Description
本出願は、2008年1月31日に出願された、Wexnerらによる「Endoscopic flexible loop for gastrointestinal polypectomy and mucosal resection using ligasureTM sealing technology」というタイトルの米国仮特許出願第61/063,158号の優先権の利益を主張し、本明細書に参考として援用される。
本開示は、内部組織の除去のための装置および方法に関し、より具体的には、摘出に先立って、組織を治療するための一対の電極を含む、スネア型装置に関する。
ポリープは、粘膜から突出する組織の異常成長である。狭小の伸長茎によって粘膜の表面に付着するポリープは、有茎性であると言われる。茎が存在しない場合、ポリープは、無茎性であると言われる。ポリープは、一般に、結腸、胃、鼻、膀胱、および子宮に見られる。また、ポリープは、粘膜が存在する身体内の至る所、例えば、頸部および小腸にも形成され得る。
ポリープを除去するために構成される装置が開示される。一実施形態では、ポリープ除去装置は、近位および遠位端を有する管状部材と、管状部材の遠位端内から動作可能に延出し、その間の組織を封着するように構成される第1および第2の電極を含むスネア部であって、封着された組織を切断するための切除部材をさらに含む、スネア部とを含む。ポリープ除去装置は、管状部材の近位端内から動作可能に延出するハンドル部をさらに含んでもよい。ハンドル部は、ユーザによる動作可能係合のために構成されてもよい。切除部材は、第1および第2の電極の遠位端間に延出してもよい。
本発明は、例えば、以下を提供する。
(項目1)
組織除去システムであって、
近位および遠位端を有する管状部材と、
該管状部材の該遠位端から動作可能に延出可能であって、自身の間の組織を治療するように構成される第1および第2の電極を含むスネア部であって、該スネア部は、該治療された組織を切断するために、該第1と第2の電極との間に位置付けられる切除部材をさらに含む、スネア部と
を備える、システム。
(項目2)
上記管状部材の上記近位端に動作可能に連結されるハンドル部をさらに備える、項目1に記載の組織除去システム。
(項目3)
上記ハンドル部は、ユーザによる動作可能な係合のために構成される、項目2に記載の組織除去システム。
(項目4)
上記切除部材は、近位方向に引っ張られる時のみ切断するように構成される、項目1に記載の組織除去システム。
(項目5)
上記組織は、ポリープであって、上記第1および第2の電極は、該ポリープの茎を把持するように構成される、項目1に記載の組織除去システム。
(項目6)
上記第1および第2の電極は、上記切除部材にヒンジ結合される、項目2に記載の組織除去システム。
(項目7)
上記スネア部は、上記管状部材内に後退させられるように構成される、項目1に記載の組織除去システム。
(項目8)
上記第1および第2の電極は各々、組織接触表面および絶縁本体部から形成される、項目1に記載の組織除去システム。
(項目9)
電気外科発電機をさらに含む、項目1に記載の組織除去システム。
(項目10)
上記第1および第2の電極は、上記電気外科発電機に電気的に連結され、該発電機は、両極の態様で該第1および第2の電極にエネルギーを送達するように構成される、項目9に記載の組織除去システム。
(項目11)
上記管状部材の上記遠位端は、上記第1および第2の電極が、互いに接触するのを防止するためのスペーサ部材を含む、項目1に記載の組織除去システム。
(項目12)
上記第1および第2の電極のうちの少なくとも1つは、該第1と第2の電極との間の接触を防止するために、その上に搭載されるスペーサを含む、項目1に記載の組織除去システム。
(項目13)
組織除去システムであって、
エネルギーを発生させ、エネルギーを組織除去装置に送達するように構成される発電機を備え、該組織除去装置は、
近位および遠位端を有する管状部材と、
該管状部材の該近位端から動作可能に延出するハンドル部と、
該管状部材内に摺動可能に支持され、該管状部材の該遠位端から延出するように構成されるスネア部であって、該スネア部は、自身の間の組織を治療するために、該発電機からのエネルギーを受容するために、そこに動作可能に搭載される第1および第2の電極を含む、スネア部と、
該第1と第2の電極との間に位置付けられ、該治療された組織を切断するように構成される、切除部材と
を備え、該第1および第2の電極のうちの少なくとも1つは、該第1と第2の電極との間の接触を防止するために、その上に搭載される少なくとも1つのスペーサを含む、システム。
(項目14)
上記スネア部は、非導電性材料から形成される、項目13に記載の組織除去システム。
(項目15)
上記エネルギーは、無線周波数、超音波、マイクロ波、およびレーザから成る群から選択される、項目13に記載の組織除去システム。
(項目16)
組織を除去する方法であって、
スネア部と、そこに動作可能に搭載される第1および第2の電極と、該第1と第2の電極との間に位置付けられる切除部材とを含む、組織除去装置を提供することと、
該スネア部を延出させ、身体の組織塊を囲繞することと、
該スネア部を後退させ、該第1と第2の電極との間の該組織塊を捕捉することと、
エネルギーを該第1および第2の電極に送達することと、
該スネア部をさらに後退させ、該身体から該組織塊を切断することと
を含む、方法。
(項目17)
エネルギーを上記第1および第2の電極に送達することは、上記組織塊を封着することをさらに含む、項目16に記載の方法。
(項目18)
エネルギー送達の間、上記組織塊の電気的特性を監視することと、
所定の条件を検出することと、
該組織塊の切断準備ができたことを合図することと
をさらに含む、項目17に記載の方法。

Claims (6)
- 組織除去システムであって、該組織除去システムは、
近位端と遠位端とを有する管状部材と、
該管状部材の該遠位端から動作可能に延出可能なスネア部であって、該スネア部は、第1の電極および第2の電極を含み、該第1の電極および該第2の電極は、その間の組織を治療するように構成され、該第1の電極および該第2の電極のうちの少なくとも1つは、該第1の電極または該第2の電極のうちのそれぞれの電極の近位端上に搭載されたスペーサを含み、該スペーサは、該第1の電極と該第2の電極との間の接触を防止するように構成され、該スネア部は、該治療された組織を切断するために、該第1の電極と該第2の電極との間に配置された鋭利表面を有する切除部材をさらに含む、スネア部と
を備える、組織除去システム。 - 前記鋭利表面は、近位方向に引っ張られるときのみ治療された組織を切除するように構成されている、請求項1に記載の組織除去システム。
- 前記組織は、ポリープであり、前記第1の電極および前記第2の電極は、ポリープの茎を把持するように構成されている、請求項1に記載の組織除去システム。
- 前記スネア部は、前記管状部材内に後退させられるように構成されている、請求項1に記載の組織除去システム。
- 前記第1の電極および前記第2の電極は各々、組織接触表面および絶縁本体部から形成されている、請求項1に記載の組織除去システム。
- 前記第1の電極および前記第2の電極は、電気外科発電機に電気的に結合され、該電気外科発電機は、両極の態様で該第1の電極および該第2の電極にエネルギーを送達するように構成されている、請求項1に記載の組織除去システム。
