Description
以下で構成される組織再生を必要とする患者を治療するための装置:
シート。シートには直径30ミクロンを超える大きさの孔が複数空いた多孔領域がある。シートには、多孔領域の右側に第1側部領域がある。シートには多孔領域の左側に第2側部領域がある。第1側部領域には孔がないか孔がある場合でも最大のもので直径30ミクロン以下。
シート。シートには直径30ミクロンを超える大きさの孔が複数空いた多孔領域がある。シートには、多孔領域の右側に第1側部領域がある。シートには多孔領域の左側に第2側部領域がある。第1および第2側部領域は多孔領域の上に配置され、多孔領域の上に軟部組織遮断領域を形成する。軟部組織遮断領域には直径20ミクロンを超える大きさの孔はない。
管。管の第1側面には直径30ミクロンを超える大きさの孔が複数空いた多孔領域がある。管の第2側面には多孔領域の上に軟部組織遮断領域が配置される。軟部組織遮断領域には第1シートと第2シートがあり、第2シートは第1シートの上に配置される。軟部組織遮断領域には直径20ミクロンを超える大きさの孔はない。
袋。袋の第1側面には直径30ミクロンを超える大きさの孔が複数空いた多孔領域がある。袋の第2側面には多孔領域の上に軟部組織遮断領域が配置される。軟部組織遮断領域には第1シートと第2シートがあり、第2シートは第1シートの上に配置される。軟部組織遮断領域には直径20ミクロンを超える大きさの孔はない。
a. シートの一領域に直径30ミクロンを超える大きさの孔を複数開け、多孔領域を形成。
b. シートを折り多孔領域の右側上部に第1側部領域を作成。
c. シートを折り多孔領域の左側で第1側部領域の上にかぶさるように第2側部領域を作成し、第1と第2側部領域で軟部組織遮断領域を形成。軟部遮断領域には直径20ミクロンを超える大きさの孔はない。
本発明の他の機能によれば、第2側部領域にある連結部は第1側部領域のスロットに挿入されている。
a. 舌骨膜組織を下顎骨舌側面から切り取り、舌骨膜組織と下顎骨舌側面の間に舌開口部を形成。
b. 舌開口部を通る骨膜下トンネルを形成。骨膜下トンネルは舌開口部から下顎骨の一部と骨膜組織の一部に沿って伸びる。
c. 舌開口部を通して骨膜下トンネルの中に袋を挿入。袋には直径30ミクロンを超える大きさの多孔領域がある。袋には軟部組織遮断領域があり直径20ミクロンを超える大きさの孔はない。袋の中には骨増生材がある。袋の多孔領域は下顎骨の少なくとも一部に向けて配置され、軟部組織遮断領域は骨膜組織の少なくとも一部に向けて配置される。
d. 舌開口部を閉じ、下顎骨の一部に隣接したまま袋を少なくとも数日間置いておき、袋の孔を通して骨組織の内部成長を可能にする。
骨膜下トンネルの中に袋が挿入される際、心膜のような引き裂き抵抗のあるメンブレン(例えばドイツのボティス社のジェイソンメンブレン)または圧縮されたコラーゲン(例えばスイスのガイストリッヒ社のバイオガイド圧縮)を使うと有利である。
顎の中で「粘膜骨膜組織」という用語は歯茎および/または歯肉のことも指している。
孔12は円形あるいはその他の形でよい。孔12は中央多孔領域11の面の少なくとも30%を占めることができる。孔12は中央多孔領域11の面の少なくとも50%および/または中央多孔領域11の面の少なくとも70%および/または中央多孔領域11の少なくとも90%を占めることが望ましい。
容器30に充填する方法としては、骨増生材を容器の中に注ぐおよび/またはスプーン状の装置を使うおよび/または注射器やじょうご、棒および/またはその他容器30の中に挿入できる装置を使うことが考えられる。
容器はオープンサイナス増生手術とも呼ばれるオープンサイナスリフト手術で使うことができる。上顎洞の側壁に開口を開けシュナイダーメンブレンを持ち上げた後、容器を外側壁の開口を通して挿入し、上顎洞の底面上かつ持ち上げられたシュナイダーメンブレンの下に配置することができる。容器を骨増生材で既に満たした状態で挿入および/または上顎洞の中に入れた後に充填することができる。上顎洞の側壁開口部および/またはシュナイダーメンブレンおよび/またはシュナイダーメンブレンの裂け目にメンブレンが2層ある側を向けたかたちで容器を配置できる。
図15は、充填された容器150と、図14の容器の端部開口部143が閉じられた様子と、図14の容器の側面開口145が第2格納式カバー139によって覆われた様子を示す。側面開口145はまた、第2格納式カバー139によって覆われるため図15には示されていないが、第1格納式カバー138によって覆われることができる。容器がサイナスリフト手術で使われる場合は、上顎洞壁の開口部を通して利用できるよう側面開口を設けることができる。
Claims (13)
- 以下で構成される組織再生を必要とする患者の治療のための装置:
直径30ミクロン以上の孔がいくつかあいた多孔領域を有するシートを有し、
前記シートには、前記多孔領域の右側に第1領域があり、前記シートの右端から前記多孔領域と前記第1領域の間にある第1折り目まで伸び、
前記シートには、前記多孔領域の左側に第2領域があり、その前記シートの左端から前記多孔領域と前記第2領域の間にある第2折り目まで伸び、
前記第1領域には孔がないか孔がある場合でも最大のもので直径30ミクロン未満であり、
前記多孔領域を覆うように前記第1領域が前記多孔領域にかぶさり、
前記第1領域を覆うように前記第2領域が前記第1領域にかぶさり、前記シート左端が前記第1折り目に隣接するものである。 - 前記第1および第2領域が一体となり軟部組織遮断領域を形成し、平行な2層が少なくとも前記多孔領域の過半を覆い、
前記軟部組織遮断領域には孔がないか孔がある場合でも最大のもので直径30ミクロン未満である、請求項1に記載の装置。 - 前記孔の直径が800~2500ミクロンである、請求項2に記載の装置。
- 前記孔の占める割合が前記多孔領域の70%を超える、請求項2~3のいずれかに記載の装置。
- 前記軟部組織遮断領域の孔が最大のもので直径10ミクロン未満である、請求項2~4のいずれかに記載の装置。
- 前記シートの少なくとも一部分が心膜である、請求項2~5のいずれかに記載の装置。
- 前記第1領域が前記第2領域と連結する、請求項2~6のいずれかに記載の装置。
- 前記第1領域が前記第2領域と非連続的に連結し、前記第1と第2領域の連結部分の間に前記第1と第2領域の非連結部分を有する、請求項2~7のいずれかに記載の装置。
- 前記第1領域にスロット、前記第2領域に連結部があり、前記第2領域の前記連結部の一部の幅が前記第1領域にある前記スロットの長さよりも大きいものであり、
前記第2領域にある前記連結部の前記一部が前記第1領域の前記スロットに挿入されたものである、請求項2~8のいずれかに記載の装置。 - 前記第2領域にスロット、前記第1領域に連結部があり、前記第1領域の前記連結部の一部の幅が前記第2領域にある前記スロットの長さよりも大きいものであり、
前記第1領域にある前記連結部の前記一部が前記第2領域の前記スロットに挿入されたものである、請求項2~8のいずれかに記載の装置。 - 前記第1領域と前記第2領域の間に前記多孔領域の第1境界、前記第1領域と前記第2領域の間に前記多孔領域の第2境界があり、
前記多孔領域の前記第1および第2境界は前記多孔領域の向かい合って位置する境界にあり、
前記第1境界は前記軟部組織遮断領域と連結し、前記多孔領域の前記第2境界に隣接する詰め物用の隙間を有した袋を形成する、請求項2~10のいずれかに記載の装置。 - 境界閉鎖拡張部が前記多孔領域から前記第1および第2折り目を境界とする前記シート面を超して伸び、
前記第1領域および前記第2領域の間に前記境界閉鎖拡張部が配置され、袋を形成するものである、請求項2~11のいずれかに記載の装置。 - 前記多孔領域の境界が前記軟部組織遮断領域と連結し袋を形成し、前記袋の中に骨補強材がある、請求項2~12のいずれかに記載の装置。
