図7−図10に、自己閉鎖組織ファスナにおける組織の処置、部位への配置、およびファスナの展開を可能とする展開デバイス内に配置された本発明の好適な実施形態の斜視図および断面図を示す。
本発明の実施形態の説明において、自己閉鎖組織ファスナの機能部分を含む材料が、説明したような変形応力を受けている状態および平面状態での必要とされる変形に耐えられる能力を基準に選択され、さらに、図1から図6で示されているがこれらに限定されない多くの実施形態の幾何形状で定義され、さらに、機械的故障および破損がなく、図7から図10の装置で説明された方法で使用されることが好ましいが、このことは実施形態の機能性に限定されない。ファスナを送出装置に装填し、それにより、ファスナ26(または、他の名称)に変形応力またはひずみを与えることにより、組織を定位置に留めることができる元々の平面形態に十分に戻ることができないくらいにファスナを恒久に変形させるような十分な大きさの変形応力を発生させることがないようにするのが好ましい。理想的には、ファスナは実質的な平面形態に戻る。しかし、ファスナのある程度の恒久変形、ならびに、組織の妨げによるある程度の非復元状態は、許容可能である。ファスナが組織を定位置に保持している限り、ファスナが完全な平面から歪むことは許容可能である。約50%の復元という数字は、本明細書では材料選定の際の基準として使用されるが、材料の選択の際に使用されるのは組織を定位置に保持するための機能性であること、ならびに、ある材料が医療移植片としての使用に適していることが知られている場合、その材料が適切であるかどうかを判断するときにはほんの簡単な試験のみが必要となることを理解されたい。
Claims (17)
- 組織を留めるためのデバイスであって、前記デバイスが、
平面の形態および円筒の形態を有する単一の閉じたリングを備え、前記リングが、ねじれエネルギーを加えることによって前記平面の形態から前記円筒の形態に変形され、
前記リングが、当該リングの円周の一部に沿ってそれぞれ延在している複数の機能領域を有し、前記機能領域が、
a)それぞれ90°以上のねじれ回転に耐えることができる複数のねじれ可能領域と、
b)少なくとも1つの組織係合突出部および相互連結のない、少なくとも1つの自由に延伸する安定化突出部が備わる中央スパインをそれぞれ有し、それぞれ一対の前記ねじれ可能領域に連結された、トルク下での変形に対して抵抗力を生じる1つまたは複数の中央領域であって、前記組織係合突出部が、前記リングの一方の側から、前記円筒の形態にある当該リングの中心軸に対して平行な軸に沿って突出し、前記安定化突出部が、前記リングの他方の側から、前記円筒の形態にある当該リングの中心軸に対して平行な軸に沿って突出する中央領域と、
c)それぞれ一対の前記ねじれ可能領域に連結された1つまたは複数の相互連結領域と
を備え、
組織と係合するための、前記円筒の形態から前記平面の形態への前記組織係合突出部の再方向付けが、前記リングに蓄積されたねじれエネルギーによってなされる、デバイス。 - 前記1つまたは複数の相互連結領域が、前記ねじれ可能領域および前記中央領域の一方の材料と同じ材料を含む、請求項1に記載のデバイス。
- 前記リングの2つ以上の場所で、90°以上のねじれ回転に耐えることができる材料から作られる、請求項1に記載のデバイス。
- 最大90°以上のねじれ回転が、前記円筒の形態から前記平面の形態への前記組織係合突出部の再方向付けに際して復元可能である、請求項1に記載のデバイス。
- 90°以上のねじれ回転が、少なくとも45°復元可能である、請求項1に記載のデバイス。
- 前記リングの内側円周の90%から105%の範囲の外側円周を有するマンドレル上に装着されることにより、前記平面の形態から前記円筒の形態にねじれ変形するように構成された、請求項1に記載のデバイス。
- 前記平面の形態にある前記リングから外側に突出している前記安定化突出部に対し、当該デバイスが前記平面の形態から前記円筒の形態に変形するまで、互いに向けて力を加えることにより、前記平面の形態から前記円筒の形態にねじれ変形するように構成された、請求項1に記載のデバイス。
- 前記安定化突出部が、当該デバイスを、管の内部において追加の拘束装置なしで前記円筒のリング形態で安定して保持するように構成され、前記管の内側円周が、前記円筒のリング形態にあるデバイスの外側円周の95%から120%の範囲にある、請求項1に記載のデバイス。
- ステンレス鋼、インコネル(商標)、ニチノール、モネル、ハステロイ(商標)、エルジロイ(商標)、タングステン、チタニウム、ならびにこれらの合金、混合物、積層物、複合物および組合せからなるグループから選択される材料から少なくとも部分的に作られる、請求項1に記載のデバイス。
- 少なくとも前記ねじれ可能領域が、超弾性材料を含む、請求項1に記載のデバイス。
- 中央空間を画定する閉じた部材を備える、組織を係合するデバイスであって、
前記閉じた部材が、
前記閉じた部材が弛緩状態からねじれ歪み状態へ変形されるように、それぞれ90°以上ねじられることができる複数のねじれ可能領域と、
一対のねじれ可能領域の間の少なくとも1つの相互連結領域と、
一対のねじれ可能領域の間の少なくとも1つの中央領域と
を備え、
前記少なくとも1つの中央領域が、トルク下での変形に対して抵抗力を生じ、安定化要素および穿孔部材が備わる中央スパインを含み、
前記安定化要素が、前記閉じた部材の一方の側から突出し、前記穿孔部材が、前記閉じた部材の他方の側から突出し、
前記穿孔部材は、前記閉じた部材が前記ねじれ歪み状態から解放されたとき、組織と係合するように構成され、
前記安定化要素は、相互連結のない、自由に延伸する突出部である、デバイス。 - 前記閉じた部材が、円形、楕円形、卵形、長方形、三角形、正方形、および多角形からなるグループから選択される形状を有する、請求項11に記載のデバイス。
- 前記ねじれ歪み状態から解放されたとき、デバイスに蓄積されたねじれエネルギーが、当該デバイスを弛緩状態に復帰させる、請求項11に記載のデバイス。
- 前記ねじれ可能領域が、超弾性材料を含む、請求項11に記載のデバイス。
- 前記弛緩状態において、前記穿孔部材が中央領域に突出する、請求項11に記載のデバイス。
- 前記穿孔部材から突出するバーブをさらに含む、請求項11に記載のデバイス。
- 前記閉じた部材が、複数の相互連結領域および複数の中央領域をさらに含む、請求項11に記載のデバイス。
