Description
カルガリーバイオフィルム装置のようなバイオフィルム装置(セリらによる、1999年米国特許7,041,470号)を例えば改造し、本発明の方法と共に用いて、生理的に許容できるアンチバイオフィルム組成物等を同定し、(a)経口で利用でき、(b)広く安全であると認識され(GRAS)、(c)胃を通過する間にも活性を維持することが知られているか又はそのことを実現でき、および(d)モデル組織(system)において破壊的なバイオフィルムにおいても活性である、酵素をスクリーニングすることができる。ヒトを悩ますバイオフィルムの研究のために適切な他の装置も用いられる。セリによって説明されているように、カルガリーバイオフィルム装置(CBD)によって、ウェルが96ある(またはその他の所望の適切な数の)標準的なプレートに96の等価のバイオフィルムを用い、そして当該バイオフィルムを検査しながら抗生物質にさらすことで、バイオフィルムの抗生物質に対する感受性の迅速かつ再現性よいアッセイを行うことができる。本発明において、ここに記述する通り、かかるスクリーニングバイオフィルムを、酵素が凝縮しているところに晒す。バイオフィルム形成は、例えば、量的微生物学および走査電子顕微鏡が追随する。
胃腸の表面でバクテリアが成長するには、バイオフィルム群落を形成するための構造的な支持となる、ポリサッカライドリッチな細胞外基質(matrix)を自己生成することがしばしば必要である。胃腸管内のこれらの微生物のバイオフィルム基質を崩壊させる酵素が、本発明での方法等の対象である。
細胞外基質の組成は複雑で可変性であり、異なるバクテリアの種類によって、あるいは同じ種類でも異なる環境条件のもとでは異なる。不均一な組成にも拘わらず、エキソポリサッカライドがバイオフィルム基質の典型的なコンパウンドであり、微生物細胞が挿入されるフレームワークを提供する。ここに記載する多くの異なるエキソポリサッカライドのうち、セルロースおよびβ−1,6−結合のN−アセチルグルコサミンが、多くの異なるバクテリアのバイオフィルム基質の、最も一般的な成分(コンポーネント)である。
生来のおよびバイオフィルムによる感染性の微生物の双方を含めて、対象となる代表的なバイオフィルムの微生物を以下に説明する。
腸球菌は、人体の胃腸管の通常の細菌叢の一部であるが、20年以上にわたって院内感染の重要な原因として認識されており、一般には、尿路感染症、菌血症、腹腔内のおよび外科手術による傷からの感染症、カテーテル関係の感染症および心内膜炎に関係している。
病原性のブドウ球菌は、バイオフィルムを形成し、これらの浮遊性(planktonic)のものより、抗生物質や免疫防御システムに対して示す耐性がより高いものとなっている。黄色ブドウ球菌は、院内感染と関係する一般的な病原菌である。それは臨床の現場に存続し続けて、バイオフィルムの形成を通して抗菌剤に対する耐性を高めることができる。黄色ブドウ球菌は、血流感染を引き起す主要な病原菌の1つであり、宿主の組織上および体内に留置した医療装置上にバイオフィルムを形成することができ、そこに存在し続けて病気を引き起こす。黄色ブドウ球菌が引き起した感染症は、抗生物質への耐性が高まっているために、治療するのがより困難になっている(例えば、バンコマイシンまたはメチシリン耐性のある黄色ブドウ球菌)。とりわけバイオフィルム環境においては、病原菌は抗菌剤に対してより高い耐性を示す。
人体への日和見病原体である緑膿菌は、重篤な病気の患者の間では感染に関係する主要な死因であり、あらゆるグラム陰性の感染症の中で最も高い患者死亡率の1つを維持する。伝統的には、重篤な病気の患者の間では、肺が緑膿菌感染症の主な部位であると考えられてきたが、これらの感染症の相当数は、胃腸管フローラによる、または腸から肺実質への血行性転位による気道の直接的な汚染の結果として発生している。緑膿菌は、免疫学的に危険にさらされた患者に重大な感染症を引き起こし、嚢胞性線維症患者においては、主要な病原体である。重要な病原性のメカニズムは、ムコイドバイオフィルムの形成である。分泌されるアルギン酸塩がムコイドバイオフィルムマトリックスの重要な構成要素である。しかしながら、緑膿菌のアルギン酸塩陰性突然変異種もまたムコイドバイオフィルムを形成することができ、アルギン酸塩過生成ムコイド緑膿菌により形成されるバイオフィルムとは異なる構造を示している(ニベンズDE、オーマンDE、ウィリアムズJ、フランクリンMJによる、Role of alginate and its O acetylation in formation of Pseudomonas aeruginosa microcolonies and biofilms ジャーナルオブバクテリオロジー(J Bacteriol) 2001年183巻1047〜57頁、ウォズニアックDJ、ウィコフTJ、スターキーM、ケイサーR、アザディP、オトゥールGA、パーセックMRによる、Alginate is not a significant component of the extracellular polysaccharide matrix of PA14 and PAO1AlPseudomonas aeruginosa biofilms、米国科学国立学会会報(Proc Natl Acad Sci USA)2003年、100巻、7907〜12頁)。
ヘリコバクターピロリは、世界の人工の50%に感染しているような一般的な人体の病原体の1つである。それは十二指潰瘍、胃潰瘍、胃炎および胃癌と関係している。ヘリコバクターピロリの治療は、多剤を処方したり長期の治療期間を伴うため困難である。再発率は10〜20%である。最近の研究によって、ヘリコバクターピロリ病の病原菌におけるバイオフィルムの重要性が記録されている(コティッチアJMらによる、Presence and density of Helicobacter pylori biofilms in human gastric mucasa in patients with peptic ulcer disease、ジャーナルオブガストロインテスティナルサージェリー(J Gastrointest Surg) 2006年、10巻、883〜9頁)。経口の多酵素製剤には、ヘリコバクターピコリのバイオフィルムを容易に除去し、ヘリコバクターピコリ病原体を根絶し、それによって胃炎、消化性潰瘍および胃癌の危険が低減できるという大きな期待がある。
食物を媒介とする病原菌リステリア菌は、25%を超える深刻な死亡率を有することがはっきりしている深刻な病気であるリステリア症の原因となる病原体である。リステリア菌(リステリア・モノサイトゲネス)は、冷凍温度、低pHおよび高濃度の塩分等のような広範囲の環境条件でも生存し、成長することができる。これによって、その病原体は、食物の保存や安全の限界を超えて人体の健康にとって潜在的な危険をもたらす。リステリア菌は、とりわけ、低温殺菌されていない牛乳、生野菜、生および調理された家禽の肉などの生の食物に見出される。リステリア菌は、低温で成長する能力があって、冷凍された食物においても成長することが可能である。リステリア菌は、動物における感染ともっぱら関係していると考えられていたが、最近、割合は小さいが、この病原種が腸管内に休止状態で単離存在しているヒトもいる(ルーケッテC、バーチェPによる、The pathogenesis of infection by Listeria monocytogenes、マイクロバイオロジア(Microbiologia)、1996年、12巻、245〜58頁)。
カンピロバクタージェジュニ(Campylobactor jejuni)は、動物の糞の中に広く観察される、湾曲した桿菌様のグラム陰性の微好気性バクテリアの一種である。該菌は、世界中において、人体の胃腸炎の最も一般的な原因の1つである。カンピロバクター種が引き起す食中毒は、ひどく衰弱させるものであるが、生命を脅かすことはほとんどない。それは、その後発達するギラン−バレー症候群(GBS)と結び付いており、通常最初の病状から2、3週間で発達する。汚染された食物が、単独の感染症の主要な原因であり、正しく調理されなかった肉や、家禽が、通常はバクテリア源となる。カンピロバクタージェジュニでの感染の結果、通常腸炎となり、その特徴は、腹痛、下痢、発熱および不快感である。主要な胃腸病原体カンピロバクタージェジュニは、培養液中の単種のバイオフィルムを3種類形成して存在することが示されている(ジョシュアGW、ガスリーアイアンC、カーリィシェフAV、レンBWによる、Biofilm formation in Campylobacter jejuni.、マイクロバイオロジー(Microbiology) 2006年、152巻(2部)387〜96頁)。
炭疽菌は、グラム陽性の、内生胞子形成細菌であり、肺、胃腸および皮膚の炭疽病の病因である病原体である。人間と家畜が触れ合う地方においては、皮膚の炭疽病の慢性的な症例が広く報告されている。現在、炭疽菌についてのバイオフィルムの生態の重要性が確認できるデータはほとんど知られていないが、他の病原性または非病原性のバシラス種、例えばそれぞれセレウス菌および枯草菌、においてバイオフィルムが特徴付けられている。炭疽菌は、容易にバイオフィルムを形成するが、それは、広く処方されている抗生物質に本来的に耐性がある(リーK、コスタートンJW、レイベルJ、オアーバックRK、ワグナーDM、カイムP、レイドJGによる、Phenotypic and functional characterization of Bacillus anthracis biofilms、マイクロバイオロジー(Microbiology) 2007年、153巻(6部)1693〜701頁)。
エルシニア症は、エルシニア属のバクテリアによって引き起される感染症である。米国では、ほとんどの人間の病気は、1つの種類のエルシニア・エンテロコリチカ菌によって引き起されている。エルシニア・エンテロコリチカ菌の感染は幼い子供に最も良く起こる。子供においての一般的な症状は、発熱、腹痛および下痢である。胃腸の症状は、エルシニア症が急性であっても慢性であっても一般的である。感染は、汚染された食物、とりわけ生か十分に調理されていない豚製品を食べることで最も良く起こる。汚染された、低温殺菌されていない牛乳または処置されていない水もまた感染を引き起す。
人間は一般に以下3つのうち1つの経路で感染する。ブルセラで汚染された何かを飲むか食べるかするか、病原体を吸い込む(吸入)か、皮膚の傷口を通してバクテリアを体内に入れてしまう経路である。最も一般的な感染経路は、汚染されたミルク製品を飲むか食べるかすることによる。
サルモネラ菌(Salmonella enterica)は、サルモネラ症を引き起す食物を媒介とする病原体であり、腸の上皮に侵入してそこで増殖するバクテリアを摂取することによって引き起される。サルモネラ菌は、バイオフィルムを形成することで知られており、真核細胞に付着し、その上で成長し、エキソポリサッカライドによって、成長が促進される(レデボアおよびジョーンズ、2005年)。サルモネラに感染するとたいていの人は、感染後12乃至72時間で下痢、発熱および腹部痙攣性腹痛を起こす。病状は通常4乃至7日続き、たいていの人は治療しなくとも回復する。しかしながら、人によっては下痢が非常に深刻となって、入院する必要がある。これらの患者では、サルモネラの感染は、腸から血流、そして他の体の部位まで広がり、その患者が適切に治療を受けなければ死に至り得る。
シゲラバクテリアには、いくつかの異なる種がある。ソンネ赤痢菌(Shigella sonnei)は、「グループD」シゲラとしても知られており、米国の細菌性赤痢は3分の2以上、これが原因である。細菌性赤痢は、シゲラと称されるバクテリアのグループによって引き起される感染症である。シゲラに感染するとたいていの人がそのバクテリアに晒されてから1日か2日後に下痢、発熱、腹部痙攣性腹痛が発現する。シゲラバクテリアの中には、抗生物質に対する耐性を有するものもある。第二の種類、フレクスナー赤痢菌(Shigella flexner)または「グループB」シゲラ(赤痢菌)は、残りのほとんど全ての病気の原因である。残りの種類のシゲラ(赤痢菌)は、開発途上国において病気の重要な原因であり続けている。開発途上国で観察される1つの種類は、1型志賀赤痢菌(Shigella dysenteriae type 1)であり、現地では、命に関わる伝染病を引き起している。
腸チフス菌は、潜在的には、致死的な病気である腸チフスを引き起こす。症状の無い保菌者が、胆嚢にバクテリアを保持している。チフス菌は、ヒトのみに住み付く。腸チフスにかかっている人は、血流中および腸管内にバクテリアを保持している。加えて、保菌者(キャリア)と称される少数の人は、腸チフスから回復しても、バクテリアを保持し続けるのである。該病人と保菌者の双方が、チフス菌を排泄物(大便)とともに放出する。チフス菌は、汚染された食物、水および飲料で伝染する。最近、ガラスカバースリップ上でサルモネラバイオフィルムの形成を分析するシステムが開発された(プラウティAM、シュエジンガーWH、ガンJSによる、Biofilm formation and interaction with the surfaces of gallstones by Salmonella spp.、インフェクションアンドイミュニティ(Infect Immun)2002年、70巻、2640〜9頁)。
腸管毒素原性大腸菌は、小腸を対象としているが、この部位は、より高い酸性度と蠕動運動のために原発部位の微生物叢の障壁効果(barrier effect)が低い。当該細菌は、粘液に付着してコロニーを形成し、病原性の効果(活性)を引き出す(ナットンS、ロイドDR、キャンディDC、マクネイシュASによる、In vitro adhesion of enterotoxigenic Escherichia coli to human intestinal epithelial cells from mucosal biopsies.、インフェクションアンドイミュニティ(Infect Immun)、1984年、44巻、514〜8頁)。これが意味するところは、病原体および/またはその毒性(病原性)は、露呈されている腸吸収細胞に直ちに付着し、宿主に侵入することができることである。
コレラ菌は、グラム陰性の、条件的(facultive)病原菌であり、コレラの原因となる病原体であり、コレラは、毎年開発途上国で何百万もの人々を苦しめている破滅的な下痢を引き起こす病気である。該菌は、貯水池におそらくはバイオフィルムの形で生存している。
赤痢アメーバによって引き起される侵襲的な腸アメーバ症は、栄養体が結腸の粘膜層に付着し、粘膜や粘液の崩壊および/または枯渇および宿主の上皮の炎症を起こしている細胞への付着および細胞崩壊で開始する。腸アメーバ症の現在の実用モデルでは、宿主の腸の微環境、とりわけ腸のムチンやバクテリアのバイオフィルムが病原体のアメーバの挙動に影響を及ぼすことが示唆されている。バクテリアのバイオフィルムを崩壊させる酵素がアメーバ症の抑制および治療に有用である。
多酵素製剤抗バイオフィルム活性
セルラーゼ−2000CU、グルコアミラーゼ50−AGU、ヘミセルラーゼ/ペクチナーゼ−300HSU、ベータ−グルカナーゼ−100BGU、w/DPP−IV活性を有するプロテアーゼ/ペプチダーゼ複合体−100,000HUT、チトサナーゼ−100ユニット、リゾチーム−200,000SHUおよびセラチアペプチダーゼ−1000ユニットからなる多酵素製剤で最初の実験を行った。これらの活性のある酵素を20mgのLロイシンを含む500mgの混合物に含有させた。その多酵素製剤を、50mg/mLから0.34mg/mLの一連の希釈度でテストした。希釈液は、殺菌した陽イオン調整済みミュラーヒントンブロス(Cation Adjusted Mueller Hinton Broth CAMHB)または酵母用のサブローデキストローズブロス(Sabouraud Dextrose Broth SDB)で製造した。
a.低温貯蔵を用い(−70℃)、上記したバクテリア微生物の最初の継代培養をトリプチケースソイ寒天培地(TSA)上にストリークする。
b.37℃で24時間インキュベートし、パラフィルムでプレートを包んで4℃で貯蔵する。
c.最初の継代培養から2番目の継代培養をTSA上にストリークする。37℃で24時間インキュベートする。2番目の継代培養は、インキュベーションから最初に取り出されたときから24時間以内に使用されなければならない。
d.2番目の継代培養を用いて、ガラスの試験管内の0.5マクファーランドスタンダード(Mc Farland Standard)(mL当たりの細胞数:1.5×108)に合致する3mLの殺菌水において、殺菌した綿棒を用いて接種物を作る。
e.この溶液をCAMHBで1:30に希釈する(または酵母用のSDBで1:10に希釈する)。
f.微生物を均一に混合するために希釈した微生物を3乃至5回反転させる。
g.TSAまたはSA上の接種物の三重のサンプルを連続して希釈し、スポットで培養することで細胞の密度を確認する。
h.希釈された微生物の残部(22mL)を96peg MBEC HTP装置の容器に入れる。
i.微生物を含む一番下のプレートに96peg MBEC装置の蓋をする。
j.該装置を、湿らせた培養器内の揺動器に、1分あたりの揺れを3乃至4回に設定して37℃で24時間置く。
k.ポリ−L−リジンのプレートを用いてカンジダパラトロピカリス(C. paratropicalis)およびカンジダアルビカンス(C. albicans)を培養する。これらは、0.1%(w/v)ポリL−リジン溶液(シグマP8920)を脱イオン水で10倍に希釈し、濾過して殺菌されることにより調製された。
a.大腸菌(Escherichia coli)O157:H7−テストされた多酵素では、MIC、MBCおよびMBECの限界点は、観察されなかった。多酵素製剤は、テストされた最も低い濃度2つを除く全てで抗バイオフィルム活性を有した。以下にデータを表で表す。
実験2では、C#18の黄色ブドウ球菌(Staphylococcus aureus)ATCC29213および黄色ブドウ球菌(Staphylococcus aureus)MRSA Uに対する抗バイオフィルム活性について、前記多酵素製剤を125mgのエチレンジアミン四酢酸ナトリウムを含まないものと含むものとについて査定した。成長媒体および条件は、TSB/TSA、好気的、および35±2℃であった。実験の設計および条件は、上記実験1について前述されている通りとした。
黄色ブドウ球菌(Staphylococcus aureus)MRSA399−EDTAを備える多酵素製剤についてのMBECはテストされた濃度では限界点を有さないことが分かった。EDTAを備える多酵素製剤について、MICおよびMBCは、1.56mg/mLで限界点を有することが観察された。EDTAを備える多酵素製剤は、多酵素のみに比べてバクテリアのバイオフィルムを根絶するのにより効力があってより有効である。これはEDTAを備える多酵素製剤の最大のLog低減は、多酵素のみの最大のLog低減よりも低い濃度であるからである。観察によると、MICおよびMBC活性のための酵素/EDTAは、著しい抗菌性ならびに抗バイオフィルム性が示される。以下にデータを表で示す。
Claims (2)
- 生理的に許容できる抗バイオフィルム組成物であって、該組成物は、
セルラーゼ、グルコアミラーゼ、ヘミセルラーゼ/ペクチナーゼ複合体、β−グルカナーゼ、DPP−IV活性を有する酵素、チトサナーゼ、リゾチーム、およびセラチアペプチダーゼ;
キレート化剤;ならびに
薬学的に許容できる担体
を含む、生理的に許容できる抗バイオフィルム組成物。 - 前記キレート化剤がEDTAである、請求項1に記載の抗バイオフィルム組成物。
