該
クリップの足部材の締め付け用内部表面の間で縫合糸が
しっかりと締め付けられる。この締め付け操作が完了し
た後、この熱と圧縮力を取り除く。
【００１８】有利に、この方法は、１本の縫合糸ばかり
でなく２本以上の隣接する縫合糸を固着させるか或はつ
なぎ止めるためにも用いられ得る。このクリップが有す
る締め付け作用と接着作用が組み合わさっていること
で、この締め付けられた糸を引き離すに必要な引っ張り
力が高いため、これを達成することができる。
【００１９】本発明の方法は、有利に、非常に短期間、
好適には１０秒未満に行われる。これは、該足部材に圧
縮力をかけながら同時に、この足部材に短い一発の熱エ
ネルギーをかけることで、該クリップの内部表面の中で
該縫合を締め付けることにより、達成され得る。これは
本発明を実施するための好適な手段であるが、これは必
ずしも、どのように本発明の方法を実施するかに関する
制限因子であるとは限らない。
【００２０】本発明の代替方法を図５に説明する。クリ
ップをつかみそして圧縮力をかける装置のシャフトを設
計して、このシャフトの上に多数のクリップを乗せて体
の中の適当な位置に運ぶようにする。縦方向シャフトの
その遠位先端の上か、或は二者択一的に、この装置のつ
かみ部材の上に、加熱用要素を位置させることができ
る。次に、このシャフトの上の最も遠い場所に位置して
いるクリップの少なくとも一部に熱をかけることで、こ
のクリップを柔らかくした後、この柔らかくなったクリ
ップをこのシャフトから離れるように押して、これが充
分にこの装置のつかみ部材に噛み合わうようにする。次
に、この装置のつかみ部材を活性化して、このクリップ
の上に圧縮力をかけることにより、縫合の回りで締め付
けを行う。逆に、残りのクリップは、このシャフトに沿
って離れるように押され、多数のクリップの連続した取
り付けが行われる。

は、クリップの足部材の１つの中を伸びている穴が備わ
っているクリップを用いる必要がないことを説明するも
のである。次の締め付けを行うための圧縮力を該足部材
にかける目的でクリップの少なくとも一部をその軟化温
度以上の温度に加熱することができることを条件とし
て、向かい合う２つの足部材が備わっている如何なるク
リップにも、何らかの方法で取り付けることが可能であ
るか或は接触させることが可能な、加熱用要素を用いる
ことも全く可能である。
【００２２】図４は、外科用クリップ１０を締め付ける
ための本発明の外科用装置３０を示している。この示す
装置は、多数のクリップを取り付ける目的で図５に示し
た代替具体例と一緒に用いられ得る。この装置には、ベ
ース３６と作動引金３７が備わっている、引金で活性化
されるハンドル３１が含まれている。利用者は、矢の示
す方向に力をかけることで上記引金を該ベースに向かっ
て押すことができる。上記力をかけた後、この引金は図
４に示す位置に戻る。このハンドルに加えて、このハン
ドルを締め付け用ジョー３２に連結させている縦方向の
管状シャフト３０が備わっている。この縦方向のシャフ
トには、内視鏡外科手術中、トロカールスリーブを下方
に置くに適切な構造を有する長いネックが備わってい
る。この締め付け用ジョーには、それぞれ３３および３
４で示す、向かい合う第一および第二締め付け用部材が
備わっており、これらは、クリップの向かい合う第一お
よび第二足部材をつかむような構造になっている。この
第一締め付け用足部材には、最遠位に位置しているクリ
ップに熱をかけるための、その上に配置されている加熱
用要素３２が備わっている。二者択一的に、この加熱用
要素は、該シャフトの遠位末端に位置していてもよく、
この場合、これは最遠位に位置しているクリップの穴を
通って伸びている。
【００２３】図４に示す外科用装置を用いるためには、
利用者は、このハンドルのベースの方向に引金を押すこ
とで、接触スイッチ３８が活性化され、これがまた適切
な電力源３９に電力を送る。この電力源は、再充電可能
な低圧バッテリーであってもよい。このバッテリーがオ
ン状態になると、該加熱用要素が活性化され、そして該

用クリップを準備し； ｂ）上記第一および第二足部材の該締め付け用内部表面
の間に、縫合糸を置き； ｃ）上記クリップの少なくとも一部に熱をかけること
で、上記クリップを構成している上記ポリマー材料を柔
らかくし； ｄ）上記クリップを軟化した状態にしながら、上記クリ
ップの上記第一および第二足部材に対して圧縮力をかけ
ることにより、上記第一および第二足部材の上記締め付
け用内部表面の間で、上記縫合糸を締め付け；そして ｅ）上記クリップから上記熱および上記圧縮力を取り除
く；段階を含む、外科用結び目を結ぶことなく、縫合糸
をしっかりとつなぎ止める方法。

行う第１項の方法。

向かい合う第一および第二足部材を有するポリマー材料
で構成されている外科用クリップを縫合の回りで締め付
けるための外科用装置。
【００４８】２３．上記ハンドルの上記ベースに、それ
の表面の上に配置されている接触スイッチが備わってい
る第２２項の装置。
【００４９】２４．上記引金と上記接触スイッチとを接
触させて上記スイッチを活性化するように上記ハンドル
の上記引金を押すことにより、上記第一および第二手段
を活性化する第２３項の装置。

Claims (3)

    【特許請求の範囲】
  1. 【請求項１】ａ）各々に、もう一方の足部材の締め付け
    用内部表面に向かい合う締め付け用内部表面が備わって
    いる、それらの基部末端で連結している第一および第二
    足部材を有するポリマー材料で構成されている外科用ク
    リップを準備し； ｂ）上記第一および第二足部材の該締め付け用内部表面
    の間に、縫合糸を置き； ｃ）上記クリップの少なくとも一部に熱をかけること
    で、上記クリップを構成している上記ポリマー材料を柔
    らかくし； ｄ）上記クリップを軟化した状態にしながら、上記クリ
    ップの上記第一および第二足部材に対して圧縮力をかけ
    ることにより、上記第一および第二足部材の上記締め付
    け用内部表面の間で、上記縫合糸を締め付け；そして ｅ）上記クリップから上記熱および上記圧縮力を取り除
    く；段階を含む、外科用結び目を結ぶことなく、縫合糸
    をしっかりとつなぎ止める方法。
  2. 【請求項２】 第一および第二足部材を含んでおり、各
    々の足部材には、外側つかみ表面、締め付け用内部表
    面、および上記外側つかみ表面を上記締め付け用内部表
    面に連結させている外側末端表面が備わっており、上記
    締め付け用内部表面はもう一方の足部材の締め付け用内
    部表面に向かい合っており、そして上記第一足部材のそ
    の中に穴が備わっている、外科用クリップ。
  3. 【請求項３】ａ）ベースおよびこのベースに対して動く
    ように取り付けられている作動引金が備わっているハン
    ドル、 ｂ）クリップの向かい合う第一および第二足部材をつか
    むような構造になっている向かい合う第一および第二締
    め付け用部材が備わっている締め付け用ジョー、 ｃ）上記第一締め付け用部材の上に配置されている加熱
    用要素、 ｄ）上記ハンドルを上記締め付け用ジョーに連結させる
    ための管状シャフト、 ｅ）上記締め付け用ジョーの中で上記クリップをつかみ
    そして上記加熱用要素を活性化するとき、上記クリップ
    を構成しているポリマー材料が柔らかくなるように上記
    加熱用要素を活性化する第一手段、 ｆ）上記クリップを構成している上記ポリマー材料が柔
    らかくなったとき、上記クリップの上記第一および第二
    足部材が縫合の回りで閉じるように、上記締め付け用ジ
    ョーの向かい合う上記第一および第二締め付け用部材に
    圧縮力を伝える第二手段、が備わっている、向かい合う
    第一および第二足部材を有するポリマー材料で構成され
    ている外科用クリップを縫合の回りで締め付けるための
    外科用装置。
