Description
本発明は、医療装置の分野、特に、経皮的に送達可能な心臓弁、および、経皮的に送達可能な心臓弁の作成方法に関する。
心臓弁膜症は、生理学的機能を低下させ、当該機能を制限する症状を引き起こす一般的な変性状態であり、世界中で数百万の患者の生命を脅かしている。根本的な原因は様々であるが、心臓弁の機能異常は、突き詰めれば、解剖学的通路が狭くなること(狭窄)による、弁の面を通過する血液の輸送不良、または、血液が再び弁を通過して戻ってきてしまい、これによって弁を通過した血液が正しい方向へ効果的に輸送されにくくなる閉鎖不良(機能不全または逆流)と表現される。これらの血流力学的状態は、1)心拍出量の欠乏、2)心臓のポンピングチャンバーに対する不都合な負荷、を招く。これらはいずれも、換言すれば、患者の機能的障害、または、有効な矯正がなされなかった場合には、しばしば早期の死を引き起こす。
本発明は様々な異なる変形例または実施形態を含み得るものであり、また、この概要は限定や包括を意図したものでないと理解されたい。この概要には、いくつかの実施形態が一般的な表現で記載されているが、他の実施形態をより具体的に記載したものも含まれている。
1つ以上の本発明の、上述した利点および特徴点、並びに、別の利点および特徴点を更に明確にするために、添付の図面に記載された特定の実施形態を参照しながら、上記1つ以上の本発明の更に具体的な記載について説明する。これらの図面は、1つ以上の本発明の特定の実施形態を説明したに過ぎず、それ故に、これらの図面は、本発明の範囲を限定しない。1つ以上の本発明は、添付されている以下の図面を用いながら、更なる特徴点および細部について記載および説明される。
図8Cは、図8Bに示す、折り畳む工程の後に形成されたひだとともに縫合された後の2枚の組織切片を示す図である(詳細は後述)。
本明細書に記載されている1つまたは複数の発明の実施形態は、人工心臓弁に関連する1つまたは複数の装置、アセンブリ、および/または方法を含んでいる。本明細書に記載されている少なくとも1つの実施形態における人工心臓弁は、カテーテルを通した患者内の移植部位への経皮的な送達のように、外科的に移植されてもよい。本明細書に記載されている人工心臓弁の1つまたは複数の実施形態は、少なくとも、構造上の欠陥および罹患した弁を含む大動脈弁および肺動脈弁の部位への適用が可能である。
(a)工程248aにおいて、上記組織を、温度が略4〜37℃の1〜37.5%のホルマリン、理想的には緩衝溶液中に、略3日間〜5週間、より好ましくは略3日間〜4週間、さらに好ましくは略3週間〜4週間浸漬する。そしてより好ましくは、温度20℃の10%のホルマリン中に6日間浸漬する。
(b)工程248bにおいて、上記組織を4〜37℃の100%のグリセロール中に最長で6週間浸漬する。そしてより好ましくは、上記組織を20℃の100%グリセロール中に略3週間浸漬する。
(c)工程248cにおいて、上記組織を、0〜37℃の0.1〜25%のグルタルアルデヒド中に略3日間〜5週間、より好ましくは略3日間〜4週間、さらに好ましくは略3週間〜4週間浸漬する。そしてより好ましくは、4℃の0.25%のグルタルアルデヒド中に7日間浸漬する。
(d)工程248dにおいて、上記組織を、0〜37℃の0.1〜25%のグルタルアルデヒド(オリゴマー成分を制限するフィルター処理済み)中に略3日間〜5週間、より好ましくは略3日間〜4週間、さらに好ましくは略3週間〜4週間浸漬する。そしてより好ましくは、4℃の0.25%のグルタルアルデヒド中に7日間浸漬する。
(e)工程248eにおいて、上記組織を、上述のホルマリン、グルタルアルデヒドまたはオリゴマーフォルター処理が施されたグルタルアルデヒド溶液にアミノ酸であるリジンおよび/またはヒスチジンを加えたものに浸漬する。なお、上記の固定剤の添加物として用いられる上記アミノ酸(L−リジンまたはヒスチジン)の濃度は略100〜100mMであり、好ましい値は684mMである。
上記に加え、上に挙げられた処理の組み合わせが実行されてもよく、例えば、工程(a)の後に工程(b)が続いてもよいし、工程(a)の後に工程(c)が続いてもよいし、工程(a)の後に工程(d)が続いてもよい。
Claims (10)
- 組織を含んでいる人工心臓弁を、部分的に圧縮して送達カテーテル上にマウントする工程と、
上記組織を、少なくとも部分的に乾燥させる工程と、
上記人工心臓弁を、さらに圧縮して送達カテーテルにマウントする工程と、
上記人工心臓弁および上記送達カテーテルを、殺菌および包装する工程と、を含むことを特徴とする方法。 - 上記無菌包装された人工心臓弁および送達カテーテルを輸送する工程をさらに含むことを特徴とする請求項1に記載の方法。
- 上記組織は、処置された心膜組織を含んでいることを特徴とする請求項1に記載の方法。
- 上記人工心臓弁を部分的に圧縮して上記送達カテーテル上にマウントする工程に先立って、上記組織は、(a)実質的に乾燥していないもの、および、(b)少なくとも部分的に水を含んでいるもの、の少なくともいずれか1つであることを特徴とする請求項1に記載の方法。
- フレームに組織を取り付ける工程と、
上記フレームを、当該フレームに取り付けられた組織とともに、部分的に圧縮して送達カテーテル上にマウントする工程と、
上記組織を、少なくとも部分的に乾燥させる工程と、
上記フレームを、当該フレームに取り付けられた組織とともに、さらに圧縮して送達カテーテル上にマウントする工程と、
上記フレームおよび上記送達カテーテルを、当該フレームに取り付けられた組織とともに、殺菌および包装する工程と、を含むことを特徴とする方法。 - 上記フレームを部分的に圧縮してマウントする工程に先立って、上記組織は、(a)実質的に乾燥していないもの、および、(b)少なくとも部分的に水を含んでいるもの、の少なくともいずれか1つであることを特徴とする請求項5に記載の方法。
- 上記送達カテーテル上にマウントされている上記無菌包装されたフレームを、当該フレームに取り付けられている組織とともに、外科用または医療用の処置設備へ輸送する工程をさらに含むことを特徴とする請求項5に記載の方法。
- 上記フレームに上記組織を取り付ける工程に先立って、上記組織が折り畳まれて、組織小葉アセンブリが形成されることを特徴とする請求項5に記載の方法。
- 上記組織小葉アセンブリは、少なくとも1つの折り返しと、少なくとも1つのひだと、を有していることを特徴とする請求項8に記載の方法。
- 上記組織は、処置された心膜組織を含んでいることを特徴とする請求項5に記載の方法。
