1.材料及び方法
1.1 試料調製
隣接白斑を有する、人間の臼歯と小臼歯を抜き取り、脱石灰部分を切断した。外側のエナメル質に対し閉じ込められている120の欠損を選んだ。各欠損の半分と切断面には、対照試料とし、マニキュアを塗った。次に、欠損を37%リン酸ゲルあるいは5%又は15%塩酸ゲルを用いて30秒から120秒(n=10)エッチングした。
試料を、一端をストッパーで閉じたシリコーンチューブの中で5分間乾燥させ、シリコーンリングを用いて分離した。次に、0.1mmol/lの蛍光色素のローダミンBイソシアネート(RITC)で標識したスパー樹脂(Spurr's resin)(Spur AR、電子顕微鏡の包埋用の低粘度のエポキシ樹脂、I Ultrastruct Res、第26巻、第31頁〜第43頁(1969))を、試料にかけ、そしてチューブを別のストッパーで閉じた。樹脂をオートクレーブ中(Ivomat IP3; イボクラールビバデントシャーン、リヒテンシュタイン)で0.8Mpa、70℃で8時間かけて硬化させた。加圧により、非常に低粘度の樹脂は、欠損の残留孔に浸透する。硬化後、試料を切断し、試料ホルダーに平行に固定し、4000グリット(イクザクトミクロシュロイフシステム; イクザクトアパラテバウ)まで研磨した。この浸透方法を、樹脂浸透による視覚化(VIRIN)と名付けた。
試料を、共焦点走査型顕微鏡(CLSM)(ライカTCS NT、ライカ、ハイデルベルグ、ドイツ)を用いて検討した。励起光がアルゴン/クリプトンレーザを用い、最大波長が560nmである。映像が、蛍光モードで記録した。蛍光のみを検出し、反射光を除くように、590nmのロングパスフィルターを用いて発光を検出した。オイルに浸漬した状態で、対物40倍で試料を観察した。観察した層は、表面から約10μm下の層である。観察中、レーザ光強度と光増幅率は一定とした。映像(250x250μm)は、解像度1024x1024ピクセル、256疑似カラーステップ(pseudo color steps)(赤/黒)で撮影し、イメージジェイプログラム(NIH、ロックビルパイク、メリーランド、米国)を用いて解析した。
対照例の表面層の厚さと、正常及び患部の組織の浸蝕部分及びエッチング部分を測定した。リン酸ゲルで30秒間エッチングしても表面層の厚さに大きな変化はなかった(p>0.05、t-検定)。しかしながら、リン酸ゲルで30秒間又は90秒間エッチングした場合に比べ、15%塩酸ゲルで90秒間エッチングした場合では、欠損における表面層の減少率が大きく増加した(p<0.05、アノーバ(ANOVA))。欠損における浸蝕深さについては、正常なエナメル質と比べて違いは認められなかった(p>0.05、t-検定)。
1.材料及び方法
この試験では、隣接白斑を有する、抜いた人間の臼歯と小臼歯を用いた。ソフトティッシュで十分に洗浄した後、歯を使用するまで20%エタノール溶液中に保存した。歯は、20倍の実体顕微鏡(ステミ(Stemi) SV 11、カールザイス、オーバーコーヘン、ドイツ)を用いて観察し、損傷や空洞のある欠損を除いた。
CLSMの映像では、浸透した樹脂は赤色蛍光を示すが、象牙質や欠損内の残存孔は緑色である(図3Aから3C)。正常なエナメル質の固体材料又は表面層は黒色である。
顕著に大きい(p<0.01、マン−ホイットニー)平均浸透深さ[65(35)μm]が認められた。塩酸エッチングを行う[20(18)μm]と、リン酸エッチングを行った場合[37(25)μm]やエッチングを行わなかったCTRグループ[42(23)μm](p>0.05、マン−ホイットニー)に比べ、表面層の厚さが大きく減少した。
この検討の目的は、エナメル質欠損に浸透させるための66個の実験用複合樹脂(浸透剤)について評価を行うことである。
エタノール(0%、10%又は20%)と、ビスGMA、UDMA、TEGDMA、そしてHEMAの内の2種のモノマーを種々の重量比(100:0、75:25、50:50、25:75、0:100)で含む66個の実験用浸透剤を調製した(表2)。各実験用樹脂は、表2に従い、10gを混合して褐色のガラス製ジャーに投入した。早期に重合が起きるのを防止するため、樹脂は使用するまで4℃で保存した。実験用浸透剤のPCを決定するために、接触角、表面張力、そして粘度を測定した。
実験用浸透剤の結果を表2に示す。樹脂の間の最も大きい差異は、粘度であった(3.
2〜6637.0mPa・s)。HEMAとTEGDMAが高含量である樹脂混合物は、低粘度で高
いPCを有していた。一方、ビスGMAとUDMAは粘度が増加し、PCが減少した。ビスGMA又
はUDMAが高含量である5個の実験用樹脂は、非常に粘度が高く、市販の装置では測定する
ことができなかった。接触角には大きな差異が認められたが(3.2〜54.2度)、P
Cへの影響は、接触角のコサインで効いてくるため、わずかであった。エタノールを添加
すると、すべの混合物の粘度、表面張力、そして接触角が減少し、すべてのモノマーの組
み合わせについて、浸透係数が増加した。TEGDMA、 HEMA、そして20%エタノールを含
む複合物が最も大きなPCを有していた。HEMAとエタノールが高含量である複合物は、十
分に硬化せず、ゴム状又は液状となった(表2)。
この生体外における実験の目的は、PCの異なる12個の実験用浸透剤の浸透指数(PQ=浸透深さ/欠損深さ)を、接着剤(エキサイト(登録商標);ビバデント)と比較することにある。
ウシの臼歯から143個の試料を調製し、エポキシ樹脂に埋め込み研磨した。試料の一部をマニキュアで覆い(対照例)、4つの窓を50日間(pH4.95、37℃)脱灰した。脱灰後、4つの窓の内の3つをリン酸で5秒間エッチングした(37℃)。上記接着剤と、12個の実験用材料(No.13〜21、4〜6)を欠損(n=11)に塗布した。余分の材料を除去した後、樹脂を30秒間で光硬化させた。試料をその表面に対し垂直に切断し、その切片を共焦点顕微鏡(CLSM)と、マイクロ放射線撮影(TMR)を用いて調べた。
CLSM[299(57)μm]とTMR[296(51)μm]の平均欠損深さ(SD)は同等であった。接着剤と比較すると、TEGDMAをベースとする3個の浸透剤は(No.4〜6;ビスGMAとTEGDMA(25:75)、そしてエタノールを含むもの(No.20,21)だけでなく)、PQが大きく増加した(p<0.05;アノーバ)。種々の材料について、図5Aは浸透指数、図5Bは絶対浸透深さを示している。
この検討の目的は、脱灰条件での、閉塞させた人工エナメル質欠損の進行度を評価することである。
130個のウシのエナメル質の試料のそれぞれについて、4個の欠損状のう蝕を作製した(pH4.5の溶液で50日の条件で脱灰した。)。各試料について、3個の欠損はリン酸で5秒間エッチングし、残りの1個の欠損にはエッチングを行わなかった。接着剤エキサイトと、PCの異なる12個の実験用複合物(No.13〜21、と4〜6;表2)のそれぞれを欠損の上に塗布した(n=10)。余分の材料を除去した後、樹脂を30秒間で光硬化させた。次に、試料をその表面に対し垂直に切断した。各試料の半分をベースラインの対照例とした。残りの半分を、さらに50日間、脱灰用溶液に曝した(効果)。試料は共焦点顕微鏡を用いて観察した。
2回目の脱灰期間中に、平均欠損深さ(SD)は、299(51)μmから418(7
6)μmに増加した(41.5%)。未処理の対照例と比較し、浸透処理した欠損は顕著
に欠損の進行が抑制された(p>0.001、t-検定)。PCが100cm/sより小さ
い浸透剤と、エキサイト(PC=31.3cm/s)は、欠損深さが顕著に増加した(図
7)。PCが高い浸透剤は、ほとんど進行しなかった(p>0.05)。閉塞させた欠損
の進行度と、PCと浸透時間の積の平方根との間に負の相関が認められた(r2=0.6
25;p<0.01;図8)。PCが高い浸透剤は、PCが低い浸透剤に比べ、欠損の進
行を抑制するのに、適していると結論できる。
1.材料
エナメル質への浸透を実行するための有効な手段は、その場で歯を分離するための分離手段と(図9)、塩酸ゲル等の状態調節剤を塗布するための塗布用片と(図10A)、浸透剤と、必要に応じて、高粘度の光硬化性モノマーの混合物と、洗浄用片(図10B)とを含んでいる。

歯科医の一般的なやり方として、治療部位を拭くために歯科用ダム(dental dam)を用い、歯同士を約300μm離間させるために分離手段を用いる。患者が行う場合には、治療部位を拭いたものは捨てることができる。

- さらに、少なくとも1つの塗布用片、及び/又は少なくとも1つの洗浄用片、及び/又は歯と歯を分離するための分離手段を含む請求項1から7のいずれか1項に記載のキット。
- (a)状態調節剤を含む塗布用片、
(b)液状複合樹脂を含む塗布用片、および
(c)少なくとも1つの洗浄用片、を含む請求項8記載のキット。 - さらに、(d)光硬化性モノマーの混合物を含む塗布用片を含む請求項8記載のキット。
- さらに、(e)歯と歯を分離するための分離手段を含む請求項7から9のいずれか1項に記載のキット。
