Description

【発明の詳細な説明】
産業上の利用分野 本発明は、投与単位21ないし28から成り、毎日
１単位投与する経口避妊用３段階製剤に関する。 従来の技術 経口避妊用多段階組合わせ製剤は、例えば西ド
イツ国特許公開第2365103号明細書及びそれをベ
ースとする他の特許明細書から既に公知である。
この多段階製剤は糖衣剤21ないし28個より成り、
かつ第１段階で各糖衣剤がエチニルエストラジオ
ール0.02〜0.05mgに相当するエストロゲン量とｄ
−ノルゼストレル0.04〜0.09mgに相当するゲスタ
ゲン量を含有する糖衣剤４〜６個、第２段階で各
糖衣剤が第１段階の１〜２倍のエストロゲン量、
例えばエチニルエストラジオール0.03〜0.05mgと
第１段階の１〜1.5倍のゲスタゲン量、例えばｄ
−ノルゲストレル0.05〜0.125mgを含有する糖衣
剤４〜６個、及び第３段階で各糖衣剤が第１段階
よりも多量かつ又は同量でありかつ第２段階より
も少量か又は同量であるエストロゲン量、例えば
エチニルエストラジオール0.025〜0.050mgと第２
段階よりも多量であるが第１段階の３倍より多量
ではないゲスタゲン量、例えばｄ−ノルゲストレ
ル0.10〜0.25mgを含有する糖衣剤９〜11個並びに
場合により第４段階でエストロゲン及びゲスタゲ
ンを含まない糖衣剤７個を含有する。エストロゲ
ン及びゲスダゲンを含有する３段階の用量単位数
は21であり、かつ28日周期に適合させるため21個
の作用物質含有単位に作用物質を含有しない７単
位を加えることができる。 周期的又は連続的適用のための公知の組合わせ
製剤に比べて、多段階製剤により更に高い認容性
及び周期制御性が達成された。 ヨーロツパ特許出願第81200240号明細書（公開
番号36229）には、多段階製剤の別形が記載され
ており、これは、第１段階の単位中にそれ以後の
段階の単位によりも高いエストロゲン量を含有す
ることを特徴としている。 多段階組合わせ製剤では、エストロゲンをエチ
ニルエストラジオール0.05mgより少量で使用する
ことができる。段階的な構成のため、ゲスダゲン
量も低く維持することもできる。このようにし
て、従来極めて低いホルモン量の避妊薬が得られ
ている。 発明が解決しようとする問題点 本発明の課題は、多段階組合わせ製剤中のゲス
タゲン量を、良好な認容性と周期制御を保持しな
がら低下させることである。 問題点を解決するための手段 ゲスタゲンとしてゲストデン（17α−エチニル
−17β−ヒドロキシ−18−メチル−エストラ−
４，15−ジエン−３−オン）を使用する場合に、
多段階組合わせ製剤中のゲスタゲン量を低下させ
得ることが判明した。用量の低下にもかかわら
ず、優れた周期制御が良好な認容性と共に達成さ
れることは全く予想外であつた。避妊効果の確実
性は常に保証されている。 それ故、本発明は投与単位21もしくは28より成
り、毎日１単位投与する経口避妊用多段階組合わ
せ製剤に関し、この製剤は４〜６単位の第１段階
で各単位が低い用量のエストロゲンと低い用量の
ゲスタゲンを含有し、４〜６単位の第２段階で各
単位が第１段階に比べて同じ用量か又は僅かに高
めた、最高２倍まで高めた用量のエストロゲン
と、同じ用量か又は僅かに高めた、最高１ 1/2倍
まで高めた用量のゲスタゲンを含有しかつ９〜11
単位の第３段階で各単位が先行段階に比べて同じ
用量か又は再度低下させた、最高で出発値に再び
低下させた用量のエストロゲンと更に高めた、最
高で出発値の３倍に高めた用量のゲスタゲンを含
有し、その際に３つの段階は合計すると21単位よ
り成りかつ場合により次いでエストロゲン及びゲ
スタゲンを含有しない７単位を更に包含してお
り、この製剤はゲスタゲンがゲストデンでありか
つエストロゲンがエチニルエストラジオールであ
り、第１段階が糖衣剤６個より成り、その際に各
糖衣剤がエチニルエストラジオール0.03mg及びゲ
ストレン0.05mgを含有し、第２段階が糖衣剤５個
より成り、その際に各糖類衣剤がエチニルエスト
ラジオール0.04mg及びゲストデン0.07mgを含有し
かつ第３段階が糖衣剤10個よりなり、その際に各
糖衣剤がエチニルエストラジオール0.03mg及びゲ
ストデン0.10mgを含有することを特徴とする。 経口避妊にあたり、記載の段階順序で毎日１投
与単位を与える。作用物質の組合わせ物を投与す
る全日数は常に21日であり、その後ホルモンなし
で毎日１為薬を投与するか又は投与単位を与えな
い７日間が続く。 エチニルエラストラジオール及びゲストデンを
殊に一緒に経口投与するか、別個に及び／又は腸
管外に投与することもできる。 エチニルエストラジオール及びゲストデンはガ
ーレン式製剤で常用の添加物、賦形剤及び／又は
嬌味剤と公知方法で常用の投与形に加工する。優
れた経口投与には特に錠剤、糖衣剤、カプセル
剤、ピル、懸濁液又は溶液が該当する。 作用物質を含有する21投与単位で作用物質を含
まない７投与単位（偽薬）で補充して、ホルモン
を投与しない日を調整することができる。このよ
うにして、１日１単位を服用する習慣をつける。
28日後に（出血開始後）新しい錠剤を包装で継続
する。 作用物質がフイルム材料中に配合されていても
よい。フイルム層を区分することにより、口腔又
は舌下投与に相応する用量の投与単位を調製する
ことができる。 それ故、本発明により多段階組合わせ製剤を一
定の順序の21又は28個の投与単位で含有する製薬
的包装も得られ、その際にその順序は毎日の投与
の段階に相当する。偽薬及び３段階の投与単位を
その色又は形状で区別すると有利である。 製薬的包装を、例えば第１段階の６糖衣剤、第
２段階の５糖衣剤、第３段階の10糖衣剤及び場合
により７偽薬を包含する特に透明包装の形で実施
することができ、それらを１個ずつ毎日、つまり
21又は28日間にわたつて取り出すことができる。 実施例 例（糖衣剤の組成） 第１段階（６糖衣剤） エチニルエストラジオール 0.030mg ゲストデン 0.050mg ラクトース 37.455mg トウモロコシデンプン 15.500mg Ca−ジ−Na−エデテート 0.065mg コリドン25 1.700mg ステアリン酸マグネシウム 0.200mg 全 量 55.000mg これに常用の糖混合物及び場合により色素を加
えて90.000mgにする。 第２段階（５糖衣剤） エチニルエストラジオール 0.040mg ゲストデン 0.070mg ラクトース 37.425mg トウモロコシデンプン 15.500mg Ca−ジ−Na−エデテート 0.065mg コリドン25 1.700mg ステアリン酸マグネシウム 0.200mg 全 量 55.000mg これに常用の糖混合物及び場合により色素を加
えて90.000mgにする。 第３段階（10糖衣剤） エチニルエストラジオール 0.030mg ゲストデン 0.100mg ラクトース 37.405mg トウモロコシデンプン 15.500mg Ca−ジ−Na−エデテート 0.065mg コリドン25 1.700mg ステアリン酸マグネシウム 0.200mg 全 量 55.000mg これに常用の糖混合物と場合により色素を加え
て90.000mgとする。 認容性及び避妊効果確実性についての臨床実験 本発明の例による３段階製剤(A)を西ドイツ国特
許公告第2365103号明細書の例による３段階製剤
(B)と比較した。 試験製剤(A)で生殖能力のある年令の婦人377人
を2123周期で試験製剤(B)で婦人362人を2088周期
で処置した。それぞれの婦人は21日間毎日１糖衣
剤を服用し、続く７日間（この間の出血開始）は
服用しない。この服用形を６周期にわたつて持続
する。ほとんどの婦人は最後まで試験に参加し
た。
【表】 Ａによる良好な周期制御は、Ａのゲスタゲン量
がＢよりも少ないので驚異的である。

Claims (1)

    【特許請求の範囲】
  1. １ 投与単位21もしくは28より成り、毎日１単位
    投与する経口避妊用３段階製剤であつて、その際
    ４〜６単位の第１段階で各単位が低い用量のエス
    トロゲンと低い用量のゲスタゲンを含有し、４〜
    ６単位の第２段階で各単位が第１段階に比べて同
    じ用量か又は僅かに高めた、最高２倍まで高めた
    用量のエストロゲンと、同じ用量か又は僅かに高
    めた最高１ 1/2倍まで高めた用量のゲスタゲンを
    含有し、かつ９〜11単位の第３段階で各単位が先
    行段階に比べて同じ用量か又は再度低下させた、
    最高で出発値に再び低下させた用量のエストロゲ
    ンと更に高めた、最高で出発値の３倍に高めた用
    量のゲスタゲンを含有し、その際に３つの段階は
    合計21単位より成りかつ次いで更にエストロゲン
    及びゲスタゲンを含有しない７単位を包含しても
    よい前記製剤において、第１段階が糖衣剤６個よ
    り成り、その際に各糖衣剤がエチニルエストラジ
    オール0.03mg及びゲストデン0.05mgを含有し、第
    ２段階が糖衣剤５個より成り、その際に各糖類衣
    剤がエチニルエストラジオール0.04mg及びゲスト
    デン0.07mgを含有しかつ第３段階が糖衣剤10個よ
    りなり、その際に各糖衣剤がエチニルエストラジ
    オール0.03mg及びゲストデン0.10mgを含有するこ
    とを特徴とする経口避妊用３段階製剤。
