Description
本発明の実施形態は全般的には、動物、好ましくは哺乳動物、特にヒトにおける細胞動員に関する。実施形態は特に、例えば造血幹細胞移植(HSCT)を含めた細胞移植に使用可能な、幹細胞、前駆細胞および/または特定の白血球の動員に関する。
動員に対するデキストラン硫酸の効果の特徴を明らかにし、適切な用量、回収時間、投与様式およびプレリキサフォル(AMD3100)とG−CSF(NEUPOGEN(登録商標))を併用した現在用いられている治療法と比較した効果に関してさらに知見を得るため、マウス用いて一連の実験を実施した。
雌DBA/2マウスをLaboratories社(オランダ)およびCharles River Laboratories社(ドイツ)から入手した。全個体ともウプサラ大学の動物施設にて機関のガイドラインに従い、標準的な条件下、自由に摂餌および摂食させて飼育した。7〜40週齢、体重17〜31gの個体を用いた。いずれの実験もスウェーデン、ウプサラ市の動物倫理委員会による承認を受けたものである。
G−CSF(NEUPOGEN(登録商標)、Amgen社、オランダ)を0.3mg/mLの無菌等張水溶液として調達し、通常の生理食塩水で50μg/mLの濃度に希釈した。第−2日および第−1日の朝および晩にG−CSFを単回皮下注射として2.5μgの用量で投与した。以下に挙げる様々な平均分子量のデキストラン硫酸を用いた:
Meito−平均分子量6939Da、名糖産業株式会社(東京、日本)から入手し、クエン酸一水和物(CAM)緩衝液に溶かした;
pKC−平均分子量5139Da、pK Chemicals A/S社(コペンハーゲン、デンマーク)から入手し、CAM緩衝液または0.9%NaCl(Fresenius Kabi社)に溶かした;およびTdB−平均分子量3.3kDa、TdB Consultancy社(ウプサラ、スウェーデン)から入手し、0.9%NaCl(Fresenius Kabi社)に溶かした。
イソフルラン麻酔下、EDTAでフラッシュした(0.5M EDTA(Rudbeck研究所によって調製された)のストック溶液を0.9%NaClで1:2.5に希釈して調製した0.2M EDTA)シリンジを用いて、致死的な心臓穿刺により末梢血の検体を採取した。
イソフルラン麻酔下、EDTAでフラッシュしたシリンジを用いた致死的な心臓穿刺により末梢血の検体を採取し、EDTA 1.6mgを含むポリプロピレンチューブ(Sarstedt社、ランズクルーナ、スウェーデン)に移した。
イソフルラン麻酔下(別途明記されない限り)、EDTAでフラッシュした(上記の通りに調製した0.2M EDTA)シリンジを用いて致死的な心臓穿刺により末梢血の検体を採取した。
データを平均値±SEMで表す。スチューデントのt検定(両側、等分散)を用いて群間比較を実施した。Microsoft Excelを用いて統計解析を実施した。p値が0.05未満の差を統計的に有意であるとした。
マウス(DBA/2N、8〜14週齢、Charles River社)をデキストラン硫酸(10mg/kg、50mg/kg、150mg/kgおよび500mg/kg、pKC)、AMD3100(5mg/kg、陽性対照)またはCAM緩衝液(陰性対照)のs.c.注射により処置した。用いたAMD3100の用量および細胞回収までの1時間は、マウスで報告されていた同薬剤の最適な投与レジメンである(Broxmeyer(2005))。血液学的分析を上記の通りに用いて血液を分析した。さらに詳細に述べると、最後の注射から3時間後(AMD3100では1時間後)、マウスを屠殺し、末梢血の血液学的鑑別分析を実施した。血清および/または血漿検体を採取し、分析まで−20℃で保存した。EDTAでフラッシュしたシリンジによる心臓穿刺により血液を採取し、細胞計数分析にはEDTAおよびレピルジンと混合し、HGF分析にはEDTAのみと混合した。
DBA/2OlaHsdマウス(7〜12週齢、Harlan社)に25〜200mg/kgのデキストラン硫酸(Meito)i.v.を用いて動員を実施した。陽性対照および陰性対照としてAMD3100(5mg/kg、s.c.)またはCAM緩衝液(i.v.)を用いた。CFCアッセイおよび血液学的分析を上記に通りに用いて血液を分析した。
DBA/2Nマウス(9〜10か月齢、Charles River社)に100mg/kgのデキストラン硫酸(Meito、i.v.およびs.c.)を用いて動員を実施した。CFCアッセイおよび血液学的分析を上記の通りに用いて血液を分析した。
DBA/2Nマウス(8〜14週齢、Charles River社)に50mg/kgのデキストラン硫酸(pKC、i.v.)または100mg/kgのデキストラン硫酸(Meito、i.v.)を用いて動員を実施した。陰性対照としてCAM緩衝液(i.v.)を用いた。CFCアッセイ、血液学的分析およびHGF−ELISAを用いて血液を分析した。
アフェレーシス前の患者に実施される標準的な治療は、G−CSFを最大1週間、連日注射することを基本とするものである。臨床的にはデキストラン硫酸とG−CSFを併用することが可能である。マウスを用いた試験では2.5μg/個体のG−CSFが1日2回(8時間置き)、2日間投与された(Broxmeyer(1999))。G−CSFとデキストラン硫酸の併用の効果を検討するため、DBA/2OlaHsdマウス(10〜15週齢、Harlan社)をG−CSFで2日間処置し(NEUPOGEN(登録商標)、2×2.5μ/日、s.c.)、第3日にデキストラン硫酸(5mg/kg、25mg/kg、100mg/kg、Meito、i.v.)、CAM(陰性対照、i.v.)またはAMD3100(陽性対照、5mg/kg、s.c.)のいずれかを注射した。CFCアッセイおよび血液学的分析を用いて血液を分析した。
動物
雌DBA/2Olaマウス(Harian社、オランダ)をウプサラ大学の動物施設にて標準的な条件下、自由に摂餌および摂食させて飼育した。体重17〜22gの個体を用いた。
DBA/2雌を以下の4つのグループ:1)賦形剤(NaCl水溶液)(n=8)、2)50mg/kgデキストラン硫酸DS3(n=5)、3)50mg/kgデキストラン硫酸DS5(n=5)および4)50mg/kgデキストラン硫酸DS5 PNB(n=5)に分けた。グループ4)は、麻酔プロトコルの変化が動員に影響を及ぼすかどうかを評価するため、イソフルランの代わりにペントバルビタールナトリウム(PNB)で鎮静させた。
DS5(平均分子量5.1kDa、pKC社(デンマーク)、バッチ31497)およびDS3(平均分子量3.3kDa、TdB Consultancy社(ウプサラ、スウェーデン)、バッチ20341)を0.9%NaCl(Fresenius Kabi社)に溶かして20mg/mLとし、20μmフィルターでろ過して無菌溶液を得た。マウスに2.5mL/kg(約50μL)を尾静脈から静脈内投与した。
結果を図10および表6に示す。DS3では、WBC、リンパ球ともに全体として有意な変化が全くみられなかったのに対して、好中球にわずかな減少が認められた。

Claims (16)
- 対象の末梢血中への前駆細胞および/または幹細胞の動員のための医薬の製造のための、平均分子量の範囲が4500〜7000Daのデキストラン硫酸またはその薬学的に許容される塩の使用。
- 前記前駆細胞および/または幹細胞が、コロニー形成単位−顆粒球、赤血球、単球、巨核球、CFU−GEMMおよび赤芽球バースト形成細胞BFU−Eからなる群より選択されるコロニー形成細胞である、請求項1に記載の使用。
- 前記デキストラン硫酸またはその薬学的に許容される塩の平均分子量の範囲が4500〜5500Daである、請求項1または2に記載の使用。
- 前記デキストラン硫酸またはその薬学的に許容される塩の平均硫黄含有量が15〜20%の範囲内にあり、好ましくは約17%である、請求項1〜3のいずれか1項に記載の使用。
- 前記デキストラン硫酸またはその薬学的に許容される塩は、水性注射液として処方される、請求項1〜4のいずれか1項に記載の使用。
- 前記デキストラン硫酸またはその薬学的に許容される塩は、前記対象の体重1kg当たり0.1〜50mgの範囲内、好ましくは前記対象の体重1kg当たり1〜50mgの範囲内、より好ましくは前記対象の体重1kg当たり5〜25mgの範囲内の用量で投与されるよう処方される、請求項1〜5のいずれか1項に記載の使用。
- 前記対象はヒトであり、前記デキストラン硫酸またはその薬学的に許容される塩は、前記ヒト対象の前記血流中に前記細胞が動員される時点の前に約0時間〜約6時間以内の間隔を置いてヒト対象に投与されるよう処方される、請求項1〜6のいずれか1項に記載の使用。
- 前記デキストラン硫酸またはその薬学的に許容される塩は、前記ヒト対象の前記血流中に前記細胞が動員される前記時点の前に約0時間〜約4時間以内の間隔を置いて前記ヒト対象に投与されるよう処方される、請求項7に記載の使用。
- 前記デキストラン硫酸またはその薬学的に許容される塩は、前記対象の前記末梢血中に前記細胞を動員するのに顆粒球コロニー刺激因子G−CSFと併用するよう処方される、請求項1〜8のいずれか1項に記載の使用。
- 前記G−CSFが、前記対象の前記血流中に前記細胞が動員される時点の2〜4日前に1回または2回、好ましくはこれに加えて前記対象の前記血流中に前記細胞が動員される前記時点の当日に、前記対象に投与するよう処方される、請求項9に記載の使用。
- 前記デキストラン硫酸またはその薬学的に許容される塩は、さらに、前記対象の前記血流中に肝細胞増殖因子HGFを誘導する、請求項1〜10のいずれか1項に記載の使用。
- 平均分子量の範囲が4500〜7000Daのデキストラン硫酸またはその薬学的に許容される塩と、顆粒球コロニー形成刺激因子G−CSFとを含む、対象の末梢血中への前駆細胞および/または幹細胞の動員のための組成物。
- 水性溶媒をさらに含む、請求項12に記載の組成物。
- 薬物として使用する、請求項12または13に記載の組成物。
- 対象の末梢血中への前駆細胞および/または幹細胞の動員のための医薬の製造のための、請求項12または13に記載の組成物の使用。
- 対象の血流中への標的白血球、特にリンパ球の動員のための医薬の製造のための、請求項12または13に記載の組成物の使用。
