背景技术
重定向T细胞疗法的潜力已由blinatumomab在血液系统恶性肿瘤中的批准证实,并且最近由靶向实体瘤(如结直肠癌和前列腺癌)的CD3双特异性抗体的早期临床活性的报告证实。CD3双特异性抗体具有作为有效癌症治疗剂的潜力,因为它们募集并激活广泛的T细胞库以对抗表达肿瘤相关细胞表面抗原的肿瘤细胞。它们绕开了T细胞受体和与抗原性肽复合的MHC I类结合的需要,而是将T细胞募集到表达细胞表面抗原的靶细胞。双特异性抗体的一个臂与肿瘤相关细胞表面抗原结合,另一臂与T细胞上的CD3蛋白结合,导致针对肿瘤细胞的细胞毒性T淋巴细胞(CTL)应答。
鉴于CD3双特异性抗体的高效力,用在正常组织中具有最小或受限表达的抗原以展现肿瘤选择性活性非常重要。叶酸受体-α (FRα,也称为FOLR1)是一种糖基磷脂酰肌醇连接的细胞表面糖蛋白,对叶酸具有高亲和力。大多数正常组织不表达FOLR1,并且生理性叶酸向大多数细胞的转运被认为是由其他几种蛋白质介导的,最显著的是还原叶酸载体。在浆液性和子宫内膜样上皮性卵巢癌、子宫内膜腺癌和腺癌亚型的非小细胞肺癌中发现了高水平的FOLR1。重要的是,FOLR1表达在卵巢癌患者的转移灶和复发癌中以及上皮性卵巢癌和子宫内膜癌化疗后得到维持。这些特性表明,FOLR1可以为全身性施用的T细胞重定向疗法的肿瘤特异性激活提供有前景的靶抗原。
发明内容
本公开提供了双特异性T细胞接合剂(BiTE)形式的FOLR1xCD3双特异性抗体FR1-V4-LLG-P329G-1.1和FR1-V4-LFLE-P329G。多种功能测定已证明双特异性T细胞接合剂形式的工程化FOLR1xCD3双特异性抗体对多种癌症(特别是FOLR1阳性癌症,例如结肠癌、肾癌、结直肠癌、肺癌、胃癌、卵巢癌、子宫内膜癌、输卵管癌、尿路上皮癌、乳腺癌、胰腺癌、前列腺癌、皮肤癌、头颈癌、脑癌、膀胱癌和肝癌)具有有效的抗肿瘤作用。

免疫球蛋白的重链可分为三个功能区:Fd区、铰链区和Fc区(可结晶片段)。Fd区包含VH和CH1结构域,并与轻链结合形成Fab (抗原结合片段)。Fc片段负责免疫球蛋白效应功能,包括例如补体结合和与效应细胞的同源Fc受体结合。在IgG、IgA和IgD免疫球蛋白类别中发现的铰链区充当柔性间隔区,允许Fab部分相对于Fc区在空间中自由移动。铰链结构域在结构上是多样的,在免疫球蛋白类别和亚类之间的序列和长度都不同。
根据晶体学研究,免疫球蛋白铰链区在结构和功能上可进一步细分为三个区域:上铰链、核心铰链和下铰链(Shin等人, Immunological Reviews 130:87, 1992)。上铰链包括从CH1的羧基末端到铰链中限制运动的第一个残基的氨基酸,通常是在两条重链之间形成链间二硫键的第一个半胱氨酸残基。上铰链区的长度与抗体的片段柔性相关。核心铰链区含有重链间二硫键。下铰链区连接CH2结构域的氨基末端,并包括CH2结构域中的残基。人IgG1的核心铰链区含有当通过二硫键形成二聚化时会产生环状八肽的序列,据信环状八肽充当枢轴,从而赋予柔性。免疫球蛋白铰链区多肽序列的结构和柔性允许的构象变化可以影响抗体Fc部分的效应功能。
“轻链可变区”(VL)或“重链可变区”(VH)由通过三个“互补决定区”或“CDR”分隔的“框架”区组成。框架区用于对齐特异性结合抗原表位的CDR。CDR包括抗体中主要负责抗原结合的氨基酸残基。VL结构域和VH结构域从氨基端到羧基端都包含以下框架区(FR)和CDR区:FR1、CDR1、FR2、CDR2、FR3、CDR3和FR4。VL结构域的CDR1、CDR2和CDR3在本文中也分别称为LCDR1、LCDR2和LCDR3;VH结构域的CDR1、CDR2和CDR3在本文中也分别称为HCDR1、HCDR2和HCDR3。

术语“表位”是指抗原上抗体结合的位点。表位可以由连续的氨基酸或由一个或多个蛋白质的三级折叠并置的非连续氨基酸形成。由连续氨基酸形成的表位(也称为线性表位)通常在变性溶剂的暴露中保留,而由三级折叠形成的表位(也称为构象表位)通常在变性溶剂的处理中丢失。表位通常包括独特的空间构象中的至少3个,更通常地至少5个或8-10个氨基酸。表位限定了抗体的最小结合位点,并因此是抗体或其抗原结合片段的特异性靶标。

双特异性抗体也可以用放射性标记的氨基酸标记。放射性标记可以用于诊断和治疗目的。例如,放射性标记可用于通过X射线、发射光谱或其他诊断技术检测靶抗原的表达。多肽标记的实例包括但不限于以下放射性同位素或放射性核苷酸:3H、14C、15N、35S、90Y、99Tc、111In、125I、131I。

生成稳定的细胞系HT1080-FR1以促进体外和体内功效研究。简而言之,将商业FOLR1重组质粒pCMV-FOLR1 (Sino Biological)用Lipofectamine™ LTX试剂和PLUS™试剂(Thermo)和转染特异性培养基Opti-MEM™ I(Gibco)瞬时转染到HT1080细胞中。细胞培养物随后补充潮霉素B以选择阳性克隆。2-3周后,逐渐分离出单个阳性克隆,并用流式细胞术进行验证。获得FOLR1阳性稳定细胞系HT1080-FR1。

实施例1.FOLR1×CD3双特异性抗体的构建和初步表征
双特异性T细胞接合剂(BiTE)是一类新型的双特异性抗体,其通过同时结合肿瘤抗原和T细胞抗原如T细胞表面CD3分子,能够引导细胞毒性T细胞杀伤癌细胞。
为了生成FOLR1×CD3 BiTE,将抗FOLR1抗体的单链Fv (scFv)经由接头融合至抗CD3 Fab的VL结构域的N端。将抗CD3 Fab进一步融合至单体Fc (例如,mFc7.2)的N端。单体Fc7.2含有能够抑制Fc同二聚化的两个氨基酸突变(T366L和Y407H),如PCT申请号PCT/US2018/016524中所述,该申请通过整体引用并入本文。FOLR1×CD3 BiTE的轻链由抗FOLR1scFv、抗CD3 VL、CL、铰链区、CH2和CH3结构域组成。重链由抗CD3 VH、CH1、铰链区、CH2和CH3结构域组成(图1)。两种FOLR1×CD3 BiTE,分别称为FR1-V4-LFLE-P329G和FR1-V4-LLG-P329G-1.1,基于不同的铰链区、CH2和CH3结构域构建。为了获得全长轻链,通过融合克隆将抗FOLR1 scFv片段克隆到含有抗CD3 hSP34 VL-CL和完整工程化Fc的pBY质粒中。将重链构建到单个载体pBY中用于在哺乳动物细胞中表达。
将含有重链和轻链基因的两种质粒共转染至293FS或CHO-S细胞。将质粒和转染剂PEI以1:3的比例混合,然后滴加到293FS或CHO-S细胞培养物中。转染后细胞继续生长5-7天。通过以8000 rpm离心20分钟收获细胞培养物。将含有靶蛋白的培养物上清液加载到蛋白A Sepharose 4 Fast Flow柱(GE Healthcare)上。根据制造商的说明进行后续纯化。
FOLR1×CD3 BiTE在瞬时转染的293 free style (293FS)或CHO-S细胞中表达良好,并分泌到培养上清液中。在非还原SDS-PAGE上,FR1-V4-LFLE-P329G的表观分子量(aMW)为约127 kDa,而FR1-V4-LLG-P329G-1.1的表观分子量(aMW)为约128 kDa。
根据Kabat编号系统,FR1-V4-LFLE-P329G和FR1-V4-LLG-P329G-1.1的CDR序列如表1所示。FR1-V4-LFLE-P329G和FR1-V4-LLG-P329G-1.1的轻链可变区(VL)和重链可变区(VH)的氨基酸序列如表2所示。FR1-V4-LFLE-P329G和FR1-V4-LLG-P329G-1.1的全轻链和重链序列如表3所示。

实施例2.FOLR1×CD3 BiTE与重组FOLR1和CD3的结合
根据标准方案进行ELISA,以确定FOLR1×CD3 BiTE对人FOLR1和人CD3的结合亲和力。简而言之,将人抗原CD3、His标签或人FOLR1包被在Corning EIA/RIA高结合96孔板(Corning)上,每孔100 ng,4°C过夜,并用3%脱脂牛奶封闭PBS (pH7.4)。加入从50 μg/mL五倍连续稀释的抗体,并在室温下孵育2 h。用含有1%脱脂牛奶的PBS清洗板。通过抗组氨酸标签抗体(HRP)(Sigma)检测结合的抗体。
该测定在室温下用TMB底物(Solarbio)显色,并用酶标仪在450 nm处检测。通过将数据拟合到Langmuir吸附等温线来计算半数最大结合(EC50)。结果显示于图2-3中。
结果表明,FR1-V4-LFLE-P329G结合人FOLR1的EC50为1614 ng/mL (图2),结合CD3的EC50为381.8 ng/mL (图3),FR1-V4-LLG-P329G-1.1结合人FOLR1的EC50为2835 ng/mL(图2),结合CD3的EC50为669.2 ng/mL (图3)。这些结果表明FR1-V4-LFLE-P329G和FR1-V4-LLG-P329G-1.1可以在体外和体内发挥良好的功能。
实施例3.FOLR1×CD3 BiTE与细胞表面相关FOLR1的结合
为了测量FOLR1×CD3 BiTE与细胞表面相关FOLR1的结合能力,使用FOLR1阳性细胞系SKOV3和HT1080-FR1进行了流式细胞术。约5×105个细胞与不同浓度的抗体(100 μg/mL、20 μg/mL、4 μg/mL、0.8 μg/mL、160 ng/mL、32 ng/mL、6.4 ng/mL、1.28 ng /mL、0.256ng/mL、0 ng/mL)在冰上孵育1 h。细胞用含有0.5%牛血清白蛋白的PBS (PBSA)洗涤一次,并重悬于100 µl PBSA中。然后加入1 µl/测试抗人IgG (γ链特异性)-R-藻红蛋白抗体(Sigma)并孵育30分钟。细胞用PBSA洗涤一次,然后用于流式细胞术分析。结果显示于图4A-4B中。
结果表明,FR1-V4-LFLE-P329G和FR1-V4-LLG-P329G-1.1均以浓度依赖性方式与SKOV3和HT1080-FR1细胞系结合良好。
实施例4.FOLR1×CD3 BiTE介导的T细胞活化
FOLR1×CD3 BiTE在靶细胞(HT1080-FR1、SKOV3、HT1080)存在的情况下激活人T细胞的能力和特异性通过使用带有TCR/CD3效应细胞(Jurkat-NFAT-CD3)的Bio-GloTM萤光素酶测定系统评估。在三个测试的靶细胞系中,HT1080细胞不表达人FOLR1,而SKOV3细胞和FOLR1转染的稳定细胞HT1080-FR1均具有高水平的FOLR1表达。TCR/CD3效应细胞(Jurkat-NFAT-CD3)表达内源性TCR和CD3受体。当效应细胞(Jurkat-NFAT-CD3)与适当的TCR/CD3配体或抗TCR/CD3抗体接合时,TCR转导细胞内信号,导致TCR介导的T细胞活化并产生增强的荧光信号。
将靶细胞以1×104(对于HT1080-FR1)或1.5×104(对于SKOV3和HT1080)细胞的密度铺板在96孔板上,每孔100 μL RMPI 1640完全培养基过夜。去上清后,每孔加入5倍梯度稀释的50 μL抗体(FR1-V4-LFLE-P329G、FR1-V4-LLG-P329G-1.1、FR1-mAb、CD3-mAb和FR1-mAb+CD3-mAb),最大浓度为100 μg/mL。然后在每孔50 μL RMPI 1640完全培养基中以1.2×105个细胞的密度加入效应细胞(Jurkat-NFAT-CD3)。将板在加湿培养箱中于37°C孵育6小时。然后,将100 μL/孔的Stable-Lite萤光素酶测定系统溶液(Vazyme)添加到每个孔中,并在室温下避光孵育10分钟。使用SpectraMax®i3x ELISA阅读器(Molecular Devices)检测发光。结果显示于图5A-5D中。
在FOLR1阳性HT1080-FR1细胞存在的情况下,Jurkat-NFAT-CD3细胞被FR1-V4-LFLE-P329G和FR1-V4-LLG-P329G-1.1有效激活,其中EC50分别为约82.98 ng/mL和97.89ng/mL (图5A)。对于FOLR1阳性SKOV3细胞,Jurkat-NFAT-CD3细胞也被FR1-V4-LFLE-P329G和FR1-V4-LLG-P329G-1.1有效激活,其中EC50分别为约1012 ng/mL和684.8 ng/mL (图5B)。然而,在FR1-mAb+CD3-mAb组合处理组以及FR1-mAb和CD3-mAb组中没有观察到明显的荧光信号(图5B)。此外,在FOLR1阴性HT1080细胞中未检测到明显的T细胞激活信号(图5C)。当FOLR1×CD3 BiTE与Jurkat-NFAT-CD3细胞单独孵育时,会发生一定程度的非特异性激活,导致微弱的荧光信号产生,但与FOLR1阳性细胞相比存在显著差异(图5D)。
结果表明,FR1-V4-LFLE-P329G和FR1-V4-LLG-P329G-1.1均可同时结合效应细胞的CD3抗原和肿瘤细胞的FOLR1抗原,导致T细胞特异性激活。并且,FR1-V4-LFLE-P329G和FR1-V4-LLG-P329G-1.1诱导的T细胞活化显著强于mAb组,表明FOLR1×CD3 BiTE的潜在抗肿瘤活性优于mAb。
实施例5.FOLR1×CD3 BiTE介导的针对人癌细胞系的杀伤
使用FOLR1转染细胞系HT1080-FR1通过CCK8测定评估FOLR1×CD3 BiTE的体外功效,并将FOLR1阴性HT1080细胞系用作阴性对照。将5×103个靶细胞接种于100 μL RPMI1640完全培养基中,孵育约20 h。然后,加入效应细胞、人PBMC (hPBMC,每孔1×105个细胞,在50 µl RPMI 1640完全培养基中)。同时,每孔加入从4 μg/ml 5倍连续稀释的50 μL抗体。孵育后48 h,从靶细胞中去除培养基,加入100 µl含有10% CCK8的RPMI 1640完全培养基,在CO2培养箱中孵育30分钟。根据制造商的说明,使用酶标仪测量细胞杀伤活性。结果显示于图6-7中。
结果表明,FR1-V4-LFLE-P329G和FR1-V4-LLG-P329G-1.1在1.6 ng/ mL至1000ng/mL的浓度范围内,针对FOLR1阳性HT1080-FR1细胞具有有效的体外T细胞依赖性细胞毒性(CTL),其中CTL EC50分别为1.70 ng/mL和3.62 ng/mL (图6)。然而,在FOLR1阴性细胞系HT1080中未观察到显著的CTL杀伤活性(图7),表明CTL杀伤效力取决于双特异性抗体同时结合肿瘤细胞表面的FOLR1抗原和T细胞上的CD3蛋白。总之,结果表明FR1-V4-LFLE-P329G和FR1-V4-LLG-P329G-1.1都具有显著的体外杀伤作用。
实施例6.FOLR1×CD3 BiTE介导的小鼠肿瘤生长抑制
使用B-NDG小鼠评估FOLR1×CD3 BiTE的体内功效。考虑到稳定细胞系HT1080-FR1的高FOLR1表达和优异的致瘤特性,通过混合HT1080-FR1和hPBMC在B-NDG小鼠中建立肿瘤模型。将1.7×106HT1080-FR1细胞(100 μL)和1.7×106新鲜分离的人PBMC (100 μL)的总计200 μL混合物皮下接种到B-NDG小鼠的右侧腹部。这些小鼠被随机分组并在100至150 mm3的肿瘤大小下分期。阴性对照组小鼠皮下注射生理盐水,同时实验组小鼠单次静脉注射FR1-V4-LFLE-P329G 500 μg/kg或FR1-V4-LLG-P329G-1.1 500 μg/kg。小鼠每周给药三次,总共处理15天。同时,测量小鼠的肿瘤体积和体重。使用以下公式计算肿瘤生长抑制率(TGI):TGI(%)=[1-(T-T0)/(C-C0)]×100,(T和T0:分别为特定实验日和随机分组日的肿瘤体积;C和C0:对照组的相应平均肿瘤体积)。值>100%表示肿瘤缩小。结果显示于图8A-8B中。
通过以500 μg/kg的剂量施用FR1-V4-LFLE-P329G或FR1-V4-LLG-P329G-1.1,两个治疗组均显示出显著的抗肿瘤作用。FR1-V4-LFLE-P329G处理15天后肿瘤生长抑制率(TGI)达到115.28%,而FR1-V4-LLG-P329G-1.1处理15天后肿瘤发生缩小,肿瘤生长完全抑制(TGI=109.79%)(图8A)。同时,所有小鼠组的体重仅出现微小变化(图8B),表明FOLR1×CD3 BiTE在体内具有低毒性优势。
综上所述,FR1-V4-LFLE-P329G和FR1-V4-LLG-P329G-1.1均具有优异的体内外药效学功能。因此,这些FOLR1×CD3 BiTE有望进行临床研究。
