Description
給によって広く用いられてきた5。しかし、いくつかの報告では、少ないパーセントのルテインしか黄斑に到達しないことが示されている6〜8。さらに、眼関門による限界のために、後眼部の治療的治療は困難である。眼は、涙膜、角膜、硝子体、血液網膜関門、及び血液房水関門によって保護されていることから、眼、特に網膜に、充分な濃度及び最小限の副作用で薬物を送達することは非常に困難である9,10。この問題を克服するために、in−situ適用が用いられてきたが、インプラントなどの徐放システムは、非常に侵襲的であり、高価である。この数年間、多くの研究の結果が、このような治療のリスクを強調している11、12。
、AMDの進行を遅延させ、網膜内皮細胞を保護することができる。眼科的使用のための様々なイオントフォレシス送達システムが作り出されており、それを用いて、ヒトの前眼部及び後眼部の両方に医薬が安全に、効果的に送達されてきた21。この技術を用いることで、高分子を含む有意な量の生物活性分子を、角膜及び強膜を通して送達することが可能である。ここで報告される研究では、ルテインエマルジョンが、イオントフォレシスにより、黄斑へ拡散送達されている22〜23。発想は、高濃度の荷電ルテインを、経強膜的に(transclerally)及び/又は経角膜的に、イオントフォレシスによって推進する最
小侵襲的方法を開発することであった。本発明者らは、強膜及び角膜イオントフォレシス適用後に、二光子顕微鏡、ラマン分光分析、及びHPLCを用いて様々な眼組織中のルテインの分布及び濃度を調べた。この手法の主たる利点は、より安全で、患者コンプライアンスがより容易である医療デバイスを利用し、頻繁な高用量の注射又は外科的インプラント術といった困難を回避することである。この手順は、手術環境の必要がなく、通常の眼の診療予約の過程で、医師の診療所にて素早く行うことができる。

物質及び方法
製剤研究. 化合物の安定性を改善するために現在用いられている様々な送達システムの中でも、リポソームは、その生体適合性、持続的放出の可能性、及び疎水性及び親水性化合物の両方を運搬する能力に起因する利点を有している16。この研究では、結晶ルテイン(Kemin Foods、FloraGLO(登録商標)結晶ルテイン ロット 1401103302)を、リン脂質90H(Lipoid GmbH、ロット 529400−2120046−12−112、CAS 308068−11−3)及びオクタデシルアミン(Sigma−Aldrich ロット BCBK6340V、CAS 1
24−30−1)を用いてリポソームにカプセル化した。脂質膜を、90Hリン脂質、オクタデシルアミン、及びCHCl3/MeOH(2:1)(Sigma−Aldrich、ロット SHBC4982V、CAS 67−66−3/Sigma−Aldrich、ロット SZBC237BV、CAS 67−56−1)に溶解したルテインを用いて作製した。溶媒を回転蒸発によって真空除去したが;溶液を、Heidolphロータリーエバポレーター(rotavapor)により40℃で真空乾燥し、ロータリーエバポレーター
の速度は、生成物のロスを引き起こす可能性のある気泡形成及び飛び散りを低減するために調節し、乾燥薄膜は、1〜2時間後に得られた。微量の溶媒をも除去するために、この薄膜を、室温で真空下に少なくとも16時間置いた。脂質膜の水和を、40〜45℃で蒸留水(超純水−AquaMaxによるMilliQ−導電率0.054uS/cm)を脂質膜へ添加して脂質を水和することによって行い、大きいリポソーム小胞を形成した。大きいリポソーム小胞のホモジナイゼーションを、Ika Works ULTRA−TURRAX T 25デジタルホモジナイザー(スタウフェン、ドイツ)を用いて達成し、リポソーム小胞のナノサイズ範囲へのサイズ低下は、大型Microfluidizer(登録商標)高流体プロセッサー(high fluid processor)M−110EHを用いて、50〜60℃及び1200バールで押出すことによって行った。このプロセスを5回繰り返した。エマルジョンの滅菌は、121℃、1気圧で20分間行った。表1は、リポソームエマルジョンの組成を示す。最終生成物のサイズ分布、ゼータ電位、重量モル浸透圧濃度、及びpHを滅菌後に分析し、表2にまとめる。

た。動物は、到着後3日間観察を続け、特に眼に注意して、病気の徴候について毎日観察した。動物は、同一環境条件下の標準的なケージに個別に収容した。温度は15〜21℃に、相対湿度は55±10%に保持した。部屋は継続的に換気した(毎時≧15空気量)。温度及び相対湿度は、継続的に制御し、記録した。動物は、12時間の明期(午前7:00から午後7:00)及び12時間の暗期の制御されたサイクルで、定期的に10〜200lxの光に暴露した(ケージ内)。実験全体を通して、動物は、餌及び水に自由にアクセス可能とした。動物には、標準的なドライペレットタイプの餌(150g/日)、LASQCdiet(登録商標)Rab−14H(LASVENDI GMBH、ゾースト、ドイツ)を与えた。定期的に分析を行った水道水を、プラスチックボトルから自由に利用可能とした。本実験計画で述べるすべての標準的な操作手順及びプロトコルは、認可された倫理委員会によるレビューを受けた。すべての動物は、実験及びその他の科学的目的に用いられる脊椎動物の保護に関する指令2016/63/UE 欧州代表者会議17、並びに眼科及び視覚研究における動物の使用に関する視覚と眼科学研究協会会議(ARVO)声明18に従って取り扱った。
有色ウサギにおけるルテインエマルジョンの眼内イオントフォレシス送達. イオントフォレシスによるルテイン送達の能力を評価するために、発明者らは、ルテインを有するリポソームを作製し(正に荷電)、このエマルジョンを、2.0mAで10分間、有色ウサギの角膜/強膜に適用した。イオントフォレシスによる送達の効果を、採取した組織の目視検査によって評価した。電流あり及びなしでのルテインエマルジョン適用後の結果を表4にまとめる。
世界中で65歳以上の人のおよそ10%がAMD疾患に罹患している19。様々な試験から、ルテインのAMD進行の遅延剤としての可能性、及び神経保護分子としての可能性も示されている13、20〜22。さらに、ルテインは、固有の黄斑指向性を有する眼の天然成分であり、それが先天的に存在する傍中心窩領域に特異的に沈積される1。これらの特徴は、AMDの制御に用いられる現行品に対する利点であり得る。この病態に利用可能である治療は、副作用を有し、患者にとって困難であり、高価である眼内注射を含み、従って、より安全で有効な治療の開発が極めて重要である。この数年の間、多くの研究結果により、硝子体内注射のリスクが強調されてきた。硝子体内注射によって薬物を高頻度で投与する必要があることにより、網膜剥離、眼内炎、及び眼圧上昇が引き起こされ得る。非感染性及び感染性炎症の両方が、硝子体内注射の合併症として報告されている。その
使用承認以来、硝子体内注射の比率の高まりと共に、感染性眼内炎の発生率が広く研究されてきた23、23。本研究において、本発明者らは、ルテインをin−situで送達するための最小侵襲的技術を初めて試験した。イオントフォレシスは、最小侵襲的方法であるという利点を有し、従って、外科的インプラント術又は頻繁な高用量の硝子体内注射といった困難を回避することでから、より安全であり、患者コンプライアンスの改善がより容易である12。実際、様々な前臨床及び臨床試験から、繰り返し行われた眼内イオントフォレシス適用の安全性が報告されている14、25、26。別の利点は、この方法が、それほど高価ではなく、手術環境の必要がなく、通常の眼の診療予約の過程で、医師の診療所にて素早く行うことができることである。様々な研究が、能動的(active)角膜移植片拒絶反応の管理27、ドライアイ疾患の治療14、28、非感染性前部ぶどう膜炎15、及び円錐角膜疾患29を例とするヒトの眼疾患の治療のためのイオントフォレシスの使用を確立した。
物質及び方法
製剤研究. 眼組織中へのより高い浸透のために、負に荷電した粒子よりも正の粒子の方が、イオントフォレシス適用のためのドラッグキャリアとしてのより良好な候補であることが示されている38。さらに、電界は、正に荷電した分子を強制的に眼膜(負に荷電)中へと移動させる39。本研究では、発明者らは、ヒトの眼に存在する膜が、生理学的pHにおいて負に荷電しており、イオントフォレシス適用によって送達されるべき正に荷電したルテインのエマルジョンの開発に適しているという事実を利用した。ルテインが高い分子量で、親油性であり、水に不溶な分子であることから、修飾を行わずにこのカロテノイドをイオントフォレシスで送達することは、ほぼ不可能である1。これを克服するために、ルテイン分子のキャリアとして挙動する正に荷電したリポソーム小胞で製剤を作製した(Lipo+)。脂質膜を、phospholipon 90H(Lipoid GmbH、ロット 529400−2120046−12−112、CAS 308068−11−3)、オクタデシルアミン(Sigma−Aldrich ロット BCBK6340V、CAS 124−30−1)、結晶ルテイン(Kemin Health、FloraGLO(登録商標)結晶ルテイン ロット 1401103302)を用いて作製した。4〜5Lの作製のために、これらの化合物を、500〜800mLのCHCl3/MeOH(1:1 体積/体積)(Sigma−Aldrich、ロット SHBC4982V、CAS 67−66−3/Sigma−Aldrich、ロット SZBC237BV、CAS 67−56−1)中に、30〜35℃に加熱することによって溶解した。表5の製剤組成を参照されたい。溶媒を回転蒸発によって真空除去したが;溶液を、Heidolphロータリーエバポレーターにより40℃で真空乾燥し、乾燥薄膜は、1〜2時間後に得られた。微量の溶媒をも完全に確実に除去するために、この薄膜を、室温で真空下に少なくとも16時間置いた。有機溶媒の含有量をガスクロマトグラフィ(GC)で分析し、25ppm未満であることを確認した。脂質膜の水和を、65℃で蒸留水(超純水−AquaMaxによるMilliQ−導電率0.054uS/cm)を脂質膜へ添加することによって行い、大きいリポソーム小胞を形成した。これらの大きいリポソーム小胞のホモジナイゼーションを、Ika Works ULTRA−TURRAX T

Lipo+分光特性. 本実験では、二光子励起波長を決定するために、Lipo+溶液の吸光度及び蛍光スペクトルを最初に取り扱った。吸収スペクトルは、300から750nmでトレースし、図1に示す。Lipo+溶液は、370nmに吸収ピークを示し、これを、480から650nmのLipo+蛍光スペクトルのトレースのための励起波長として用いた(図2)。Lip+は、500〜530nm及び540〜570nmの2つの蛍光バンドピークを示した。これらの結果に基づいて、二光子実験のために選択したフィルターは、550/88であった。

前記カロテノイドが、ルテイン及びゼアキサンチンから成る群より選択され、
前記リポソームが、正のゼータ電位を有する、
組成物。 - 前記リポソームが、さらにオクタデシルアミンを含有する、請求項1に記載の組成物。
- 眼疾患又は眼機能障害を治療、寛解、又は予防するための、請求項1または請求項2に記載の組成物。
- 眼疾患若しくは眼機能障害を治療、寛解、又は予防することが、加齢黄斑変性の発症を遅延することである、請求項3に記載の組成物。
- 前記眼疾患又は眼機能障害が、加齢黄斑変性である、請求項3に記載の組成物。
- 健康な対象に適用される、請求項3に記載の組成物。
