影響を受ける試験抗体による体液性免疫応答のCA125による抑制を効果的に抑制することができるMCBドメイン融合タンパク質(薬剤)の組成物、キット、およびそのような薬剤を特定するための方法が本明細書に提供される。本明細書に記載の最適なMCBドメイン融合タンパク質を特定するための方法のいくつかの実施形態では、方法は、アンカータンパク質に遺伝的に融合された、アミノ酸スペーサーによって連結された2つ以上のMCBドメインを含有するMCBドメイン融合タンパク質を生成することと、影響を受ける試験抗体またはADCが、ADCC、CDC、または毒素により誘導された標的細胞の殺傷を媒介するために使用された場合、大幅に改善された生物学的活性を有する能力を試験することとを伴う。いくつかの実施形態では、MCBドメイン融合タンパク質は、ヒトIgG1 Fc重鎖ドメインに融合された2つのMCBドメインからなり、重鎖は、ヒンジ領域内にジスルフィド架橋ドメインを保持する。他の実施形態では、MCBドメイン融合タンパク質は、免疫グロブリンIgG1以外のタンパク質に由来するアンカータンパク質に融合された、アミノ酸スペーサーによって連結された2つ以上のMCBドメインからなり、アンカータンパク質は、腎クリアランスを回避し、PK半減期を延長するために、単量体の場合、分子量が60kDa以上であるか、またはアンカータンパク質が二量体またはより高次の多量体を形成する場合、30kDa以上である。他の実施形態では、MCBドメイン融合体は、アミノ酸スペーサーによって連結され、かつペグ化され、血清アルブミンにコンジュゲートされ、抗体によって結合され、かつ/または他の修飾によって修飾されて分子量が60kDa以上の融合分子を作成する、少なくとも2つのMCBドメインからなる。さらに他の実施形態では、MCBドメイン融合タンパク質は、アンカータンパク質に融合された、アミノ酸スペーサーによって連結された3つ以上のMCBドメインを有する。融合は、アンカータンパク質のNまたはC末端への遺伝子融合によってであり得、MCBドメインの構成は、タンデム配列であってもよいし、図1Dに例示されているようにアンカータンパク質のN末端およびC末端に連結することによって分離されていてもよい。タンデムなMCBドメイン間の連結は、アミノ酸リンカー単位GGGGSをコードする遺伝子リンカーを介してである。連結は、1つまたは複数の単位を介してであり得る。最適なスペーサー単位は、影響を受ける試験抗体、CA125、および標的細胞の存在下で、機能的なADCC、CDC、またはADC殺傷アッセイを使用して決定され得る。例を図1に概略的に示す。キットは、当業者によって使用されるADCC、CDC、またはADC殺傷アッセイを用いることにより、影響を受ける試験抗体に対するCA125による体液性免疫抑制を大幅に克服することができる、MCBドメイン融合タンパク質で構成される。
CA125による影響を受ける抗体の体液性免疫抑制活性を克服するのに好適なMCBドメイン融合タンパク質を作製するためのキットが、本明細書でさらに提供される。
図1は、アンカータンパク質に遺伝的に融合された、アミノ酸スペーサーによって遺伝的に連結された2つのMCBドメイン(A)、アンカータンパク質のN末端側またはC末端側のドメインに融合された、アミノ酸スペーサーによって遺伝的に連結された3つ以上のMCBドメイン(B)、ヒトIgG1 Fc重鎖ドメインに遺伝的に融合された、アミノ酸スペーサーによって遺伝的に連結された2つのMCBドメイン(C)、ならびにアンカータンパク質のN末端側およびC末端側のドメインに融合された、アミノ酸スペーサーによって遺伝的に連結された3つ以上のMCBドメイン(D)を含む、MCBドメイン融合タンパク質の概略図を示す。アンカータンパク質は、腎排泄および短い血清半減期を回避するのに有効な分子量サイズを維持するため、単量体として存在する60kDa以上のタンパク質、または多量体(二量体以上の高次構造)として存在する30kDa以上のタンパク質からなる。HSAの場合、連結はNまたはC末端においてであり得、他の分子をコンジュゲートするために使用されかつ当業者によって使用される内部遊離システイン(一部の参考文献ではCys34と称される)にも連結され得る(Sugio S,et al.Protein Engineering 12:439-446,1999)。あるいは、限定されないが、ペグ化などの化学修飾を、より小さなアンカータンパク質またはMCBドメイン自体の多量体と共に使用して、より大きい分子量の融合タンパク質を作成することができる。本明細書に示される連結されたMCBドメインは、スペーサー単位GGGGCを使用して遺伝的連結によって結合している。用いられるスペーサー単位には、単一のスペーサー単位の使用、および複数のスペーサー単位を含有する融合タンパク質が含まれる。
抗体薬物コンジュゲート(ADC)の使用は、Adcetris(登録商標)およびKadcyla(登録商標)の商業化の成功により長年にわたって広がっている(Hedrich WD,et al.Clin Pharmacokinet 57:687-703,2018)。これらのADCが機能する主な機構は、がん細胞の細胞表面抗原を、それに結合してから内在化する抗原特異的抗体構成要素でターゲティングすることであり、細胞傷害性ペイロードは、内部タンパク質分解酵素によって抗体から遊離する。内在化の速度は、所与のADCの有効性に深刻な影響を与える可能性がある(Chalouni C and Doll S.J Exp Clin Cancer Res 37:20-32,2018)。本明細書では、CA125がCA125産生がん細胞を標的とするADCの有効性に大きな影響を与える可能性があることを教示する。図6は、高レベルのCA125を発現する腫瘍細胞を治療する場合に、ADCの細胞傷害性がどのように低減するかの例を示す。これらの研究では、トポイソメラーゼ阻害剤SN38を含有する抗メソセリンADCを、メソセリンおよびCA125を発現するヒト卵巣がん細胞株OVCAR3、およびshRNA(Sigma-Aldrich)を使用してCA125をノックダウンした操作された同質遺伝子細胞株(OVCAR-KOと称される)に対して使用した。同様の株が以前に発表され、親OVCAR細胞とは対照的に、CA125の影響を受ける抗体による抗体媒介性体液性応答が大幅に増強されることが示されている(Kline JB,et al.OncoTarget 8:52045-52060,2017、Nicolaides NC,et al.Cancer Biol Ther 13:1-22,2018)。図6の左側のバーに示されているように、抗メソセリンADCは、同質遺伝子のOVCAR-KO株と比較して、OVCAR3親細胞株を殺傷する効果が低かった。CA125発現腫瘍細胞のADCによる殺傷およびCA125の影響を受ける抗体を改善するためのMCBドメイン融合タンパク質の使用を決定するために、OVCAR3細胞を96ウェルフォーマットにプレーティングし、MCBドメイン-2X融合タンパク質(配列番号2)の存在下または不在下でOVCAR3親細胞を抗メソセリンADCで処理した。図6の右側のバーに示されているように、ADCを用いたMCBドメイン-2X融合タンパク質の投与は、ADCのみで処理した細胞と比較してADCによる殺傷が大幅に増強された。
Claims (42)
- 患者における体液性免疫応答のCA125による抑制を阻害するための、融合タンパク質を含む医薬であって、
前記融合タンパク質が、(a)配列番号1に示されるメソセリンのアミノ酸296~359を含むメソセリンの最小CA125結合(MCB)ドメインの少なくとも2つのコピーと、(b)アンカータンパク質と、を含み、
前記融合タンパク質が単量体の場合、少なくとも60kDaの単量体の分子量を有し、前記融合タンパク質が多量体の場合、少なくとも30kDaの単量体の分子量を有する、
医薬。 - 前記体液性免疫応答が、外因的に投与された治療用抗体によって媒介される、請求項1に記載の医薬。
- 前記体液性免疫応答が、内因的に産生される自己抗体によって媒介される、請求項1に記載の医薬。
- CA125によって悪影響を受ける治療用抗体と組み合わせて用いられる、請求項1~3のいずれか一項に記載の医薬。
- 健康なヒト集団と比較して高いレベルのCA125を発現する疾患を有する患者を治療する方法において使用するための、融合タンパク質を含む医薬であって、
前記融合タンパク質が、(a)配列番号1に示されるメソセリンのアミノ酸296~359を含むメソセリンの最小CA125結合(MCB)ドメインの少なくとも2つのコピーと、(b)アンカータンパク質と、を含み、

- 前記治療用抗体が、抗体-薬物コンジュゲートの一部である、請求項7に記載の医薬。
- 前記疾患が、卵巣がんである、請求項5に記載の医薬。
- 前記疾患が、中皮腫、乳がん、肺がん、結腸直腸がん、胃腸がん、子宮内膜がん、および膵臓がんからなる群から選択されるがんである、請求項5に記載の医薬。
- 前記融合タンパク質が、細胞傷害性薬物または放射性核種に結合している、請求項5に記載の医薬。
- 患者においてCA125を発現する腫瘍を監視するための方法において使用するための、融合タンパク質を含む医薬であって、
