Description

Die Erfindung betrifft ein Hilfsmittel zum Befestigen einer Endoprothese mittels eines mineralischen Zementes in einem Knochen-Hohlraum.
Bisher verwendete man zum Befestigen einer Endoprothese Kunststoff-Knochenzemente, die in der feuchten Umgebung des Knochen-Hohlraumes abbinden. Dies hat jedoch den Nachteil, daß der Kunststoff-Knochenzement im Verlauf von einigen Jahren altert, so daß sich die Endoprothese lockert.
Weiterhin hat die Verwendung von Kunststoff-Knochenzementen mehr oder weniger starke Vergiftungserscheinungen zur Folge, die das umliegende Gewebe zerstören und schlimmstenfalls sogar zum Tode des Patienten führen können.
Auch entwickelt der Kunststoff-Knochenzement •beim Abbinden eine Wärme von 70 bis 80° C in der Grenzfläche zum Knochen; bei dieser Temperatur wird das Zellgewebe des Knochens zerstört und muß Während eines mehrmonatigen Heilungsprozesses wieder hergestellt werden.
Andererseits ist es bekannt, mineralische Zemente Zu benutzen, die den Nachteil haben, daß sie nur in trockener Umgebung abbinden. Beispielsweise verwendet man in der Baustoffindustrie Bauzemente Und in der Zahnmedizin Zinkphosphat-Zemcnte, Silikat-Zemente, Stein- oder Silico-Phosphat-Zemente. All diese mineralischen Zemente wären im Prinzip turn Befestigen von Endoprothesen in Knochen-Hohlräumen verwendbar, haben jedoch den Nach- •eil, daß das in den Knochen-Hohlraum nachfliegende Blut sowie sonstige Körperflüssigkeiten das Abbinden der mineralischen Zemente slören.
Der Erfindung liegt die Aufgabe zugrunde, ein Hilfsmittel anzugeben, welches · die Verwendung mineralischer Zemente zum Befestigen einer Endoprothese in einem Knochen-Hohlraum ermöglicht
Diese Aufgabe wird erfindungsgemäß durch ein Hilfsmittel gelöst, welches gekennzeichnet ist durch eine in biologischer Umgebung resorbierbare Folie. Diese im Handel erhältliche Folie wird in den Knochen-Hohlraum eingelegt, wo sie mindestens für die Dauer des Aushärtens des Zementes diesen Hohlraum sicher gegenüber dem Knochen abdichtet, aus welchem Blut oder sonstige Körperflüssigkeiten austreten.
Die erfindungsgemäße Folie ermöglicht die Verwendung bisher nicht verwendbarer Zemente, die über längere Zeiträume hinweg besser mit dem Knochengewebe verträglich sind und weitaus weniger altern, d.h. im Laufe vieler Jahre nicht brüchig werden. Auch könnte man dank der erfindungs2emäßen Folie mineralische Zemente verwenden, die im Laufe von einigen Jahren vom Körper abgebaut und durch körpereigenes Gewebe ersetzt werden; die Verwendung derartiger Zemente scheiterte bisher daran, daß ihr komplizierter Abbindemechanismus durch die Gewebeflüssigkeit, beispielsweise das Blut, gestört wurde.
Vorteilhaft ist die Folie entsprechend der Form des Knochen-Hohlraumes, beispielsweise als Säckchen, vorgeformt.
Auch empfiehlt es sich, die Folie mit einem vorstehenden Rand zu versehen, damit dieser den die öffnung des Knochen-Hohlraumes umgebenden Knochenbereich abdeckt und gegen die Berührung mit dem mineralischen Zement schützt. Bei einem Säckchen ist dieser vorstehende Abdeck-Rand dann also an der öffnung angeordnet.
Die Erfindung wird in den Zeichnungen am Beispiel der Anwendung bei einem Femur-Kopfteil einer Hüftgelenkendoprothese erläutert:
In F i g. 3 ist der obere Teil des Oberschenkelknochens dargestellt. Dabei wurde bereits der Femur-Kopf abgetrennt und der Markraum zum Einbringen der Prothese geöffnet. Mit 1 ist der Knochen bezeichnet und mit 2 der Knochen-Hohlraum. Nach dem öffnen des Markraumes und seiner korrekten Ausformung mittels eines Raspatoriums wird die Folie 3 (Fig. 2) in den Knochen-Hohlraum2 eingelegt. Um einen straffen Sitz der Folie 3 zu erzielen, kann ein. weiteres Hilfsmittel, z. B. eine bekannte Probierprothese, benutzt werden. Der überstehende Rand 4 der Folie 3 wird auf die Fläche 5 des Oberschenkelknochens 1 aufgelegt. Der Innenraum der Folie 3 wird anschließend wie bisher üblich mit Zement gefüllt. Allerdings wird, im Gegensatz zur bisherigen Operationstechnik, hierfür ein mineralischer Zement benutzt. Im richtigen Zeitpunkt vor dem Aushärten des Zementes wird der Schaft6 der in Fig. 1 dargestellten Prothese in den Hohlraum 2 eingeführt und dort mit den bisherigen Methoden zum festen Sitz gebracht.

Claims (4)

Patentansprüche:
1. Hilfsmittel zum Befestigen einer Endoprothese mittels eines mineralischen Zementes in S einem Knochen-Hohlraum, gekennzeichnet durch eine in biologischer Umgebung resorbierbare Folie (3) zur Einlage in den Knochenhohlraum (2),
2. Hilfsmittel nach Anspruch 1, dadurch gekennzeichnet, daß die Folie entsprechend der Form des Knochen-Hohlraumes (2), beispiels weise als Säckchen (3), vorgeformt ist
3. Hilfsmittel nach Anspruch 1 oder 2, dadurch gekennzeichnet, daß die Folie einen Rand (4) aufweist, der über den Knochen-Hohlraum (2) hinausreicht
4. Hilfsmittel nach Anspruch 2, dadurch ge kennzeichnet, daß das Säckchen (3) an seiner öffnung einen vorstehenden Rand (4) aufweist.
