図1A~図1Dは、ハンドル組立体(12)と、ハンドル組立体(12)から遠位に延在するシャフト組立体(14)と、シャフト組立体(14)の遠位端にあるエンドエフェクタ(16)とを含む例示的な外科用ステープラ(10)を示す。本明細書では、「近位」、「遠位」、「右」、「左」といった用語は、外科用ステープラ(10)のハンドル組立体(12)を握っている臨床医を基準として使用されることを理解されたい。したがって、エンドエフェクタ(16)は、比較的近位にあるハンドル組立体(12)に対して遠位にある。本明細書に別途記載のある場合を除き、外科用ステープラ(10)は、その開示が参照により本明細書に組み込まれる米国特許出願公開第2005/0143759号、発明の名称「Curved Cutter Stapler Shaped for Male Pelvis」(2005年6月30日公開)、及び/又はその開示が参照により本明細書に組み込まれる米国特許出願公開第2017/0027571号、発明の名称「Surgical Instrument Comprising Systems for Assuring the Proper Sequential Operation of the Surgical Instrument」(2017年2月2日公開)の教示の少なくとも一部に従って構成され、動作可能であってもよい。
図1Aに示すように、外科用ステープラ(10)のハンドル組立体(12)は、ハンドルハウジング(34)と、ハンドルハウジング(34)内に収容された近位部分(図示せず)と、シャフト組立体(14)に沿って遠位に延在する細長い遠位部分とを有する一対のハンドルフレームプレート(35、36)とを含む。簡潔に上述したように、ハンドル組立体(12)は、鞍状の摺動部(18)と、閉鎖トリガ(20)と、発射トリガ(22)とを更に含む。ハンドルハウジング(34)は、操作者が少なくとも片手の掌で把持することができるハンドグリップ(38)を画定する。本実施例のハンドルハウジング(34)は、右シュラウドハンドル部分(40)及び左シュラウドハンドル部分(42)によって形成される。閉鎖トリガ(20)は発射トリガ(22)に対して近位に位置付けられる。各トリガ(20、22)はフレームプレート(35、36)に枢動可能に取り付けられ、ハンドルハウジング(34)の下面を通って露出されて、操作者の指によって操作される。図1Aは、エンドエフェクタ(16)の閉鎖、及びステープル(図示せず)及び湾曲したナイフ(32)の発射の前の、非作動位置にある閉鎖及び発射トリガ(20、22)を示す。これに応じて、カートリッジハウジング(28)とアンビル(26)との間の間隙(25)内に組織を受容するために、カートリッジハウジング(28)はアンビル(26)から離間している。
図3~図5に最もよく示されるように、本実施例のエンドエフェクタ(16)は、C字形の支持構造体(128)と、C字形の支持構造体(128)によって取り外し可能に受容される交換可能なカートリッジユニット(24)とを含む。本実施例の支持構造体(128)は、ショルダリベット(129)及び一対のポスト(130)によって、シャフト組立体(14)の遠位端においてハンドルフレームプレート(35、36)の遠位端に固定される。用語「C字形」という用語は、本明細書では、それぞれが互いに対向する凹状の第1の外側部及び凸状の第2の外側部を有する支持構造体(128)及びカートリッジユニット(24)の湾曲に関して使用される。このような構造は、機能性及び患者内の組織へのアクセス性を高める。あくまで一例として、支持構造体(128)及びカートリッジユニット(24)のC字形構造は、例えば、直腸結腸切除術において低位前方切除術(「lower anterior resection、LAR」)を行うために、エンドエフェクタ(16)が患者の骨盤の凹み内の結腸下部に容易にアクセスすることを可能にし得る。したがって、本明細書で使用するとき、「C字形」という用語は、外科用ステープル留め及び切断器具の機能性を同様に向上させるであろう様々な凹形状を含むと解釈されるべきである。
カートリッジユニット(24)を含む、上記の外科用ステープラ(10)の様々な構造的特徴について説明してきたが、ここで外科手術中の外科用ステープラ(10)の例示的な作動について以下に説明する。外科用ステープラ(10)は、最初に、患者の体腔内で適切に操作されて、アンビル(26)とカートリッジハウジング(28)との間の間隙(25)(図1A参照)内に患者組織を位置付ける。図1Bに示すように、次に、摺動部(18)が遠位に作動されてプッシュロッド(50)を遠位に駆動し、それによって組織保持ピン(30)をカートリッジハウジング(28)からアンビル(26)に向かって遠位に駆動する。組織保持ピン(30)の尖った遠位先端は、組織をしっかりと係合し(例えば、穿孔し)、それによって組織を間隙(25)内に捕捉する。
