Description
MSI試験は、労働集約的であり、専門の分子病理学的サービス(molecular pathologic service)を必要とし、MSIは、LSの特徴ではあるが、LSに特有のものではない。およそ10〜25%の散発性CRCおよび多くの結腸外の癌もMSIを示す。さらに、LS関連および散発性MSI陽性CRCは、多くの共通の病理組織学的特徴を有しているが、散発性MSI CRCは、家族歴陽性と関連づけられない点で異なる。
本発明の目的は、対象のLS病歴または癌の予備知識なしでMMR効率の低下を検出するための正確で単純で安価な臨床的に適した試験を実現するための新規の方法および手段を提供することであり、それにより、診断法の現在の方法の課題を解決する。本発明は、ヒト対象がDNAミスマッチ修復機能障害を有しているかどうかを判別するための定量的方法であって、a)前記ヒト対象から得られたサンプルから核抽出物を生成すること、b)それぞれ陽性および陰性対照として、MMR能力のあるおよびMMR欠損のある核タンパク質抽出物を準備すること、c)個別のバイアル中で各核抽出物を少なくとも1つのミスマッチ含有ヘテロ二重鎖DNA基質と組み合わせること、d)前記サンプルおよび対照核タンパク質抽出物ならびに前記基質に対してミスマッチ修復アッセイを実施すること、ならびにe)前記サンプル核抽出物が前記基質DNA分子を修復できるかどうかを判別することを含む、方法に関する。好ましくは、前記サンプルは、構成組織から得られた正常な細胞、より好ましくは、線維芽細胞または血液由来細胞を含む。
検査される組織サンプルは、任意の正常な組織であってもよい。これは、容易に得られ、処理される。本発明の特定の一態様において、最適な組織サンプルは、ヒト皮膚パンチまたは切除生検であり、これは、医療分野の当業者に周知の任意の方法によって得られていてもよい。
細胞のミスマッチ修復(MMR)の能力を調べるために、そこから核タンパク質を抽出する前に十分な量の細胞が増殖させられなければならない。細胞は、使用される細胞の種類に適しており、当該技術分野において周知の標準的な細胞培養条件において増殖させられ、その後、穏やかにそれを遠心沈殿することによって回収される。
MMRタンパク質は、細胞の核から抽出される。細胞は、遠心沈殿され、計数され、あらゆる残存トリプシンを除去するために生理食塩水緩衝液で洗浄される。細胞は、遠心沈殿される前に等張緩衝液でさらに洗浄され、続く低張の洗浄の浸透作用により膨張させられる。顕微鏡下で大半の細胞が溶解したことが確認されるまで細胞膜を破壊するために細い針が使用される。
本発明によるMMRアッセイに使用するための環状ヘテロ二重鎖プラスミド基質は、一本鎖DNA(ssDNA)と1つ非相同部位をもつ変性された対応するプラスミドDNAの再アニーリングによって作製される。
本発明によるMMRアッセイは、試験サンプルのMMR機能が欠損しているかどうかを判別することのみに使用されてもよいが、上記のとおり、MMR欠損細胞から抽出された核タンパク質のMMR能力と、MMR能力のある細胞から抽出されたMMR能力とを区別することもできる。MMR欠損遺伝子を特定するために、サンプル核抽出物は(X)は、MLH1(Y1)およびMSH2/MSH6(Y2)欠損核抽出物を別々に補完するために使用される。

線維芽細胞を、Dulbecco’s Modified Eagle Medium(Invitrogen、カタログ#31966−021)中、37℃、5%CO2のインキュベーター内で増殖させる。その増殖培地に、10%のウシ胎仔血清(Invitrogen、カタログ#10106−169)、2×非必須アミノ酸(Invitrogen、カタログ#10370−070)および2%のペニシリン−ストレプトマイシン(Invitrogen、カタログ#15070−063)を補充する。細胞を、およそ80〜90%コンフルエントのときに、トリプシン0.25%EDTA(Invitrogen、カタログ#25200−072)を用いて1:2〜1:6に継代する。MLH1−欠乏誘導細胞株(depleted derivative cell line)FB−M1またはMSH2−欠乏誘導細胞株FB−SH13の発現抑制を維持するために、150μg/μlのハイグロマイシンB(Invitrogen、カタログ#10687−010)を増殖培地に添加する。核タンパク質の抽出のために、およそ5×108〜109細胞が収集されるまで、その細胞をフィルターでキャップされた175cm2ボトル(NUNC、カタログ#145−178883)で処理された培地中で増殖させる。
付着性のヒト癌細胞株HeLa、LoVoおよびHCT116(American Type Culture Collection、マナッサス、VA、米国)を、製造業者の取扱説明書に従って培養する。HeLa細胞(ATCC CCL−2)は、女性個人の子宮頸部由来のMMR能力のある上皮細胞であるのに対して、HCT116(ATCC CCL−247)およびLoVo細胞(ATCC CCL−229)は、男性個人の結腸由来のMMR欠損上皮細胞である。HCT116細胞は、MLH1が欠如しているのに対して、LoVo細胞では、MSH2遺伝子が不活化しており、MSH2、MSH3およびMSH6タンパク質の欠損を引き起こす。MSH2の欠如は、その対応物MSH3およびMSH6のタンパク質分解と関連づけられている。
1. トリプシンにより5×108〜109の対数期細胞を回収する。
ドデシル硫酸ナトリウムポリアクリルアミドゲル電気泳動による50mgの核抽出物(NE:nuclear extract)および0.1〜5μlの野生型総タンパク質抽出物を使用したウエスタンブロット分析によって、NEのタンパク質発現量を調査した。そのタンパク質をニトロセルロース膜(Amersham Hypond(商標)、PVDF、Amersham Pharmacia Biotech、ウプサラ、スウェーデン)にブロットし、それを、その後、モノクローナル抗体、抗−MSH2(Calbiochem、サンディエゴ、CA、米国、MSH2−Ab1、NA−26、0.2mg/ml)、抗−MSH3(BD Transduction Laboratories、レキシントン、KY、米国、M94120、250mg/ml)、抗−MSH6(BD Transduction Laboratories、clone 44、0.02mg/ml)、抗−PMS2(Calbiochem/Oncogene Research、サンディエゴ、CA、米国、Ab−1、0.2mg/ml)および抗−MLH1(BD Biosciences/Pharmingen、サンディエゴ、CA、米国、clone 168−15、0.5mg/ml)とともにインキュベートした。ローディングコントロール(loading control)として偏在的に発現されるα−チューブリンを使用して、抽出物中のMMRタンパク質量を概算した(抗−α−チューブリン;Sigma、ルイス、MO、米国、DM1A、0.2mg/ml)。
ヘテロ二重鎖分子の調製。ヘテロ二重鎖DNA分子は、ミスマッチの部位から445bp上流に一本鎖の切れ目をもつ環状の3192bp長の分子である。これは、完全なBglII制限部位を含む。pGEM−IDL40プラスミドからの一本鎖DNA(図6)を、同じ構築物ベースのエラー導入プラスミド(error introducing plasmid)(5’GT基質のためのpGEM−IDL GTまたは5’IDL1基質のためのpGEM−IDL−39)とともにアニールすることによってこの分子を作製する。
本発明は、以前に示されたインビトロMMRアッセイの修正版であり(Nystrom−Lahtiら、2002)、核抽出物のMMR能力を直接アッセイすることを可能にする。さらに、表1に示されるように特徴づけられたMMR欠損抽出物を補完することによって、サンプル中の離脱されたMMR遺伝子を特徴づけるために相補性が使用できる。修復反応は、合計75〜100μgのNEを含むよう統一する。過量のヘテロ二重鎖DNA基質(5’GTまたは5’IDL1)は100ngとする。修復反応を、20μlの体積で以下の試薬により実施する:
−100ngのヘテロ二重鎖基質
−75〜100μg核抽出物
−2μlの10×MMR緩衝液(200mMのトリス−HCL、pH7.6、400mMのKCl、50mMのMgCl2、10mMのグルタチオン、BSA 500μg/ml、1mMの各dNTP、15mMのATP)
−110mMの最終濃度になるまでのKCl
−20μl以下のH2Oの添加。
この例は、MMRアッセイが正常な組織、たとえば、線維芽細胞に対して実施できることを示すために実施した。例2に記載されるように調製された野生型線維芽細胞核抽出物(FB−WT:Wild-type fibroblast nuclear extract)、MMR能力のあるHeLa細胞からの核抽出物およびタンパク質を含まない陰性対照(MOCK)を、例3に記載されるような本発明によるMMRアッセイに供した。
この例は、それぞれMLH1遺伝子またはMSH2遺伝子が(sh)RNA技術によって部分的に発現抑制されたヒト線維芽細胞株FB−M1およびFB−SH13(例2に記載される)が、MMR能力に明らかな低下を示すことを示している。
MOCK−核抽出物を含まない陰性アッセイ対照;
FB−WT−野生型ヒト線維芽細胞核抽出物(陽性対照);および
FB−M1−部分的に発現抑制されたMLH1をもつヒト線維芽細胞からの核抽出物;または
FB−SH13−部分的に発現抑制されたMSH2をもつヒト線維芽細胞からの核抽出物。
本発明によるキットは、MMRアッセイを実施するための全試薬を予め入れたバイアルとして好ましくは提供される。試験サンプル核抽出物のみが添加される。本発明の一態様では、検査される異なるMMR欠損のために個別のバイアルが提供される。
−基質のヘテロ二重鎖
−核抽出物(HCT116、LoVoまたはFB−WT)
−10×MMR緩衝液(200mMのトリス−HCL、pH7.6、400mMのKCl、50mMのMgCl2、10mMのグルタチオン、BSA 500μg/ml、1mMの各dNTP、15mMのATP)。
例6に記載されるような取り扱いおよびインキュベーションに適した任意の種類のバイアル、たとえば、eppendorf(登録商標)チューブが適している。
以下に、本願の種々の実施態様を付記する。
[1]ヒト対象がDNAミスマッチ修復機能障害を有しているかどうかを判別するための定量的方法であって、a)前記ヒト対象から得られたサンプルから核タンパク質抽出物を生成すること、b)陽性および陰性対照としてそれぞれ、MMR能力のあるおよびMMR欠損のある核タンパク質抽出物を準備すること、c)個別のバイアル中で各核抽出物を少なくとも1つのミスマッチ含有ヘテロ二重鎖DNA基質と組み合わせること、d)前記サンプルおよび対照核タンパク質抽出物ならびに前記基質に対してミスマッチ修復アッセイを実施すること、ならびにe)前記サンプル核抽出物が前記基質DNA分子を修復できるかどうかを判別することを含む、方法。
[2]前記サンプルは、構成組織から得られた正常な細胞を含む、[1]に記載の方法。
[3]前記正常な細胞は、線維芽細胞である、[2]に記載の方法。
[4]前記正常な細胞は、血液由来細胞である、[2]に記載の方法。
[5]前記基質は、配列番号1を有するプラスミドに由来する、[1]〜[4]の何れか1に記載の方法。
[6]前記基質は、配列番号2および配列番号3からなる群から選択される核酸配列をさらに含む、[5]に記載の方法。
[7]前記MMR欠損核抽出物は、HCT116およびLoVo細胞からなる群から選択される細胞株に由来する、[1]〜[6]の何れか1に記載の方法。
[8]バイアル当たりの核抽出物の総量は、50〜500μg、好ましくは50〜200μg、より好ましくは75〜100μgの範囲である、[7]に記載の方法。
[9]機能欠損を有するミスマッチ修復遺伝子を特定するための定量的方法であって、a)ヒト対象から得られたサンプルから核タンパク質抽出物を生成すること、b)陽性および陰性対照としてそれぞれ、MMR能力のあるおよびMMR欠損のある核タンパク質抽出物を準備すること、c)少なくとも1つのMMR欠損核タンパク質抽出物を前記サンプル抽出物と混合すること、d)個別のバイアル中でステップa〜cにおいて準備された各核抽出物を少なくとも1つのミスマッチ含有ヘテロ二重鎖DNA基質と組み合わせること、e)前記核抽出物および前記基質に対してミスマッチ修復アッセイを実施すること、ならびにf)前記サンプル核抽出物が前記基質を修復でき、前記MMR欠損核抽出物を補完できるかどうかを判別することによって前記欠損遺伝子を特定することを含む、方法。
[10]前記サンプルは、構成組織から得られた正常な細胞を含む、[9]に記載の方法。
[11]前記正常な細胞は、線維芽細胞である、[10]に記載の方法。
[12]前記正常な細胞は、血液由来細胞である、[10]に記載の方法。
[13]前記基質は、配列番号1を有するプラスミドに由来する、[9]〜[12]の何れか1に記載の方法。
[14]前記基質は、配列番号2および配列番号3からなる群から選択される核酸配列をさらに含む、[13]に記載の方法。
[15]前記MMR欠損核抽出物は、HCT116およびLoVo細胞からなる群から選択される細胞株に由来する、[9]〜[14]の何れか1に記載の方法。
[16]バイアル当たりの核抽出物の総量は、50〜500μg、好ましくは50〜200μg、より好ましくは75〜100μgの範囲である、[15]に記載の方法。
[17][9]〜[16]の何れか1に記載の方法を使用して欠損DNAミスマッチ修復遺伝子を特定することにより、ヒトのリンチ症候群を診断するための方法であって、前記サンプルは、ヒト対象の構成組織から得られた臨床サンプルである、方法。
[18][1]〜[17]の何れか1に記載の方法に使用するためのキットであって、少なくとも1つのミスマッチ含有ヘテロ二重鎖DNA基質、少なくとも1つのMMR欠損核抽出物および陽性対照としてMMR能力のある核抽出物を含む、キット。
[19]前記MMR欠損核抽出物は、MLH1、MSH2またはMSH6のいずれかについてのMMR欠損を有する細胞株から得た、[18]に記載のキット。
[20]少なくとも2つのMMR欠損核抽出物を含む、[19]に記載のキット。
[21]前記MMR欠損核抽出物は、それぞれMLH1およびMSH2/MSH6についてのMMR欠損を有する細胞株に由来する、[20]に記載のキット。
[22]前記MMR欠損核抽出物は、それぞれHCT116およびLoVo細胞に由来する、[18]〜[21]の何れか1に記載のキット。
[23]前記MMRアッセイを実施するために必要な追加の試薬をさらに含む、[18]〜[22]の何れか1に記載のキット。
[24]200mMのトリス−HCL、pH7.6、400mMのKCl、50mMのMgCl2、10mMのグルタチオン、BSA 500μg/ml、1mMの各dNTPおよび15mMのATPを含む10×MMR緩衝液;ならびに42mMのEDTA、1.2%のSDS、50.4μg/mlのプロテイナーゼKを含むMMRアッセイ用停止液をさらに含む、[23]に記載のキット。
[25]前記サンプルを清澄にするための試薬、たとえば、TE緩衝液、フェノール/クロロホルム/イソアミルアルコールおよびクロロホルム;前記サンプルを沈殿させるための試薬、たとえば、NaClおよびエタノール;ならびに修復を検出するための試薬、たとえば、RNase Aならびに緩衝液とともに制限酵素BglIIおよびEco31Iならびにローディングダイを含む、[23]〜[24]の何れか1に記載のキット。
Claims (14)
- ヒト対象が遺伝性DNAミスマッチ修復機能障害MMR+/-を有しているかどうかを判別するための定量的方法であって、
a)前記ヒト対象の正常な構成組織サンプルから初代細胞を準備すること、
b)前記初代細胞を培養すること、
c)ヒト対象の培養された前記初代細胞から核タンパク質抽出物を生成すること、
d)陽性および陰性対照としてそれぞれ、MMR能力のあるMMR+/+およびMMR欠損のあるMMR-/-核タンパク質抽出物を細胞株から準備すること、
e)個別のバイアル中で工程cの初代細胞核タンパク質抽出物と、工程dのMMR能力のある核タンパク質抽出物およびMMR欠損のある核タンパク質抽出物を少なくとも1つのミスマッチ含有ヘテロ二重鎖DNA基質と組み合わせること、
f)初代細胞の核タンパク質抽出物に対しておよび対照核タンパク質抽出物に対して、前記DNA基質と共にミスマッチ修復アッセイを実施すること、ならびに
g)初代細胞の核タンパク質抽出物が前記基質DNA分子を修復する能力を定量的に判別し、それによってヒト対象が遺伝性DNAミスマッチ修復機能障害(MMR+/-)を有しているかどうかを判別することを含む、方法。 - 遺伝性DNAミスマッチ修復機能障害を有するミスマッチ修復遺伝子を特定するための請求項1記載の定量的方法であって、該方法は
工程d)の後に、工程d)の少なくとも1つのMMR欠損核タンパク質抽出物を、工程c)の前記抽出物と混合すること、および
工程e)において、個別のバイアル中で、工程d)の少なくとも1つのMMR欠損核タンパク質抽出物と工程c)の前記抽出物の混合物を、少なくとも1つのミスマッチ含有ヘテロ二重鎖DNA基質と混合することを含み、更に
h)工程d)のMMR欠損核タンパク質抽出物を工程c)の前記抽出物により相補し、相補された核抽出物が前記DNA基質を修復できるかを判別することにより欠損遺伝子を同定すること、を含む方法。 - 前記初代細胞は、線維芽細胞である、請求項1または2に記載の方法。
- 前記初代細胞は、血液由来細胞である、請求項1または2に記載の方法。
- 前記基質は、配列番号1を有するプラスミドに由来する、請求項1〜4の何れか1項に記載の方法。
- 前記基質は、配列番号2および配列番号3からなる群から選択される核酸配列をさらに含む、請求項5に記載の方法。
- 請求項2〜6の何れか1項に記載の方法を使用して遺伝性DNAミスマッチ修復機能障害を有するミスマッチ修復遺伝子を特定することにより、ヒトのリンチ症候群を診断するための方法であって、前記初代細胞は、ヒト対象の構成組織から得られた臨床サンプルに由来する、方法。
- DNAミスマッチ修復遺伝子が、MLH1、MSH2、MSH6、MLH3、MSH3、PMS1、PMS2およびEXO1からなる群から選択される、請求項7に記載の方法。
- 請求項1〜8の何れか1項に記載の方法に使用するためのキットであって、バイアルが、少なくとも1つのミスマッチ含有ヘテロ二重鎖DNA基質、ならびに、少なくとも1つのMMR欠損核抽出物および/または陽性対照としてMMR能力のある核抽出物を含む、キット。
- 前記MMR欠損核抽出物は、MLH1、MSH2、MSH6またはPMS2のいずれかについてのMMR欠損を有する細胞株から得た、請求項9に記載のキット。
- 少なくとも2つのMMR欠損核抽出物を含む、請求項10に記載のキット。
- 前記MMR欠損核抽出物は、それぞれMLH1およびMSH2/MSH6についてのMMR欠損を有する細胞株に由来する、請求項11に記載のキット。
- 200mMのトリス−HCL、pH7.6、400mMのKCl、50mMのMgCl2、10mMのグルタチオン、BSA 500μg/ml、1mMの各dNTPおよび15mMのATPを含む10×MMR緩衝液;ならびに42mMのEDTA、1.2%のSDS、50.4μg/mlのプロテイナーゼKを含むMMRアッセイ用停止液をさらに含む、請求項9〜12の何れか1項に記載のキット。
