Description
本出願は、2012年11月5日に出願された米国仮出願第61/722,633号および2013年3月15日に出願された第61/794,367号に基づく優先権および利益を主張し、これらそれぞれの内容が本明細書によって本明細書中で参考として援用される。

一つの局面において、本発明は、インビボで血液がんの幹細胞を排除する方法であって:(A)血液がんの患者に、有効量の抗がん剤を投与することと;(B)この患者に有効量のシグナル伝達兼転写活性化因子5(STAT5)アンタゴニストを投与することと、を含む方法を提供する。
本発明の実施形態において、例えば以下の項目が提供される。

K562細胞に対する2つの異なるSTAT5アンタゴニストの抗増殖効果が、標的特異性の指標の一つである段階的用量反応曲線に従うことを初めて決定した(図5a)。特異性は、PPARγのmRNAに対するshRNAを含有することによってさらに支持され(図5b)。その後、健康な提供者からの臍帯血CD34+細胞に、p210 BCR−ABLを発現するレンチウイルスベクターを形質導入した。

上記のインビトロの研究から、薬理学的用量でのピオグリタゾンは、PPARγ/STAT5経路の活性化を通じてBcr−Abl陽性細胞株K562の細胞増殖を阻害することを実証した。さらなるインビトロの研究を実行し、これは、BCR−ABLが発現されているとき、ピオグリタゾンの存在下で、コロニー形成細胞(CFC)における減少の度合いは1.9倍大きい(p<0.002)ということを示した(図1a)。ピオグリタゾンとイマチニブとを組み合わせることは、3倍(p<0.0001)の阻害的効果をもたらした(図1a)。

そのCD34+細胞が90%より多くPh1+であった診断時の慢性期(CP)CML患者29人のコホートをテストした。観測されたクローン原性欠損は、実施例2において記載したそれと同様であったが、上記薬物の組み合わせの効果は、より明白であった(6倍、p<0.0001)(図1b)。サイトカインの非存在下でのCP−CML CD34+細胞の液体培養において、類似の傾向が観察されたが、単一の薬について低い反応を示した人および高い反応を示した人(7人の患者が試験された)がいた(図1cおよび表1)。しかし、イマチニブとピオグリタゾンとを組み合わせたとき、反応はより大きく、そしてより小さく分散した(図1c)。
イマチニブ単独は、CP−CMLのLTC−ICの頻度を有意に減少させることができなかった(p=0.067およびp=0.02)。しかし、ピオグリタゾンは、単一の薬として2.4倍(p=0.008)だけか、またはイマチニブの存在下で相乗効果によって3.5倍(p<0.001)だけもしくはダサチニブの存在下で相乗効果によって5倍((p=0.0038)だけか、のいずれかでそうすることができた(図2aおよびb)。ピオグリタゾンは、静止期から出ること、そしてより大きな速度の細胞増殖を引き起こすことによって細胞周期の状態に対してイマチニブと反対の効果を有した(図2c−e)。さらに、ピオグリタゾンとイマチニブとを組み合わせることは、細胞周期の状態に対するイマチニブの有害な効果を大いに打消し、そしてそのために相乗的に効果的に作用して増殖細胞と非増殖細胞との両方を枯渇させた(図2dおよびe)。

CMLのLSCに対するピオグリタゾンの活性を媒介する可能な分子経路を調査した。シグナル伝達兼転写活性化因子5(STAT5)は考慮すべき標的であった。PPARγは、STAT5(AおよびB)の負の転写調節因子であり得、そしてHIV感染宿主およびSIV感染宿主においてクローン原性欠損を引き起こし得る。STAT5(AおよびB)は、対立遺伝子用量依存的様式においてマウスにおける正常HSCの維持およびフィットネス(fitness)のために決定的であり得る。CML細胞において、STAT5の活性はBCR−ABLキナーゼの活性に依存するが、STAT5発現のノックダウンまたはこれのリン酸化の阻害は、インビトロでのCML細胞のアポトーシスに対する非依存性およびサイトカイン独立性を抑制する。STAT5の抑止は、そうでなければBCR−ABLのレトロウイルスの導入によってもたらされるCML状態の開始および維持を防ぎ得る。STAT5の発現レベルは、マウスモデルにおける疾患の進行の度合い、およびヒトにおける初期TKI感受性の度合いと相関する。

10日間、ピオグリタゾンとともにかまたはなしかで、イマチニブに曝露させた後、CMLのLSCに関係するSTAT5および/またはPPARΓの推定上の下流転写標的7つのmRNA発現レベルを用いて、10人の患者からのCP−CMLのCD34+細胞のサイトカインなしでの培養物を検査した。これらはSTAT3、SIRT1、GLUT1、OCT1、PML1、MDR1およびALOX5を含む。OCT1のmRNAレベルだけが、イマチニブ単独と比較してピオグリタゾン+イマチニブの存在下での培養の後、中程度に(3.75倍;p=0.049)増加させられることを見出した(図6)。OCT1発現の上方制御はイマチニブの細胞取り込みを増加させ得るが、CMLのLSCは、BCR−ABLキナーゼ活性に軽度に依存していることが公知である。
図4は、CML患者におけるピオグリタゾン誘導CMRおよび退薬後の維持を示す。国際(international)(IS)比での患者の有核血液細胞についてのBCR−ABL/ABLのためのRT−qPCRアッセイ。3人の患者は、様々な期間(水平方向の矢印)でt=0でピオグリタゾンを受けて、継続的なイマチニブ治療(灰色のブロック)にもかかわらず3から6年の間BCR−ABLのmRNA+血液細胞が持続して存在することを示した。ピオグリタゾン処置は18から54か月前に停止したが、患者1は糖尿病のために短期間処置を再び続けた。患者2は1年間のCMRの後に退薬した。患者3はまた6か月間分子的再発なくイマチニブ処置を停止した。全ての患者は、12より多くの月から56か月の間にCMR(4.5log)に到達した。
27人の患者を臨床試験に登録し、そして24人が評価可能であった(1人はCMR4.5なので除外し、1人の患者はMMRでないので除外し、そして1人の患者は同意退薬した)。年齢の中央値は61.6歳(24.1〜79)であり、そして組み入れ後の経過観察の中央値は13か月(9.8〜21)であった。全ての評価可能な患者は計画通りにピオグリタゾンを始めた。7人の患者(29.2%)は12か月の前にピオグリタゾンを中止し、有害事象のために中止した患者はいなかった。中止は3か月目と9か月目との間に起った。ピオグリタゾン治療期間の中央値は11.2か月であり(2.6〜15.4)、1日量の中央値は39.9mgであった。ピオグリタゾン開始の前(中央値850ng/ml)および後(中央値927ng/ml)の通しのレベル(through level)の面からイマチニブとピオグリタゾンとの間に相互作用は観測されなかった(p=ns)。主な有害事象は、3人の患者における体重増加およ体液貯留の悪化であった。3人の患者(14%)では、確認された検出不可能なレベルのBCR−ABL転写物が取得された。CMR4.5の1年間の累積発生率は57%であった。Stat5のmRNAの定量は、ベースラインの値と比較してM6およびM12での患者試料において有意に減少し、そしてクローン原性潜在力の減少もまたM6およびM12での骨髄細胞において観測された。「対照患者」を同様の特徴によって集めた(n=20)。ピオグリタゾン群における57%と比較すると(p=0.02)、この対照群におけるCMR4.5の累積発生率は27%であった。
図6は、ピオグリタゾンとイマチニブとの組み合わせに反応したCP−CML細胞における標的遺伝子の発現を示す。a,イマチニブ単独(1μM)またはイマチニブおよびピオグリタゾン(それぞれ1μMおよび10μM)のいずれかによって7日間、サイトカインなしの無血清培地中でCP−CMLのCD34+細胞を培養した。その後、ヒトOCT1、MDR1、SIRT1、STAT3、ALOX5およびPML1に特異的なプライマーおよびプローブを用いたRT−qPCRアッセイのために細胞を処理した PPARγアゴニストによって上方制御されることが公知であるCD36を正の対照として使用した。結果をGAPDHのmRNAレベルに対して正規化し、そして「イマチニブ単独」条件についてのmRNA発現に関して表した(それぞれの評価した遺伝子について11人の患者の平均およびSD)。b,GLUT1の細胞表面発現はフローサイトメトリーによって定量した(5人の患者の平均)。
イマチニブと組み合わせられたとき、STAT5の下方制御は、転写因子HIF2aの異常に高い発現を減少させ、休止期のCML幹細胞(LSC)の除去をもたらすことを示すために研究を実行した。HIF2aのsiRNAを、単独およびイマチニブとの組み合わせで、CD34+幹細胞における発現を阻害するために使用した。図9に7日目での結果を示す。HIF2a阻害とイマチニブとの組み合わせが幹細胞の有意な衰退を引き起こすことが示される。LSCは、STAT5によって制御されるHIF2aに依存するので、STAT5阻害剤(例えば、ピオグリタゾンのようなPPARアゴニスト)によるHIF2aの抑制はLSCの増殖を誘発し、イマチニブ治療の効果を増強する。
ピオグリタゾンによってSTAT5の発現を標的にすることが、インビトロで慢性骨髄性白血病(CML)細胞のクローン原性活性に影響し、インビボで分子的反応の改善をもたらすかどうかを評価するために以下の研究を実行した。
予備的なインビトロの研究で、CML患者からのCD34+初代細胞の生存率とコロニー形成能とに影響を与えるピオグリタゾンの能力を試験した。クローン原性研究およびLTC−IC研究を行った。培養したPh+−CD34+細胞をfacsによって特徴付け、そしてCSFEアッセイによって解析した。BCR−ABLおよびSTAT5の発現をリアルタイムPCRによって定量した。対照実験は、Stat5およびPPAR−γについてのレンチウイルスベクターおよびsiRNAアッセイを使用して行った。
インビトロの研究から、初めに、薬理学的用量でのピオグリタゾンはPPAR−γ/STAT5経路の活性化を通じてBcr−Abl陽性細胞系列K562の細胞増殖を阻害することが実証された。次に、PPAR−γ/STAT5経路がCML患者からのCD34+細胞においてクローン原性欠損を誘発することが示された。さらに、PPAR−γ/STAT5経路の活性化は、CMLのLTC−ICにおけるクローン原性および増殖性欠損もまた誘発した。その後、イマチニブが非感受性の休止期のCML細胞の選択を誘発することが確認され、そしてこの効果がPPAR−γ/STAT5経路の活性化によって阻害されることが示された。
本インビボ研究は、PPAR−γアゴニストがCMLのCD34+細胞におけるSTAT5の下方制御をもたらし、そしてCFCおよびLTC−ICアッセイにおいてそれらのクローン原性かつ長期の潜在力を優先的に減少させるということを示した以前のインビトロの結果を拡張した。特に、本研究は、ピオグリタゾンとイマチニブとの組み合わせを用いて処置された患者の半分より多くにおいてMMRを達成したことによってこれらの効果がインビボに変換できることを実証し、インビボで休止期のCML細胞を標的とすることが可能であり得ることを示唆し、そして幹細胞プールの衰退という考えを支持した。
Claims (20)
- 患者におけるBCR−ABL融合タンパク質の発現により特徴付けられる白血病の再発を防ぐ方法において使用するための組み合わせ物であって:該組み合わせ物は、
(A)有効量の抗がん剤であって、BCR−ABLのリン酸化を阻害するチロシンキナーゼ阻害薬(TKI)である抗がん剤と;
(B)有効量のペルオキシソーム増殖剤活性化受容体ガンマ(PPARγ)アゴニストであって、シグナル伝達兼転写活性化因子5(STAT5)の転写を阻害するチアゾリジンジオン化合物であるPPARγアゴニストと;
を含み、
該抗がん剤が、該患者における反応の安定した累積発生率が達成されるまで、PPARγアゴニストより前に投与されることを特徴とする、組み合わせ物。 - 前記チアゾリジンジオン化合物が、ピオグリタゾン、ロシグリタゾン、トログリタゾン、エングリタゾン、シグリタゾンおよびネトグリタゾンからなる群から選択される、請求項1に記載の組み合わせ物。
- 前記チアゾリジンジオン化合物がピオグリタゾンである、請求項1または2に記載の組み合わせ物。
- 前記患者が慢性骨髄性白血病(CML)または急性骨髄性白血病を患っている、請求項1〜3のいずれかに記載の組み合わせ物。
- 前記TKIが、イマチニブ、ダサチニブ、ニロチニブ、ボスチニブ、およびポナチニブからなる群から選択される、請求項1〜4のいずれかに記載の組み合わせ物。
- 前記TKIが、イマチニブである、請求項1〜5のいずれか一項に記載の組み合わせ物。
- 前記チアゾリジンジオン化合物が約15〜60mg/日の用量で投与されることを特徴とする、請求項1〜6のいずれかに記載の組み合わせ物。
- 前記チアゾリジンジオン化合物が約30〜50mg/日の用量で投与されることを特徴とする、請求項1〜7のいずれかに記載の組み合わせ物。
- 前記TKIが約300〜800mg/日の用量で投与されることを特徴とする、請求項1〜8のいずれかに記載の組み合わせ物。
- 前記TKIが約300〜400mg/日の用量で投与されることを特徴とする、請求項1〜9のいずれかに記載の組み合わせ物。
- 前記反応が、血液学的完全寛解(CHR)、細胞遺伝学的大寛解(MCR)、細胞遺伝学的完全寛解(CCR)、分子遺伝学的効果(MMR)および分子遺伝学的完全寛解(CMR)からなる群から選択される、請求項1〜10のいずれかに記載の組み合わせ物。
- 前記TKIが、前記チアゾリジンジオン化合物の投与の前に少なくとも3か月間投与されることを特徴とする、請求項1〜11のいずれかに記載の組み合わせ物。
- 前記チアゾリジンジオン化合物が前記TKIと併用して2〜12か月の間投与されることを特徴とする、請求項1〜12のいずれかに記載の組み合わせ物。
- 前記TKIの投与が、前記チアゾリジンジオン化合物の投与が中止された後に継続されることを特徴とする、請求項1〜13のいずれかに記載の組み合わせ物。
- 前記TKIが、約300〜400mg/日の用量で投与され、前記チアゾリジンジオン化合物が前記患者に約30〜50mg/日の用量で投与されことを特徴とする、請求項1〜14のいずれかに記載の組み合わせ物。
- 前記チアゾリジンジオン化合物が、ピオグリタゾン、ロシグリタゾン、トログリタゾン、エングリタゾン、シグリタゾンおよびネトグリタゾンからなる群から選択され、前記チアゾリジンジオン化合物が、CMRが達成されるまで約2〜12か月間投与されることを特徴とする、請求項15に記載の組み合わせ物。
- 前記チアゾリジンジオン化合物が、2か月の間約30mg/日の用量で投与され、そしてその後に合計12か月の間約45mg/日の用量で投与されることを特徴とする、請求項16に記載の組み合わせ物。
- 前記患者が、t(9;22)(q34;q11)相互転座(der22またはPh+染色体)およびBCR−ABL融合タンパク質の発現によって特徴付けられるCMLを有し、前記TKIがBCR−ABLのリン酸化を阻害し、約300〜400mg/日の用量で投与され、前記PPARγアゴニストがチアゾリジンジオン化合物であり、分子遺伝学的完全寛解(CMR)が達成されるまで約30〜50mg/日の用量で投与されることを特徴とする、請求項1〜17のいずれかに記載の組み合わせ物。
- (a)前記TKIが、イマチニブ、ダサチニブ、ニロチニブ、ボスチニブ、およびポナチニブからなる群から選択され、
(b)前記チアゾリジンジオン化合物が、ピオグリタゾン、ロシグリタゾン、トログリタゾン、エングリタゾン、シグリタゾンおよびネトグリタゾンからなる群から選択される、
請求項1〜18のいずれかに記載の組み合わせ物。 - 前記TKIがイマチニブであり、前記チアゾリジンジオン化合物がピオグリタゾンである、請求項1〜19のいずれかに記載の組み合わせ物。
