Description
有効成分として組み換えマイコバクテリウムを含み、
前記組み換えマイコバクテリウムは、
ウレアーゼ欠損であり、

前記組み換えマイコバクテリウムは、
ウレアーゼ欠損であり、
(a)マイコバクテリウム抗原またはその免疫原性フラグメント、および(b)ファゴリソソームエスケープドメインを含む融合ポリペプチドをコードする組み換え核酸分子を含む。
(a) 配列番号1に示されるヌクレオチド211−1722を含むヌクレオチド配列;
(b) (a)からの配列と同一のアミノ酸配列をコードするヌクレオチド配列;
および、
(c) (a)または(b)からの配列とストリンジェントな条件下でハイブリダイズするヌクレオチド配列;
から選択される核酸分子でコードされる。
配列番号2:配列番号1の核酸分子の対応するアミノ酸配列を示す。
VPM1002は、rBCG ΔureC::Hly+::Hyg+として特徴付けられる、Mycobacterium bovis BCGのDanish株の亜型Pragueに由来する、遺伝子組み換えされたBCGワクチンである。VPM1002は、生きているMycobacterium bovis BCGΔureC::Hly+::Hyg+の凍結乾燥された固形物(lyophilised cake)として入手された。1バイアルは5×106 CFU(範囲2−8×106 CFU)のVPM1002を含んでいた。
本研究の第一の目的は、単回のVPM1002の安全性を調査することであった。
これは、ヒトへのVPM1002の最初の適用であった。その研究は、公開の、無作為化され、管理された、用量漸増計画に従い、単回のVPM1002の安全性および免疫原性を評価した。
四十(40)のBCG未投与およびBCG前ワクチン接種をされている(またはPPD陽性の)40対象が、本研究に含まれるように計画された。追跡不能となった1対象を除き、全80対象が計画どおりに研究を完了した。

免疫原性パラメーター:
・リンパ球刺激試験(LST):細胞あたりのインターフェロン(IFN)−γの量
・酵素結合免疫スポット技術(ELIspot):末梢血単核細胞(PBMC)の総数あたりの、IFN−γを分泌するPBMCの数
・全血アッセイ(WBA):リンパ球の数あたりのIFN−γの量
・細胞内サイトカイン染色(ICS)(蛍光活性化細胞選別(FACS)−分析):リンパ球の総数あたりの、CD4+およびCD8+リンパ球の数;それらはxxx−bright、xxx−brightおよびxxxbright(「三重陽性細胞(triple−positive−cells)」)であった。
第一のエンドポイント:
身体検査、バイタルサイン、ECG、肝臓超音波検査、胸部X線、検査の安全性パラメーター(血液学、凝固、臨床化学および検尿を含む)、忍容性、併用薬の記録および有害事象のモニタリングにより評価されたとおりに、単回のVPM1002の安全性を評価すること。
・ツベルクリン(PPD)に関するLSTに続く、PBMCの上清におけるIFN−γ特異的酵素結合免疫吸着測定法(ELISA);
・PPDで刺激後のIFN−γ分泌PBMC数に特異的なELISpot;
・PPDで3日間、細胞を刺激し、ELISAにより血漿中IFN−γを測定するWBA;
により評価される免疫原性。
・PPDで一晩刺激したCD4+およびCD8+リンパ球における、IFN−γ、腫瘍壊死因子(TNF)−αおよびインターロイキン(IL)−2に関するICSのFACS−分析;
・PPDで一晩刺激したCD4+およびCD8+リンパ球における、カルボキシフルオレセイン ジアセテート スクシンイミジル エステル(carboxyfluorescein diacetate succinimidyl esters)(CFSE)で細胞内染色したFACS−分析;
・結核抗原(TB−Ag)85bペプチドカクテルでの刺激に関するLST、ELISpot、ICSおよびWBA;
・PPDまたはTB−Ag85Bペプチドカクテルに対する血清抗体の濃度;これらの血清抗体の免疫グロブリン(Ig)G−サブタイプの定量化;
により評価される免疫原性。
記述統計学が、安全性パラメーターの評価のために用いられた。
以下の統計的試験方法が、免疫原性データのために用いられた。
・BCGグループと比較した、繰り返し測定環境での、ベースラインからの調整された変化から用量反応関係を検出するためのヨンクヒール・タプストラ検定(Jonckheere Terpstra test)(α=0.05)
・事前に定義された推定の、治療因子の共変量および余因子を有するそれぞれのパラメーターでの、ベースラインから、ベースラインの後の個々の訪問値(visits)への変化を調整するための線形回帰モデル(後退的選択)
・グループ内、およびVPM1002グループとBCGグループとの比較の両方での、95%信頼区間を用いた、治療効果(ベースラインからの変化)の推定
・調整回帰モデルにおける、統計学的に無関係の共変数/余因子の変数減少(backward elimination)
・2つの治療グループ間の試験的比較に関するX2検定、t検定、U検定
・ノンパラメトリック分析の感度を推定するヨンクヒール・タプストラ検定の代わりの多変量線形回帰
全80対象は、安全性母集団および包括解析(Intention to Treat)(ITT)母集団に含まれた。全80対象は、免疫原性パラメーターに関して正当で解釈可能な評価を提供し、大幅な手順の逸脱は無く;したがって、全ての対象は、免疫原性(Immunogenicity)(IM)および治験実施計画(Per−Protocol)(PP)母集団として有効であった。
本研究の第二の目的は、VPM1002の免疫原性を示すことであった。第二の目的は達成された。本研究は、VPM1002が、「未投与」およびBCG「前免疫」の対象の両層において、量的および質的に、非常に良好な細胞免疫応答を誘導することを示す。
全ての観測データは、VPM1002によって誘導された、明確なTh1タイプの免疫応答を示す。その特定のワクチンVPM1002株の開発の当初の目標は、細胞媒介性の免疫応答を増大させること、および、BCGよりも質的により良い免疫応答を誘導することであった。これらの目標は達成可能であった。加えて、それは、BCGによって誘導された既存の免疫応答へのブーストワクチン接種の可能性も示す。
この第I相臨床試験の第一のエンドポイントは、VPM1002の安全性評価であった。実際、本研究はVPM1002に関して安全性に対する懸念を少しも見せなかった。
ADRsは全ての対象で報告された。ほぼ全てのADRが注射部位の障害であった(全ADRsの98.0%)。
以前にBCGワクチン接種歴のある対象のグループにおいては、AEsの数は、BCGグループにおける72AEsおよび5×103 CFUと5×104 CFU VPM1002グループの両方における61 AEsと比較して、5×105 CFU VPM1002グループにおいて最も高かった(97 AEs)。
AEが原因で中断した対象はなかった。
バイタルサインおよびECGパラメーターは、時間−または用量−に相関した差異を示さなかった。
薬力学(第二の試験目的)
・第二の試験目的は達成された。
・VPM1002は、用量依存的なIFN−γ刺激によって検出されたように免疫原性を示す。その結果を図3および4に示す。
・VPM1002は、BCGとは量的および質的に異なる免疫応答を誘導する。
・VPM1002は、既存のBCG誘導の免疫状態を強化する効果を有する。
・多機能CD4+ T細胞は、VPM1002(5×105 CFU)の全群において、上方調節された。その結果を図5に示す。
・薬剤関連と考えられた有害事象は、ほとんど常に注射部位での障害であった。AEsの数は、用量とともに増加し、5×105 CFU VPM1002と5×105 CFU BCGの参照ワクチンの後で、同様であった。
・局所的反応の数と強度は、VPM1002の用量とともに増加し、最も高い用量において、局所的反応の発生率は、BCGでのワクチン接種後に観察されたものと同様であった。
・VPM1002の全体的な忍容性は、対象ごとに、常に、良いまたは非常に良いと評価された。
・検査値、バイタルサインおよびECGデータは、臨床的に関連する時間−または用量−に相関した差異を示さなかった。
VPM1002の安全性プロフィールは良好であった。VPM1002は免疫原性を示した。VPM1002の免疫原性プロフィールは、BCGのそれとは異なる。そのベネフィット/リスク比は、このワクチン候補の臨床開発を継続することを可能にする。
