より大きい長さ636および632を有する。孔630は、主な孔部分632と、楔形の
延長部636とを含み、主本体部分632から離れた点638に向かって主本体部分63
2から延在する2つのまっすぐな面またはややまっすぐな(semi−straight
)面635および637を含む。いくつかの実施例では、孔630は、楔形の延長部の点
638が孔の基端側にあるような向きにされる。カテーテルが患者の皮膚を通して挿入さ
れるとき、皮膚は、自然に孔の中に沈むことができる。カテーテルが進められるにつれて
、まっすぐな面635および637は、次第に皮膚を孔630から押し出し、孔の基端側
が挿入の方向と直角をなす大きな平坦な面を備える場合に発生しうる皮膚の出っ張りを防
止する。図19は、いくつかの実施例に従った、丸い楔形の延長部646と、主な孔部分
644と、孔の幅とを有する別の涙滴形状の孔640を示す。丸い形の延長部646は、
孔640の全長644および646を減少させ、カテーテル本体の強度を増大させる。
診断に必要な造影剤の量を減少させるために、血液の単位体積あたりの造影剤の濃度はカテーテルの先端速度を上昇させることなく、造影剤の体積流量を増大させることによって増大する必要がある。本発明の要素は、以下の実施例で示されるように、これらの必要な目的を達成する。
標準的なカテーテルの先端における噴流速度は、5(ml/秒)の体積流量設定で1,000(インチ/秒)を超えるため、患者の静脈壁に大きな力が加えられることになる。この力は、静脈構造が最適でない患者にとって危険であり、流量増大による溢出または内膜損傷の可能性が高くなる。
標準的な22GAX1.00”カテーテルを使った注入システムと、図4A、4Bに関連して上述した、複数の拡散用孔を含むように変更した22GAX1.00”カテーテル(P_inj#1とP_inj#2)を使った注入システムとの間において、注入システム内の内圧を比較した。
カテーテルの内壁面に関して約45°で穿孔された複数の拡散用孔を含むように変更された標準的な22GAX1.00”カテーテルについて、数値流体力学解析を実行した。解析の結果、カテーテルの内壁面に関して90°で穿孔された複数の拡散用孔を有する標準的な22GAX1.00”カテーテルと比較した場合、主流から拡散用孔へのバルク流の流れの向きの変更はさらに6%増大したことが明らかになった。解析により、標準的カテーテルのまっすぐの孔と比較して、拡散用孔450の断面を通る流体流492は大きく増大したことがわかった。本発明の拡散用孔450には確かに若干の再循環渦494が発生したが、再循環渦494は、標準的なカテーテルの循環渦392と比較するとずっと弱かった。流体流492の代表的な図を図4Bに示す。
標準的な末梢静脈カテーテルでは、カテーテルの穴にカテーテルの先端に向かってテーパが付けられるため、収縮によって注入物が加速すると、反動力が発生する。この力は、消火ホースを握るときに感じられる力に似ている。消火ホースと同様に、圧縮的な反動力を受けるカテーテルの先端は不安定となって、静脈内で激しく振動することがあり(catheter whip現象と呼ばれる)、前述のような静脈損傷の原因となりうる。十分な注入物が拡散用孔を通じて軸線方向から転向されると、反動力は負となり、実際にカテーテルの先端が引っ張られ、カテーテルの先端が緊張状態にあると、挿入されたカテーテルが大幅に安定化する。したがって、いくつかの実施例では、細孔の角度を戦略的に選択し、拡散用孔からの流量の増大と、拡散用孔を通じた注入物の軸線方向の流量の減少によるカテーテルの先端の反動力の低下とのバランスを取る。
Claims (11)
- 基端と、末端と、該基端と該末端との間に延びる穴と、末端の穴とを有するカテーテル
本体であって、患者の末梢静脈に到達するのに十分な切頭の長さをさらに有するカテーテ
ル本体と、
前記カテーテル本体の前記末端上に位置決めされた少なくとも1つの孔であって、前記
カテーテル本体の肉厚の中に形成され、前記穴と連通し、内壁面および内側部分を有する
少なくとも1つの孔と、
前記少なくとも1つの孔の前記内壁面に、前記内側部分に延びる内向きの突起と、
を含む末梢用カテーテル。 - 前記少なくとも1つの孔は、少なくとも2つの孔を含み、第1の孔は、第1の孔軸線を
有し、第2の孔は、第2の孔軸線を有し、
前記第1の孔の向きは、前記第1の孔軸線が前記第2の孔軸線と実質的に交差するよう
な前記第1の孔軸線の配置を含む請求項1に記載の末梢用カテーテル。 - 前記少なくとも1つの孔は、末端および基端を含み、該末端は、略半円形であり、該基
端は、先細である請求項1に記載の末梢用カテーテル。 - 前記孔の形状は、楔形の延長部を含む請求項3に記載の末梢用カテーテル。
